    SummaryMonosomy 7 and interstitial deletion of 7q (−7/7q−) are well-recognized nonrandom chromosomal abnormalities frequently found among patients with myelodysplastic syndromes (MDSs) and myeloid leukemias. We previously identified candidate myeloid tumor suppressor genes (SAMD9, SAMD9-like = SAMD9L, and Miki) in the 7q21.3 subband. We established SAMD9L-deficient mice and found that SAMD9L+/− mice as well as SAMD9L−/− mice develop myeloid diseases resembling human diseases associated with −7/7q−. SAMD9L-deficient hematopoietic stem cells showed enhanced colony formation potential and in vivo reconstitution ability. SAMD9L localizes in early endosomes. SAMD9L-deficient cells showed delays in homotypic endosome fusion, resulting in persistence of ligand-bound cytokine receptors. These findings suggest that haploinsufficiency of SAMD9L and/or SAMD9 gene(s) contributes to myeloid transformation

    Inappropriate implantable cardioverter defibrillator shocks—incidence, effect, and implications for driver licensing

    PurposePatients with implantable cardioverter defibrillators (ICDs) have an ongoing risk of sudden incapacitation that may cause traffic accidents. However, there are limited data on the magnitude of this risk after inappropriate ICD therapies. We studied the rate of syncope associated with inappropriate ICD therapies to provide a scientific basis for formulating driving restrictions.MethodsInappropriate ICD therapy event data between 1997 and 2014 from 50 Japanese institutions were analyzed retrospectively. The annual risk of harm (RH) to others posed by a driver with an ICD was calculated for private driving habits. We used a commonly employed annual RH to others of 5 in 100,000 (0.005%) as an acceptable risk threshold.ResultsOf the 4089 patients, 772 inappropriate ICD therapies occurred in 417 patients (age 61 ± 15 years, 74% male, and 65% secondary prevention). Patients experiencing inappropriate therapies had a mean number of 1.8 ± 1.5 therapy episodes during a median follow-up period of 3.9 years. No significant differences were found in the age, sex, or number of inappropriate therapies between patients receiving ICDs for primary or secondary prevention. Only three patients (0.7%) experienced syncope associated with inappropriate therapies. The maximum annual RH to others after the first therapy in primary and secondary prevention patients was calculated to be 0.11 in 100,000 and 0.12 in 100,000, respectively.ConclusionsWe found that the annual RH from driving was far below the commonly cited acceptable risk threshold. Our data provide useful information to supplement current recommendations on driving restrictions in ICD patients with private driving habits

    研究論文Original Articles 【目的】本研究の目的は,急性期病院退院後の軽症脳梗塞患者の身体活動量に影響する退院時の要因を明らかにすることであった.【方法】研究デザインは,前向きコホート研究である.軽症脳梗塞患者を対象に身体活動量に関連する要因を退院時に測定し,退院2 週後と3 か月後の身体活動量をオムロン活動量計Active style Pro HJA-750C を用いて測定し,退院後の身体活動量に影響する退院時の要因を検討した.【結果】重回帰分析の結果,退院3 か月後の身体活動量に影響する要因として6 分間歩行距離(β= 0.675,p < 0.001)のみが抽出された(自由度調整済みR2 = 0.432,p < 0.001).【結論】急性期病院退院後の軽症脳梗塞患者の身体活動量には,退院時の6 分間歩行距離が影響することが明らかになった.退院3 か月後の身体活動量の向上には,退院時の6 分間歩行距離を高めることが重要であることが示唆された.【Object】The purpose of this study was to clarify the factors at the time of discharge that affect the physical activity level in patients with minor ischemic stroke after discharge from the acute care hospital.【Methods】This is a prospective cohort study. We measured factors related to physical activity level at the time of discharge for patients with minor ischemic stroke, and used OMRON\u27s Active Style Pro HJA-750C to measure physical activity level 2 weeks and 3 months after discharge. We used multiple regression analysis to examine factors at discharge that affect physical activity level after discharge.【Result】As a result of multiple regression analysis, only 6-minute walking distance ( β = 0.675, p <0.001) was extracted as a factor affecting physical activity level 3 months after discharge.【Conclusion】It was clarified that the 6-minute walking distance at the time of discharge affected the physical activity level of patients with minor ischemic stroke after discharge from the acute hospital. These results suggest that it is important to increase the walking distance for 6 minutes at the time of discharge in order to improve physical activity level 3 months after discharge

    Emerging concepts in biomarker discovery; The US-Japan workshop on immunological molecular markers in oncology

    Supported by the Office of International Affairs, National Cancer Institute (NCI), the "US-Japan Workshop on Immunological Biomarkers in Oncology" was held in March 2009. The workshop was related to a task force launched by the International Society for the Biological Therapy of Cancer (iSBTc) and the United States Food and Drug Administration (FDA) to identify strategies for biomarker discovery and validation in the field of biotherapy. The effort will culminate on October 28th 2009 in the "iSBTc-FDA-NCI Workshop on Prognostic and Predictive Immunologic Biomarkers in Cancer", which will be held in Washington DC in association with the Annual Meeting. The purposes of the US-Japan workshop were a) to discuss novel approaches to enhance the discovery of predictive and/or prognostic markers in cancer immunotherapy; b) to define the state of the science in biomarker discovery and validation. The participation of Japanese and US scientists provided the opportunity to identify shared or discordant themes across the distinct immune genetic background and the diverse prevalence of disease between the two Nations
