    Las Asociaciones civiles de mujeres en México han tomado auge a través de los años ya que desempeñan una función vital al promover en la toma de decisiones la incorporación de las opiniones de las personas involucradas, la presente investigación muestra un diagnóstico de necesidades de capacitación para una Asociación Civil de Mujeres del Sur del Estado de Sonora, ya que este tipo de organizaciones ha ido aumentando en el país y cada vez su participación es más impactante a nivel nacional e internacional, se aplicó un instrumento hibrido tomado de INEGI a 12 grupos de agremiadas, dando como resultado datos interesantes acerca del manejo de sus finanzas, de la escolaridad de las participantes, de sus movimientos financieros y sobre todo áreas de oportunidad para ser capacitadas

    The purpose of the study was to design a mobile application for planning tourist destinations based on experiences, which allows generating the profile of the tourist or visitor to provide the appropriate options to their preferences and generate a space for interaction for service providers, taking as case study the community of Masiaca located in the south of the State of Sonora. For this, it was necessary to investigate the current situation to identify the areas of focus of the problem, similar cases and existing solutions, to know the main tourist applications that exist in the market, as well as the characteristics they have, an analysis was developed needs and perception of the different actors to create user stories, in order that the requirements of each of the actors are clearly identified, applying an instrument to know the perception of the importance of considering requirements or not, which consisted In a quantitative, cross-sectional descriptive study, where a non-probabilistic intentional sample was worked at convenience, considering 30 service providers and 30 tourists and visitors, for this purpose a questionnaire with a Likert scale was designed for tourists and visitors consisting of 13 items and another for service providers of 11 items, valid given by five experts on the subject. Finally, the solution and the high-fidelity prototype of the mobile application were designed.El propósito del estudio fue diseñar una aplicación móvil para la planificación de destinos turísticos basados en experiencias, que permita generar el perfil del turista o visitante para proporcionar las opciones adecuadas a sus preferencias y generar un espacio de interacción para los prestadores de servicios, tomando como caso de estudio la comunidad de Masiaca ubicada en el sur del Estado de Sonora. Para ello, fue necesario investigar la situación actual para identificar las áreas de enfoque de la problemática, casos similares y soluciones existentes, para conocer las principales aplicaciones turísticas que existen en el mercado, así como las características con las que cuentan, se desarrolló un análisis de necesidades y percepción de los diferentes actores para crear las historias de usuario, con la finalidad de que los requerimientos de cada uno de los actores estén claramente identificados, aplicando un instrumento para conocer la percepción de la importancia de considerar o no requerimientos, que consistió en un estudio cuantitativo, transversal descriptivo, donde se trabajó con una muestra no probabilística intencional a conveniencia considerando a 30 prestadores de servicio y a 30 turistas y visitantes, para ello se diseñó un cuestionario con escala de Likert dirigido para turistas y visitantes que consta de 13 ítems y otro para los prestadores de servicios de 11 ítems, validado por cinco expertos en el tema. Finalmente, se diseñó la solución y el prototipo de alta fidelidad de la aplicación móvil

    Según el diario el Mundo, por primera vez los estudiantes universitarios españoles que tienen pensado crear un negocio (26,8%) supera a los que se ven trabajando en la Administración Pública el (25%), ese fue el resultados de las 8600 encuestas realizadas por GAD3 para el estudio encargado por Educa 2020, donde Ingeniería y Arquitectura son las ramas con mayores porcentajes de alumnado que quieren crear una empresa. Con estos datos, se pretende trabajar en esta línea para estimular al alumnado en la generación de ideas de negocio y dar soporte para llevarlas a cabo. Además, a través del trabajo coordinado con el Vicerrectorado de Coordinación Institucional e Infraestructuras se intentará acercar al alumnado cómo es el proceso de licitación de obras, servicios o equipamiento en una Administración Pública como es la UCO, mostrando la transparencia del proceso. En la experiencia piloto del anterior curso académico, el alumnado valoró muy positivamente la asistencia a un acto público de apertura de ofertas económicas de un proceso de licitación real desarrollado por la UCO, donde se mostraron participativos y vieron de primera mano cómo todo el proceso es transparente y sigue rigurosamente la Ley de Contratos del Sector Público.According to the El Mundo newspaper, for the first time, Spanish university students who plan to create a business (26.8%) exceed those who are working in the Public Administration (25%), that was the result of the 8600 surveys conducted by GAD3 for the study commissioned by Educa 2020, where Engineering and Architecture are the branches with the highest percentages of students that want to start a business. With these data, it is recommended to work in this line to stimulate the student in the generation of business ideas and give support to carry them out. In addition, through coordinated work with the Office of the Vice Chancellor for Institutional Coordination and Infrastructures, students are approached about the process of bidding for works, services or equipment in a Public Administration such as the UCO, showing the transparency of the process. In the pilot experience of the previous academic course, the students positively valued attendance at a public act of opening economic offers of a real bidding process developed by the UCO, where they participated and showed how the whole transparent process rigorously follows the Law of Public Sector Contracts

    Effectiveness and Safety of the Sequential Use of a Second and Third Anti-TNF Agent in Patients With Inflammatory Bowel Disease: Results From the Eneida Registry

