    Volunteer experiences in community housing during the Great Hanshin-Awaji Earthquake, Japan

    The recovery phase of disasters is long term and the victims require intensive support. Experiences in disasters can be traumatic and affect people physically and psychologically. Recovery is not only about the rebuilding of infrastructure in the affected area but also about the rehabilitation of people and the rebuilding of their life. The importance of long-term mental health care after disasters has been emphasized in the literature and the authors’ volunteer participation during the Japanese Great Hanshin-Awaji Earthquake in 1995 reinforced its importance. This article reports on the authors’ experiences as health professional volunteers caring for dislocated people living in temporary housing after the earthquake, when kodokushi (death alone and, initially, unnoticed) and alcoholism claimed the life of some of these people

    Application of hot spring pool training in outpatients with chronic diseases and its clinical evaluation

    1996年4月より6月までの3ヵ月間に,三朝分院内の温泉プールで水泳ないし歩行訓練を受けた内科外来患者は延べ2109例(実数213例)で,外来患者総数(延べ6848例)に対する利用率は30.8%であった。これらの対象症例について,リハビリテーション受診カードにより,性別,年令別,疾患別の検討を行った。その結果,性別では女性が多く(72.3%),年齢別では50才以上の患者が多い(80.3%)傾向が見られた。また,地域別検討では,鳥取県内からの患 者が多い(88.3%)傾向であった。さらに,1996年12月より1997年1月へかけて,アンケート調査を行い(集計数;64例)継続期間,利用回数,効果を感じた期間,効果のあった症状などについて疾患別に比較検討した。その結果,慢性閉塞性肺疾患を主とした呼吸器疾患では,週に1~2回の利用回数(50.0%)で長期間(3年以上:52.9%)続けている患者が多い傾向であった。また慢性関節リウマチ,腰痛などの慢性疼痛性疾患では,訓練期間は比較的短期間であっ たが(3年以下:52.9%),利用回数は週に3~5回(38.0%)と多い傾向が見られた。温泉プール訓練では,疾患本来の症状改善に加えて,全身状態の改善,精神的リラックス,気分転換などを感じていることが示唆された。For three months from April to June in 1996, 2109(30.8%) of 6848 outpatients (the total number) had swimming and walking traning in a hot spring pool. Of the 213 patients (the actual number), the numbers of female patients (72.3%), of patients over the age of 50 (80.3%), and of those coming inside Tottori prefecture (88.3%) were larger in analysis by the consultation cards for rehabillitation. The results of questionnaire from the patients performed from December 1996 to January 1997 demonstrated that patients with chronic respiratory disease had pool training 1 to 2 times a week (50.0%) and for longer term (more than 3 years, 52.9%), and patients with chronic pain disease had the training 3 to 5 times a week (38.0%) and for shoter term (less than 3 years, 61.9%). It was suggested from the results that tha training in a hot spring pool improved subjective and objective symptoms of each disease, and, furthermore, brought about improvement of general condition, psychological relaxation, and a change of feeling

    Spatiotemporal expression of TRPM4 in the mouse cochlea

    The present study was conducted to elucidate the presence of the transient receptor potential cation channel subfamily M member 4, TRPM4, in the mouse inner ear. TRPM4 immunoreactivity (IR) was found in the cell body of inner hair cells (IHCs) in the organ of Corti in the apical side of marginal cells of the stria vascularis, in the apical portion of the dark cells of the vestibule, and in a subset of the type II neurons in the spiral ganglion. Subsequently, changes in the distribution and expression of TRPM4 in the inner ear during embryonic and postnatal developments were also evaluated. Immunohistochemical localization demonstrated that the emergence of the TRPM4-IR in IHCs occurs shortly before the onset of hearing, whereas that in the marginal cells happens earlier, at the time of birth, coinciding with the onset of endolymph formation. Furthermore, semiquantitative real-time PCR assay showed that expressions of TRPM4 in the organ of Corti and in the stria vascularis increased dramatically at the onset of hearing. Because TRPM4 is a Ca2+-activated monovalent-selective cation channel, these findings imply that TRPM4 contributes to potassium ion transport, essential for the signal transduction in IHCs and the formation of endolymph by marginal cells. © 2014 Wiley Periodicals, Inc

