    Metronidazole Appears Not to Be a Human Teratogen: Review of Literature

    Metronidazole is used to treat trichomoniasis, bacterial vaginosis, and other diseases. As is the case with many drugs, physicians often hesitate to use it during pregnancy, particularly in the first trimester. A review of the nearly four decades' worth of published literature on metronidazole use in pregnant women indicates that it is not teratogenic, regardless of the trimester in which it is used. On the other hand, a number of published studies indicate that bacterial vaginosis and trichomoniasis are associated with preterm birth and low birth weight. Treatment of these conditions with metronidazole during pregnancy may decrease the incidence of preterm birth and low birth weight, thus potentially decreasing the complications that can result from prematurity

    The use of photographic caricatures to examine the development of a perception of facial attractiveness

    Throughout the human population, there is remarkable agreement as to what constitutes an attractive face. The consistency of attractiveness ratings across age, gender and culture has led to a search for an underlying construct that determines facial attractiveness. Langlois and Roggman (1990) proposed the average is attractive hypothesis arguing that facial attractiveness is determined by the level of averageness of facial features. Langlois and Roggman (1990) created composite faces to examine this hypothesis but their methodology was criticised, particularly because the technique used to create the composites tended to remove facial flaws and blemishes. This led to the argument that the increased attractiveness of the composite faces was the result of the smoothing of the faces rather than from their increased averageness. This study used photographic quality caricatures, which retain facial texture, to further examine the average is attractive hypothesis. From a digitised photograph, faces shifted away from the average by +18% and +36% (caricatures), and faces shifted closer to the average by -18% and -36% (anticaricatures) were produced. Along with the original photograph this provided five different versions of the same face varying only on averageness. Forty-eight of these face sets were created: six male and six female sets for the ages 6-, 8-, 10-year-olds and adults. Twenty participants in each of the age groups 6-, 8-, 1 0-year-olds and adults were asked to select the most and the least attractive face from each set. Examination of the mean caricature level chosen by each group found an overall preference for average faces providing support for the average is attractive hypothesis. The preference for average faces was present in the youngest age groups but the strength of the effect increased with age. There is, however, a suggestion that absolute averageness is not preferred, with some support for the idea of an optimum level of averageness

    Incorporation of privacy elements in space station design

    Privacy exists to the extent that individuals can control the degree of social contact that they have with one another. The opportunity to withdraw from other people serves a number of important psychological and social functions, and is in the interests of safety, high performance, and high quality of human life. Privacy requirements for Space Station crew members are reviewed, and architectual and other guidelines for helping astronauts achieve desired levels of privacy are suggested. In turn, four dimensions of privacy are discussed: the separation of activities by areas within the Space Station, controlling the extent to which astronauts have visual contact with one another, controlling the extent to which astronauts have auditory contact with one another, and odor control. Each section presents a statement of the problem, a review of general solutions, and specific recommendations. The report is concluded with a brief consideration of how selection, training, and other procedures can also help Space Station occupants achieve satisfactory levels of seclusion

    Tadalafil in patients with chronic obstructive pulmonary disease:a randomised, double-blind, parallel-group, placebo-controlled trial

    SummaryBackgroundPhosphodiesterase-5 (PDE5) inhibitors improve exercise capacity and quality of life in patients with idiopathic pulmonary arterial hypertension. However, whether such beneficial effects take place in selected populations with chronic obstructive pulmonary disease (COPD) remains uncertain. We aimed to assess the effects of tadalafil—a PDE5 inhibitor—on exercise capacity and quality of life in patients with COPD and mild pulmonary hypertension.MethodsWe did a randomised, double-blind, parallel-group, placebo-controlled trial at three centres in Scotland, UK, between Sept 1, 2010, and Sept 1, 2012. Patients with moderate to severe COPD were randomly assigned (1:1), via centralised randomisation with a computer-generated sequence and block sizes of four, to receive daily tadalafil 10 mg or placebo for 12 weeks. Patients, study investigators, outcome assessors, and those administering drugs were masked to group allocation. The primary endpoint was the mean placebo-corrected difference between the baseline and final 6 min walk distance after 12 weeks. We measured change in quality of life at baseline, 8 weeks, and 12 weeks, with standardised questionnaires. Analysis was per protocol and by intention to treat. This trial is registered with ClinicalTrials.gov, number NCT01197469.Findings120 patients were randomly assigned to receive tadalafil (n=60) or placebo (n=60), of whom 56 (93%) versus 57 (95%) completed the study. At 12 weeks the difference in 6 min walking distance between the tadalafil and placebo groups was 0·5 m (95% CI −11·6 to 12·5; p=0·937). We recorded no statistically significant changes in quality of life (between-group difference on the St George's Respiratory Questionnaire −2·64 [95% CI −6·43 to 1·15]; Research and Development version 1 short-form-36 4·08 [–1·35 to 9·52]; Minnesota Living with Heart Failure questionnaire −2·31 [–7·06 to 2·45]). 19 (32%) of 60 patients in the treatment group had dyspepsia; the severity of dyspepsia ranged from mild to severe, with four (21%) of 19 patients needing a proton-pump inhibitor. Five (8%) of 60 participants had dyspepsia in the placebo group. Headache was noted in 17 (28%) patients in the treatment group versus 5 (8%) in the placebo group, but was mild in all patients. Two (3%) patients in the treatment group had facial flushing, which resulted in one withdrawal. Other withdrawals within the tadalafil group happened after a transient ischaemic attack and two deaths (ruptured abdominal aortic aneurysm and pneumonia).InterpretationTadalafil does not improve exercise capacity or quality of life despite exerting pulmonary vasodilation.FundingChief Scientist Office for Scotland

