    The aims of this study are to know the role of organizational communication in the improvement of service quality in the West Bolang Itang District, the impact of organizational communication satisfaction, and the factors supporting the communication services organization in the process. The method used in this study put more emphasis on qualitative research methods. The instruments used in this study were a list of questions with emphasis on the various lists of questions that have been circulated to the informant in a structured or unstructured. The data collection techniques were observation and interviews. The observation was conducted in a participatory manner, in which researchers directed the targeted research communities. The analytical techniques used in this study were more emphasis on qualitative analysis. From this study it can be concluded that a) the role of organizational communication in the organization's service process can be applied through the relationship between superiors and subordinates, to maximize the timeliness of the work, affecting the attitude of openness subordinates in performing duties, affecting the administrative service, increasing the level of professionalism through education, and maximize the implementation of the discipline, b) good organizational communication satisfaction can affect the quality of service that can be seen from the procedure of service satisfaction, satisfaction in meeting the requirements of the various forms of service, satisfaction in the provision of cost of service, and satisfaction with services completion time, c) factors that supports communication between other organizations through service accountability, transparency of service, knowledge and skill factors and factors of discipline in conducting the service. It can be suggested that it is necessary to create a good working relationship between superior and subordinate, between subordinates with subordinates as well as between the parties nor the providers of service recipients, as well as a need for improvement in facilities and infrastructure to support the intensity of service activity

    AbstractIt is increasingly recognised that leadership skills are a key requirement in being successful in surgery, regardless of speciality and at all levels of experience and seniority. Where the emphasis was previously on technical ability, knowledge and diagnostic acumen, we now know that non-technical skills such as communication and leadership contribute significantly to patient safety, experience and outcomes, and should be valued.The operating theatre is a unique micro-environment which is often busier, noisier, more stressful and more physically demanding than the clinic or ward setting. As a result surgeons and their trainers, who are striving to develop leadership skills require an in-depth awareness of the challenges in this environment and the opportunities that arise from them to develop leadership effectively. This article outlines why leadership learning is so beneficial in the operating theatre, both for the team and the patient as well as what elements of daily routine activity such as the WHO checklist use, list-planning and audit can be exploited to transform the average busy operating theatre into a rich, learning environment for future leaders in surgery

    . Local Government Financial Management is a subsystem of State Government Financia Management. It is also a substantial element in operating local governance effectively, efficiently and economically by fulfilling main pillars of government operation. Those main pillars are transparency, accountability, and participative. These pillars are determined by the Indonesian Government Regulation Number 58 year 2005 and Permendagri Number 13 year 2006. The stages include planning and budgeting process, starting from absorption of people\u27s aspiration, setting up and the implementation of detailed planning and budgeting as regulated by constitutions. There are obstacles in the planning and budgeting process in Local Government of Bolaang Mongondow. Those obstacles are in Musrenbang stage, Local Government Budgeting Team, process of setting up KUA-PPAS and RKA-SKPD, budget discussion between DPRD members and APBD budgeting staff. This research also discusses their efforts to overcome the obstacles in the process of planning and budgeting of Local Government of BolaangMongondow APBD. This is a qualitative case study research. Data were collected by interviews and observation. Respondents are staff involving in the process of planning and budgeting. They are Government employees and members of DPRD in the Local Government of Bolaang Mongondow. This study undertook more than 2 months for collecting its data. Results indicate that the process planning and budgeting of APBD in the Local Government of Bolaang Mongondow has not fulfilled yet the regulations. Results also show that transparency is not yet conducted sufficiently and people\u27s aspirations have not been absorbed optimally, either bottom up or top down approach. It is, thus, efforts are needed to overcome such obstacles in the process of planning and budgeting of APBD in the Local Government of BolaangMongondow

