    Large-Scale Modeling and DR Control of Electric Water Heaters With Energy Star and CTA-2045 Control Types in Distribution Power Systems

    The paper proposes a generalized energy storage (GES) model for battery energy storage systems (BESS), electric water heaters (EWH) and heating, ventilation, and air-conditioning (HVAC) systems to enable demand response control complying to Energy Star and CTA-2045 standards. The demand response control has been implemented in the DER integration testbed, which was originally developed by EPRI, to demonstrate that the “energy content” and “energy take” for BESS and EWH with mixing valve technology are comparable for typical residential ratings. A distribution power system was modeled using the modified IEEE 123-bus feeder system, measured residential loads, and EWH power simulated based on realistic hot water draws from CBECC-Res software. The demand response control, which complies to CTA-2045 standards was implemented to the EWHs considering the energy take values. Results demonstrate that the EWHs can be controlled to postpone the peak power at the distribution system level and provide a large amount of energy storage, while maintaining system robustness. The impact on occupant comfort was also analyzed

    The paper proposes a generalized energy storage (GES) model for battery energy storage systems (BESS), electric water heaters (EWH) and heating, ventilation, and air-conditioning (HVAC) systems. The analogies, including state of charge versus water temperature differential, are identified and explained, and models-in-the-loop (MIL) are introduced, which are compatible with the Energy Star and CTA-2045 general specifications and command types. Emphasis is placed on the proposed EWH model as it needfully fulfills a gap in present literature. The corresponding MIL has been implemented in the DER integration testbed, which was originally developed by EPRI, and satisfactorily validated against experimental results. A case study is included to illustrate that the daily “energy content” and “energy take” for BESS and EWH with mixing valve technology are comparable for typical residential ratings. The BESS, which requires more initial investment, has advantages in terms of flexibility for contributing to grid services, which are illustrated through a combined simulation and experimental study based on data collected from a field demonstration site with four smart homes

    Improving the quality of care of children in community clinics: an intervention and evaluation in Bangladesh.

    Community health care providers (CHCPs) in 40 rural community clinics of Comilla district, Bangladesh, were trained using a newly developed case-management job aid based on the World Health Organization Integrated Management of Childhood Illness and a communication guide.To assess 1) the change in knowledge of the CHCPs after training; 2) the absolute quality of care provided by the CHCPs (determined as the proportion of children aged <5 years [under-fives] correctly diagnosed, treated and referred); and 3) the consultation behaviour of the CHCPs.Change in knowledge was assessed by tests pre-and post-training. The quality of care was determined by reassessments at the clinic exit by a medical officer, without a baseline comparison. Consultation behaviour was assessed through direct observation. The study was performed during 2014-2015.The mean standard knowledge score of the CH-CPs increased from 19 to 25 (P < 0.001). Of 1490 under-fives examined, 91% were correctly diagnosed, 86% were correctly treated and 99.5% received a correct referral decision. The CHCPs performed well on most of the measures of good communication, although one third did not explain the diagnosis and treatment to patients.The training was effective in changing knowledge. The CHCPs applied the knowledge gained and provided good quality care. Following these results, the Bangladesh Ministry of Health and Family Welfare has scaled up the training nationwide. The lessons learnt should be useful for other countries

    Annual Biomass Production, Chemical Composition and In- sacco Degradability of Different Cultivars of Moringa oleifera

    Types of plant cultivars and seasons often affect production and productivity of fodder biomass and nutritional quality to animals. Selection of suitable cultivars and better understanding of year round biomass production are indispensable for improving quality feed supply to animals. Black Seed Moringa (BSM-L) and White Seed Moringa (WSM), the two local cultivars and Black Seed Moringa (BSM-T) cultivar of Thailand origin of Moringa oleifera were cultivated in the fodder research field of the Bangladesh Livestock Research Institute (BLRI) during the period of 19 August 2014 to 23 December 2015. An agronomical trial was conducted to determine the biomass yield of the three cultivars in different seasons of a year under common agronomical practices. The effect of the cultivars on the daily relative growth rate (RGR), chemical composition and in-sacco dry matter (DM) degradability were also evaluated. The cultivar response to biomass production performances, chemical composition and nutritional values were analyzed in an ANOVA of a Randomized Block Design (RBD), while the differences in the rate and extent of the DM degradability in-sacco determined using three rumen cannulated bulls were analyzed in an ANOVA of 3x3 Latin Square Design. The annual biomass yield of BSM-L tops (114.5 t/ha fresh; 22.7 t/ha DM) was significantly higher than that of WSM (29.0 t/ha fresh; 5.80 t/ha DM) or BSM-T (83.5 t/ha fresh; 16.0 t/ha DM). No significant difference in chemical composition (224.9, 222.4 & 223.8 g.kg-1 DM of crude protein (CP), respectively, and 450.9, 455.3 & 435.4 g.kg-1 DM of neutral detergent fiber, respectively) or nutritional value (47.4, 46.7 & 45.3% of potential, and 62.8, 64.2 and 63.6% of effective degradability of dry matter) was found for the cultivars. BSM-L had a significantly higher survivability (97.2%), prune number per plant (3.50) and RGR (15.6 mg DM/day) than WSM (25.0%, 2.30 & 4.20 mg DM/day) or BSM-T (55.6%, 3.10 & 10.8 mg DM/day) respectively. The hot and dry, and hot and humid climate having a Heat Index (HI) range of 25o to 35o F and monthly total rainfall of 130 mm to 332 mm were suitable for cultivation of all the Moringa cultivars. It was concluded that considering biomass production and its quality in terms of chemical composition and nutritional values, Black Seed Moringa (Moringa oleifera) may be cultivated as a plant fodder crop for the production of feed for ruminant animals

