    Hand washing Compliance - Is It A Reality?

    Background: Transmission of microorganisms from the hands of health care workers is the main source of cross-infection in hospitals and can be prevented by hand washing. The aim of this study was to identify predictors of noncompliance with hand washing during routine patient care. Materials And Methods: This is an observational study. The participants in the study were Health Care Workers (HCWs). Doctors, nurses and ward aides working in different wards of the hospital who were observed for compliance with hand washing. Results: In 270 observed opportunities for hand washing, average compliance was 63.3%. Noncompliance was highest among doctors followed by nurses. Ward aides were most compliant. Conclusions: Compliance with hand washing was moderate. Variation across the hospital ward and type of HCW suggests that targeted educational programs may be useful. Noncompliance suggests that understaffing may decrease quality of patient care

    Overcoming catastrophic interference in connectionist networks using Gram-Schmidt orthogonalization.

    Connectionist models of memory storage have been studied for many years, and aim to provide insight into potential mechanisms of memory storage by the brain. A problem faced by these systems is that as the number of items to be stored increases across a finite set of neurons/synapses, the cumulative changes in synaptic weight eventually lead to a sudden and dramatic loss of the stored information (catastrophic interference, CI) as the previous changes in synaptic weight are effectively lost. This effect does not occur in the brain, where information loss is gradual. Various attempts have been made to overcome the effects of CI, but these generally use schemes that impose restrictions on the system or its inputs rather than allowing the system to intrinsically cope with increasing storage demands. We show here that catastrophic interference occurs as a result of interference among patterns that lead to catastrophic effects when the number of patterns stored exceeds a critical limit. However, when Gram-Schmidt orthogonalization is combined with the Hebb-Hopfield model, the model attains the ability to eliminate CI. This approach differs from previous orthogonalisation schemes used in connectionist networks which essentially reflect sparse coding of the input. Here CI is avoided in a network of a fixed size without setting limits on the rate or number of patterns encoded, and without separating encoding and retrieval, thus offering the advantage of allowing associations between incoming and stored patterns.     Blood manufacturing methods affect red blood cell product characteristics and immunomodulatory activity

    Transfusion of red cell concentrates (RCCs) is associated with increased risk of adverse outcomes that may be affected by different blood manufacturing methods and the presence of extracellular vesicles (EVs). We investigated the effect of different manufacturing methods on hemolysis, residual cells, cell-derived EVs, and immunomodulatory effects on monocyte activity. Thirty-two RCC units produced using whole blood filtration (WBF), red cell filtration (RCF), apheresis-derived (AD), and whole blood-derived (WBD) methods were examined (n = 8 per method). Residual platelet and white blood cells (WBCs) and the concentration, cell of origin, and characterization of EVs in RCC supernatants were assessed in fresh and stored supernatants. Immunomodulatory activity of RCC supernatants was assessed by quantifying monocyte cytokine production capacity in an in vitro transfusion model. RCF units yielded the lowest number of platelet and WBC-derived EVs, whereas the highest number of platelet EVs was in AD (day 5) and in WBD (day 42). The number of small EVs (<200 nm) was greater than large EVs (≥200 nm) in all tested supernatants, and the highest level of small EVs were in AD units. Immunomodulatory activity was mixed, with evidence of both inflammatory and immunosuppressive effects. Monocytes produced more inflammatory interleukin-8 after exposure to fresh WBF or expired WBD supernatants. Exposure to supernatants from AD and WBD RCC suppressed monocyte lipopolysaccharide-induced cytokine production. Manufacturing methods significantly affect RCC unit EV characteristics and are associated with an immunomodulatory effect of RCC supernatants, which may affect the quality and safety of RCCs

    Isolation, Identification and Characterization of Keratin degrading microorganisms from Poultry soil and their Feather degradation Potential