    Metabolome analyses assisted by capillary electrophoresis-mass spectrometry (CE-MS) have allowed us to systematically grasp changes in small molecular metabolites under disease conditions. We applied CE-MS to mine out biomarkers in hepatic ischemia-reperfusion. Rat livers were exposed to ischemia by clamping of the portal inlet followed by reperfusion. Metabolomic profiling revealed that l contents of taurine in liver and plasma were significantly increased. Of interest is an elevation of hypotaurine, collectively suggesting significance of hypotaurine/taurine in post-ischemic responses. Considering the anti-oxidative capacity of hypotaurine, we examined if supplementation of the compound or its precursor amino acids could affect hepatocellular viability and contents of taurine in liver and plasma. Administration of hypotaurine, N-acetylcysteine or methionine upon reperfusion comparablly attenuated the post-ischemic hepatocellular injury but with different metabolomic profiling among groups: rats treated with methionine or N-acetylcysteine but not those treated with hypotaurine, exhibited significant elevation of hepatic lactate generation without notable recovery of the energy charge. Furthermore, the group treated with hypotaurine exhibited elevation of the plasma taurine, suggesting that the exogenously administered compound was utilized as an antioxidant. These results suggest that taurine serves as a surrogate marker for ischemia-reperfusion indicating effectiveness of hypotaurine as an energy-saving hepatoprotective amino acid

    How a disabled person's family recovered resilience through intervention by a daughter-in-law who became a companion for the disabled person <Article>

    The purpose of this research was to study the family resilience recovery processes by which a woman who became the sister of a disabled person through marriage was able to improve the family's situation. The subject of the research was a 44 year-old female. She married her husband 25 years ago. Her husband's younger sister was disabled. In this research a single case study was done, through a semi-structured interview. The subject said that she had intervened on a problem of cruelty that the disabled person's family face. The family had been insular, but the subject led it to become one open to society. Furthermore, the disabled person was able to maintain a place in society, which also contributed to the recovery of her physical and emotional functions. In this way, the subject took on the role of recovering the family's resilience. However, this worsened the relation between her mother-in-law and the disabled person. This suggests that the subject's bearing everything on her shoulders was a major burden for her. In the future it is necessary to investigate the subject's life history and the husband's family and increase the number of case studies

    8-Prenylnaringenin tissue distribution and pharmacokinetics in mice and its binding to human serum albumin and cellular uptake in human embryonic kidney cells

    8-Prenylnaringenin (8-PN), a hop flavonoid, is a promising food substance with health benefits. Compared with nonprenylated naringenin, 8-PN exhibits stronger estrogenic activity and prevents muscle atrophy. Moreover, 8-PN prevents hot flushes and bone loss. Considering that prenylation reportedly improves the bioavailability of flavonoids, we compared the parameters related to the bioavailability [pharmacokinetics and tissue distribution in C57/BL6 mice, binding affinity to human serum albumin (HSA), and cellular uptake in HEK293 cells] of 8-PN and its mother (non-prenylated) compound naringenin. C57/BL6 mice were fed an 8-PN or naringenin mixed diet for 22 days. The amount of 8-PN (nmol/g tissue) in the kidneys (16.8 ± 9.20), liver (14.8 ± 2.58), muscles (3.33 ± 0.60), lungs (2.07 ± 0.68), pancreas (1.80 ± 0.38), heart (1.71 ± 0.27), spleen (1.36 ± 0.29), and brain (0.31 ± 0.09) was higher than that of naringenin. A pharmacokinetic study in mice demonstrated that the Cmax of 8-PN (50 mg/kg body weight) was lower than that of naringenin; however, the plasma concentration of 8-PN 8 h after ingestion was higher than that of naringenin. The binding affinity of 8-PN to HSA and cellular uptake in HEK293 cells were higher than those of naringenin. 8-PN bioavailability features assessed in mouse or human model experiments were obviously different from those of naringenin

    A novel S-sulfhydrated human serum albumin preparation suppresses melanin synthesis

