При проектировании технологического процесса было выполнено следующее: выбраны базы и схемы установки; разработана маршрутная и операционная технология; определены технологические допуски, припуски на операционные размеры; выбрано оборудование, приспособления и инструмент; рассчитаны режимы резания. В конструкторской части были сконструированы два новых приспособления для закрепления детали.In the course of the project, an analysis of the existing technological process of manufacturing the frame has been made. Positive and negative aspects have been identified. When designing the technological process, the following has been done: the database and scheme installation have been selected; the route and the operating technology have been designed; the technological tolerances, allowances and operating dimensions have been defined, equipment, devices and tools have been chosen, cutting data have been calculated. In the design part two new fixtures to secure the part have been developed

    Eosinophilic gastroenteritis with refractory ulcer disease and gastrointestinal bleeding as a rare manifestation of seronegative gastrointestinal food allergy

    Gastrointestinal bleeding and iron deficiency anaemia may cause severe symptoms and may require extensive diagnostics and substantial amounts of health resources. This case report focuses on the clinical presentation of a 22 year old patient with recurrent gastrointestinal bleeding from multilocular non-healing ulcers of the stomach, duodenum and jejunum over a period of four years. Extensive gastroenterological and allergological standard diagnostic procedures showed benign ulcerative lesions with tissue eosinophilia, but no conclusive diagnosis. Multiple diagnostic procedures were performed, until finally, endoscopically guided segmental gut lavage identified locally produced, intestinal IgE antibodies by fluoro-enzyme-immunoassay. IgE antibody concentrations at the intestinal level were found to be more-fold increased for total IgE and food-specific IgE against nuts, rye flour, wheat flour, pork, beef and egg yolk compared with healthy controls. Thus, a diet eliminating these allergens was introduced along with antihistamines and administration of a hypoallergenic formula, which resulted in complete healing of the multilocular ulcers with resolution of gastrointestinal bleeding. All gastrointestinal lesions disappeared and total serum IgE levels dropped to normal within 9 months. The patient has been in remission now for more than two years. Eosinophilic gastroenteritis (EG) is well known to induce refractory ulcer disease. In this case, the mechanisms for intestinal damage and gastrointestinal bleeding were identified as local gastrointestinal type I allergy. Therefore, future diagnostics in EG should also be focused on the intestinal level as identification of causative food-specific IgE antibodies proved to be effective to induce remission in this patient

    Open-label parallel dose tolerability study of three subcutaneous immunotherapy regimens in house dust mite allergic patients

    Background The current maintenance dose (10,000 AUeq/monthly) of a subcutaneous allergoid for house dust mite (HDM) immunotherapy has previously shown significant clinical efficacy in patients with HDM induced allergic rhinitis or rhinoconjunctivitis. In order to comply with the 2009 EMA guidelines on immunotherapy products, a study was conducted to evaluate the safety, tolerability and short-term treatment effects of up-dosing regimens with high doses (up to 40,000 AUeq) of allergoid HDM immunotherapy. Methods In total 48 patients with HDM-allergic rhinitis or rhinoconjunctivitis (29 M/19 F; 18–53 years) were included and enrolled into one of three up-dosing regimens (1:4:4): 1) a regular regimen with up-dosing to 40,000 AUeq followed by two maintenance doses (total duration 17 weeks), 2) an intermediate regimen (14 weeks) or 3) a fast regimen (11 weeks). Safety and tolerability were evaluated by monitoring of early and late local reactions and systemic reactions. In addition, short-term effects were assessed by conjunctival provocation test (CPT) and levels of serum allergen-specific IgE, IgG and IgG4. Results Thirty-nine patients completed the study according to protocol. No early local reactions occurred. Late local reactions (LLR) were observed in 12% of the injections. In total, 31 systemic reactions, all grade 1, were reported of which two needed oral antihistamine treatment. No grade 2 or higher systemic reactions were observed. Six patients (15%) did not reach the highest dose due to LLR and/or systemic reactions needing antihistamines (20% in the regular regimen, 16% in the intermediate regimen and 13% in the fast regimen). At the end of the study, an improvement in the CPT was observed in 82.1% of patients, indirectly indicating an early treatment effect at the current dose and higher doses. In addition, IgG4 immunoglobulin levels were significantly increased in all groups following treatment. Conclusions In this open-label study, allergoid HDM immunotherapy in doses up to 40,000 AUeq was generally well tolerated and no clinically relevant safety issues were identified. In the safety aspects of the three up-dosing regimens no clinically relevant differences were encountered. Therefore, these dose ranges and up-dosing regimens can be safely included in future dose-finding efficacy studies

    URL:http://link.aps.org/doi/10.1103/PhysRevLett.88.115502 DOI:10.1103/PhysRevLett.88.115502Small-angle x-ray scattering, nitrogen adsorption, and scanning tunneling microscopy show that a series of activated carbons host an extended fractal network of channels with dimension Dp = 2.8-3.0 (pore fractal), channel width 15-20Å (lower end of scaling), network diameter 3000-3400Å (upper end of scaling), and porosity of 0.3-0.6. We interpret the network as a stack of quasiplanar invasion percolation clusters, formed by oxidative removal of walls between closed voids of diameter of ∼10Å and held in registry by fibrils of the biological precursor, and point out unique applications.This work was supported by the Petroleum Research Fund, Grant No. 30602-AC9,5 (P.     Testing the Properties of Beam-Dose Monitors for VHEE-FLASH Radiation Therapy

    Very High Energy Electrons (VHEE) of 50 - 250 MeV are an attractive choice for FLASH radiation therapy (RT). Before VHEE-FLASH RT can be considered for clinical use, a reliable dosimetric and beam monitoring system needs to be developed, able to measure the dose delivered to the patient in real-time and cut off the beam in the event of a machine fault to prevent overdosing the patient. Ionisation chambers are the standard monitors in conventional RT; however, their response saturates at the high dose rates required for FLASH. Therefore, a new dosimetry method is needed that can provide reliable measurements of the delivered dose in these conditions. Experiments using 200 MeV electrons were done at the CLEAR facility at CERN to investigate the properties of detectors such as diamond beam loss detectors, GEM foil detectors, and Timepix3 ASIC chips.
