    Effect of femoral head size, subject weight and activity level on acetabular cement mantle stress following total hip arthroplasty

    In cases where cemented components are used in total hip arthroplasty, damage or disruption of the cement mantle can lead to aseptic loosening and joint failure. Currently, the relationship between subject activity level, obesity and prosthetic femoral head size and the risk of aseptic loosening of the acetabular component in cemented THA is not well understood. This study aims to provide an insight into this. Finite element models, validated with experimental data, were developed to investigate stresses in the acetabular cement mantle and pelvic bone resulting from the use of three prosthetic femoral head sizes, during a variety of daily activities and one high impact activity (stumbling) for a range of subject body weights. We found that stresses in the superior quadrants of the cortical bone‐cement interface increased with prosthetic head size, patient weight and activity level. In stumbling, average von Mises stresses (22.4 MPa) exceeded the bone cement yield strength for an obese subject (143 kg) indicating that the cement mantle would fail. Our results support the view that obesity and activity level are potential risk factors for aseptic loosening of the acetabular component and provide insight into the increased risk of joint failure associated with larger prosthetic femoral heads

    Rotating BPS black holes in matter-coupled AdS(4) supergravity

    Using the general recipe given in arXiv:0804.0009, where all timelike supersymmetric solutions of N=2, D=4 gauged supergravity coupled to abelian vector multiplets were classified, we construct genuine rotating supersymmetric black holes in AdS(4) with nonconstant scalar fields. This is done for the SU(1,1)/U(1) model with prepotential F=-iX^0X^1. In the static case, the black holes are uplifted to eleven dimensions, and generalize the solution found in hep-th/0105250 corresponding to membranes wrapping holomorphic curves in a Calabi-Yau five-fold. The constructed rotating black holes preserve one quarter of the supersymmetry, whereas their near-horizon geometry is one half BPS. Moreover, for constant scalars, we generalize (a supersymmetric subclass of) the Plebanski-Demianski solution of cosmological Einstein-Maxwell theory to an arbitrary number of vector multiplets. Remarkably, the latter turns out to be related to the dimensionally reduced gravitational Chern-Simons action.Comment: 23 pages, uses JHEP3.cl

    Order-of-magnitude speedup for steady states and traveling waves via Stokes preconditioning in Channelflow and Openpipeflow

    Steady states and traveling waves play a fundamental role in understanding hydrodynamic problems. Even when unstable, these states provide the bifurcation-theoretic explanation for the origin of the observed states. In turbulent wall-bounded shear flows, these states have been hypothesized to be saddle points organizing the trajectories within a chaotic attractor. These states must be computed with Newton's method or one of its generalizations, since time-integration cannot converge to unstable equilibria. The bottleneck is the solution of linear systems involving the Jacobian of the Navier-Stokes or Boussinesq equations. Originally such computations were carried out by constructing and directly inverting the Jacobian, but this is unfeasible for the matrices arising from three-dimensional hydrodynamic configurations in large domains. A popular method is to seek states that are invariant under numerical time integration. Surprisingly, equilibria may also be found by seeking flows that are invariant under a single very large Backwards-Euler Forwards-Euler timestep. We show that this method, called Stokes preconditioning, is 10 to 50 times faster at computing steady states in plane Couette flow and traveling waves in pipe flow. Moreover, it can be carried out using Channelflow (by Gibson) and Openpipeflow (by Willis) without any changes to these popular spectral codes. We explain the convergence rate as a function of the integration period and Reynolds number by computing the full spectra of the operators corresponding to the Jacobians of both methods.Comment: in Computational Modelling of Bifurcations and Instabilities in Fluid Dynamics, ed.     Maximum Likelihood Finite-Element Model Updating of Civil Engineering Structures Using Nature-Inspired Computational Algorithms

Such updating process is usually performed under the maximum likelihood method in practical engineering applications. According to this, the updating problem is transformed into an optimization problem. The objective function of this problem is usually defined in terms of the relative differences between the numerical and the experimental modal properties of the structure. To this aim, either (1) a single-objective or (2) a multi-objective approach may be adopted. Due to the complexity of the problem, global optimizers are usually considered for its solution. Among these algorithms, nature-inspired computational algorithms have been widely employed. Nevertheless, such model updating approach presents two main limitations: (1) a clear dependence between the updated model and the objective function considered; and (2) a high computational cost. In order to overcome these drawbacks, a detailed study has been performed herein both to establish the most adequate objective function to tackle the problem and to further assist in the selection of the most efficient computational algorithm among several well-known ones.     Finite-element-model updating of civil engineering structures using a hybrid UKF-HS algorithm