    Community engagement interventions for communicable disease control in low- and lower- middle-income countries : evidence from a review of systematic reviews

    BACKGROUND: Community engagement (CE) interventions include a range of approaches to involve communities in the improvement of their health and wellbeing. Working with communities defined by location or some other shared interest, these interventions may be important in assisting equity and reach of communicable disease control (CDC) in low and lower-middle income countries (LLMIC). We conducted an umbrella review to identify approaches to CE in communicable disease control, effectiveness of these approaches, mechanisms and factors influencing success. METHODS: We included systematic reviews that: i) focussed on CE interventions; ii) involved adult community members; iii) included outcomes relevant to communicable diseases in LLMIC; iv) were written in English. Quantitative results were extracted and synthesised narratively. A qualitative synthesis process enabled identification of mechanisms of effect and influencing factors. We followed guidance from the Joanna Briggs Institute, assessed quality with the DARE tool and reported according to standard systematic review methodology. RESULTS: Thirteen systematic reviews of medium-to-high quality were identified between June and July 2017. Reviews covered the following outcomes: HIV and STIs (6); malaria (2); TB (1); child and maternal health (3) and mixed (1). Approaches included: CE through peer education and community health workers, community empowerment interventions and more general community participation or mobilisation. Techniques included sensitisation with the community and involvement in the identification of resources, intervention development and delivery. Evidence of effectiveness of CE on health outcomes was mixed and quality of primary studies variable. We found: i) significantly reduced neonatal mortality following women's participatory learning and action groups; ii) significant reductions in HIV and other STIs with empowerment and mobilisation interventions with marginalised groups; iii) significant reductions in malaria incidence or prevalence in a small number of primary studies; iv) significant reductions in infant diarrhoea following community health worker interventions. Mechanisms of impact commonly occurred through social and behavioural processes, particularly: changing social norms, increasing social cohesion and social capacity. Factors influencing effectiveness of CE interventions included extent of population coverage, shared leadership and community control over outcomes. CONCLUSION: Community engagement interventions may be effective in supporting CDC in LLMIC. Careful design of CE interventions appropriate to context, disease and community is vital

    Patient Feedback Systems at the Primary Level of Health Care Centres in Bangladesh: A Mixed Methods Study

    This paper documents, and reflects on key strengths and weaknesses of, existing patient feedback management systems at primary health care in Bangladesh and proposes key implications for future policy and practice. A mixed-method study was conducted in two Upazila (sub-district) Health Complexes (UHC) within one district in Bangladesh. It reports qualitative data from thematic analysis of in-depth interviews (n = 15) with key stakeholders; non-participant observations of feedback environment at UHCs; document review; and a stakeholder workshop. Patient feedback data from publicly available web portals were also analyzed. Multiple parallel patient feedback systems exist at health facilities. Key strengths across all systems included common goals of ensuring accountability and patient voice and high-level commitment. Common weaknesses included lack of documented processes, limited awareness of available channels among patients and a lack of documented actions following feedback. The findings helped to provide a few implications for future policy and practice on patient feedback management

    Sour Ageusia in Two Individuals Implicates Ion Channels of the ASIC and PKD Families in Human Sour Taste Perception at the Anterior Tongue

