    Preformed metal crowns for decayed primary molar teeth

    BackgroundPreformed metal crowns (PMCs) are recommended by the British Society of Paediatric Dentistry (BSPD) for restoring badly broken down primary molar teeth. However, few dental practitioners adopt this technique in clinical practice, citing cost and clinical difficulty as reasons for this. Whilst there is a subjective impression by clinical academics that PMCs provide a more durable restoration than filling materials, there appears to be little evidence within the literature to support this.ObjectivesThe primary aim of this systematic review was to compare clinical outcomes for primary molar teeth restored using PMCs compared to those restored with filling materials.Search methodsThe literature was searched using: the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2005, Issue 3); MEDLINE (1966 to August 2005); EMBASE (1980 to August 2005); System for Information on Grey Literature in Europe (SIGLE) (1976 to August 2005). Relevant publications' reference lists were reviewed for relevant articles. The most recent search was carried out on 24 August 2005.Selection criteriaRandomised controlled trials (RCTs) that assessed the effectiveness of PMCs compared with filling materials or where there had been no treatment in children with untreated tooth decay in one or more primary molar teeth.Data collection and analysisTwo review authors independently assessed the title and abstracts for each article from the search results to decide whether it was likely to be relevant. Full papers were obtained for relevant articles and all three review authors studied these.Main resultsForty‐seven records were retrieved by the search strategies of which some were duplicates. Of these, 14 studies were scrutinised. No studies met the inclusion criteria and six studies were excluded from the review as they were either retrospective in design or reported as prospective outcomes but not randomised. No data were available for extraction and analysis and therefore, no conclusion could be made as to whether PMCs were more successful than filling materials for restoring primary molar teeth.Authors' conclusionsNo RCTs were available for appraisal. Whilst this technique is recommended by the BSPD for use in clinical practice, the evidence to support this is not strong, consisting mainly of case reports and uncontrolled studies. It is important that the absence of evidence for PMCs is not misinterpreted as evidence for their lack of efficacy.There is a strong need for prospective RCTs comparing PMCs and fillings for managing decayed primary molar teeth. The lower levels of evidence that have been produced, however, have strength in that the clinical outcomes are consistently in favour of PMCs, despite many of the studies placing PMCs on the most damaged of the pair of teeth being analysed

    Exploring the Structure and Biochemistry of Oxidation-Mediated Inhibitation of the Peptidyl-Prolyl Isomerase PIN1

    Pin1 is a phosphorylation-dependent peptidyl-prolyl isomerase that has been shown to be neuroprotective in aging-related neurodegenerative diseases such as Alzheimer\u27s disease (AD). However, it is not active in AD brain, and a recent proteomic screen of Mild Cognitive Impairment (MCI) brain samples revealed that Pin1 is oxidized in the brains of these pre-AD patients. This suggests that this oxidation may be the cause of the loss of the neuroprotective Pin1 function in AD. The Pin1 active site contains a functionally critical cysteine residue (Cys113) with a low predicted pKa, making it highly susceptible to oxidation. We hypothesize that Pin1 is inhibited by oxidation of this cysteine. This hypothesis was tested using enzyme activity assays, mutational analysis, and structural studies. We conclude that the loss of Pin1 activity due to oxidation can be isolated to the oxidative modification of Cys113, and reveals a potential mechanism for countering this loss of function

    Analytical, circle-to-circle low-thrust transfer trajectories with plane change

    Orbit averaging techniques are used to develop analytical approximations of circle-to-circle low-thrust trajectory transfers with plane-change about the Sun. Separate expressions are developed for constant acceleration, or thrust, electric propulsion, solar sail propulsion and combined, or hybrid electric (constant acceleration or thrust) / solar sail propulsion. The analytical expressions uniquely allow the structure of circle-to-circle low-thrust trajectory transfers with plane-change about the Sun to be understood, and the optimal trajectory structure is analytically derived for each propulsion system considered. It is found that the optimal fixed thrust electric propulsion transfer reduces the orbit radius with no plane change and then performs the plane-change, while the optimal solar sail and hybrid transfers combine the reduction of orbit radius with some plane change, before then completing the plane change. The optimal level of plane change during the reduction of orbit radius is derived and it is found the analytically-derived minimum time solar sail transfer is within 1% of the numerically-derived optimal transfer. It is also found that, under the conditions considered, a sail characteristic acceleration of less than 0.5 mm/s2 can, in 5-years, attain a solar orbit that maintains the observer-to-solar pole zenith angle below 40 degrees for 25 days; the approximate sidereal rotation period of the Sun. However, a sail characteristic acceleration of more than 0.5 mm/s2 is required to attain an observer-to-solar pole zenith angle below 30 degrees for 25 days within 5-years of launch. Finally, it was found that the hybridization of electric propulsion and solar sail propulsion was, typically, of more benefit when the system was thrust constrained than when it was mass constrained

