    Outsourcing a High Speed Internet Access Project: An Information Technology Class Case Study in Three Parts

    In early 2004, the Hilton Hotels Corporation (HHC) required that all of its hotels (both owned and franchised) install highspeed Internet access (HSIA) in all of their rooms by June 2004. This case focuses on how one of its franchise properties located on the northern gulf coast of Florida (the Hilton Sandestin Beach Golf Resort & Spa—hereinafter referred to as the HSB) responded to this mandate. The (A) part of the HSB case includes the industry situation for the HSB in 2004 plus the details surrounding the initial phase of the HSIA project. Also presented in this part are descriptions of how and why the HSB management made the decision to use a wireless (versus wired) solution as well as to outsource both the installation of the HSIA wireless hardware and the customer support aspects of the HSIA project to what turned out to be a less than experienced vendor. The (B) part of this case describes the situation at HSB after the high speed internet system was installed and made operational, as well as the various problems that the system and its lack of customer support created for the hotel. Part (C) details the actions of the new HSB General Manager who arrived in late 2004 after the initial HSIA implementation and how he resolved the issues that had been created by his predecessor

    Dosing strategies for switching from oral risperidone to paliperidone palmitate: Effects on clinical outcomes.

    IntroductionThere are currently no guidelines for switching patients from oral risperidone to paliperidone palmitate (Invega Sustenna®). Furthermore, the paliperidone long-acting injectable (LAI) package insert does not recommend bridging with oral antipsychotics, which may result in inadequate serum concentrations in patients on ≥4 mg/d risperidone.MethodsThis study evaluated the effects of suboptimal dosing and bridging in patients switched from oral risperidone to paliperidone LAI on hospitalization days, emergency department (ED)/mental health urgent care visits, and no-shows/cancellations to mental health appointments. Patients were categorized into optimal or suboptimal dosing based on their loading and maintenance paliperidone doses. Patients on risperidone ≥4 mg/d were categorized as bridged if they received risperidone for ≥7 days after the first paliperidone injection.ResultsThere were no significant differences in outcomes between optimally and suboptimally dosed patients. There were statistically significant reductions in hospitalization days in patients who were bridged compared with patients who were not bridged. There were statistically significant reductions in hospitalization days and ED/mental health urgent care visits after switching to paliperidone LAI.DiscussionThe results of this study indicate that bridging patients who are on ≥4 mg/d risperidone, when converting to paliperidone LAI, is associated with reductions in hospitalization days. However, more research is required to determine the optimal dose and duration of the bridge. The results also indicate that switching patients from oral risperidone to paliperidone LAI, even if the dose is suboptimal, is associated with reductions in hospitalization days and ED/mental health urgent care visits

    The remarkable rate of development of medical knowledge and pharmacology affects all medical specialties and in particular general practice, since the latter embraces various aspects of different medical fields. Certain areas of medical practice, such as Hormone Replacement Therapy (HRT), hypercholesterolaemia and drugs affecting the Renin Angiotensin System (RAS) feature in a vast amount of literature which is constantly being updated. The increasing awareness of the previously unknown effects of HRT on one side, and the widespread prevalence of cardiovascular disease in the case of hypercholesterolaemia and drugs affecting the RAS have contributed to this large amount of studies. On the other hand, certain areas of practice such as atopic eczema and anti-thrombotic therapy have been rather quiescent as far as developments are concerned, but revolutionary treatments have recently been introduced in both areas, namely the calcineurin antagonists and ximelagratan. The latter is a very promising drug which can replace warfarin, while the former are the first steroid-sparing medications which effectively control atopic eczema. Keeping abreast with recent developments is a further hurdle which the modern general practioner has to contend with in order to offer valid treatment options, and to be able to answer questions by increasingly well-informed patients.peer-reviewe

    Conducting Research with Community Groups

    Nurse scientists are increasingly recognizing the necessity of conducting research with community groups to effectively address complex health problems and successfully translate scientific advancements into the community. While several barriers to conducting research with community groups exist, community based participatory research (CBPR) has the potential to mitigate these barriers. CBPR has been employed in programs of research that respond in culturally sensitive ways to identify community needs and thereby address current health disparities. This manuscript presents case studies that demonstrate how CBPR principles guided the development of: (a) a healthy body weight program for urban, underserved African-American women, (b) a reproductive health educational intervention for urban, low-income, underserved, ethnically diverse women, and (c) a pilot anxiety/depression intervention for urban, low-income, underserved, ethnically diverse women. These case studies illustrate the potential of CBPR as an orientation to research that can be employed effectively in non-research intensive academic environments
