    Akreditasi sekolah adalah kegiatan penilaian (asesmen) sekolah secara sistematis dan komprehensif melalui kegiatan evaluasi diri dan evaluasi eksternal (visitasi) untuk menentukan kelayakan dan kinerja sekolah. Tujuan pelaksanaan akreditasi sekolah adalah untuk : (a) menentukan tingkat kelayakan suatu sekolah dalam menyelenggarakan layanan pendidikan dan (b) memperoleh gambaran tentang kinerja sekolah. Pengetahuan tentang akreditasi sekolah dan tujuan daripada pelaksanaan akreditasi sekolah belumlah begitu dipahami oleh masyarakat dikarenakan selama ini kurang mendapat sosialisasi tentang akreditasi sekolah. Bahwa akreditasi sekolah perlu diketahui oleh masyarakat supaya masyarakat dapat memilih sekolah yang unggul bagi anaknya/ siswa agarĀ  mendapat pengalaman belajar yang berkualitas karena sekolah ya ng mempunyai nilai akreditasi A (= Unggul) berarti telah memenuhi syarat delapan Standar Nasional Pendidikan yang ditetapkan pemerintah sebagai acuan penilaian akreditasi sekolah dengan sangat baik sehingga dapat memberikan layanan pendidikan yang unggul juga. Sosialisasi urgensi akreditasi dilaksanakan dengan melibatkanĀ  Ā mitra, yakni Desa Pematang Johar Kecamatan Labuhan Deli Kabupaten Deli Serdang. Alasan dipilihnya mitra ini adalah masih minimnya sekolah dengan akreditasi yang unggul di wilayah ini. Dengan diadakannyaĀ  sosialisasi tentang urgensi akreditasi sekolah bagi masyarakat ini,Ā  Ā maka diharapkan dapat menumbuhkan pengetahuan tentang pentingnya nilai akreditasi yang unggul bagi sekolah yang berlanjut pada peningkatan kualitas sekolah dan kualitas layanan pendidikan yang unggul danĀ  Ā menghasilkan lulusan yang unggul pula. Selain itu, dengan adanya sosialisasi urgensi akreditasi sekolah/madrasahĀ  diharapkan dapat menggerakkanĀ  partisipasi masyarakat melalui komite untuk membantu sekolah dalam meningkatkan kualitas akreditasinya. SekaligusĀ  memotivasi sekolah untuk terus berupaya mendapatkan nilai akreditasi yang unggul karena masyarakat akan memilih sekolah yang unggul bagi tempat belajar anak-anaknya

    Regulation is the cornerstone for utilizing marine fisheries resources, and thus Indonesian Government had implemented its Constitution as basic laws and rules. Those regulations need further elaboration. Therefore, Indonesian Government established sets of laws related to the utilization of marine fisheries resources, and in its implementation, those laws and regulations should never deviate from the Constitution of the Republic of Indonesia. The utilization of marine fisheries resources in Indonesia's Exclusive Economic Zone enables the possibility of cooperation and colaboration with other countries. Therefore, Indonesia is required to seek by evaluating and complementing the laws in accordance with its Constitution. Pengaturan merupakan landasan utama pemanfaatan sumber daya perikanan, Indonesia telah menyiapkan aturan dasar yaitu Landasan Konstitusional. Pengaturan tersebut perlu penjabaran lebih lanjut. Oleh karena itu Indonesia membentuk aturan-aturan yang terkait dengan pemanfaatan sumber daya ikan dan dalam implementasinya aturan tersebut tidak boleh menyimpang dari aturan yang paling dasar. Pemanfaatan sumber daya ikan di Zona Ekonomi Eksklusif adalah pemanfaatan yang memungkinkan adanya pemanfaatan berbagi dengan negara lain maka Indonesia dituntut untuk segera dapat mencari dengan cara mengevaluasi dan melengkapi aturan sesuai dengan Landasan Konstitusional

    In addition to the established association between high lipoprotein(a) [Lp(a)] concentrations and coronary artery disease, an association between Lp(a) and venous thromboembolism (VTE) has also been described. Lp(a) is controlled by genetic variants in LPA gene, coding for apolipoprotein(a), including the kringle-IV type 2 (KIV-2) size polymorphism. Aim of the study was to investigate the role of LPA gene KIV-2 size polymorphism and single nucleotide polymorphisms (SNPs) (rs1853021, rs1800769, rs3798220, rs10455872) in modulating VTE susceptibility. Five hundred and sixteen patients with VTE without hereditary and acquired thrombophilia and 1117 healthy control subjects, comparable for age and sex, were investigated. LPA KIV-2 polymorphism, rs3798220 and rs10455872 SNPs were genotyped by TaqMan technology. Concerning rs1853021 and rs1800769 SNPs, PCR-RFLP assay was used. LPA KIV-2 repeat number was significantly lower in patients than in controls [median (interquartile range) 11(6-17) vs 15(9-25), p<0.0001]. A significantly higher prevalence of KIV-2 repeat number ā‰¤7 was observed in patients than in controls (33.5% vs 15.5%, p<0.0001). KIV-2 repeat number was independently associated with VTE (p = 4.36 x10-9), as evidenced by the general linear model analysis adjusted for transient risk factors. No significant difference in allele frequency for all SNPs investigated was observed. Haplotype analysis showed that LPA haplotypes rather than individual SNPs influenced disease susceptibility. Receiver operating characteristic curves analysis showed that a combined risk prediction model, including KIV-2 size polymorphism and clinical variables, had a higher performance in identifying subjects at VTE risk than a clinical-only model, also separately in men and women

