    DIFFERENT TECHNICAL STRATEGIES AND BIOMECHANICAL ASPECTS OF DOUBLE POLlNG IN ELITE CROSS-COUNTRY SKIING

    The purpose of this study was to analyse double poling (DP) regarding biomechanical performance determinants and different strategies. Eleven elite cross-country skiers performed DP at 85% of their maximal DP velocity (V85%) during roller skiing (treadmill; 1° inclination) while pole forces and selected joint angles were recorded. A 2D video evaluation categorised skiers into two different DP strategy groups. Strategy A group showed higher elbow (p < 0.01) and hip flexion angular velocities, smaller minimum elbow, knee and hip angles, higher peak pole force, shorter time to peak pole force and a longer relati.ve recovery time (p < 0.05), variables to which V85% was significantly correlated (p < 0.05). DP strategy A provides an effective model for technique and specific strength training while its physiological economy has to be further investigated

    Radiation to supraclavicular and internal mammary lymph nodes in breast cancer increases the risk of stroke

    The aim of this study was to assess whether adjuvant treatment of breast cancer (BC) affects the risk of stroke, and to explore radiation targets and fraction doses regarding risk and location of stroke. In a Swedish BC cohort diagnosed during 1970–2003, we carried out a nested case–control study of stroke after BC, with relevant details extracted from medical records. The odds ratio (OR) for radiotherapy (RT) vs that of no RT did not differ between cases and controls (OR=0.85; confidence interval, CI=0.6–1.3). Radiotherapy to internal mammary chain (IMC) and supraclavicular (SCL) lymph nodes vs that of no RT was associated with a higher, although not statistically significant, risk of stroke (OR=1.3; CI=0.8–2.2). In a pooled analysis, RT to IMC and SCL vs the pooled group of no RT and RT to breast/chest wall/axilla (but not IMC and SCL), showed a significant increase of stroke (OR=1.8; CI=1.1–2.8). There were no associations between cancer laterality, targets of RT, and location of stroke. The radiation targets, IMC and SCL, showed a statistically significant trend for an increased risk of stroke with daily fraction dose. Our finding of a target-specific increased risk of stroke and a dose-response relationship for daily fraction dose, indicate that there may be a causal link between RT to the IMC and SCL and risk of stroke

    A Qualitative Study of Healers and Their Clients in Germany

    Background. The aim of this study was to investigate the nature of the relationships between healers and their clients in Germany. Methods. An interdisciplinary research team performed semistructured interviews with healers and clients and participatory observation of healing sessions. All interviews were digitally recorded, transcribed, and analyzed using content analysis. Results. Fifteen healers and sixteen clients were included. The healer-client relationship was described as a profound and unique experience, which brought forth interpersonal and spiritual connections. The healers were seen as role models for healing to occur and support for being connected spiritually. The clients had to be open-minded and responsible. The importance of the healers’ empathy was emphasized. Discussion. The relationship between healer and client can be seen as a triangular connection between client, healer, and a transcendent source which is not the case in typical patient- doctor relationships. The spiritual connection is also said to enhance the empathetic understanding of the healer. The personality and a partner-like attitude of the healer supported the client in giving a more positive meaning to his life, in reconnecting to his spirituality, and in taking responsibility. Future studies should address the role of spirituality in health care and the development of enduring healer-client relationships

    A scientific perspective on reducing ski-snow friction to improve performance in Olympic Cross-Country Skiing, the Biathlon and Nordic Combined

    Of the medals awarded at the 2022 Winter Olympics in Beijing, 24% were for events involving cross-country skiing, the biathlon and Nordic combined. Although much research has focused on physiological and biomechanical characteristics that determine success in these sports, considerably less is yet known about the resistive forces. Here, we specifically describe what is presently known about ski-snow friction, one of the major resistive forces. Today, elite ski races take place on natural and/or machine-made snow. Prior to each race, several pairs of skis with different grinding and waxing of the base are tested against one another with respect to key parameters, such as how rapidly and for how long the ski glides, which is dependent on ski-snow friction. This friction arises from a combination of factors, including compaction, plowing, adhesion, viscous drag, and water bridging, as well as contaminants and dirt on the surface of and within the snow. In this context the stiffness of the ski, shape of its camber, and material composition and topography of the base exert a major influence. An understanding of the interactions between these factors, in combination with information concerning the temperature and humidity of both the air and snow, as well as the nature of the snow, provides a basis for designing specific strategies to minimize ski-snow friction. In conclusion, although performance on "narrow skis" has improved considerably in recent decades, future insights into how best to reduce ski-snow friction offer great promise for even further advances