Among the different methods to tackle the updating problem, the maximum likelihood method has been widely used for practical engineering applications. In this method, the updating problem is transformed into an optimization problem where the relative differences between the numerical and experimental modal properties of the structure are reduced via the modification of the most relevant physical parameters of the model. However, this method often presents the drawback of requiring high simulation times in order to perform the updating process when dealing with complex structures. To overcome this limitation, in this paper a novel hybrid Unscented Kalman Filter – Harmony Search (UKF-HS) algorithm is proposed and its implementation details are discussed. In order to validate such hybrid algorithm and further illustrate its performance, the finite-element-model updating of a benchmark footbridge is performed using two different approaches (single-objective and multi-objective) and three different computational algorithms, namely: (i) genetic algorithms; (ii) harmony search; and (iii) the novel UKF-HS hybrid algorithm.     A probabilistic unified approach for power indices in simple games

A unified probabilistic way to define some of these power indices is considered in this paper. We show that six well-known power indices are obtained under such a probabilistic approach.     Citrulline as a marker of intestinal function and absorption in clinical settings: A systematic review and meta-analysis

    Background: Citrulline has been described as a marker of intestinal function or absorption but evidence varies according to clinical settings. Objective: The objective of this article is to examine the evidence of plasma citrulline as a marker of intestinal function and absorption in various clinical settings. Methods: Studies were examined for p values, means and standard deviations, correlation coefficients or other metrics depicting the association of citrulline with intestinal function. A random effects model was used to produce a pooled estimate. A hierarchical summary receiver operating curve model was fitted for diagnostic accuracy measures. Results: Citrulline levels are correlated strongly with small bowel length in short bowel syndrome patients (r = 0.67). Citrulline is strongly negatively correlated (r = –0.56) with intestinal disease severity with regards to enteropathies (coeliac disease, tropical enteropathy, Crohn’s disease, mucositis, acute rejection in intestinal transplantation). Citrulline cut-off levels have an overall sensitivity and specificity of 80% and 84% respectively. Citrulline levels in untreated coeliac patients compared to controls were reduced by 10 µmol/l. Citrulline levels increase with gluten-free diet and with improvement of enteropathy. Citrulline is decreased in critical illness and sepsis. Conclusion: These findings allow us to advocate quite reasonably that citrulline is a marker of acute and chronic intestinal insufficiency

    Vitamin food fortification today

    Historically, food fortification has served as a tool to address population-wide nutrient deficiencies such as rickets by vitamin D fortified milk. This article discusses the different policy strategies to be used today. Mandatory or voluntary fortification and fortified foods, which the consumer needs, also have to comply with nutritional, regulatory, food safety and technical issues. The ‘worldwide map of vitamin fortification’ is analysed, including differences between develop and developing countries. The vitamins, folate and vitamin D, are taken as practical examples in the review of the beneficial effect of different strategies on public health. The importance of the risk–benefit aspect, as well as how to identify the risk groups, and the food vehicles for fortification is discussed

    The epidemiology of malaria in adults in a rural area of southern Mozambique

    BACKGROUND: Epidemiological studies of malaria in adults who live in malaria endemic areas are scarce. More attention to the natural history of malaria affecting adults is needed to understand the dynamics of malaria infection and its interaction with the immune system. The present study was undertaken to investigate the clinical, parasitological and haematological status of adults exposed to malaria, and to characterize parasites in these individuals who progressively acquire protective immunity. METHODS: A cross-sectional survey of 249 adults was conducted in a malaria endemic area of Mozambique. Clinical, parasitological and haematological status of the study population was recorded. Sub-microscopic infections and multiplicity of infections were investigated using polymerase chain reaction (PCR) and restriction fragment length polymorphism of Plasmodium falciparum merozoite surface protein 2 (msp2). RESULTS: Prevalence of P. falciparum infection by microscopy (14%) and PCR (42%) decreased progressively during adulthood, in parallel with an increase in the prevalence of sub-microscopic infections. Anaemia was only related to parasitaemia as detected by PCR. Multiplicity of infection decreased with age and was higher in subjects with high P. falciparum densities, highlighting density-dependent constraints upon the PCR technique. CONCLUSION: Adults of Manhiça progressively develop non-sterile, protective immunity against P. falciparum malaria. The method of parasite detection has a significant effect on the observed natural history of malaria infections. A more sensitive definition of malaria in adults should be formulated, considering symptoms such as diarrhoea, shivering and headache, combined with the presence of parasitaemia

    Plasticity and dystonia: a hypothesis shrouded in variability.

    Studying plasticity mechanisms with Professor John Rothwell was a shared highlight of our careers. In this article, we discuss non-invasive brain stimulation techniques which aim to induce and quantify plasticity, the mechanisms and nature of their inherent variability and use such observations to review the idea that excessive and abnormal plasticity is a pathophysiological substrate of dystonia. We have tried to define the tone of our review by a couple of Professor John Rothwell's many inspiring characteristics; his endless curiosity to refine knowledge and disease models by scientific exploration and his wise yet humble readiness to revise scientific doctrines when the evidence is supportive. We conclude that high variability of response to non-invasive brain stimulation plasticity protocols significantly clouds the interpretation of historical findings in dystonia research. There is an opportunity to wipe the slate clean of assumptions and armed with an informative literature in health, re-evaluate whether excessive plasticity has a causal role in the pathophysiology of dystonia
