Astma ja allergia lievenevät Suomessa - apteekkien allergiabarometri 2010-2016

Teema : Astman tutkimukset ja hoidotLähtökohdat Apteekeissa on seurattu barometrikyselyillä astma- tai allergialääkkeitä ostavien potilaiden oireita sekä lääkkeiden ja terveydenhuollon käyttöä. Menetelmät Kyselyyn osallistui 956 ostajaa vuonna 2016 ja 1 114 vuonna 2010; Vastaajista 60 %:lla ja 59 %:lla oli oikeus erityiskorvaukseen pysyvästä astmalääkityksestä. Vertasimme heidän ilmoittamaansa oireiden vaikeutta. Tulokset Vuonna 2016 vastaajista 2,5 % arvioi astmansa vaikeaksi, vuonna 2010 luku oli 3,8 %. Vaikeita allergiaoireita, hengenahdistusta ja hengityksen vinkumista oli merkitsevästi vähemmän vuonna 2016. Lähes joka yö astmaoireisiin heränneiden määrä väheni 15 %:sta 8 %:iin. Päivystyskäyntejä astman vuoksi oli vuoden aikana 16 %:lla ja sairaalahoitoja 6 %:lla vuonna 2016 vastanneista. Päätelmät Astmaa sairastavien vointi on Suomessa parantunut ja vain pienellä osalla tauti on vaikea. ¬Kansallisen allergiaohjelman aikana 2010–16 oireilu on edelleen hieman lieventynyt. Päivystyskäyntejä ja sairaalahoidon tarvetta voidaan vielä vähentää.Peer reviewe

Astman ja allergian kustannusten myönteinen kehitys jatkui

Vertaisarvioitu. English summary.Lähtökohdat : Astman ja allergian kustannuksia selvitettiin vuodelta 2018 ja arvioitiin muutoksia Kansallisen allergiaohjelman (2008–2018) aikana. Yhtenä ohjelman tavoitteena oli vähentää kustannuksia 20 % ¬verrattuna ohjelmaa edeltäneeseen tasoon. Menetelmät : Sairaanhoidon kustannuksia, kuntoutusta, työkyvyttömyyttä ja niiden muutoksia tarkasteltiin hoitoilmoitusrekisterin, tilastojen ja tietokantahakujen avulla. Sairauspoissaolojen ja työkyvyn heikkenemisen kustannuksia arvioitiin kyselytutkimusten avulla. Tulokset : Vuonna 2018 astman ja allergian kustannukset olivat 1,5–1,8 miljardia euroa. Suorat kustannukset olivat 332 miljoonaa ja epäsuorat kustannukset 1,2–1,5 miljardia euroa. Astman osuus oli 205 (62 %) ja lääkehoidon 190 miljoonaa euroa (57 %). Allergiaohjelman aikana vertailukelpoiset suorat kustannukset vähenivät ¬5 % ja kaikki kustannukset yhteensä 30 %. Sairaalahoidon, työkyvyttömyyseläkkeiden ja ¬kuntoutuksen kustannukset vähenivät puoleen. Vain avohoitokäyntien kustannukset sekä matkakustannukset kasvoivat. Päätelmät : Astman ja allergisten sairauksien myönteinen kehitys jatkui allergiaohjelman jälkipuoliskolla. Sairaalahoitoa tarvitaan vähemmän, työkyvyttömyyseläkkeelle jäädään harvemmin ja kustannukset vähenevät. Ohjelma saavutti kustannustavoitteensa.Peer reviewe

Chronic Comorbidities Contribute to the Burden and Costs of Persistent Asthma

Background. We aimed to study the prevalence of chronic comorbidities in asthma patients and the costs of health care use associated with asthma with comorbidities. Material and Methods. We analysed the prevalence of the four most common chronic diseases in asthma patients in 2008-2014 in Finland. Prevalence of coronary artery disease, diabetes and dyslipidaemia, hypertension, epilepsy, inflammatory bowel disease, rheumatic diseases, and severe psychiatric disease was studied by register of the Social Insurance Institution of Finland. The costs of health care services were collected from the registries maintained by the National Institute for Health and Welfare (THL). Results. Prevalence of asthma was 4.6% in 2014. Diabetes was among the four most common comorbidities in all the age groups. The other common comorbidities were hypertension (>= 46 years; 12.9-37.6%), severe psychiatric disorders (age groups of 16-59 years; 1.4-3.5%), and ischaemic heart disease (>= 60 years; 10-25%). In patients with both asthma and diabetes, the costs of hospitalization were approximately 169% compared with patients with asthma alone. Conclusions. Prevalence of asthma increases by tenfold when aging. The comorbidity diversity and rate are age-dependent. Prevalence of diabetes as comorbidity in asthma has increased. Costs of hospitalizations in asthma approximately double with chronic comorbidities.Peer reviewe