    Three Different Types of Galaxy Alignment within Dark Matter Halos

    Using a large galaxy group catalogue based on the Sloan Digital Sky Survey Data Release 4 we measure three different types of intrinsic galaxy alignment within groups: halo alignment between the orientation of the brightest group galaxies (BGG) and the distribution of its satellite galaxies, radial alignment between the orientation of a satellite galaxy and the direction towards its BGG, and direct alignment between the orientation of the BGG and that of its satellites. In agreement with previous studies we find that satellite galaxies are preferentially located along the major axis. In addition, on scales r < 0.7 Rvir we find that red satellites are preferentially aligned radially with the direction to the BGG. The orientations of blue satellites, however, are perfectly consistent with being isotropic. Finally, on scales r < 0.1 \Rvir, we find a weak but significant indication for direct alignment between satellites and BGGs.     Glomerular sclerosis in kidneys with congenital nephrotic syndrome (NPHS1)

    Congenital nephrotic syndrome of the Finnish type (NPHS1) is a rare genetic disease caused by mutations in the NPHS1 gene encoding a major podocyte slit-diaphragm protein, nephrin. Patients with NPHS1 have severe nephrotic syndrome from birth and develop renal fibrosis in early childhood. In this work, we studied the development of glomerular sclerosis in kidneys removed from 4- to 44-month-old NPHS1 patients. The pathological lesions and expression of glomerular cell markers were studied in nephrectomized NPHS1 and control kidneys using light and electron microscopy and immunohistochemistry. An analysis of 1528 glomeruli from 20 patients revealed progressive mesangial sclerosis and capillary obliteration. Although few inflammatory cells were detected in the mesangial area, paraglomerular inflammation and fibrosis was common. The podocytes showed severe ultrastructural changes and hypertrophy with the upregulation of cyclins A and D1. Podocyte proliferation, however, was rare. Apoptosis was hardly detected and the expression of antiapoptotic B-cell lymphoma-2 and proapoptotic p53 were comparable to controls. Moderate amounts of podocytes were secreted into the urine of NPHS1 patients. Shrinkage of the glomerular tuft was common, whereas occlusion of tubular opening or protrusion of the glomerular tuft into subepithelial space or through the Bowman's capsule were not detected. The results indicate that, in NPHS1 kidneys, the damaged podocytes induce progressive mesangial expansion and capillary obliteration. Podocyte depletion, glomerular tuft adhesion, and misdirected filtration, however, seem to play a minor role in the nephron destruction

    Hip fracture, mortality risk, and cause of death over two decades

    Summary: Men and women with hip fracture have higher short-term mortality. This study investigated mortality risk over two decades post-fracture; excess mortality remained high in women up to 10 years and in men up to 20 years. Cardiovascular disease (CVD) and pneumonia were leading causes of death with a long-term doubling of risk. Introduction: Hip fractures are associated with increased mortality, particularly short term. In this study with a two-decade follow-up, we examined mortality and cause of death compared to the background population. Methods: We followed 1013 hip fracture patients and 2026 matched community controls for 22 years. Mortality, excess mortality, and cause of death were analyzed and stratified for age and sex. Hazard ratio (HR) was estimated by Cox regression. A competing risk model was fitted to estimate HR for common causes of death (CVD, cancer, pneumonia) in the short and long term (>1 year). Results: For both sexes and at all ages, mortality was higher in hip fracture patients across the observation period with men losing most life years (p <0.001). Mortality risk was higher for up to 15 years (women (risk ratio (RR) 1.9 [95 % confidence interval (CI) 1.7–2.1]); men (RR 2.8 [2.2–3.5])) and until end of follow-up ((RR 1.8 [1.6–2.0]); (RR 2.7 [2.1–3.3])).     Spatial and kinematic alignments between central and satellite halos

    Based on a cosmological N-body simulation we analyze spatial and kinematic alignments of satellite halos within six times the virial radius of group size host halos (Rvir). We measure three different types of spatial alignment: halo alignment between the orientation of the group central substructure (GCS) and the distribution of its satellites, radial alignment between the orientation of a satellite and the direction towards its GCS, and direct alignment between the orientation of the GCS and that of its satellites. In analogy we use the directions of satellite velocities and probe three further types of alignment: the radial velocity alignment between the satellite velocity and connecting line between satellite and GCS, the halo velocity alignment between the orientation of the GCS and satellite velocities and the auto velocity alignment between the satellites orientations and their velocities. We find that satellites are preferentially located along the major axis of the GCS within at least 6 Rvir (the range probed here). Furthermore, satellites preferentially point towards the GCS. The most pronounced signal is detected on small scales but a detectable signal extends out to 6 Rvir. The direct alignment signal is weaker, however a systematic trend is visible at distances < 2 Rvir. All velocity alignments are highly significant on small scales. Our results suggest that the halo alignment reflects the filamentary large scale structure which extends far beyond the virial radii of the groups. In contrast, the main contribution to the radial alignment arises from the adjustment of the satellite orientations in the group tidal field.