    BACKGROUND:The perception of sour taste in humans is incompletely understood at the receptor cell level. We report here on two patients with an acquired sour ageusia. Each patient was unresponsive to sour stimuli, but both showed normal responses to bitter, sweet, and salty stimuli. METHODS AND FINDINGS:Lingual fungiform papillae, containing taste cells, were obtained by biopsy from the two patients, and from three sour-normal individuals, and analyzed by RT-PCR. The following transcripts were undetectable in the patients, even after 50 cycles of amplification, but readily detectable in the sour-normal subjects: acid sensing ion channels (ASICs) 1a, 1beta, 2a, 2b, and 3; and polycystic kidney disease (PKD) channels PKD1L3 and PKD2L1. Patients and sour-normals expressed the taste-related phospholipase C-beta2, the delta-subunit of epithelial sodium channel (ENaC) and the bitter receptor T2R14, as well as beta-actin. Genomic analysis of one patient, using buccal tissue, did not show absence of the genes for ASIC1a and PKD2L1. Immunohistochemistry of fungiform papillae from sour-normal subjects revealed labeling of taste bud cells by antibodies to ASICs 1a and 1beta, PKD2L1, phospholipase C-beta2, and delta-ENaC. An antibody to PKD1L3 labeled tissue outside taste bud cells. CONCLUSIONS:These data suggest a role for ASICs and PKDs in human sour perception. This is the first report of sour ageusia in humans, and the very existence of such individuals ("natural knockouts") suggests a cell lineage for sour that is independent of the other taste modalities

    Understanding demand for, and feasibility of, centre-based child-care for poor urban households: a mixed methods study in Dhaka, Bangladesh

    Background: Centre-based child-care has potential to provide multiple health and development benefits to children, families and societies. With rapid urbanisation, increasing numbers of low-income women work with reduced support from extended family, leaving a child-care vacuum in many low- and middle-income countries. We aimed to understand perceptions of, and demand for, centre-based child-care in Dhaka, Bangladesh among poor, urban households, and test the feasibility of delivering sustainable centre-based child-care. Methods: We used sequential mixed methods including a household survey (n = 222) and qualitative interviews with care-givers (n = 16), community leaders (n = 5) and policy-makers (n = 5). We co-produced and piloted a centre-based child-care model over ten-months, documenting implementation. A co-design focus group with mothers, parents’ meetings, and qualitative interviews with child-care centre users (n = 5), non-users (n = 3), ex-users (n = 3) and staff (2) were used to refine the model and identify implementation issues. Results: We found 24% (95% CI: 16,37%) of care-givers reported turning-down paid work due to lack of child-care and 84% (95% CI:74, 91%) reported wishing to use centre-based child-care and were willing to pay up to 283 Takka (~$3.30) per month. Adjusted odds of reported need for child-care among slum households were 3.8 times those of non-slum households (95% CI: 1.4, 10). Implementation highlighted that poor households needed free child-care with food provided, presenting feasibility challenges. Meta-inference across quantitative and qualitative findings identified the impact of the urban environment on child-care through long working hours, low social capital and fears for child safety. These influences interacted with religious and social norms resulting in caution in using centre-based child-care despite evident need. Conclusion: Sustainable provision of centre-based care that focuses on early childhood development requires subsidy and careful design sensitive to the working lives of poor families, particularly women and must respond to the dynamics of the urban environment and community values. We recommend increased research and policy focus on the evaluation and scale-up of quality centre-based child-care, emphasising early-childhood development, to support low-income working families in urban areas

    Rethinking health systems in the context of urbanisation: challenges from four rapidly urbanising low-income and middle-income countries

    The world is now predominantly urban; rapid and uncontrolled urbanisation continues across low-income and middle-income countries (LMICs). Health systems are struggling to respond to the challenges that urbanisation brings. While better-off urbanites can reap the benefits from the ‘urban advantage’, the poorest, particularly slum dwellers and the homeless, frequently experience worse health outcomes than their rural counterparts. In this position paper, we analyse the challenges urbanisation presents to health systems by drawing on examples from four LMICs: Nigeria, Ghana, Nepal and Bangladesh. Key challenges include: responding to the rising tide of non-communicable diseases and to the wider determinants of health, strengthening urban health governance to enable multisectoral responses, provision of accessible, quality primary healthcare and prevention from a plurality of providers. We consider how these challenges necessitate a rethink of our conceptualisation of health systems. We propose an urban health systems model that focuses on: multisectoral approaches that look beyond the health sector to act on the determinants of health; accountability to, and engagement with, urban residents through participatory decision making; and responses that recognise the plurality of health service providers. Within this model, we explicitly recognise the role of data and evidence to act as glue holding together this complex system and allowing incremental progress in equitable improvement in the health of urban populations

    Fluorofluorophores: Fluorescent Fluorous Chemical Tools Spanning the Visible Spectrum

    “Fluoro” refers to both fluorescent and fluorinated compounds. Despite the shared prefix, there are very few fluorescent molecules that are soluble in perfluorinated solvents. This paucity is surprising, given that optical microscopy is a ubiquitous technique throughout the physical sciences and the orthogonality of fluorous materials is a commonly exploited strategy in synthetic chemistry, materials science, and chemical biology. We have addressed this shortage by synthesizing a panel of “fluorofluorophores,” fluorescent molecules containing high weight percent fluorine with optical properties spanning the visible spectrum.