    Role of TGF-?1 in Urinary Bladder Carcinoma

    The present study aimed to evaluate sera TGF- ?1 concentration in patients with urinary bladder carcinoma (UBC). All malignant of them was transitional cell carcinoma (TCC) type , patients with urinary bladder disorders (UBD ) and healthy control , and to study the correlation between sera TGF-?1 levels and tumor stages and grades in UBC patients . A direct ELISA test was used to quantify the seraTGF-?1 concentrations in sera of 58 patients with urinary bladder carcinoma UBC of different grades (G) and stages (T) all malignant of them was transitional cell carcinoma (TCC) type , 15 from patients with UBD and 15 healthy subjects . Sera levels of TGF-?1 were elevated in patients with UBC and UBD compared to healthy (P ? 0.01) .There was no association between tumor stages and serum level of TGF-?1 (P?0.05) . Whereas there was an association between serum levels of TFG-?1 and tumor grades (p ?0.05) . Increased serum level was found in high grades G?2 of patient with UBC

    The Contribution of the Pineal Gland on Daily Rhythms and Masking in Diurnal Grass Rats, Arvicanthis niloticus

    Melatonin is a hormone rhythmically secreted at night by the pineal gland in vertebrates. In diurnal mammals, melatonin is present during the inactive phase of the rest/activity cycle, and in primates it directly facilitates sleep and decreases body temperature. However, the role of the pineal gland for the promotion of sleep at night has not yet been studied in non-primate diurnal mammalian species. Here, the authors directly examined the hypothesis that the pineal gland contributes to diurnality in Nile grass rats by decreasing activity and increasing sleep at night, and that this could occur via effects on circadian mechanisms or masking, or both. Removing the pineal gland had no effect on the hourly distribution of activity across a 12:12 light-dark (LD) cycle or on the patterns of sleep-like behavior at night. Masking effects of light at night on activity were also not significantly different in pinealectomized and control grass rats, as 1h pulses of light stimulated increases in activity of sham and pinealectomized animals to a similar extent. In addition, the circadian regulation of activity was unaffected by the surgical condition of the animals. Our results suggest that the pineal gland does not contribute to diurnality in the grass rat, thus highlighting the complexity of temporal niche transitions. The current data raise interesting questions about how and why genetic and neural mechanisms linking melatonin to sleep regulatory systems might vary among mammals that reached a diurnal niche via parallel and independent pathways

    Cardiovascular Complications of Systemic Lupus Erythematosus: Impact of Risk Factors and Therapeutic efficacy--a Tertiary Centre Experience in an Appalachian State

    OBJECTIVES: Cardiovascular complications became a notable cause of morbidity and mortality in patients with lupus as therapeutic advancements became more efficient at managing other complications. The Appalachian community in Kentucky has a higher prevalence of traditional cardiovascular risk factors, predisposing them to cardiovascular events. Namely, the mean body mass index of the members of the Kentucky Appalachian community was reported at 33 kg/m2 and 94.3% of male members of this community use tobacco. We sought to identify risk factors that predispose patients with lupus to cardiovascular morbidities and examine the effect of immunomodulatory drugs. METHODS: We identified 20 UKHS patients having both a lupus diagnosis and experienced at least one cardiovascular event. We chose three controls matched for birth-year ±5 years to each case. In a case-control design, we analysed lupus manifestations, cardiovascular risk factors and immunosuppressive therapies. We collected Systemic Lupus Erythematosus Disease Activity Index 2000 disease activity index during the cardiovascular event. RESULTS: We identified 308 patients with lupus from among all University of Kentucky Health System patients. 20 (6.5%) of such patients with lupus were confirmed to cardiovascular complication. Of those 20, 7 (35%) had experienced myocardial infarction, 10 (50%) had experienced stroke and 4 (20%) had peripheral ischaemia. Tobacco use and male gender were the only traditional cardiovascular risk factors higher in the cases group. Hydroxychloroquine and steroids were less utilised in the cases than in the controls (70% vs 100% in hydroxychloroquine, 30% vs 82% in steroids). Venous thrombosis was found to be significantly higher in the cases. On multivariate analysis, venous thrombosis remained significant. CONCLUSION: Despite tobacco use partially explaining the increased risk of cardiovascular disease among the cases group, the higher prevalence of venous thrombosis in the cases group suggests lupus as a potential additional risk factor of cardiovascular morbidity among patients with lupus in this Appalachian community

    Cytomegalovirus-mediated T cell receptor repertoire perturbation is present in early life