    The rising number of people living with HIV/AIDS (PLWHA) worldwide has made health care professionals and policy makers search for accessible health care that will meet the needs of people who are suffering from the disease and enhance their quality of life (QoL).This study investigated met and unmet palliative care needs of PLWHA in selected areas in Rwanda.The study sample included 306 participants: PLWHA, health care professionals and coordinators of HIV/AIDS units. Quantitative and qualitative methodologies were used.The data were analysed separately and then triangulated. In the findings, over 50% of PLWHA had symptoms related to HIV/AIDS most of the time, with the most common symptom being pain.     A study of social relations in the recording of popular music

    The production of recordings is examined from a social production perspective. It is argued that "conventional sociology of art" presents a partisan view of creative activity which prevents it acknowledging the reality of cultural production today as exemplified by the recording of popular music. Some recent developments in related intellectual traditions show how "art" and "artists" are social constructions and lead towards a more inclusive, phenomenologically influenced, "social production" perspective. It is argued that the production of recordings takes place in the shadow of earlier work, within a structure of aesthetics and concepts of creativity created by the various institutions of the "art world", especially those of the cultural market place. The development of recording as a business in the U.K. is traced and contexted within the contemporary development of both national and international entertainment and cultural industries. The impact of business arrangements on the production and distribution of recordings is examined. Wider social concerns are shown to be assimilated into the finished recording through the structure of the work organisation responsible for its production. This incorporates both the characteristic capitalist division of labour and the related artistic division of labour, which affect the finished recording through the impact of specific working relations and practices on the distribution of opportunities for decision making on aesthetic matters amongst recording personnel. Similarly, the technology of recording which has a profound effect on the shape of the finished artifact is shown to mediate the priorities of capitalist organisations. Differing aesthetics adopted by recording personnel are shown to be related to the dominant technology of the time

    1. Using chicken models to avoid unnecessary harm, this study examined the relationship between naturally-occurring maxillary (top) beak shapes and their ability to cause pecking damage.2. A selection of 24 Lohmann Brown laying hens from a total population of 100 were sorted into two groups based on their maxillary beak shape, where 12 were classified as having sharp beaks (SB) and 12 as having blunt beaks (BB).3. All hens were recorded six times in a test pen which contained a chicken model (foam block covered with feathered chicken skin) and a video camera. During each test session, the number of feathers removed from the model, the change in skin and block weight (proxies for tissue damage) and the percentage of successful pecks (resulting in feather and/or tissue removal) were recorded.4. SB hens removed more feathers from the model and had a greater change in skin weight than BB hens. The mean number of pecks made at the model did not differ between the beak shape groups; however, SB hens had a greater percentage of successful pecks, resulting in feather and/or tissue removal, compared to BB hens.5. In conclusion, SB hens were more capable of removing feathers and causing damage. Birds performed more successful pecks resulting in feather and/or tissue removal as they gained experience pecking at the model.</p

    Pretreatment with beta-blockers and the frequency of hypokalemia in patients with acute chest pain

    Plasma potassium concentration was measured at admission in 1234 patients who presented with acute chest pain. One hundred and ninety five patients were on P blockers before admission. The potassium concentrations of patients admitted early (within four hours of onsetof symptoms) were compared with those admitted later (4-18 hours after onset of symptoms). There was a transient fall in plasma potassium concentrations in patients not pre-treated with , B blockers. This was not seen in patients who had been on P blockers before admission. Nonselective, B blockers were more effective than cardioselective agents in maintaining concentrationsof plasma potassium. These findings suggest a mechanism for the beneficial effects of ,B blockers on morbidity and mortality in acute myocardial infarction
