    Kesesuaian Penyelenggaraan Pelayanan Kefarmasian di Puskesmas

    Abstract Pharmaceutical services have been gradually applied in primary health services both in terms of pharmaceutical management and clinical pharmacy services. In order to support the implementation, the standard has been amended several times, resulting Permenkes Number 74 of 2016 Concerning the Pharmaceutical Services Standard in Public Health Centre (puskesmas) as the most updated one. This study aimed to determine the suitability of the implementation of pharmaceutical service standards in the management of medicine and clinical pharmacy at the puskesmas. The cross-sectional research design was conducted in February-November 2017. The selection of provincial locations was carried out purposively. Data collection tools were questionnaires and a list of standard pharmacy services at the puskesmas. Data were analyzed descriptively. The results showed that 54.5% of the puskesmas did not have pharmacists as the responsible person and only 18.2% of the puskesmas had sufficient pharmacist and pharmaceutical technical staff for both drug management activities and clinical pharmacy services. Comprehensive drug management activities in accordance with pharmaceutical service standards at the puskesmas have been implemented in 96.7% of puskesmas with pharmacists. Comprehensive clinical pharmacy service activities according to pharmacy service standards at puskesmas are only applied in 23.3% of puskesmas with pharmacists. The availability and ability of pharmacists in providing clinical pharmacy services in puskesmas need to be a concern. Abstrak Pelayanan kefarmasian secara bertahap telah mulai diterapkan di pelayanan kesehatan dasar, baik dalam kewajiban pengelolaan farmasi maupun pelayanan farmasi klinis. Untuk mendukung hal tersebut, standar pelayanan kefarmasian di puskesmas telah beberapa kali mengalami perubahan dan terakhir diperbaharui dengan Permenkes Nomor 74 Tahun 2016 Tentang Standar Pelayanan Kefarmasian di puskesmas. Studi ini bertujuan mengetahui kesesuaian pelaksanaan standar pelayanan kefarmasian dalam pengelolaan obat dan farmasi klinik di puskesmas. Desain penelitian potong lintang dilakukan pada bulan Februari-November 2017. Pemilihan lokasi provinsi dilakukan secara purposif. Alat pengumpul data berupa kuesioner dan daftar tilik standar pelayanan kefarmasian di puskesmas. Analisis data dilakukan secara deskriptif. Hasil studi menunjukkan bahwa sebanyak 54,5% puskesmas belum mempunyai tenaga apoteker sebagai penanggung jawabnya dan hanya 18,2% puskesmas yang jumlah apoteker dan tenaga teknis kefarmasiannya mencukupi untuk kegiatan pengelolaan obat dan pelayanan farmasi klinik. Kegiatan pengelolaan obat yang komprehensif sesuai dengan standar pelayanan kefarmasian di puskesmas sudah diterapkan di 96,7% puskesmas yang memiliki apoteker. Kegiatan pelayanan farmasi klinis yang komprehensif sesuai standar pelayanan kefarmasian di puskesmas hanya diterapkan di 23,3% puskesmas yang memiliki apoteker. Ketersediaan dan kemampuan apoteker dalam memberikan pelayanan farmasi klinik di puskesmas perlu menjadi perhatian

    Papillomavirus E2 protein is regulated by specific fibroblast growth factor receptors

    The papillomavirus (PV) E2 protein activates transcription and replication by recruiting cellular proteins and the E1 DNA helicase to their binding sites in the viral genome. We recently demonstrated that phosphorylation of tyrosine 102 in the bovine papillomavirus (BPV-1) E2 protein restricts these activities and that fibroblast growth factor receptor-3 (FGFR3) tyrosine kinase binds PV E2. Expression of FGFR3 decreased viral replication with both wild-type and the phenylalanine substitution at position 102, inferring that another kinase targets Y102. Here we tested FGFR- 1, −2 and −4 for association with PV E2 proteins. FGFR2 but not FGFR1 or FGFR4 co-immunoprecipitated with BPV-1 E2. We found that FGFR2 suppressed replication but did not depend on phosphorylation of BPV-1 Y102. HPV-16 and −31 E2 interacted with FGFR1, −2, and −4. These results imply that the expression and activity of FGF receptors in epithelial cells can regulate the function of E2 in viral replication