    Keratinolytic microorganisms have a great importance in poultry waste degradation and its bioconversion to compost or animal feed. The aim of this study was to isolate keratin degrading bacteria and fungi from poultry farm soil, and to study their ability to degrade chicken feathers. The poultry farm soil samples were added in basal medium with feathers as a source of carbon and nitrogen. Five bacterial cultures were isolated. Bacteria were grown in basal media with feathers meal and showed feather degrading capacity. Bacterial strains were identified as Aeromicrobium spp., Exiguobacter spp., Marinococcus spp. and Bacillus spp. 1 Bacillus spp 2.These bacteria showed keratinolytic enzyme activity in the cell free culture supernatants. The highest biodegradation of feathers was obtained using Aeromicrobium (KD1-72.55%), among the isolated cultures. Two fungal cultures (F1 and F2) were also isolated by Hair Bait Technique, out of which F1 showed good keratinolytic activity. The good ability of selected microorganisms to degrade feathers can be utilized for their potential biotechnological application in processing of feather waste from poultry industry

    Unconventional Fermi surface in an insulating state

    Insulators occur in more than one guise, a recent finding was a class of topological insulators, which host a conducting surface juxtaposed with an insulating bulk. Here we report the observation of an unusual insulating state with an electrically insulating bulk that simultaneously yields bulk quantum oscillations with characteristics of an unconventional Fermi liquid. We present quantum oscillation measurements of magnetic torque in high purity single crystals of the Kondo insulator SmB6, which reveal quantum oscillation frequencies characteristic of a large three-dimensional conduction electron Fermi surface similar to the metallic rare earth hexaborides such as PrB6 and LaB6. The quantum oscillation amplitude strongly increases at low temperatures, appearing strikingly at variance with conventional metallic behaviour

    Do pediatric gastroenterology doctors address pediatric obesity?

    Objectives: To assess how often obesity is acknowledged at pediatric gastroenterology outpatient visits. Methods: A retrospective chart review was performed to identify obese children seen at a gastroenterology subspecialty clinic over a 1-year period of time; 132 children were identified. Demographics, obesity comorbidities, reasons for referral, diagnosis of obesity, and a plan to address obesity were abstracted. Chi-square or Fisher’s exact tests were used to examine statistical associations. Results: Only 49% of children were given a diagnosis of obesity. In total, 52% of children were given a body mass index reduction plan. Those diagnosed with obesity were more likely to receive a body mass index reduction plan (p \u3c 0.0001). Younger children and males were more likely to receive an obesity diagnosis (p = 0.002 and p = 0.02, respectively). Diagnosis of obesity was more likely in patients with obesity-related comorbidities (p = 0.0004) and those referred for obesity or related comorbidities (p = 0.01). Conclusion: Obesity is diagnosed less than 50% of the time in pediatric gastroenterology outpatient clinics. To increase opportunities for addressing childhood obesity in the pediatric gastroenterology outpatient setting, further investigation of barriers and optimal provider education is urgently required

    An X-Ray Survey of the Open Cluster NGC 6475 (M7) with ROSAT

    A ROSAT x-ray survey, with complimentary optical photometry, of the open cluster NGC 6475 has enabled the detection of approx. 50 late-F to K0 and approx. 70 K/M dwarf new candidate members, providing the first reliable detection of low-mass stars in this low. galactic latitude, 220 Myr old cluster. The x-ray observations reported here have a typical limiting sensitivity of L(sub x) approx. equal to 10(exp 29) erg/s. The detection frequency of early type cluster members is consistent with the hypothesis that the x-ray emitting early type stars are binary systems with an unseen, low-mass secondary producing the x rays. The ratio between x-ray and bolometric luminosity among NGC 6475 members saturates at a spectral-type/color which is intermediate between that in much younger and in much older clusters, consistent with rotational spindown of solar-type stars upon their arrival on the ZAMS. The upper envelope of x-ray luminosity as a function of spectral type is comparable to that of the Pleiades, with the observed spread in x-ray luminosity among low-mass members being likely due to the presence of binaries and relatively rapid rotators. However, the list of x-ray selected candidate members is likely biased against low-mass, slowly rotating single stars. While some preliminary spectroscopic information is given in an appendix, further spectroscopic observations of the new candidate members will aid in interpreting the coronal activity among solar-type NGC 6475 members and their relation to similar stars in older and younger open clusters