    Air Pollution, Smoking, and Plasma Homocysteine

    BACKGROUND: Mild hyperhomocysteinemia is independently associated with an increased risk of cardiovascular disease. Air pollution exposure induces short-term inflammatory changes that may determine hyperhomocysteinemia, particularly in the presence of a preexisting proinflammatory status such as that found in cigarette smokers. OBJECTIVE: We examined the relation of air pollution levels with fasting and postmethionine-load total homocysteine (tHcy) in 1,213 normal subjects from Lombardia, Italy. METHODS: We obtained hourly concentrations of particulate matter < 10 Ī¼m in aerodynamic diameter (PM(10)) and gaseous pollutants (carbon monoxide, nitrogen dioxide, sulfur dioxide(,) ozone) from 53 monitoring sites covering the study area. We applied generalized additive models to compute standardized regression coefficients controlled for age, sex, body mass index, smoking, alcohol, hormone use, temperature, day of the year, and long-term trends. RESULTS: The estimated difference in tHcy associated with an interquartile increase in average PM(10) concentrations in the 24 hr before the study was nonsignificant [0.4%; 95% confidence interval (CI), āˆ’2.4 to 3.3 for fasting; and 1.1%, 95% CI, āˆ’1.5 to 3.7 for postmethionine-load tHcy]. In smokers, 24-hr PM(10) levels were associated with 6.3% (95% CI, 1.3 to 11.6; p < 0.05) and 4.9% (95% CI, 0.5 to 9.6; p < 0.05) increases in fasting and postmethionine-load tHcy, respectively, but no association was seen in nonsmokers (p-interaction = 0.005 for fasting and 0.039 for postmethionine-load tHcy). Average 24-hr O(3) concentrations were associated with significant differences in fasting tHcy (6.7%; 95% CI, 0.9 to 12.8; p < 0.05), but no consistent associations were found when postmethionine-load tHcy and/or 7-day average O(3) concentrations were considered. CONCLUSIONS: Air particles may interact with cigarette smoking and increase plasma homocysteine in healthy subjects

    Low-molecular-weight heparin for prevention of placenta-mediated pregnancy complications: protocol for a systematic review and individual patient data meta-analysis (AFFIRM)

    BACKGROUND: Placenta-mediated pregnancy complications include pre-eclampsia, late pregnancy loss, placental abruption, and the small-for-gestational age newborn. They are leading causes of maternal, fetal, and neonatal morbidity and mortality in developed nations. Women who have experienced these complications are at an elevated risk of recurrence in subsequent pregnancies. However, despite decades of research no effective strategies to prevent recurrence have been identified, until recently. We completed a pooled summary-based meta-analysis that strongly suggests that low-molecular-weight heparin reduces the risk of recurrent placenta-mediated complications. The proposed individual patient data meta-analysis builds on this successful collaboration. The project is called AFFIRM, An individual patient data meta-analysis oF low-molecular-weight heparin For prevention of placenta-medIated pRegnancy coMplications. &nbsp; METHODS/DESIGN: We conducted a systematic review to identify randomized controlled trials with a low-molecular-weight heparin intervention for the prevention of recurrent placenta-mediated pregnancy complications. Investigators and statisticians representing eight trials met to discuss the outcomes and analysis plan for an individual patient data meta-analysis. An additional trial has since been added for a total of nine eligible trials. The primary analyses from the original trials will be replicated for quality assurance prior to recoding the data from each trial and combining it into a common dataset for analysis. Using the anonymized combined data we will conduct logistic regression and subgroup analyses aimed at identifying which women with previous pregnancy complications benefit most from treatment with low-molecular-weight heparin during pregnancy. &nbsp; DISCUSSION: The goal of the proposed individual patient data meta-analysis is a thorough estimation of treatment effects in patients with prior individual placenta-mediated pregnancy complications and exploration of which complications are specifically prevented by low-molecular-weight heparin. &nbsp; SYSTEMATIC REVIEW REGISTRATION: PROSPERO (International Prospective Registry of Systematic Reviews) 23 December 2013, CRD42013006249.</div

    Treatment Decision-Making of Secondary Prevention After Venous Thromboembolism: Data From the Real-Life START2-POST-VTE Register

    Patients with venous thromboembolism (VTE) should receive a decision on the duration of anticoagulant treatment (AT) that is often not easy to make. Sixteen Italian clinical centers included patients with recent VTE in the START2-POST-VTE register and reported the decisions taken on duration of AT in each patient and the reasons for them. At the moment of this report, 472 (66.9%) of the 705 patients included in the registry were told to stop AT in 59.3% and to extend it in 40.7% of patients. Anticoagulant treatment lasted 653 months in &gt;90% of patients and was extended in patients with proximal deep vein thrombosis because considered at high risk of recurrence or had thrombophilic abnormalities. d-dimer testing, assessment of residual thrombus, and patient preference were also indicated among the criteria influencing the decision. In conclusion, Italian doctors stuck to the minimum 3 months AT after VTE, while the secondary or unprovoked nature of the event was not seen as the prevalent factor influencing AT duration which instead was the result of a complex and multifactorial evaluation of each patient
