    Changing stroke mortality trends in middle-aged people: an age-period-cohort analysis of routine mortality data in persons aged 40 to 69 in England

    Background: In the UK, overall stroke mortality has declined. A similar trend has been seen in coronary heart disease, although recent reports suggest this decline might be levelling off in middle-aged adults. Aim: To investigate recent trends in stroke mortality among those aged 40–69 years in England. Methods: The authors used routine annual aggregated stroke death and population data for England for the years 1979–2005 to investigate time trends in gender-specific mortalities for adults aged 40 to 69 years. The authors applied log-linear modelling to isolate effects attributable to age, linear ‘drift’ over time, time period and birth cohort. Results; Between 1979 and 2005, age-standardised stroke mortality aged 40 to 69 years dropped from 93 to 30 per 100 000 in men and from 62 to 18 per 100 000 in women. Mortality was higher in older age groups, but the difference between the older and younger age groups appears to have decreased over time for both sexes. Modelling of the data suggests an average annual reduction in stroke deaths of 4.0% in men and 4.3% in women, although this decrease has been particularly marked in the last few years. However, we also observed a relative rate increase in mortality among those born since the mid-1940s compared with earlier cohorts; this appears to have been sustained in men, which explains the levelling off in the rate of mortality decline observed in recent years in the younger middle-aged. Conclusions: If observed trends in middle-aged adults continue, overall stroke mortalities may start to increase again

    Effect of forward motion on engine noise

    Methods used to determine a procedure for correcting static engine data for the effects of forward motion are described. Data were analyzed from airplane flyover and static-engine tests with a JT8D-109 low-bypass-ratio turbofan engine installed on a DC-9-30, with a CF6-6D high-bypass-ratio turbofan engine installed on a DC-10-10, and with a JT9D-59A high-bypass-ratio turbofan engine installed on a DC-10-40. The observed differences between the static and the flyover data bases are discussed in terms of noise generation, convective amplification, atmospheric propagation, and engine installation. The results indicate that each noise source must be adjusted separately for forward-motion and installation effects and then projected to flight conditions as a function of source-path angle, directivity angle, and acoustic range relative to the microphones on the ground

    Sound-Induced Flash Illusion is Resistant to Feedback Training

    A single flash accompanied by two auditory beeps tends to be perceived as two flashes (Shams et al. Nature 408:788, 2000, Cogn Brain Res 14:147–152, 2002). This phenomenon is known as ‘sound-induced flash illusion.’ Previous neuroimaging studies have shown that this illusion is correlated with modulation of activity in early visual cortical areas (Arden et al. Vision Res 43(23):2469–2478, 2003; Bhattacharya et al. NeuroReport 13:1727–1730, 2002; Shams et al. NeuroReport 12(17):3849–3852, 2001, Neurosci Lett 378(2):76–81, 2005; Watkins et al. Neuroimage 31:1247–1256, 2006, Neuroimage 37:572–578, 2007; Mishra et al. J Neurosci 27(15):4120–4131, 2007). We examined how robust the illusion is by testing whether the frequency of the illusion can be reduced by providing feedback. We found that the sound-induced flash illusion was resistant to feedback training, except when the amount of monetary reward was made dependent on accuracy in performance. However, even in the latter case the participants reported that they still perceived illusory two flashes even though they correctly reported single flash. Moreover, the feedback training effect seemed to disappear once the participants were no longer provided with feedback suggesting a short-lived refinement of discrimination between illusory and physical double flashes rather than vanishing of the illusory percept. These findings indicate that the effect of sound on the perceptual representation of visual stimuli is strong and robust to feedback training, and provide further evidence against decision factors accounting for the sound-induced flash illusion

