US Caribbean fish trap fishery socioeconomic study
Concerns over the potential impacts of trap fishing on coral reefs and associated habitats prompted a socioeconomic study to characterize the U.S. Caribbean fish trap fishery in anticipation of management actions. Stratified random interviews of one hundred fishermen revealed the presence of a diverse fishery, with appreciable inter-island differences in levels of fishing dependence, fishing practices, and capital investment. High levels of fishing dependence Were observed among fishermen in the U.S. Virgin Islands, whereas Puerto Rican fishermen exhibited a more diversified livelihood strategy. Fishermen from St. Croix derived 62% of their household income from fish traps, significantly more than fishermen from St. Thomas/St. John and Puerto Rico, who derived 45% and 41%, respectively, of their household incomes from fish traps. The St. Thomas/St. John fleet was also larger and more capital-intensive than the Crucian and Puerto Rican fleets. This structural heterogeneity suggests that fishermen from the various islands may respond differently to the same regulatory constraint. Thus, targeted policies may be necessary to improve the socioeconomic performance of the fishery and the political acceptability of management actions
Patient-maintained sedation for oral surgery using a target-controlled infusion of propofol - a pilot study
OBJECTIVE: To assess the safety and efficacy of a new patient-maintained propofol system for conscious sedation in dentistry. DESIGN: Prospective clinical trial SETTING: Department of Sedation, Glasgow Dental Hospital and School, 2001 SUBJECTS AND METHODS: Patients scheduled for oral surgery with conscious sedation. Exclusions included ASA IV -V, inability to use the handset, opioid use and severe respiratory disease. INTERVENTIONS: Patients were given intravenous propofol to a level of 1.0 microg/ml (reducing from 1.5 microg/ml) using a target controlled infusion system, they then controlled their sedation level by double-clicking a handset which on each activation increased the propofol concentration by 0.2 microg/ml. MAIN OUTCOME MEASURES: Oxygen saturation, patient satisfaction, and surgeon satisfaction. RESULTS: Twenty patients were recruited, 16 female and four male. Nineteen patients completed sedation and treatment successfully. Mean lowest oxygen saturation was 94%. No patients were over-sedated. All patients successfully used the system to maintain a level of sedation adequate for their comfort. Patient and surgeon satisfaction were consistently high. CONCLUSIONS: Initial experience with this novel system has confirmed safety, patient satisfaction and surgeon satisfaction
The application of a VUV Fourier transform spectrometer and synchrotron radiation source to measurements of: II. The δ(1,0) band of NO
Line-by-line photoabsorption cross-sections of the NO δ(1,0) band were measured with the VUV Fourier transform spectrometer from Imperial College, London, using synchrotron radiation at Photon Factory, KEK, Japan, as a continuum light source. The analysis of the NO δ(1,0) band provides accurate rotational line positions and term values as well as the photoabsorption cross-sections. The molecular constants of the C(1)2 II level are found to be T0 = 54 690.155±0.03 cm–1, Bv = 1.944 06±0.000 62 cm–1, Dv = (5.91±0.42)×10–5 cm–1, AD = –0.0187±0.0050 cm–1, p = –0.0189±0.0037 cm–1, and q = –0.015 21±0.000 20 cm–1. The sum of the line strengths for all rotational transitions of the NO δ(1,0) band is determined as 4.80×10–15 cm2 cm–1, corresponding to a band oscillator strength of 0.0054±0.0003.published_or_final_versio
The application of a VUV Fourier transform spectrometer and synchrotron radiation source to measurements of: I. the β(9,0) band of NO
State-of-the-art models of the vacuum ultraviolet (VUV) absorbing properties of the atmosphere call for absorption cross sections with detail on the scale of the Doppler widths. As a consequence, spectroscopic data at resolving powers of the order of 10 6 are needed. To meet these requirements in the vacuum ultraviolet region, we have used the VUV Fourier transform spectrometer from Imperial College, London, at the synchrotron radiation facility at Photon Factory, KEK, Japan, to measure photoabsorption cross sections of NO from 195 to 160 nm, and of O 2 from 185 to 175 nm. The analysis of the β(9,0) band (B 2Πr-X 2Πr) of NO provides accurate rotational line positions and term values. Molecular constants of the B(9) 2Πlevel are T 0=54205.097±0.012cm -1, A=45.320±0.021cm -1, B υ=1.01672±0.00016cm -1, D υ=(10.61±0.32)×10 -6cm -1, and A D=0.00122±0.00011cm -1. Health-state utilities in a prisoner population : a cross-sectional survey
Background: Health-state utilities for prisoners have not been described.
