    Case Report: Generalized Mutual Information (GMI) Analysis of Sensory Motor Rhythm in a Subject Affected by Facioscapulohumeral Muscular Dystrophy after Ken Ware Treatment

    In this case report we study the dynamics of the SMR band in a subject affected from Facioscapulohumeral Muscular Dystrophy and subjected to Ken Ware Neuro Physics treatment. We use the Generalized Mutual Information (GMI) to analyze in detail the SMR band at rest during the treatment. Brain dynamics responds to a chaotic-deterministic regime with a complex behaviour that constantly self-rearranges and self-organizes such dynamics in function of the outside requirements. We demonstrate that the SMR chaotic dynamics responds directly to such regime and that also decreasing in EEG during muscular activity really increases its ability of self-arrangement and self-organization in brain. The proposed novel method of the GMI is arranged by us so that it may be used in several cases of clinical interest. In the case of muscular dystrophy here examined, GMI enables us to quantify with accuracy the improvement that the subject realizes during such treatment

    The use of antimicrobials in Italian heavy pig fattening farms

    Data on antimicrobial use (AMU) in heavy pig production (>150 kg) are limited. The aim of this study was to investigate the AMU in this production. Data from 2015 were collected for 143 fattening farms. The AMU was estimated through a treatment index per 100 days (TI100) using the defined daily dose animal for Italy (DDDAit). When possible, a comparison with the European Medicines Agency's defined daily doses for animals (DDDvet) was performed. The median TI100 was 10.7 (range, 0.2-49.5). Group treatments represented 94.6% of overall consumption. The AMU calculated using DDDAit and DDDvet were strongly correlated (rho = 0.976; p < 0.001). The AMU was negatively correlated with injectables use (rho = -0.46, p < 0.001) and positively correlated with oral products (rho = 0.21, p = 0.014), premixes (rho = 0.26, p = 0.002), and mortality (rho = 0.18; p = 0.027). Farm size was negatively correlated with AMU (rho = -0.29, p < 0.001). Smaller farms were more frequently above the median TI100 (odds ratio = 2.3, 95% confidence interval = 1.2-4.7), suggesting that they may have lower biosecurity and management standards. The results of this study should provide useful insights for the development of an Italian monitoring system

    Development and test of a visual-only meat inspection system for heavy pigs in Northern Italy

    Abstract Background There is a general consensus in recognizing that traditional meat inspection is no longer able to address the hazards related to meat consumption. Moreover, it has been shown that invasive procedures, such as palpation and incision, can increase microbial contamination in carcasses. For these reasons, legislations all over the world are changing meat inspection techniques, moving towards visual-only techniques. Hence, there was also the need to test visual-only inspection in pigs in Italy. Results A protocol for visual-only post-mortem inspection was produced together with a 24-class scheme used to record pathological lesions. A list of guidelines needed for univocal interpretation and classification of lesions was developed. To record lesions at the slaughtering line, a light instrument that is resistant to the slaughter environment was designed and then produced in collaboration with an electro-medical company. Six contracted veterinarians were chosen and trained. They performed visual-only post-mortem inspections on 231.673 heavy pigs in three different slaughterhouses of Northern Italy. Visual-only inspection was compared to traditional inspection on 38.819 pig carcasses. No relevant differences were found between the two systems. Conclusions The comparison between traditional and visual-only inspection showed that visual-only inspection can be adopted in pig slaughterhouse. The analysis of the performance of the veterinarians stressed the importance of standardization and continuous education for veterinarians working in this field

    Efficacy and non invasive treatment of sialorrhea in the goldenhar syndrome

    Background and Objective: Goldenhar syndrome (ocular-auricular-vertebral syndrome), a rare congenital condition arising from defects in the first and second brachial arches, consists in clinical variety of features ranging from facial abnormalities, ear-eye abnormalities, vertebral defects and congenital heart problems and severe obstructive sleep apnea. Due to craniofacial abnormalities, patients presents mechanical obstructive phenomena and sialorrhea that cause prone position, language’s fastening, use of nasopharyngeal cannulas and tracheal intubation. Methods: In this article, we report a case of a 16 years old child affected by Goldenhar syndrome and sialorrhea to demonstrate improvement of the daily patient management, through inoculations of botulinum toxin type A. Due to severe sialorrhea which caused tracheobronchial daily aspirations, the caregivers used an external aspirators. Results: In the first infiltration (August 2016) the parotid and submandibular glands bilaterally were inoculated with incobotulinum toxin type A (Xeomin®, Merz Pharma) with dosages of 5 UI for each of them, for a total of 20 UI without clinical efficacy (no quantitative and qualitative saliva reducing during 3 months). In the second (November 2016) and third (February 2017) infiltrations each parotid and each submandibular glands were injected with a (dosage of 7 UI and 5 UI respectively (total of 24 UI of incobotulinumtoxin A) with important clinical results (saliva production and tracheo-bronchial aspirations reduced). Conclusion: Therefore, botulinum toxin type A could be a good and non invasive treatment of sialorrhea in Goldenhar syndrome to improve oral hygiene and daily patient management