    The Dynamics of Radiative Shock Waves: Linear and Nonlinear Evolution

    The stability properties of one-dimensional radiative shocks with a power-law cooling function of the form Λ∝ρ2Tα\Lambda \propto \rho^2T^\alpha are the main subject of this work. The linear analysis originally presented by Chevalier & Imamura, is thoroughfully reviewed for several values of the cooling index α\alpha and higher overtone modes. Consistently with previous results, it is shown that the spectrum of the linear operator consists in a series of modes with increasing oscillation frequency. For each mode a critical value of the cooling index, αc\alpha_\textrm{c}, can be defined so that modes with α<αc\alpha < \alpha_\textrm{c} are unstable, while modes with α>αc\alpha > \alpha_\textrm{c} are stable. The perturbative analysis is complemented by several numerical simulations to follow the time-dependent evolution of the system for different values of α\alpha. Particular attention is given to the comparison between numerical and analytical results (during the early phases of the evolution) and to the role played by different boundary conditions. It is shown that an appropriate treatment of the lower boundary yields results that closely follow the predicted linear behavior. During the nonlinear regime, the shock oscillations saturate at a finite amplitude and tend to a quasi-periodic cycle. The modes of oscillations during this phase do not necessarily coincide with those predicted by linear theory, but may be accounted for by mode-mode coupling.Comment: 33 pages, 12 figures, accepted for publication on the Astrophysical Journa

    Jets or high velocity flows revealed in high-cadence spectrometer and imager co-observations?

    We report on active region EUV dynamic events observed simultaneously at high-cadence with SUMER/SoHO and TRACE. Although the features appear in the TRACE Fe ix/x 171A images as jets seen in projection on the solar disk, the SUMER spectral line profiles suggest that the plasma has been driven along a curved large scale magnetic structure, a pre-existing loop. The SUMER observations were carried out in spectral lines covering a large temperature range from 10^4 K to 10^6 K. The spectral analysis revealed that a sudden heating from an energy deposition is followed by a high velocity plasma flow. The Doppler velocities were found to be in the range from 90 to 160 km/s. The heating process has a duration which is below the SUMER exposure time of 25 s while the lifetime of the events is from 5 to 15 min. The additional check on soft X-ray Yohkoh images shows that the features most probably reach 3 MK (X-ray) temperatures. The spectroscopic analysis showed no existence of cold material during the events

    EIS/Hinode observations of Doppler flow seen through the 40 arcsec wide slit

    The Extreme ultraviolet Imaging Spectrometer (EIS) on board Hinode is the first solar telescope to obtain wide slit spectral images that can be used for detecting Doppler flows in transition region and coronal lines on the Sun and to relate them to their surrounding small scale dynamics. We select EIS lines covering the temperature range 6x10^4 K to 2x10^6 K that give spectrally pure images of the Sun with the 40 arcsec slit. In these images Doppler shifts are seen as horizontal brightenings. Inside the image it is difficult to distinguish shifts from horizontal structures but emission beyond the image edge can be unambiguously identified as a line shift in several lines separated from others on their blue or red side by more than the width of the spectrometer slit (40 pixels). In the blue wing of He II, we find a large number of events with properties (size and lifetime) similar to the well-studied explosive events seen in the ultraviolet spectral range. Comparison with X-Ray Telescope (XRT) images shows many Doppler shift events at the footpoints of small X-ray loops. The most spectacular event observed showed a strong blue shift in transition region and lower corona lines from a small X-ray spot that lasted less than 7 min. The emission appears to be near a cool coronal loop connecting an X-ray bright point to an adjacent region of quiet Sun. The width of the emission implies a line-of-sight velocity of 220 km/s. In addition, we show an example of an Fe XV shift with a velocity about 120 km/s, coming from what looks like a narrow loop leg connecting a small X-ray brightening to a larger region of X-ray emission.Comment: 12 pages, 8 figures, to be published in Solar Physic

    Evidence of continued injecting drug use after attaining sustained treatment-induced clearance of the hepatitis C virus: implications for reinfection

    Background: People who inject drugs (PWID) are at the greatest risk of hepatitis C virus (HCV) infection, yet are often denied immediate treatment due to fears of on-going risk behaviour. Our principal objective was to examine evidence of continued injecting drug use among PWID following successful treatment for HCV and attainment of a sustained viral response (SVR). Methods: PWID who attained SVR between 1992 and June 2012 were selected from the National Scottish Hepatitis C Clinical Database. Hospitalisation and mortality records were sourced for these patients using record linkage techniques. Our primary outcome variable was any hospitalisation or death, which was indicative of injecting drugs post-SVR. Results: The cohort comprised 1170 PWID (mean age at SVR 39.6y; 76% male). The Kaplan Meier estimate of incurring the primary outcome after three years of SVR was 10.59% (95% CI, 8.75–12.79) After adjusting for confounding, the risk of an injection related hospital episode or death post-SVR was significantly increased with advancing year of SVR: AHR:1.07 per year (95% CI, 1.01–1.14), having a pre-SVR acute alcohol intoxication-related hospital episode: AHR:1.83 (95% CI, 1.29–2.60), and having a pre-SVR opiate or injection-related hospital episode: AHR:2.59 (95% CI, 1.84–3.64). Conclusion: Despite attaining the optimal treatment outcome, these data indicate that an increasing significant minority of PWID continue to inject post-SVR at an intensity which leads to either hospitalisation or death and increased risk of reinfection
