    Lidar measurement of stratospheric aerosol at Syowa Station, Antarctica

    Lidar measurement on Antarctic aerosols were made during the Antarctic Middle Atmosphere (AMA) period, 1983 to 1985, at Syowa Station. Topics measured are winter enhancement aerosol layer and volcanic effect of El Chichon on the Antarctic stratosphere aerosols. The large depolarization ratio (maximum value was about 0.8) seems to support sublimation growth of ice crystals. The lidar measurements showed a meaningful time lag between aerosol content increase and depolarization ratio increase. Considering the balloon observations made in early winter, it was speculated that an increase in large particle number concentration also contributed to the winter enhancement. The El Chichon cloud spread to the Antarctic region by the beginning of 1983. The temporal change of integrated backscatter coefficient shows a clear decay pattern, although strong winter enhancement superposes

    The mesospheric sodium layer has been observed at Syowa Station in Antarctica during the wintering period of the 26th Japanese Antarctic Research Expedition. A lidar observation of the polar middle atmosphere at the station has been performed as a part of the Middle Atmosphere Program since 1983. At first stratospheric aerosols have been observed by the system based on a ruby laser. In 1985 a new transmitting system consisting of a tunable dye laser was added to that system to observe the sodium layer, too. The characteristics of the lidar system are given. The results of the mesospheric sodium layer observed on 42 nights during the period from March to October, 1985 are also given

    Successful Late Management of Esophageal Perforation with T-Tube Drainage

    The late management of spontaneous esophageal perforation is the most challenging problem for the surgeon. In this paper, we present a case in whom a spontaneous esophageal perforation was successfully treated by T-tube drainage after unsuccessful conservative treatment. The patient, a 68-year-old male, was admitted to the hospital with sudden upper abdominal pain. After 2 days, esophageal perforation was diagnosed, and conservative management was begun. Thereafter, the subcutaneous emphysema disappeared, and the patient's temperature decreased. However, on day 13, the patient's temperature spiked above 38°C, and computed tomography showed a mediastinal abscess. An emergency left thoracotomy with laparotomy was performed. Since a 2-cm longitudinal perforation with severe inflammatory reactions was observed, the T-tube drainage method was performed. The patient was discharged without postoperative complications and has not experienced any gastrointestinal symptoms, such as gastroesophageal reflux or dysphagia. In conclusion, the T-tube drainage method appears to be a simple and effective method for the late management of esophageal perforation with severe inflammatory reaction

    Binocular visual function after staged implantation of extended-depth-of-focus intraocular lens targeting emmetropia and -0.5 diopter: A prospective comparison

    The prospective comparative case series aimed to evaluate the binocular uncorrected visual acuities (BUCVAs) after staged implantations of extended-depth-of-focus intraocular lenses (EDOF IOLs) targeting emmetropia and -0.5 diopter (D). Diffractive EDOF IOLs with an add power of +1.75 D were implanted in the first eyes targeting emmetropia or -0.5 D according to the patients' preferences, then the targets for the second eyes were determined 1 week or longer after the implantation. IOL powers were determined with the SRK/T formula. Consequently, the subjects were divided into 3 groups: those with emmetropia targeted bilaterally (group EE, 22 patients), those with -0.5 D targeted bilaterally (group MM, 21 patients), and those with monovision of emmetropia and -0.5 D (group EM, 21 patients). Manifest refraction spherical equivalent (MRSE), BUCVA from 0.3 to 5 meters, spectacle use, and questionnaire regarding photic symptoms and patient satisfaction were assessed 3 months postoperatively. No significant differences were seen in the mean BUCVAs at any distance (P > 0.23), spectacle use (P = 0.13), or photic symptoms and patient satisfaction (P>0.65). When the EE and MM groups were assigned based on the MRSE, the EE group was better at 5 m (P = 0.005) while the MM group at 0.5 m (P = 0.031). The effect of different targeted refractions was not identified due to insufficient accuracy in the use of the SRK/T power calculation
