Study protocol of a randomized controlled trial to assess safety of teleconsultation compared with face-to-face consultation: the ECASeT study

BackgroundThe use of remote consultation modalities has exponentially grown in the past few years, particularly since the onset of the COVID-19 pandemic. Although a huge body of the literature has described the use of phone (tele) and video consultations, very few of the studies correspond to randomized controlled trials, and none of them has assessed the safety of these consultation modalities as the primary objective. The primary objective of this trial was to assess the safety of remote consultations (both video and teleconsultation) in the follow-up of patients in the hospital setting.MethodsMulticenter, randomized controlled trial being conducted in four centers of an administrative healthcare area in Catalonia (North-East Spain). Participants will be screened from all individuals, irrespective of age and sex, who require follow-up in outpatient consultations of any of the departments involved in the study. Eligibility criteria have been established based on the local guidelines for screening patients for remote consultation. Participants will be randomly allocated into one of the two study arms: conventional face-to-face consultation (control) and remote consultation, either teleconsultation or video consultation (intervention). Routine follow-up visits will be scheduled at a frequency determined by the physician based on the diagnostic and therapy of the baseline disease (the one triggering enrollment). The primary outcome will be the number of adverse reactions and complications related to the baseline disease. Secondary outcomes will include non-scheduled visits and hospitalizations, as well as usability features of remote consultations. All data will either be recorded in an electronic clinical report form or retrieved from local electronic health records. Based on the complications and adverse reaction rates reported in the literature, we established a target sample size of 1068 participants per arm. Recruitment started in May 2022 and is expected to end in May 2024.DiscussionThe scarcity of precedents on the assessment of remote consultation modalities using randomized controlled designs challenges making design decisions, including recruitment, selection criteria, and outcome definition, which are discussed in the manuscript.Trial registrationNCT05094180. Curs 2016-2017L’objectiu principal d’aquest treball és aplicar els coneixements adquirits durant la carrera, i no hi ha millor manera de dur la teoria a la pràctica, que la d’haver de realitzar una traducció treballada i justificada d’un tema en concret, en aquest cas un testament, i tenir les eines necessàries per raonar i defensar les decisions preses durant el procés de traducció. El tema que he escollit forma part del vast àmbit de la traducció jurídica, la qual es caracteritza per un llenguatge molt ric i unes expressions concretes. El meu treball de fi de grau es basa en la traducció del testament de Paul Newman, un filantrop, actor i director de cinema estatunidenc. Tot seguit trobareu una breu biografia, l’explicació i definició de l’estructura del dret successori nord-americà i el català i una comparativa d’ambdós; el text original en anglès i la seva traducció al català, un comentari traductològic, les conclusions i les fitxes terminològiques que inclouen vocabulari específic i paraules clau de la traducció.The main aim of this project is to put into practice the knowledge we have acquired through the Translation and Interpreting degree. Creating a well crafted and justified translation of a specific topic—in this case a Last Will and Testament—and knowing how to justify every decision that has been made throughout the translating process is the best way of demonstrating our well-grounded learning. The selected topic falls under the broad field of Legal Translation, characterized by a rich language and highly precise expressions. My final project is based on the translation of Paul Newman’s Last Will and Testament, a North American philanthropist, actor and film director. Marco conceptual para el monitoreo de servicios de agua potable y saneamiento en entornos metropolitanos desde una perspectiva de derechos humanos. Aplicación a Lima (Perú).

La presente investigación utiliza el enfoque del Derecho Humano al Agua y al Saneamiento para analizar el sector de los servicios de agua y saneamiento y realizar un diagnóstico en entornos periurbanos, y en específico para Lima Metropolitana. Actualmente, para el caso de Lima existe una brecha importante entre los conectados y los no conectados al servicio de agua y saneamiento y se observa una mayor dificultad en cerrar esa brecha por la baja planificación urbana del gobierno y la llegada continua de nueva población. Se desarrolla una propuesta de marco conceptual que mide el nivel de servicio para los titulares de derecho según el enfoque de las cinco dimensiones del DHAS para la zona periurbana de Lima. Para ello, se lleva a cabo una investigación cualitativa en la zona de Collique, un pueblo joven de más de 100 mil habitantes ubicado en el distrito de Comas situado en el Cono Norte de Lima. La información es recopilada a partir de talleres participativos y de entrevistas detalladas que permiten conocer la realidad y las necesidades de la población para formular la propuesta de marco conceptual. Paralelamente se llevan a cabo entrevistas bilaterales a diferentes actores del sector que posibilitan la comprensión del sector y ayudan a realizar un pre-análisis de viabilidad para la propuesta de marco presentada