The Finnish Allergy Program 2008-2018 : Society-wide proactive program for change of management to mitigate allergy burden

A 10-year national program to improve prevention and management of allergic diseases and asthma was implemented in Finland (population 5.5. million) in 2008-2018. The main aim was to reduce the long-term burden of these conditions. The strategy was changed from traditional avoidance to tolerance and resilience of the population. Health was endorsed instead of medicalization of mild symptoms. Disease severity was reevaluated, and disabling clinical manifestations were given high priority. For health care, 5 quantitative goals and 1 qualitative goal were set. For each of the goals, specific tasks, tools, and outcome evaluation were stipulated. During the program, 376 educational sessions gathered 24,000 health care participants. An information campaign targeted the lay public, and social media was used to contact people. In the 10 years of the program, the prevalence of allergic diseases and asthma leveled off. Asthma caused fewer symptoms and less disability, and 50% fewer hospital days. Food allergy diets in day care and schools decreased by half. Occupational allergies were reduced by 45%. In 2018, the direct and indirect costs of allergic diseases and asthma ranged from (sic)1.5 billion to (sic)1.8 billion, with the 2018 figures being 30% less than in the respective figures in 2007. The Finnish proactive and real-world intervention markedly reduced the public health burden of allergic disorders. The allergy paradigm was revisited to improve management with systematic education.Peer reviewe

Finnish nationwide allergy programme 2008–2018 changed attitudes and reduced morbidity

English summary.Lähtökohdat : Allergiaohjelma 2008–2018 on kansallinen kansanterveysohjelma, jonka avulla välttö¬strategia on käännetty sietostrategiaksi ja painotettu allergiaterveyttä. Raportoimme 10 vuoden tulokset. Menetelmät : Ohjelmalla oli kuusi tavoitetta, joiden toteuttamiseksi määriteltiin tehtävät, työkalut ja mittarit. Ohjelmaa toteutettiin kouluttamalla terveydenhuoltoa ja viestimällä väestölle. Tulokset : Astman ja allergisen nuhan esiintyvyys tasoittui asevelvollisissa ja Helsingin aikuisväestössä. Helsingin aikuisista astmaatikoista 41 % oli ollut vuoden 2016 kyselyä edeltäneen vuoden oireettomia (31 % 2006). Lasten allergiaruokavaliot vähenivät koko maassa noin puoleen. Työperäiset allergiset sairaudet vähenivät 45 %. Astman sairaalahoidon tarve puolittui, mutta päivystyskäynnit vähenivät oleellisesti vain lapsilla. Anafylaksia aiheutti aiempaa enemmän päivystyskäyntejä. Allergiasta ja astmasta aiheutuvat vuosittaiset suorat ja epäsuorat kustannukset vähenivät 200 miljoonaa euroa ¬(30 %) verrattaessa vuosia 2007 ja 2018. Päätelmät : Allergian ja astman aiheuttama sairastavuus ja niistä koituvat kustannukset vähenivät merkittävästi. Haitat vähenivät aluksi nopeasti, myöhemmin hitaammin. Ammattilaiset ja suuri yleisö hyväksyivät uuden suunnan, jossa painottuivat sietokyky ja terveys allergiasta huolimatta. An inventory of ready-to-use and publicly available tools for the safety assessment of nanomaterials

Legislation addressing environmental, health and safety aspects of nanomaterials in consumer products and ensuring their safe use is being continuously updated in the European Union and globally. This leads to a growing need for tools to implement this developing legislation. A freely accessible inventory of ready-to-use and publicly available tools that together cover the tasks within a nanomaterial safety assessment process was built in the presented work. This inventory is a unique metadata set in Excel® format: the 'NANoREG Toolbox', which assembles information needed for selecting and accessing instruments that meet specific goals. The recorded tools are categorised according to their purpose, type and regulatory status. The Toolbox covers an unprecedented and broad range of over 500 current tools, developed in Europe and beyond. While NANoREG focussed on safety assessment under the EU Regulation on Registration, Evaluation, Authorisation and Restriction of Chemicals(REACH), the instruments in the Toolbox are relevant and useful for nanomaterial safety assessments worldwide.JRC.F.2-Consumer Products Safet