    Pyk2 Regulates Human Papillomavirus Replication by Tyrosine Phosphorylation of the E2 Protein

    The human papillomavirus (HPV) E2 protein is a key regulator of viral transcription and replication. In this study, we demonstrate that the nonreceptor tyrosine kinase Pyk2 phosphorylates tyrosine 131 in the E2 transactivation domain. Both depletion of Pyk2 and treatment with a Pyk2 kinase inhibitor increased viral DNA content in keratinocytes that maintain viral episomes. The tyrosine-to-glutamic acid (E) mutant Y131E, which may mimic phosphotyrosine, failed to stimulate transient DNA replication, and genomes with this mutation were unable to establish stable episomes in keratinocytes. Using coimmunoprecipitation assays, we demonstrate that the Y131E is defective for binding to the C-terminal motif (CTM) of Bromodomain-containing protein 4 (Brd4). These data imply that HPV replication depends on E2 Y131 interaction with the pTEFb binding domain of Brd4.IMPORTANCE Human papillomaviruses are the major causative agents of cervical, oral, and anal cancers. The present study demonstrates that the Pyk2 tyrosine kinase phosphorylates E2 at tyrosine 131, interfering with genome replication. We provide evidence that phosphorylation of E2 prevents binding to the Brd4-CTM. Our findings add to the understanding of molecular pathways utilized by the virus during its vegetative life cycle and offers insights into the host-virus interactome

    The human papillomavirus (HPV) is a DNA tumor virus that infects cutaneous and mucosal epithelia where high-risk (HR) HPV infections lead to cervical, oropharyngeal, and anogenital cancers. Worldwide, nearly 5% of all cancers are caused by HR HPV. The viral E2 protein is essential for episomal replication throughout the viral lifecycle. The E2 protein is regulated by phosphorylation, acetylation, sumoylation, and ubiquitination.     Quaternionic and Octonionic Spinors. A Classification

    Quaternionic and octonionic realizations of Clifford algebras and spinors are classified and explicitly constructed in terms of recursive formulas. The most general free dynamics in arbitrary signature space-times for both quaternionic and octonionic spinors is presented. In the octonionic case we further provide a systematic list of results and tables expressing, e.g., the relations of the octonionic Clifford algebras with the G2G_2 cosets over the Lorentz algebras, the identities satisfied by the higher-rank antisymmetric octonionic tensors and so on. Applications of these results range from the classification of octonionic generalized supersymmetries, the construction of octonionic superstrings, as well as the investigations concerning the recently discovered octonionic MM-superalgebra and its superconformal extension.Comment: 24 pages, LaTe

    Predicting the current distribution of the chacoan peccary (catagonus wagneri) in the gran Chaco

    The Chacoan peccary (Catagonus wagneri), or Tagua, an endemic species living in the Chaco eco¬region, is endangered by highly increasing deforestation rates across the region, particularly in the last decade. This situation highlights the need to better understand the current distribution of the species, as well as how environmental conditions affect habitat suitability. This study predicts the distribution of the Chacoan peccary and evaluates the current environmental conditions in the Chaco for this species. Using six environmental variables and 177 confirmed occurrence records (from 2000 to 2015) provided by researchers, we developed a Species Distribution Model (SDM) applying the Maxent algorithm. The final model was highly accurate and significant (p < 0.001; AUC 0.860 ± 0.0268; omission error 1.82 %; post¬hoc validation of omission error using independent presence¬only records 1.33 %), predicting that 46.24 % of the Chaco is suitable habitat for the Chacoan peccary, with the most important areas concentrated in the middle of Paraguay and northern Argentina. Land cover, isothermality and elevation were the variables that better explained the habitat suitability for the Chacoan peccary. Despite some portions of suitable areas occurring inside protected areas, the borders and the central portions of suitable areas have recently suffered from intensive deforestation and development, and most of the highly suitable areas for the species are not under protection.
