    Multiscale Kinetic Monte-Carlo for Simulating Epitaxial Growth

    We present a fast Monte-Carlo algorithm for simulating epitaxial surface growth, based on the continuous-time Monte-Carlo algorithm of Bortz, Kalos and Lebowitz. When simulating realistic growth regimes, much computational time is consumed by the relatively fast dynamics of the adatoms. Continuum and continuum-discrete hybrid methods have been developed to approach this issue; however in many situations, the density of adatoms is too low to efficiently and accurately simulate as a continuum. To solve the problem of fast adatom dynamics, we allow adatoms to take larger steps, effectively reducing the number of transitions required.     Pengaruh Ekstrak Kunyit, Kencur, Jahe, Dan Lengkuas Terhadap Penyakit Bulai Pada Tanaman Jagung Manis (Zea Mays Saccharata)

    Salah satu penyakit penting dalam budidaya jagung manis adalah penyakit bulai yang disebabkan oleh jamur Peronosclerospora maydis. Penelitian bertujuan untuk mengetahui pengaruh ekstrak rimpang (kunyit, kencur, jahe, dan lengkuas) untuk mengendalikan penyakit bulai (P. maydis) pada tanaman jagung manis. Penelittian dilaksanakan dari Mei-Agustus 2012 di Laboratorium Penyakit Tanaman, Fakultas Pertanian, Universitas Lampung. Perlakuan dalam percobaan ini disusun dalam Rancangan Acak Lengkap (RAL) dengan lima ulangan. Perlakuan terdiri atas kontrol berupa air steril (P0), ekstrak kunyit (P1), ekstrak kencur (P2), ekstrak jahe (P3), ekstrak lengkuas (P4), dan fungisida sintetik (dimetomorf 0,5 g/l) sebagai pembanding (P5). Peubah yang diamati adalah keterjadian penyakit, masa inkubasi, tinggi, bobot kering dan basah tanaman. Pengamatan dilakukan setiap hari selama tiga minggu. Data hasil pengamatan kemudian dianalisis dengan sidik ragam. Hasil penelitian menunjukan ekstrak kunyit, kencur, jahe, dan lengkuas tidak efektif dalam menekan keterjadian penyakit bulai pada jagung manis

    Research on the application of cooperative learning model Time Token has been conducted to improve student learning achievement on the topic stoichiometry at class X SMAN 2 Siak Hulu. This research is experimental research with randomized control group pretest-posttest design. The sample took in the month February – maret 2013. The sample consisted of two class, X3 class as control class. Experimental class is the with cooperative learning model Time Token and Control Class is the class with cooperative learning model Time Token. Analyze of data by used t-test formula. Result of data processing obtained t count>t table is 2,07 > 1,67. It means that the application of cooperative learning model Time Token can improve student achievement on the topic redoks at class X SMAN 2 Siak Hulu with the increasing percentage of 6,299%

    A 16-year-old male was brought to the emergency department by ambulance after being found lying unresponsive on an overturned motorcycle. He was orotracheally intubated. He had palpable subcutaneous crepitus over the chest and abdomen with massive scrotal swelling, and his back exam revealed multiple penetrating wounds (Figure 1). Autopsy results found five rightsided posterior thoracic gunshot wounds. The path of one bullet shattered the right seventh rib, entering the right lung and exiting though the main pulmonary artery before ending at the left clavicle. The presence of intra-scrotal air or gas is a rare clinical entity formed when air reaches the scrotum through tissue planes and cavities via the path of least resistance.     Predicting erythropoietin resistance in hemodialysis patients with type 2 diabetes

    <p>Background: Resistance to ESAs (erythropoietin stimulating agents) is highly prevalent in hemodialysis patients with diabetes and associated with an increased mortality. The aim of this study was to identify predictors for ESA resistance and to develop a prediction model for the risk stratification in these patients.</p> <p>Methods: A post-hoc analysis was conducted of the 4D study, including 1015 patients with type 2 diabetes undergoing hemodialysis. Determinants of ESA resistance were identified by univariate logistic regression analyses. Subsequently, multivariate models were performed with stepwise inclusion of significant predictors from clinical parameters, routine laboratory and specific biomarkers.</p> <p>Results: In the model restricted to clinical parameters, male sex, shorter dialysis vintage, lower BMI, history of CHF, use of ACE-inhibitors and a higher heart rate were identified as independent predictors of ESA resistance. In regard to routine laboratory markers, lower albumin, lower iron saturation, higher creatinine and higher potassium levels were independently associated with ESA resistance. With respect to specific biomarkers, higher ADMA and CRP levels as well as lower Osteocalcin levels were predictors of ESA resistance.</p> <p>Conclusions: Easily obtainable clinical parameters and routine laboratory parameters can predict ESA resistance in diabetic hemodialysis patients with good discrimination. Specific biomarkers did not meaningfully further improve the risk prediction of ESA resistance. Routinely assessed data can be used in clinical practice to stratify patients according to the risk of ESA resistance, which may help to assign appropriate treatment strategies.</p&gt

    Mammary Paget's disease occurring after mastectomy

    BACKGROUND: Mammary Paget's disease and extramammary Paget's disease are neoplastic conditions, in which there is intraepithelial (usually intraepidermal) infiltration by neoplastic cells showing glandular differentiation. Mammary Paget's disease occurs exclusively on the nipple/areola complex from where it may spread to the surrounding skin. CASE PRESENTATION: We here describe a case of Paget's disease occurring on the thoracic wall site of a previous simple mastectomy, and also briefly summarise the most important aspects leading to a diagnosis of mammary Paget's disease. CONCLUSION: To the best of our knowledge, this is the first reported case of mammary Paget's disease occurring after mastectomy. The absence of the nipple/areola complex obviously raised some questions concerning whether it was mammary or extra-mammary Paget's disease, and how it could occur in the absence of the nipple/areola complex
