    Evolution and stoichiometry of heterogeneous processing in the Antarctic stratosphere

    Simultaneous in situ measurements of HCl and ClO have been made for the first time in the southern hemisphere, allowing a systematic study of the processes governing chlorine activation between 15 and 20 km in the 1994 Antarctic winter. Data for several other gases (O_3, NO, NO_y, OH, HO_2, N_(2)O, CH_4, CO, H_(2)O, CFCs), particulates, and meteorological parameters were collected from the ER-2 aircraft out of New Zealand as part of the 1994 Airborne Southern Hemisphere Ozone Experiment/Measurements of Atmospheric Effects of Stratospheric Aircraft (ASHOE/MAESA) campaign. Observations from the ER-2 in the fall (April–May), prior to polar night, show that chlorine activation begins with 60–75% of inorganic chlorine as HCl. By midwinter (July–August), near-total removal of HCl is observed. The wintertime loss of HCl in air recently exposed to extreme temperatures is found to be correlated with high levels of reactive chlorine (ClO and its dimer, Cl_(2)O_2) in the linear fashion expected from the stoichiometry of the heterogeneous reaction of hydrochloric acid with chlorine nitrate on polar stratospheric clouds (PSCs): HCl + ClONO_2 → Cl_2 + HNO_3. To constrain the role of different heterogeneous reactions and PSC types, we have used a photochemical trajectory model which includes heterogeneous sulfate and PSC chemistry. Model calculations of the evolution of reactive gases are compared with the in situ observations. In addition, simultaneous measurements of OH and HO_2 are used as a diagnostic for the occurrence of the heterogeneous reaction HOCl + HCl → Cl_2 + H_(2)O, which contributes to suppressed levels of HO_x inside the vortex. It is shown that the amount of chlorine activation is not strongly dependent on the composition of PSCs. However, HO_x levels exhibit different signatures depending on the type of heterogeneous surfaces that affected chlorine activation. Furthermore, this analysis implies that in the edge region of the Antarctic vortex, the observed near-total removal of HCl can result from latitudinal excursions of air parcels in and out of sunlight during the winter, which photochemically resupply HOCl and ClONO_2 as oxidation partners for HCl

This paper reviews the epidemiological and clinical data on how medications (non-steroidal anti-inflammatory drugs, estrogens and antibiotics), lifestyle factors (smoking, psychological stress, diet and air pollution) may precipitate clinical relapses and recurrence. Potential biological mechanisms include: increasing thrombotic tendency, imbalances in prostaglandin synthesis, alterations in the composition of gut microbiota, and mucosal damage causing increased permeability

    Improving PBL in Empowering Meta cognitive Skill of Students

    Abstract Objective: Problem-Based Learning (PBL) is a potential constructivist learning strategy that empowers students’ Meta cognitive skill. PBL focuses on problem, involves thinking activity to solve problems, and correlates to cognitive function of students. Methods: The implementation of PBL reveals various benefits, but there are also some weaknesses in this learning strategy. Thus, it is necessary to implement a certain learning strategy that can cover the PBL weaknesses, such as Reading, Questioning, and Answering (RQA) learning strategy. RQA is a new learning strategy developed based on a fact that almost all students do not read the next lecture materials, causing failure of learning strategy planned and finally the students’ comprehension becomes low. RQA is also potential to empower students’ Meta cognitive skill. Findings: The integration of RQA and PBL learning strategy is called PBL-RQA learning strategy. This study was a quasi-experimental study designed to compare the effect of PBL, RQA, and PBL-RQA learning strategies on the students’ Meta cognitive skill of Faculty of Mathematics and Science, State University of Makassar. Application: The results of the study showed that the potency of PBL learning strategy in empowering the students’ Meta cognitive skill has been increased by integrating it to RQA learning strategy. The meta cognitive skill mean score of the students taught by PBL-RQA learning strategy was 21% higher than that of the students taught by PBL and 26.9% higher than that of the students taught by RQA learning strategy.     Idiopathic pulmonary fibrosis in the United States: time to diagnosis and treatment

    Abstract Objective Create a timeline of diagnosis and treatment for IPF in the US. Design, setting, and participants A retrospective analysis was performed in collaboration with the OptumLabs Data Warehouse using an administrative claims database of Medicare Fee for Service beneficiaries. Adults 50 and over with IPF were included (2014 to 2019). Exposure To focus on IPF, the following diagnoses were excluded: post-inflammatory fibrosis, hypersensitivity pneumonitis, rheumatoid arthritis, sarcoidosis, scleroderma, and connective tissue disease. Main outcomes and measures Data were collected from periods prior, during, and following initial clinical diagnosis of IPF. This included prior respiratory diagnoses, number of respiratory-related hospitalizations, anti-fibrotic and oxygen use, and survival. Results A total of 44,891 with IPF were identified. The most common diagnoses prior to diagnosis of IPF were upper respiratory infections (47%), acute bronchitis (13%), other respiratory disease (10%), chronic obstructive pulmonary disease and bronchiectasis (7%), and pneumonia (6%). The average time to a diagnosis of IPF was 2.7 years after initial respiratory diagnosis. Half of patients had two or more respiratory-related hospitalizations prior to IPF diagnosis. Also, 37% of patients were prescribed oxygen prior to diagnosis of IPF. These observations suggest delayed diagnosis. We also observed only 10.4% were treated with anti-fibrotics. Overall survival declined each year after diagnosis with median survival of 2.80 years. Conclusions and relevance Our retrospective cohort demonstrates that IPF is often diagnosed late, usually preceded by other respiratory diagnoses and hospitalizations. Use of available therapies is low and outcomes remain poor
