    Intraoperative cholangiography: Learning curve in a General Surgery residency

    Antecedentes: el uso de la colangiograf铆a intraoperatoria din谩mica (CIOd) durante la colecistectom铆a laparosc贸pica (Colelap) sigue siendo un tema en discusi贸n. Objetivos: Este trabajo tiene como objetivo describir y evaluar la curva de aprendizaje y los hallazgos en la CIOd durante las colecistectom铆as laparosc贸picas realizadas por residentes de Cirug铆a General, incluy茅ndola como herramienta para una colecistectom铆a segura, as铆 como entrenamiento para el desarrollo de habilidades y destrezas. Material y m茅todos: se incluyeron pacientes con indicaci贸n de colecistectom铆a laparosc贸pica programada o de urgencia. En las cirug铆as se realiz贸 tracci贸n seg煤n Hunter, visi贸n cr铆tica de seguridad y CIOd sistem谩tica, por un residente mayor y la CIOd por un residente inferior, tutorizado por cirujano de planta. Se evaluaron curva de aprendizaje, tiempos operatorios, relaci贸n del tiempo de CIOd con el tiempo de duraci贸n de la Colelap (CIO/CX), redisecci贸n del c铆stico y litiasis c铆stica y coledociana. Resultados: se operaron 456 pacientes durante un a帽o (2017-2018). Se observ贸 que, independientemente de quien realice la CIOd, los residentes pudieron mejorar su curva de aprendizaje, objetiv谩ndose tiempos m谩s cortos para la Colelap, CIOd y la relaci贸n CIO/CX. Los coeficientes de aprendizaje fueron mejores en cirug铆as m谩s complejas en relaci贸n con el semestre. El 5,26% present贸 litiasis coledociana (n = 24); de estas, 66,7% ten铆an litiasis c铆stica (n = 16) y 25% colecistitis (n = 6) asociadas. Todas se resolvieron por v铆a transc铆stica. No hubo conversiones y se realiz贸 CIOd en el 100%. Conclusi贸n: la CIOd es un procedimiento ideal para ser practicado de manera sistem谩tica durante la Residencia, porque da el entrenamiento necesario para el manejo de la v铆a transc铆stica, permite evitar una lesi贸n quir煤rgica de v铆a biliar mayor y el diagn贸stico de coledocolitiasis.Background: The use of dynamic intra-operative cholangiography (dIOC) during laparoscopic cholecystectomy (Lap Chole) remains a topic under discussion. Objectives: This study aims to describe and evaluate the learning curve and findings in the dIOC during laparoscopic cholecystectomies performed by Residents of General Surgery, including it as a tool for a safe cholecystectomy, as well as training for the development of skills and abilities. Material and methods: Patients with indication of scheduled or emergency laparoscopic cholecystectomy were included. In the surgeries, traction was performed according to Hunter, critical safety vision and systematic dIOC, by a senior Resident and the dIOC by a less trained resident, tutored by a staff surgeon. Learning curve, operative times, dIOC time relationship with Lap Chole duration time (IOC/LC), repeated cystic dissection, cystic lithiasis and choledocholithiasis were evaluated. Results: 456 patients were operated for one year (2017-2018). It was observed that regardless of who performs the dIOC, they were able to improve their learning curve, objectifying shorter times for Lap Chole, dIOC and the IOC/LC relationship. The learning coefficients were better in complex surgeries in relation to the semester. 5.26 % had choledocholithiasis (n = 24), of these, 66.7% had cystic lithiasis (n = 16) and 25% associated cholecystitis (n = 6). All were resolved trancystically. There were no conversions and dIOC was performed in 100% of cases. Conclusion: The dIOC is an ideal procedure to be practiced systematically during residency. Because it gives the necessary training for the management of the transcystic pathway, allows avoiding an upper bile duct injury and the diagnosis of choledocholithiasis.Facultad de Ciencias M茅dica

    Surveillance of human rotaviruses in 2007-2011, Hungary: exploring the genetic relatedness between vaccine and field strains

    BACKGROUND: The availability of rotavirus vaccines has resulted in an intensification of post vaccine strain surveillance efforts worldwide to gain information on the impact of vaccines on prevalence of circulating rotavirus strains. OBJECTIVES: In this study, the distribution of human rotavirus G and P types in Hungary is reported. In addition, the VP4 and VP7 genes of G1P[8] strains were sequenced to monitor if vaccine-derived strains were introduced and/or some strains/lineages were selected against. STUDY DESIGN: The study was conducted in 8 geographic areas of Hungary between 2007 and 2011. Rotavirus positive stool samples were collected from diarrheic patients mostly <5years of age. Viral RNA was amplified by multiplex genotyping RT-PCR assay, targeting the medically most important G and P types. When needed, sequencing of the VP7 and VP4 genes was performed. RESULTS: In total, 2380 strains were genotyped. During the 5-year surveillance we observed the dominating prevalence of genotype G1P[8] (44.87%) strains, followed by G4P[8] (23.4%), G2P[4] (14.75%) and G9P[8] (6.81%) genotypes. Uncommon strains were identified in a low percentage of samples (4.12%). Phylogenetic analysis of 318 G1P[8] strains identified 55 strains similar to the Rotarix strain (nt sequence identities; VP7, up to 97.9%; VP4, up to 98.5%) although their vaccine origin was unlikely. CONCLUSIONS: Current vaccines would have protected against the majority of identified rotavirus genotypes.
