    Incorporating usability evaluation into iterative development of an online platform to support research participation in Parkinson\u27s disease: A mixed methods protocol

Introduction Many people with Parkinson\u27s (PwP) are not given the opportunity or do not have adequate access to participate in clinical research. To address this, we have codeveloped with users an online platform that connects PwP to clinical studies in their local area. It enables site staff to communicate with potential participants and aims to increase the participation of the Parkinson\u27s community in research. This protocol outlines the mixed methods study protocol for the usability testing of the platform. Methods and analysis We will seek user input to finalise the platform\u27s design, which will then be deployed in a limited launch for beta testing. The beta version will be used as a recruitment tool for up to three studies with multiple UK sites. Usability data will be collected from the three intended user groups: PwP, care partners acting on their behalf and site study coordinators. Usability questionnaires and website analytics will be used to capture user experience quantitatively, and a purposive sample of users will be invited to provide further feedback via semistructured interviews. Quantitative data will be analysed using descriptive statistics, and a thematic analysis undertaken for interview data. Data from this study will inform future platform iterations. Ethics and dissemination Ethical approval was obtained from the University of Plymouth (3291; 3 May 2022).     Temperatures in Excess of Critical Thresholds Threaten Nestling Growth and Survival in A Rapidly-Warming Arid Savanna: A Study of Common Fiscals

    Frequency, duration, and intensity of hot-weather events are all predicted to increase with climate warming. Despite this, mechanisms by which temperature increases affect individual fitness and drive population-level changes are poorly understood. We investigated the link between daily maximum air temperature (t max ) and breeding success of Kalahari common fiscals ( Lanius collaris ) in terms of the daily effect on nestling body-mass gain, and the cumulative effect on size and age of fledglings. High t max reduced mass gain of younger, but not older nestlings and average nestling-period t max did not affect fledgling size. Instead, the frequency with which t max exceeded critical thresholds (t crit s) significantly reduced fledging body mass (t crit  = 33°C) and tarsus length (t crit  = 37°C), as well as delaying fledging (t crit  = 35°C). Nest failure risk was 4.2% per day therefore delays reduced fledging probability. Smaller size at fledging often correlates with reduced lifetime fitness and might also underlie documented adult body-size reductions in desert birds in relation to climate warming. Temperature thresholds above which organisms incur fitness costs are probably common, as physiological responses to temperature are non-linear. Understanding the shape of the relationship between temperature and fitness has implications for our ability to predict species’ responses to climate change

    Host specificity and co-speciation in avian haemosporidia in the Western Cape, South Africa

    Host and pathogen ecology are often closely linked, with evolutionary processes often leading to the development of host specificity traits in some pathogens. Host specificity may range from ‘generalist’, where pathogens infect any available competent host; to ‘specialist’, where pathogens repeatedly infect specific host species or families. Avian malaria ecology in the region remains largely unexplored, despite the presence of vulnerable endemic avian species. We analysed the expression of host specificity in avian haemosporidia, by applying a previously developed host specificity index to lineages isolated from wetland passerines in the Western Cape, South Africa. Parasite lineages were isolated using PCR and identified when possible using matching lineages deposited in GenBank and in MalAvi. Parasitic clades were constructed from phylogenetic trees consisting of Plasmodium and Haemoproteus lineages. Isolated lineages matched some strains of Plasmodium relictum , P. elongatum , Haemoproteus sylvae and H. lanii . Plasmodium lineages infected a wide range of hosts from several avian families in a generalist pattern of infection. Plasmodium spp. also exhibited an infection trend according to host abundance rather than host species. By contrast, Haemoproteus lineages were typically restricted to one or two host species or families, and displayed higher host fidelity than Plasmodium spp. The findings confirm that a range of host specificity traits are exhibited by avian haemosporidia in the region. The traits show the potential to not only impact infection prevalence within specific host species, but also to affect patterns of infection at the community level