Methods: We used data from a 1996 cross-sectional survey of Australian prisoners (n = 734).
Respondent-level SF-36 data was transformed into utility scores by both the SF-6D and Nichol's
method. Socio-demographic and clinical predictors of SF-6D utility were assessed in univariate
analyses and a multivariate general linear model.
Results: The overall mean SF-6D utility was 0.725 (SD 0.119). When subdivided by various medical
conditions, prisoner SF-6D utilities ranged from 0.620 for angina to 0.764 for those with none/mild
depressive symptoms. Utilities derived by the Nichol's method were higher than SF-6D scores,
often by more than 0.1. In multivariate analysis, significant independent predictors of worse utility
included female gender, increasing age, increasing number of comorbidities and more severe
depressive symptoms.
Conclusion: The utilities presented may prove useful for future economic and decision models
evaluating prison-based health programs
Plant but not animal sourced nitrate intake is associated with lower dementia-related mortality in the Australian Diabetes, Obesity, and Lifestyle Study
Introduction: Dietary nitrate is potentially beneficial for cardiovascular, cerebrovascular, and nervous systems due to its role as a nitric oxide (NO) precursor. Increased nitrate intake improves cardiovascular health and therefore could protect against dementia, given the cardiovascular-dementia link. Objective: To investigate the association between source-dependent nitrate intake and dementia-related mortality. As individuals with diabetes are at higher risk of dementia, a secondary aim was to investigate if the associations between nitrate and dementia varied by diabetes mellitus (DM) and pre-diabetes status. Methods: This study involved 9,149 participants aged ≥25 years from the well-characterised Australian Diabetes, Obesity, and Lifestyle (AusDiab) Study followed over a period of 17 years. Intakes of plant-sourced, vegetable-sourced, naturally occurring animal-sourced nitrate, and processed meat (where nitrate is an allowed additive)-sourced nitrate were assessed from a 74-item food frequency questionnaire completed by participants at baseline and nitrate databases were used to estimate nitrate from these different dietary sources. Associations between source-dependent nitrate intake and dementia-related mortality were assessed using multivariable-adjusted Cox proportional hazards models adjusted for demographics, lifestyle, and dietary factors. Results: Over 17 years of follow-up, 93 (1.0%) dementia-related deaths occurred of 1,237 (13.5%) total deaths. In multivariable-adjusted models, participants with the highest intakes of plant-sourced nitrate (median intake 98 mg/day) had a 57% lower risk of dementia-related mortality [HR (95% CI): 0.43 (0.22, 0.87)] compared to participants with lowest intakes of plant-sourced nitrate (median intake 35 mg/day). A 66% lower risk was also seen for higher intakes of vegetable-sourced nitrate [HR (95% CI): 0.34 (0.17, 0.66)]. No association was observed for animal-sourced nitrate, but the risk was two times higher amongst those who consumed the most processed meat-sourced nitrate intake [HR (95%): 2.10 (1.07, 4.12)]. The highest intake of vegetable-sourced nitrate was associated with a lower risk of dementia-related mortality for those with and without DM and pre-diabetes. Conclusion: Encouraging the intake of nitrate-rich vegetables, such as green leafy vegetables and beetroot, may lower the risk of dementia-related mortality, particularly in individuals with (pre-) diabetes who are at a higher dementia risk
The depression in visual impairment trial (DEPVIT): trial design and protocol
<b>Background</b>
The prevalence of depression in people with a visual disability is high but screening for depression and referral for treatment is not yet an integral part of visual rehabilitation service provision. One reason for this may be that there is no good evidence about the effectiveness of treatments in this patient group. This study is the first to evaluate the effect of depression treatments on people with a visual impairment and co morbid depression.<p></p>
<b>Methods/design</b>
