    A Prospective Randomized Study Analyzing Preoperative Opioid Counseling in Pain Management After Carpal Tunnel Release Surgery.

    PURPOSE: Prescription opioid misuse has become increasingly prevalent in the United States. Preoperative opioid counseling has been proposed to decrease opioid consumption after surgery. This study aimed to evaluate the effect of preoperative opioid counseling on patients\u27 pain experience and opioid consumption after carpal tunnel release (CTR) surgery. METHODS: A prospective comparison of consecutive patients scheduled to undergo CTR surgery was conducted. Patients were randomized to receive either formal preoperative opioid counseling or no counseling. All operations were performed with the same miniopen CTR surgical technique, and the same number of opioids were prescribed after surgery. Daily opioid pill consumption, pain levels, and any adverse reactions were recorded. RESULTS: During the day of surgery and the first day following surgery, patients in the group with counseling reported significantly fewer prescribed opioid pills consumed compared with patients in the group without counseling, while experiencing no significant difference in pain level experience. In addition, patients in the group with counseling reported a significantly lower number of total pain pills consumed over the course of the study than the group without counseling. No major adverse reactions were noted in either group. CONCLUSIONS: Preoperative opioid counseling was found to result in a significant decrease in overall opioid consumption after surgery.     Complications Associated with Volar Locking Plate Fixation of Distal Radial Fractures.

    Volar locked plating is the most frequently utilized method for internal fixation of distal radial fractures. The overall complication rate for volar plating of distal radial fractures appears to be relatively low compared with other operative fixation methods. Carpal tunnel syndrome is the most commonly reported complication. However, this may occur after a distal radial fracture regardless of treatment method, with reported rates ranging from 0% to 20% with conservative management and 0% to 14% with volar plating. Extensor tendon rupture has been reported at rates of 0% to 4% and is the most frequent complication requiring plate removal. Variable-angle volar locking plates may be associated with fewer implant-related complications as a result of their greater degree of screw placement customization compared with fixed-angle volar locking plates

    Experimental Analysis of Ultra Wideband In Vivo Radio Channel

    In this paper, we present the experimental analysis of in vivo wireless channel response on Ultra-Wideband (UWB) with the frequencies between 3.1-10.6 GHz. The analysis proves the location dependent based characteristics of in vivo channel. The results clearly show the highly multipath scenario. It can also be observed that the multipath effect of the channel is much higher in the denser areas, i.e. an antenna placed within the intestine area or inside the stomach. Results prove that in vivo channel is different from a conventional communication channel and therefore extensive studies need to be done to understand the channel

    This article discusses of KKG in theological perspective al-din al-Qayyim in alQuran. In this case, this perspective is something new developed. The old conception that developed in the community is always viewed from the perspective of law or jurisprudence. Theological concept that carried the authors in this case done in depth through the verses found in the Qur'an in the context of al-din al-Qayyim. Jurisprudence approach commonly used for this are not able to address the true essence of the problem. Such an approach makes fiqh not able to uncover the goals and ideals of the Qur'an in realizing real goodness in all areas of life, including the nine areas set out in the Bill.     Mathematical modeling of ultra wideband in vivo radio channel

    This paper proposes a novel mathematical model for an in vivo radio channel at ultra-wideband frequencies (3.1–10.6 GHz), which can be used as a reference model for in vivo channel response without performing intensive experiments or simulations. The statistics of error prediction between experimental and proposed model is RMSE = 5.29, which show the high accuracy of the proposed model. Also, the proposed model was applied to the blind data, and the statistics of error prediction is RMSE = 7.76, which also shows a reasonable accuracy of the model. This model will save the time and cost on simulations and experiments, and will help in designing an accurate link budget calculation for a future enhanced system for ultra-wideband body-centric wireless systems

    Community Structure and Ecological Conditions of Sea Cucumbers (Holothuroidea) in The Coastal Village of Pasokan, Walea Besar District, Central Sulawesi

    Sea cucumber communities in the wild have different population sizes because there is a high species interaction within the community. Based on the biological organization that is used to express community structure, species diversity is unique to the community level. The objective is to ascertain the structure of sea cucumber communities along the coast of Pasok Village in the Walea Besar District of Central Sulawesi. Data are collected using transect lines, which are divided into three observation stations with ten sampling plots at each. The research method is descriptive-quantitative. The community structure can be known from the domination index (ID), diversity index (H'), and abundance index (J) used to process the data. The results of the study found the diversity index showed the numbers 1.092, 1.056 and 1.089. The dominance of Holothuria scraba species was 0.2 Indv/m2, Holothuria atra 0.14 Indv/m2, and Bohadscia marmorata 0.08 Indv/m2. The highest abundance value belonged to the species Holothuria atra 0.377 Indv/m2 and the lowest to Bohadscia marmorata 0.305 indv/m2. The results showed that the structure of the Holothuroidea community in Pasokan Pantai Village, Walea Besar District, Central Sulawesi, was in the present category

    Genetic characterization of clinical and environmental Vibrio parahaemolyticus from the Northeast USA reveals emerging resident and non-indigenous pathogen lineages

    Gastric infections caused by the environmentally transmitted pathogen, Vibrio parahaemolyticus, have increased over the last two decades, including in many parts of the United States (US). However, until recently, infections linked to shellfish from the cool northeastern US waters were rare. Cases have risen in the Northeast, consistent with changes in local V. parahaemolyticus populations toward greater abundance or a shift in constituent pathogens. We examined 94 clinical isolates from a period of increasing disease in the region and compared them to 200 environmental counterparts to identify resident and non-indigenous lineages and to gain insight into the emergence of pathogenic types. Genotyping and multi-locus sequence analysis (MLSA) of clinical isolates collected from 2010 to 2013 in Massachusetts, New Hampshire, and Maine revealed their polyphyletic nature. Although 80% of the clinical isolates harbored the trh hemolysin either alone or with tdh, and were urease positive, 14% harbored neither hemolysin exposing a limitation for these traits in pathogen detection. Resident sequence type (ST) 631 strains caused seven infections, and show a relatively recent history of recombination with other clinical and environmental lineages present in the region. ST34 and ST674 strains were each linked to a single infection and these strain types were also identified from the environment as isolates harboring hemolysin genes. Forty-two ST36 isolates were identified from the clinical collection, consistent with reports that this strain type caused a rise in regional infections starting in 2012. Whole-genome phylogenies that included three ST36 outbreak isolates traced to at least two local sources demonstrated that the US Atlantic coastal population of this strain type was indeed derived from the Pacific population. This study lays the foundation for understanding dynamics within natural populations associated with emergence and invasion of pathogenic strain types in the region