    Impaired automatic but intact volitional inhibition in primary tic disorders

    The defining character of tics is that they can be transiently suppressed by volitional effort of will, and at a behavioural level this has led to the concept that tics result from a failure of inhibition. However, this logic conflates the mechanism responsible for the production of tics with that used in suppressing them. Volitional inhibition of motor output could be increased to prevent the tic from reaching the threshold for expression, although this has been extensively investigated with conflicting results. Alternatively, automatic inhibition could prevent the initial excitation of the striatal tic focus—a hypothesis we have previously introduced. To reconcile these competing hypotheses, we examined different types of motor inhibition in a group of 19 patients with primary tic disorders and 15 healthy volunteers. We probed proactive and reactive inhibition using the conditional stop-signal task, and applied transcranial magnetic stimulation to the motor cortex, to assess movement preparation and execution. We assessed automatic motor inhibition with the masked priming task. We found that volitional movement preparation, execution and inhibition (proactive and reactive) were not impaired in tic disorders. We speculate that these mechanisms are recruited during volitional tic suppression, and that they prevent expression of the tic by inhibiting the nascent excitation released by the tic generator. In contrast, automatic inhibition was abnormal/impaired in patients with tic disorders. In the masked priming task, positive and negative compatibility effects were found for healthy controls, whereas patients with tics exhibited strong positive compatibility effects, but no negative compatibility effect indicative of impaired automatic inhibition. Patients also made more errors on the masked priming task than healthy control subjects and the types of errors were consistent with impaired automatic inhibition. Errors associated with impaired automatic inhibition were positively correlated with tic severity. We conclude that voluntary movement preparation/generation and volitional inhibition are normal in tic disorders, whereas automatic inhibition is impaired—a deficit that correlated with tic severity and thus may constitute a potential mechanism by which tics are generated

    What’s sex got to do with it? A family-based investigation of growing up heterosexual during the twentieth century

    This paper explores findings from a cross-generational study of the making of heterosexual relationships in East Yorkshire, which has interviewed women and men within extended families. Using a feminist perspective, it examines the relationship between heterosexuality and adulthood, focussing on sexual attraction, courtship, first kisses, first love and first sex, as mediated within family relationships, and at different historical moments. In this way, the contemporary experiences of young people growing up are compared and contrasted with those of mid-lifers and older adults who formed heterosexual relationships within the context of the changing social and sexual mores of the 1960s/1970s, and the upheavals of World War Two

    Senior medical students as assistants in medicine in COVID-19 crisis : a realist evaluation protocol

    Introduction The assistant in medicine is a new and paid role for final-year medical students that has been established in New South Wales, Australia, as part of the surge workforce management response to the COVID-19 pandemic. Eligibility requires the applicant to be a final-year medical student in an Australian Medical Council-accredited university and registered with the Australian Health Practitioner Regulation Agency. While there are roles with some similarities to the assistant in medicine role, such as assistantships (the UK) and physician assistants adopted internationally, this is completely new in Australia. Little is known about the functionality and success factors of this role within the health practitioner landscape, particularly within the context of the COVID-19 pandemic. Given the complexity of this role, a realist approach to evaluation has been undertaken as described in this protocol, which sets out a study design spanning from August 2020 to June 2021. Methods and analysis The intention of conducting a realist review is to identify the circumstances and mechanisms that determine the outcomes of the assistant in medicine intervention. We will start by developing an initial programme theory to explore the potential function of the assistant in medicine role through realist syntheses of critically appraised summaries of existing literature using relevant databases and journals. Other data sources such as interviews and surveys with key stakeholders will contribute to the refinements of the programme theory. Using this method, we will develop a set of hypotheses on how and why the Australian assistants in medicine intervention might â € work' to achieve a variety of outcomes based on examples of related international interventions. These hypotheses will be tested against the qualitative and quantitative evidence gathered from all relevant stakeholders. Ethics and dissemination Ethics approval for the larger study was obtained from the Western Sydney Local Health District (2020/ETH01745). The findings of this review will provide useful information for hospital managers, academics and policymakers, who can apply the findings in their context when deciding how to implement and support the introduction of assistants in medicine into the health system. We will publish our findings in reports to policymakers, peer-reviewed journals and international conferences