    Human cytomegalovirus (CMV) is a highly prevalent herpesvirus, particularly in sub-Saharan Africa, where it is endemic from infancy. The T cell response against CMV is important in keeping the virus in check, with CD8 T cells playing a major role in the control of CMV viraemia. Human leukocyte antigen (HLA) B*44:03-positive individuals raise a robust response against the NEGVKAAW (NW8) epitope, derived from the immediate-early-2 (IE-2) protein. We previously showed that the T cell receptor (TCR) repertoire raised against the NW8-HLA-B*44:03 complex was oligoclonal and characterised by superdominant clones, which were shared amongst unrelated individuals (i.e., “public”). Here, we address the question of how stable the CMV-specific TCR repertoire is over the course of infection, and whether substantial differences are evident in TCR repertoires in children, compared with adults. We present a longitudinal study of four HIV/CMV co-infected mother-child pairs, who in each case express HLA-B*44:03 and make responses to the NW8 epitope, and analyse their TCR repertoire over a period spanning more than 10 years. Using high-throughput sequencing, the paediatric CMV-specific repertoire was found to be highly diverse. In addition, paediatric repertoires were remarkably similar to adults, with public TCR responses being shared amongst children and adults alike. The CMV-specific repertoire in both adults and children displayed strong fluctuations in TCR clonality and repertoire architecture over time. Previously characterised superdominant clonotypes were readily identifiable in the children at high frequency, suggesting that the distortion of the CMV-specific repertoire is incurred as a direct result of CMV infection rather than a product of age-related “memory inflation.” Early distortion of the TCR repertoire was particularly apparent in the case of the TCR-β chain, where oligoclonality was low in children and positively correlated with age, a feature we did not observe for TCR-α. This discrepancy between TCR-α and -β chain repertoire may reflect differential contribution to NW8 recognition. Altogether, the results of the present study provide insight into the formation of the TCR repertoire in early life and pave the way to better understanding of CD8 T cell responses to CMV at the molecular level

    Current dynamics of world energy supply have driven various innovations by the industry as well as research communities. Fossil fuels, although experiencing a declining interest due to sustainability issues, remain undeniably crucial since they are nearly irreplaceable in some sectors including electricity generation, it is necessary to continuously establish efforts to improve efficiency of those kinds of fuel. In this work, the authors evaluate the potential of locally sourced essential oils, namely citronella, as a fuel additive in a vision of raising the fuel economy of gasoline RON 90. Citronella oil was selected based on a positive hypothesis deduced from its chemical properties, as mentioned in multiple published works. Tests were made on a generator-set powered by gasoline engine using the mixture of RON 90 and citronella oil of various concentrations as the fuels. In addition, a commercial synthetic additive was also tested alongside the essential oil to provide a comparative figure. Meanwhile other investigators suggest a favorable effect of essential oils, our results show that citronella oil additions lead to higher fuel consumption at the same power level. A similar negative effect was also demonstrated by the synthetic additive. The only sector showing positive results is in terms of exhaust temperature where experiments with citronella additives create lower exhaust temperature as compared to pure gasoline and synthetic additives. However, rooms for innovation remain open by exploring other variables such as higher additive concentrations or combining different kinds of essential oils

    Genetic networks controlling retinal injury.

    PURPOSE: The present study defines genomic loci underlying coordinate changes in gene expression following retinal injury. METHODS: A group of acute phase genes expressed in diverse nervous system tissues was defined by combining microarray results from injury studies from rat retina, brain, and spinal cord. Genomic loci regulating the brain expression of acute phase genes were identified using a panel of BXD recombinant inbred (RI) mouse strains. Candidate upstream regulators within a locus were defined using single nucleotide polymorphism databases and promoter motif databases. RESULTS: The acute phase response of rat retina, brain, and spinal cord was dominated by transcription factors. Three genomic loci control transcript expression of acute phase genes in brains of BXD RI mouse strains. One locus was identified on chromosome 12 and was highly correlated with the expression of classic acute phase genes. Within the locus we identified the inhibitor of DNA binding 2 (Id2) as a candidate upstream regulator. Id2 was upregulated as an acute phase transcript in injury models of rat retina, brain, and spinal cord. CONCLUSIONS: We defined a group of transcriptional changes associated with the retinal acute injury response. Using genetic linkage analysis of natural transcript variation, we identified regulatory loci and candidate regulators that control transcript levels of acute phase genes