The study is an exploratory, multicentre, individually randomised waiting list controlled trial. Participants will be randomised to receive Problem Solving Therapy (PST), a ‘referral to the GP’ requesting treatment according to the NICE’s ‘stepped care’ recommendations or the waiting list arm of the trial. The primary outcome measure is change (from randomisation) in depressive symptoms as measured by the Beck’s Depression Inventory (BDI-II) at 6 months. Secondary outcomes include change in depressive symptoms at 3 months, change in visual function as measured with the near vision subscale of the VFQ-48 and 7 item NEI-VFQ at 3 and 6 months, change in generic health related quality of life (EQ5D), the costs associated with PST, estimates of incremental cost effectiveness, and recruitment rate estimation.<p></p>
<b>Discussion</b>
Depression is prevalent in people with disabling visual impairment. This exploratory study will establish depression screening and referral for treatment in visual rehabilitation clinics in the UK. It will be the first to explore the efficacy of PST and the effectiveness of NICE’s ‘stepped care’ approach to the treatment of depression in people with a visual impairment.<p></p>
The Impact of HAART on the Respiratory Complications of HIV Infection: Longitudinal Trends in the MACS and WIHS Cohorts
Objective: To review the incidence of respiratory conditions and their effect on mortality in HIV-infected and uninfected individuals prior to and during the era of highly active antiretroviral therapy (HAART). Design: Two large observational cohorts of HIV-infected and HIV-uninfected men (Multicenter AIDS Cohort Study [MACS]) and women (Women's Interagency HIV Study [WIHS]), followed since 1984 and 1994, respectively. Methods: Adjusted odds or hazards ratios for incident respiratory infections or non-infectious respiratory diagnoses, respectively, in HIV-infected compared to HIV-uninfected individuals in both the pre-HAART (MACS only) and HAART eras; and adjusted Cox proportional hazard ratios for mortality in HIV-infected persons with lung disease during the HAART era. Results: Compared to HIV-uninfected participants, HIV-infected individuals had more incident respiratory infections both pre-HAART (MACS, odds ratio [adjusted-OR], 2.4; 95% confidence interval [CI], 2.2-2.7; p<0.001) and after HAART availability (MACS, adjusted-OR, 1.5; 95%CI 1.3-1.7; p<0.001; WIHS adjusted-OR, 2.2; 95%CI 1.8-2.7; p<0.001). Chronic obstructive pulmonary disease was more common in MACS HIV-infected vs. HIV-uninfected participants pre-HAART (hazard ratio [adjusted-HR] 2.9; 95%CI, 1.02-8.4; p = 0.046). After HAART availability, non-infectious lung diseases were not significantly more common in HIV-infected participants in either MACS or WIHS participants. HIV-infected participants in the HAART era with respiratory infections had an increased risk of death compared to those without infections (MACS adjusted-HR, 1.5; 95%CI, 1.3-1.7; p<0.001; WIHS adjusted-HR, 1.9; 95%CI, 1.5-2.4; p<0.001). Conclusion: HIV infection remained a significant risk for infectious respiratory diseases after the introduction of HAART, and infectious respiratory diseases were associated with an increased risk of mortality. © 2013 Gingo et al
Cognitive and behavioral predictors of light therapy use
Objective: Although light therapy is effective in the treatment of seasonal affective disorder (SAD) and other mood disorders, only 53-79% of individuals with SAD meet remission criteria after light therapy. Perhaps more importantly, only 12-41% of individuals with SAD continue to use the treatment even after a previous winter of successful treatment. Method: Participants completed surveys regarding (1) social, cognitive, and behavioral variables used to evaluate treatment adherence for other health-related issues, expectations and credibility of light therapy, (2) a depression symptoms scale, and (3) self-reported light therapy use. Results: Individuals age 18 or older responded (n = 40), all reporting having been diagnosed with a mood disorder for which light therapy is indicated. Social support and self-efficacy scores were predictive of light therapy use (p's<.05). Conclusion: The findings suggest that testing social support and self-efficacy in a diagnosed patient population may identify factors related to the decision to use light therapy. Treatments that impact social support and self-efficacy may improve treatment response to light therapy in SAD. © 2012 Roecklein et al