    Products of ultraviolet (UV) irradiation such as reactive oxygen species (ROS) and nitric oxide (NO) stimulate melanin synthesis. Reactive sulfur species (RSS) have been shown to have strong ROS and NO scavenging effects. However, the instability and low retention of RSS limit their use as inhibitors of melanin synthesis. The free thiol at Cys34 on human serum albumin (HSA) is highly stable, has a long retention and possess a high reactivity for RSS. We report herein on the development of an HSA based RSS delivery system. Sulfane sulfur derivatives released from sodium polysulfides (Na2Sn) react readily with HSA. An assay for estimating the elimination of sulfide from polysulfide showed that almost all of the sulfur released from Na2Sn bound to HSA. The Na2Sn-treated HSA was found to efficiently scavenge ROS and NO produced from chemical reagents. The Na2Sn-treated HSA was also found to inhibit melanin synthesis in B16 melanoma cells and this inhibition was independent of the number of added sulfur atoms. In B16 melanoma cells, the Na2Sn-treated HSA also inhibited the levels of ROS and NO induced by UV radiation. Finally, the Na2Sn-treated HSA inhibited melanin synthesis from L-DOPA and mushroom tyrosinase and suppressed the extent of aggregation of melanin pigments. These data suggest that Na2Sn-treated HSA inhibits tyrosinase activity for melanin synthesis via two pathways; by directly inhibiting ROS signaling and by scavenging NO. These findings indicate that Na2Sn-treated HSA has potential to be an attractive and effective candidate for use as a skin whitening agent

    O conceito de qualidade do cliente aplicado ao serviço de alimentação e nutrição de hospital oncológico

    O Serviço de Alimentação e Nutrição hospitalar é responsável não somente por produzir e distribuir refeições, é também sua função atender às necessidades do cliente hospitalizado. O objetivo deste estudo foi avaliar resultados de estratégias para melhoria da qualidade do Serviço de Alimentação e Nutrição de um hospital oncológico a partir da análise de fatores determinantes da qualidade na visão deste cliente. Após aplicação de questionário para conhecer graus de importância atribuídos às características da qualidade da alimentação, foram listados objetivos, resultados esperados e ações. Houve comparação da satisfação do cliente hospitalizado antes e após a implementação das estratégias. A frequência de respostas “satisfatório” relativas à aparência das refeições, atendimento da copeira e horário de refeições aumentou. Não houve  aumento do percentual de aceitação da dieta, mas o motivo de não aceitação “características organolépticas” e “quantidade exagerada de alimentos”, dependentes do serviço de alimentação ofertado, apresentou redução da frequência. Dentre os motivos de não aceitação relacionados a doença,  foi possível identificar prevalência sintomas consequentes ao tratamento quimioterápico, radioterápico ou cirúrgico a que o paciente oncológico é submetido como a falta de apetite e presença de distúrbios gastrointestinais nos dois momentos  porém, com aumento de 73,73% para 79,29%, confirmando que a aceitação alimentar recebeu grande influência de fatores relacionados à doença

    Distribution of Polysulfide in Human Biological Fluids and Their Association with Amylase and Sperm Activities

    Intracellular polysulfide could regulate the redox balance via its anti-oxidant activity. However, the existence of polysulfide in biological fluids still remains unknown. Recently, we developed a quantitative analytical method for polysulfide and discovered that polysulfide exists in plasma and responds to oxidative stress. In this study, we confirmed the presence of polysulfide in other biological fluids, such as semen and nasal discharge. The levels of polysulfide in these biological fluids from healthy volunteers (n = 9) with identical characteristics were compared. Additionally, the circadian rhythm of plasma polysulfide was also investigated. The polysulfide levels detected from nasal discharge and seminal fluid were approximately 400 and 600 μM, respectively. No correlation could be found between plasma polysulfide and the polysulfide levels of tear, saliva, and nasal discharge. On the other hand, seminal polysulfide was positively correlated with plasma polysulfide, and almost all polysulfide contained in semen was found in seminal fluid. Intriguingly, saliva and seminal polysulfide strongly correlated with salivary amylase and sperm activities, respectively. These results provide a foundation for scientific breakthroughs in various research areas like infertility and the digestive system process
