    Helium Neon laser therapy for post mastectomy lymphedema and shoulder mobility

    The aim of this work was to evaluate the efficacy of Helium Neon (He–Ne) laser therapy on post mastectomy lymphedema and shoulder mobility. Thirty female patients with axillary lymph node dissection (ALND), with or without radiotherapy had been participated in this study. The patients were randomly divided into two groups of equal numbers. Group A received He–Ne laser therapy and decongestive lymphatic therapy. Group B received placebo laser therapy in addition to decongestive lymphatic therapy. Measurements of limb volume and shoulder mobility (by tape measurement and standard goniometer) were collected before treatment and after six months of treatment. The mean values of limb volume and Shoulder mobility after 6 months of treatment showed a significant improvement (p< 0.05) for the two groups of the study with a greater improvement for patients in group A. Conclusion: Helium Neon laser therapy has a positive effect in reducing post mastectomy lymphedema and increasing range of motion of shoulder joint.Keywords: Helium Neon laser; Lymphedema; Mastectomy; Shoulder mobilityThe Egyptian Journal of Medical Human Genetics (2013) 14, 195–19

    Preliminary phytochemical and antimicrobial screening of they stem bark extracts of Bauhinia rufescence Lam using some selected pathogens

    Bauhinia rufescens Lam (Leguminosea-Caesalpinoideae) stem bark was extracted using methanol and fractionated using ethylacetate, butanol and water. The extract and fractions were subjected to preliminary phytochemical screening using standard procedures followed by antimicrobial screening using disc diffusion and broth dilution techniques. The extract and fractions showed the presence of carbohydrate, tannins, flavonoids, saponins, terpenes and steroids. The antimicrobial screening of the extract and fractions against clinical isolates of Staphylococcus aureus, Streptococcus pyogenes, Bacillus subtilis, Pseudomonas aeruginosa, Escherichia coli and Candida albicans, using disc diffusion method at disc potency of 100μg/disc showed inhibitory activity on the test isolates with zone of inhibition ranging from 16-37mm. The methanolic and ethylacetate extracts showed the least MIC of 1.25mg/mL. The result of the study confirms the traditional use of the stem bark of B. rufescens in the treatment of infections caused by susceptible microorganisms.     Hydrogen production using Al-Sn alloys prepared by rapid solidification

    Air pollution produced by the direct burning of fossil fuels is a serious problem. Therefore, there is a vital demand for renewable and clean fuel replacements for future energy source. Hydrogen, which exhibits high calorific value and is a clean burning product, could be the first choice in the future, as it is a good-looking fuel for fuel cells where the electric energy is directly gotten by the electrochemical reactions of hydrogen and oxygen. Rapidly solidified Al100-x-Snx alloy X= (0, 25, 55, 75 all in wt.%) was prepared using melt spinning technique at 800 ºC. The structural and microstructural evolutions of the phases have been studied using X-ray powder diffraction (XRD) and the Rietveld method. Al45Sn55 alloy shows the best volume of hydrogen generation which is (531 ml), this alloy has the highest number of grains (1096) than other prepared alloys in its surface which examined through a Scanning electron microscope (SEM) and analyzed using an SPM data visualization and analysis tool (Gwyddion 2.32). Differential thermal analysis (DTA) was used for calculating melting temperature for all prepared alloys. Melting temperature reduced from 933.47 K for Al pure to 500.95 K in Al45Sn55 alloy. Temperature dependence of resistivity had been performed for all prepared alloys. Electrical properties of prepared alloys show the decrease in volume of the unit cell V with increasing VEC, which means that the volume of the first Brillouin zone increases by increasing VEC since they are inversely proportional to each other

    Quality of life and physical functioning of the diabetic middle aged and older adults

    Objective: To compare the physical functioning and quality of life for the diabetic middle aged and older adults.Methodology: A descriptive study design was used, the study was conducted at the outpatient clinics of two hospitals in Alexandria, Egypt. A total number of 118 diabetic patients diagnosed with diabetes at least for one year, aged 20-59 for the middle aged adults group & ≥60 for the elderly group, had no current physical disabilities or mental impairments were included. Four measures were used in this study; the socio-demographic and clinical data structured interview questionnaire, the Katz Activities of Daily Living (ADL) Scale, the International Physical Activity Questionnaire-Short version (IPAQ), and the Short Form 36 General Health questionnaire (SF-36).Results: A statistical difference was observed between the two groups in all domains of generic health related quality of life except role limitations due to emotional problems, and social functioning. Adults' group with normal fasting blood sugar test had better quality of life and physical functioning than the elderly group.Conclusion& recommendations: The elderly patients with diabetes had poorer quality of life and functional status in comparison with the adults. These findings suggest that, health education programs that stress a balanced diet and increased activity should be a public health priority for all ages to control diabetes mellitus and its complications