    Microbiological contamination in three large-scale pig slaughterhouses in Northern Italy

    The aim of this survey was to obtain data on microbiological contamination of pig carcasses and environments in three large-scale Italian slaughterhouses (identified as A-B-C) located in Northern Italy. Each slaughterhouse was visited six times. Five carcasses and three different sites of the slaughterhouse (before and during slaughter) were sampled on each sampling day. A single pooled caecal sample was taken on each sampling day. A total of 90 carcasses, 108 environmental samples and 18 caecal samples were collected. Samples from pig carcasses and slaughterhouse environment were analyzed for Total Viable Count (TVC), Enterobacteriaceae count (EBC) and Salmonella. The caecal contents were examined for Salmonella. Carcasses from slaughterhouse A presented the greatest TVC and EBC mean log value, whereas environmental samples collected during slaughter activities from slaughterhouse C showed the greatest TVC and EBC mean log value. As far as the environmental samples collected before slaughter activities are concerned, an average up to 6 log10 CFU/cm2 TVC in two slaughter plants (A and C) and 5 log10 CFU/cm2 TVC in one slaughter plant (B) was detected. Salmonella was recovered in two slaughterhouses (A and B). Four different Salmonella serotypes were detected in the positive samples (11). Within serotype S. Rissen and S. Typhimurium monophasic-variant isolates, two PFGE patterns were identified. The findings in this survey suggest that carcass contamination is influenced by the slaughterhouse plant and this could be a result of differences in line speed. The results of environmental sampling have not shown an association with the slaughterhouse plant

    We analyze muscular dystrophy recorded by sEMG and use standard methodologies and nonlinear chaotic methods here including the RQA. We reach sufficient evidence that the sEMG signal con-tains a large chaotic component. We have estimated the correlation dimension (fractal measure), the largest Lyapunov exponent, the LZ complexity and the %Rec and %Det of the RQA demon-strating that such indexes are able to detect the presence of repetitive hidden patterns in sEMG which, in turn, senses the level of MU synchronization within the muscle. The results give also an interesting methodological indication in the sense that it evidences the manner in which nonlin-ear methods and RQA must be arranged and applied in clinical routine in order to obtain results of clinical interest.     Analysis of Brain-Neuromuscular Synchronization and Coupling Strength in Muscular Dystrophy after NPT Treatment

    We perform an analysis of brain-neuromuscular synchronization and coupling strength in muscular dystrophy before and after NPT treatment. In order to estimate with accuracy the level of brain synchronization, we introduce and use the method of the cross GMI that was elaborated by Pompe. The finality is to account for the nonlinear chaotic dynamic contributions that regulate the dynamics. We find that this method is excellent and it may be applied in such case of sEMG and EEG study as well as at a general neurological level. By it we arrive to quantify the coupling strength and synchronization first between two muscular sections (left and right trapezes) and after between Brain activity, as recorded by the EEG, and the trapezes. We compare the results before and after the NPT treatment and we find that the improvement obtained following such treatment is very consistent. We have now in progress other studies relating in particular the application of such new methodologies to other more serious pathologies as the HSP (Hereditary Spastic Paraplegia)

    Use Of External Intrathecal Infusion Pumps In The Management Of Septic Complications: A Case Report

    Spasticity is a motor disorder with an increased muscle tone, typically associated with spasms, weakness and lack of coordination. It is an invalidating and debilitating pathology, characterized by pain, limited autonomy in activities of daily living, development of severe lesions. Spasticity can be adequately treated with physiotherapy, muscle relaxants drugs or topical treatment with botulinic toxin type A. Intrathecal baclofen therapy is very effective in the treatment of severe and generalized spasticity. Sometimes, soft tissues adjacent to the implant intrathecal infusion become infected; removing intrathecal infusion and systemic antibiotic therapy are best solution for clinical cure. However, removing intrathecal baclofen therapy could increase muscle spasticity with enhancement of pain and clonus that can worsen quality of life.
