    An overview of chickpea breeding programs in Kenya

    Chickpea is a new crop in Kenya and its potential has not been fully utilized. The chickpea grain yields generally range between 1.2 to 3.5 tons/ha at farmers‟ fields, indicating that chickpea has a potential of becoming an important export crop in Kenya. The chickpea breeding program in Kenya is still at infant stage and being established with support from International Crops Research Institute for the Semi-Arid Tropics (ICRISAT). Four chickpea varieties have been recently released from the breeding material supplied by ICRISAT. Efforts are being made on evaluation of germplasm and breeding lines, application of modern molecular breeding tools and techniques in chickpea breeding and establishment of effective seed system for establishing a sustainable chickpea production system in the country

    Tracking the Feeding Patterns of Tsetse Flies (Glossina Genus) by Analysis of Bloodmeals Using Mitochondrial Cytochromes Genes

    Tsetse flies are notoriously difficult to observe in nature, particularly when populations densities are low. It is therefore difficult to observe them on their hosts in nature; hence their vertebrate species can very often only be determined indirectly by analysis of their gut contents. This knowledge is a critical component of the information on which control tactics can be developed. The objective of this study was to determine the sources of tsetse bloodmeals, hence investigate their feeding preferences. We used mitochondrial cytochrome c oxidase 1 (COI) and cytochrome b (cytb) gene sequences for identification of tsetse fly blood meals, in order to provide a foundation for rational decisions to guide control of trypanosomiasis, and their vectors. Glossina swynnertoni were sampled from Serengeti (Tanzania) and G. pallidipes from Kenya (Nguruman and Busia), and Uganda. Sequences were used to query public databases, and the percentage identities obtained used to identify hosts. An initial assay showed that the feeds were from single sources. Hosts identified from blood fed flies collected in Serengeti ecosystem, included buffaloes (25/40), giraffes (8/40), warthogs (3/40), elephants (3/40) and one spotted hyena. In Nguruman, where G. pallidipes flies were analyzed, the feeds were from elephants (6/13) and warthogs (5/13), while buffaloes and baboons accounted for one bloodmeal each. Only cattle blood was detected in flies caught in Busia and Uganda. Out of four flies tested in Mbita Point, Suba District in western Kenya, one had fed on cattle, the other three on the Nile monitor lizard. These results demonstrate that cattle will form an integral part of a control strategy for trypanosomiasis in Busia and Uganda, while different approaches are required for Serengeti and Nguruman ecosystems, where wildlife abound and are the major component of the tsetse fly food source

    The cost‐effectiveness of prophylaxis strategies for individuals with advanced HIV starting treatment in Africa

    Introduction Many HIV‐positive individuals in Africa have advanced disease when initiating antiretroviral therapy (ART) so have high risks of opportunistic infections and death. The REALITY trial found that an enhanced‐prophylaxis package including fluconazole reduced mortality by 27% in individuals starting ART with CD4 <100 cells/mm3. We investigated the cost‐effectiveness of this enhanced‐prophylaxis package versus other strategies, including using cryptococcal antigen (CrAg) testing, in individuals with CD4 <200 cells/mm3 or <100 cells/mm3 at ART initiation and all individuals regardless of CD4 count. Methods The REALITY trial enrolled from June 2013 to April 2015. A decision‐analytic model was developed to estimate the cost‐effectiveness of six management strategies in individuals initiating ART in the REALITY trial countries. Strategies included standard‐prophylaxis, enhanced‐prophylaxis, standard‐prophylaxis with fluconazole; and three CrAg testing strategies, the first stratifying individuals to enhanced‐prophylaxis (CrAg‐positive) or standard‐prophylaxis (CrAg‐negative), the second to enhanced‐prophylaxis (CrAg‐positive) or enhanced‐prophylaxis without fluconazole (CrAg‐negative) and the third to standard‐prophylaxis with fluconazole (CrAg‐positive) or without fluconazole (CrAg‐negative). The model estimated costs, life‐years and quality‐adjusted life‐years (QALY) over 48 weeks using three competing mortality risks: cryptococcal meningitis; tuberculosis, serious bacterial infection or other known cause; and unknown cause. Results Enhanced‐prophylaxis was cost‐effective at cost‐effectiveness thresholds of US300andUS300 and US500 per QALY with an incremental cost‐effectiveness ratio (ICER) of US157perQALYintheCD4<200cells/mm3populationprovidingenhanced‐prophylaxiscomponentsaresourcedatlowestavailableprices.TheICERreducedinmoreseverelyimmunosuppressedindividuals(US157 per QALY in the CD4 <200 cells/mm3 population providing enhanced‐prophylaxis components are sourced at lowest available prices. The ICER reduced in more severely immunosuppressed individuals (US113 per QALY in the CD4 <100 cells/mm3 population) and increased in all individuals regardless of CD4 count (US722perQALY).Resultsweresensitivetopricesoftheenhanced‐prophylaxiscomponents.Enhanced‐prophylaxiswasmoreeffectiveandlesscostlythanallCrAgtestingstrategiesasenhanced‐prophylaxisstillconveyedhealthgainsinCrAg‐negativepatientsandsavingsfromtargetingprophylaxisbasedonCrAgstatusdidnotcompensateforcostsofCrAgtesting.CrAgtestingstrategiesdidnotbecomecost‐effectiveunlessthepriceofCrAgtestingfellbelowUS722 per QALY). Results were sensitive to prices of the enhanced‐prophylaxis components. Enhanced‐prophylaxis was more effective and less costly than all CrAg testing strategies as enhanced‐prophylaxis still conveyed health gains in CrAg‐negative patients and savings from targeting prophylaxis based on CrAg status did not compensate for costs of CrAg testing. CrAg testing strategies did not become cost‐effective unless the price of CrAg testing fell below US2.30. Conclusions The REALITY enhanced‐prophylaxis package in individuals with advanced HIV starting ART reduces morbidity and mortality, is practical to administer and is cost‐effective. Efforts should continue to ensure that components are accessed at lowest available prices