    Rapidly Progressive IgA Nephropathy in One of a Pair of Identical Twins

    Introduction: IgA nephropathy (IgAN) is the most common cause of primary glomerulonephritis worldwide. It was considered a benign condition for many years but long term follow up showed that it might progress to end stage renal disease (ESRD). The cause of primary IgAN is unknown and no consistent genetic abnormalities that predict the development or progression of IgAN have been identified. A variety of observations suggest an as-sociation to an unknown environmental antigen, familial clustering, or infectious agent. The patient presented here is the first report of a child with IgAN in Sudan. Case report: a seven years old boy was referred to our center for further evaluation and management of sudden onset macroscopic hematuria and renal impairment. He was born after an uneventful pregnancy and breast fed. He developed normally and was healthy before this illness. He had eight siblings who were all healthy, including his identical twin brother. Physical examination and laboratory tests led to the diagnosis of IgAN. The patient was treated with peritoneal dialysis and pulses of methylprednisolone for three consecutive days. He showed a remarkable response and regained normal renal function. He was then kept on alternate day’s steroids, ACE inhibitors, and Azathioprine. The other twin is being closely monitored. Conclusion: This report provides an indirect support for the hypothesis that environmental factors play a role in the pathogenesis of IgAN. However, follow up of the currently healthy twin is necessary, since affection with the disease may be expressed at a later time. Key words: IgA nephropathy, identical twins, macroscopic hematuri

    Introduction: Tonsillectomy is a common surgical procedure done by  totolaryngologists. Tonsillectomy is a relatively simple procedure. The concept of implementing it as a day case operation has become increasingly popular.Material and Methods: This is a cross sectional study done in Alahsa city, eastern province, Saudi Arabia during the period from January 2014 to March 2015,This study reported the postoperative hemorrhage after Bipolar diathermy and Cold dissection surgical techniques to evaluate the incidence of the hemorrhage and to identify the possible risk factors associated with its occurrence.Results: Postoperative bleeding occurred in 45 (3.6%) out of 1232 patients. Post-tonsillectomy hemorrhage according to operation technique was significantly higher among patients who underwent bipolar diathermy than cold dissection technique (p< 0.05).Conclusion: Bleeding after operation by bipolar diathermy technique was occurring more frequently within the first five days. Hemoglobin level was significantly decreased in posttonsillectomy hemorrhage

    Characterization of α-Amylase Produced by Different Bacillus spp

    The production of  the amylase enzyme from different Bacillus spp. was carried out in culture fermentation (SCF). Production of the amylase was carried out for the potential use of the enzyme for industrial purposes. The bacteria used  were isolated and taxonomic characterized from Khartoum state. These bacteria were identified as B megaterium, B. subtilis, B. licheniformis and B. amyloliquefaciens. It was found that Bacillus licheniformis had 6.5 cm zone of clearance, .,B megaterium  cm, B. amyloliquefaciens  cm where as B. subtilis resulted in much lesser  cm. Amylase produced by Bacillus licheniformis B megaterium B. amyloliquefaciens  and  B. subtilis  was 42.64  mg/ml, 38.11 mg/ml, 35.14 mg/ml, and  37.93 mg/ml glucose, respectively

    Although Anglo-Egyptian negotiations began in 1920, it was only in 1936 that a treaty was concluded. This was mainly due to the radical change of the Wafd's tactics in the 1930's from extremism to moderation. Chapter I suggests the reasons for this and describes the successful attempts of the wafd to persuade the Egyptian public to accept an alliance with Britain. Though signs of the Fascist imperialist aims had begun to appear in the mid 1930's, Nahhas and his colleagues do not appear to have been convinced of a real and immediate Italian danger, and may have used the Abyssinian crisis as a face-saving pretext. Chapter II, discusses these topics and also the reasons for the desire in Britain to have a treaty with Egypt, Chapter III is devoted to a discussion of the reasons of the Palace and minority parties in Egypt, and of some extreme Conservatives in Britain, for opposing the 1936 negotiations, and their attempts to wreck them. Chapters IV, V, and VI give a detailed account of the military, Sudan, and the civil clauses of the treaty, and reactions to them in Britain and Egypt. The Appendix gives the texts of the 1936 treaty (with a map printed at the War Office to illustrate it) and an important memorandum handed by Nahhas, the Prime Minister, to Lampson, the High Commissioner, on 1 June 1936

    Persuasion is part and parcel of human interaction. The human persuaders in society have been always exit, masters of rhetoric skilled of changing our minds, or at least our behaviors. Leaders, mothers, salesmen, and teachers are clear examples of persuaders. Persuaders often turn to technology and digital media to amplify their persuasive ends. Besides, our lives and how we lead them influenced by technologies and digital media,but for the most part, their effects on our attitudes and behaviors have been incidental, even accidental. Although, nowadays, the use of computers to sell products and services considered as the most frequent application of persuasive technology. In this short paper, based on an extensive review of literatures, we aim to give a brief introduction to persuasive technology, and how it can play a role and contribute to enhance and deliver the best practice of IT. Some challenges of persuasive technology have been discussed.