    The influence of seminal plasma on offspring development and health

    The concept that a father’s wellbeing at the time of conception influences the development and long-term health of his offspring is now well established. However, the mechanisms underlying the paternal programming of offspring health are not fully defined. While sperm-mediated effects on offspring development have been investigated in detail, the significance of seminal plasma has been over-looked. Typically, the seminal plasma is viewed as a simple medium, with a main role to transport sperm into the female reproductive tract at the time of conception. However, a more sophisticated role for seminal plasma in the modulation of the maternal periconception cell-signalling, inflammatory and immunological physiology is emerging. Seminal plasma comprises a complex mix of nutrients, proteins, signalling molecules and cell-free genetic material which all interact with the endometrium to regulate gene expression, vascular remodelling, leukocyte recruitment and the priming of regulatory T cells (Tregs). These seminal plasma effects on the maternal periconception environment all act to facilitate uterine remodelling, embryo implantation and fetal development. Evidence is now emerging that poor paternal lifestyle factors such as diet, can modify these essential uterine responses, altering fetal development and ultimately long-term offspring health. The use of animal models has enhanced our understanding of the effects of seminal plasma on maternal uterine physiology, embryo development and offspring health. However, further studies are needed to define the interaction between seminal plasma components and female reproductive tissues in humans. Such studies will be central in providing better information and infertility treatments to intending parents

    Ka-band Ga-As FET noise receiver/device development

    The development of technology for a 30 GHz low noise receiver utilizing GaAs FET devices exclusively is discussed. This program required single and dual-gate FET devices, low noise FET amplifiers, dual-gate FET mixers, and FET oscillators operating at Ka-band frequencies. A 0.25 micrometer gate FET device, developed with a minimum noise figure of 3.3 dB at 29 GHz and an associated gain of 7.4 dB, was used to fabricate a 3-stage amplifier with a minimum noise figure and associated gain of 4.4 dB and 17 dB, respectively. The 1-dB gain bandwidth of this amplifier extended from below 26.5 GHz to 30.5 GHz. A dual-gate mixer with a 2 dB conversion loss and a minimum noise figure of 10 dB at 29 GHz as well as a dielectric resonator stabilized FET oscillator at 25 GHz for the receiver L0. From these components, a hybrid microwave integrated circuit receiver was constructed which demonstrates a minimum single-side band noise figure of 4.6 dB at 29 GHz with a conversion gain of 17 dB. The output power at the 1-dB gain compression point was -5 dBm

    The role of ECL2 in CGRP receptor activation: a combined modelling and experimental approach

    The calcitonin gene-related peptide (CGRP) receptor is a complex of a calcitonin receptor-like receptor (CLR), which is a family B G-protein-coupled receptor (GPCR) and receptor activity modifying protein 1. The role of the second extracellular loop (ECL2) of CLR in binding CGRP and coupling to Gs was investigated using a combination of mutagenesis and modelling. An alanine scan of residues 271–294 of CLR showed that the ability of CGRP to produce cAMP was impaired by point mutations at 13 residues; most of these also impaired the response to adrenomedullin (AM). These data were used to select probable ECL2-modelled conformations that are involved in agonist binding, allowing the identification of the likely contacts between the peptide and receptor. The implications of the most likely structures for receptor activation are discussed.</jats:p

    ‘At school I got myself a certificate’: HIV/AIDS Orphanhood and Secondary Education: a Qualitative Study of Risk and Protective Factors

    Secondary school is a period during which risk of school dropout is highest. To date, little research has examined reasons for school dropout amongst HIV/AIDS-orphaned children, who are affected economically, psychosocially and educationally. HIV/AIDS orphanhood can perpetuate poverty and increase school dropout in a range of ways, including inability to pay school fees, family disruption and stigma. Related research mostly focuses on school performance and completion and, more recently, on context-specific approaches to measure educational outcomes. The primary aim of this qualitative study was to examine how HIV/AIDS orphanhood influences participation in secondary education in South Africa and to investigate why some HIV/AIDS-orphaned adolescents find it easier to stay in school than others. Specifically, the study aimed to explore, interpret and elicit the perceptions of South African HIV/AIDS-orphaned adolescents (N = 243, aged 13–22, 53 % female, 47 % male) towards potential risk and protective factors influencing their secondary school attendance. Findings suggest complex and interconnecting multiple risk factors such as poverty pre- and post-parental death, crisis-fosterage often accompanied by further traumatic events, changing schools sporadically, recurring household migration and forced employment all of which can create obstacles to secondary education. Protective factors that emerged included extra-curricular activities and teacher/community support. These findings highlight the need for evidence-informed school and community policies, which consider the effects of household poverty, migration and living arrangements, if secondary school participation for HIV/AIDS-orphaned adolescents is to be promoted and sustained