    Late Presentation With HIV in Africa: Phenotypes, Risk, and Risk Stratification in the REALITY Trial.

    This article has been accepted for publication in Clinical Infectious Diseases Published by Oxford University PressBackground: Severely immunocompromised human immunodeficiency virus (HIV)-infected individuals have high mortality shortly after starting antiretroviral therapy (ART). We investigated predictors of early mortality and "late presenter" phenotypes. Methods: The Reduction of EArly MortaLITY (REALITY) trial enrolled ART-naive adults and children ≥5 years of age with CD4 counts .1). Results: Among 1711 included participants, 203 (12%) died. Mortality was independently higher with older age; lower CD4 count, albumin, hemoglobin, and grip strength; presence of World Health Organization stage 3/4 weight loss, fever, or vomiting; and problems with mobility or self-care at baseline (all P < .04). Receiving enhanced antimicrobial prophylaxis independently reduced mortality (P = .02). Of five late-presenter phenotypes, Group 1 (n = 355) had highest mortality (25%; median CD4 count, 28 cells/µL), with high symptom burden, weight loss, poor mobility, and low albumin and hemoglobin. Group 2 (n = 394; 11% mortality; 43 cells/µL) also had weight loss, with high white cell, platelet, and neutrophil counts suggesting underlying inflammation/infection. Group 3 (n = 218; 10% mortality) had low CD4 counts (27 cells/µL), but low symptom burden and maintained fat mass. The remaining groups had 4%-6% mortality. Conclusions: Clinical and laboratory features identified groups with highest mortality following ART initiation. A screening tool could identify patients with low CD4 counts for prioritizing same-day ART initiation, enhanced prophylaxis, and intensive follow-up.     Combining Ability Effects for Forage Residue Traits in Grain Sorghum Hybrids

    Combining ability effects of 10 female and 10 male inbred lines were evaluated for agronomic, grain, and forage residue traits in 100 grain sorghum [Sorghum bicolor (L.) Moench] F1 hybrids for 2 years. Traits studied were height, flowering, tillering, seed weight, yield, protein percentage, protein yield of grain, leaves, and stems; and in vitro dry matter disappearance (IVDMD) percentage and digestible dry matter (DDM) yield of leaves and stems. General combining ability (GCA) effects exceeded specific combining ability (SCA) effects for all traits except height in females and grain and leaf yields in males. Both GCA and SCA were low for tillering and stem protein percentage. Genetic ratios, resembling heritability, were higher for GCA than SCA except for height in females and tillering in males. Grain yield generally had no extremely strong negative phenotypic correlations with any forage residue trait although grain yield correlated with stem protein and stem IVDMD percentages produced negative values that would warrant monitoring in a breeding program. Grain yield, stem and leaf yields, and stem and leaf DDM yields all had moderate to strong positive relationships with each other. A few potentially useful parents were identified having high GCA effects for forage quality characters. Data on protein and IVDMD percentages of leaf and stem fractions from grain sorghum breeding nurseries and hybrid yield trials should be routinely collected