    Background: The effectiveness of the switch to another anti-tumor necrosis factor (anti-TNF) agent is not known. The aim of this study was to analyze the effectiveness and safety of treatment with a second and third anti-TNF drug after intolerance to or failure of a previous anti-TNF agent in inflammatory bowel disease (IBD) patients. Methods: We included patients diagnosed with IBD from the ENEIDA registry who received another anti-TNF after intolerance to or failure of a prior anti-TNF agent. Results: A total of 1122 patients were included. In the short term, remission was achieved in 55% of the patients with the second anti-TNF. The incidence of loss of response was 19% per patient-year with the second anti-TNF. Combination therapy (hazard ratio [HR], 2.4; 95% confidence interval [CI], 1.8-3; P < 0.0001) and ulcerative colitis vs Crohn's disease (HR, 1.6; 95% CI, 1.1-2.1; P = 0.005) were associated with a higher probability of loss of response. Fifteen percent of the patients had adverse events, and 10% had to discontinue the second anti-TNF. Of the 71 patients who received a third anti-TNF, 55% achieved remission. The incidence of loss of response was 22% per patient-year with a third anti-TNF. Adverse events occurred in 7 patients (11%), but only 1 stopped the drug. Conclusions: Approximately half of the patients who received a second anti-TNF achieved remission; nevertheless, a significant proportion of them subsequently lost response. Combination therapy and type of IBD were associated with loss of response. Remission was achieved in almost 50% of patients who received a third anti-TNF; nevertheless, a significant proportion of them subsequently lost response

    Long-Term Real-World Effectiveness and Safety of Ustekinumab in Crohn’s Disease Patients: The SUSTAIN Study

    Background Large real-world-evidence studies are required to confirm the durability of response, effectiveness, and safety of ustekinumab in Crohn’s disease (CD) patients in real-world clinical practice. Methods A retrospective, multicentre study was conducted in Spain in patients with active CD who had received ≥1 intravenous dose of ustekinumab for ≥6 months. Primary outcome was ustekinumab retention rate; secondary outcomes were to identify predictive factors for drug retention, short-term remission (week 16), loss of response and predictive factors for short-term efficacy and loss of response, and ustekinumab safety. Results A total of 463 patients were included. Mean baseline Harvey-Bradshaw Index was 8.4. A total of 447 (96.5%) patients had received prior biologic therapy, 141 (30.5%) of whom had received ≥3 agents. In addition, 35.2% received concomitant immunosuppressants, and 47.1% had ≥1 abdominal surgery. At week 16, 56% had remission, 70% had response, and 26.1% required dose escalation or intensification; of these, 24.8% did not subsequently reduce dose. After a median follow-up of 15 months, 356 (77%) patients continued treatment. The incidence rate of ustekinumab discontinuation was 18% per patient-year of follow-up. Previous intestinal surgery and concomitant steroid treatment were associated with higher risk of ustekinumab discontinuation, while a maintenance schedule every 12 weeks had a lower risk; neither concomitant immunosuppressants nor the number of previous biologics were associated with ustekinumab discontinuation risk. Fifty adverse events were reported in 39 (8.4%) patients; 4 of them were severe (2 infections, 1 malignancy, and 1 fever). Conclusions Ustekinumab is effective and safe as short- and long-term treatment in a refractory cohort of CD patients in real-world clinical practice

    Using Interpretable Machine Learning to Identify Baseline Predictive Factors of Remission and Drug Durability in Crohn’s Disease Patients on Ustekinumab

    Ustekinumab has shown efficacy in Crohn's Disease (CD) patients. To identify patient profiles of those who benefit the most from this treatment would help to position this drug in the therapeutic paradigm of CD and generate hypotheses for future trials. The objective of this analysis was to determine whether baseline patient characteristics are predictive of remission and the drug durability of ustekinumab, and whether its positioning with respect to prior use of biologics has a significant effect after correcting for disease severity and phenotype at baseline using interpretable machine learning. Patients' data from SUSTAIN, a retrospective multicenter single-arm cohort study, were used. Disease phenotype, baseline laboratory data, and prior treatment characteristics were documented. Clinical remission was defined as the Harvey Bradshaw Index <= 4 and was tracked longitudinally. Drug durability was defined as the time until a patient discontinued treatment. A total of 439 participants from 60 centers were included and a total of 20 baseline covariates considered. Less exposure to previous biologics had a positive effect on remission, even after controlling for baseline disease severity using a non-linear, additive, multivariable model. Additionally, age, body mass index, and fecal calprotectin at baseline were found to be statistically significant as independent negative risk factors for both remission and drug survival, with further risk factors identified for remission

    Risk Factors for COVID-19 in Inflammatory Bowel Disease: A National, ENEIDA-Based Case–Control Study (COVID-19-EII)

    (1) Scant information is available concerning the characteristics that may favour the acquisition of COVID-19 in patients with inflammatory bowel disease (IBD). Therefore, the aim of this study was to assess these differences between infected and noninfected patients with IBD. (2) This nationwide case-control study evaluated patients with inflammatory bowel disease with COVID-19 (cases) and without COVID-19 (controls) during the period March-July 2020 included in the ENEIDA of GETECCU. (3) A total of 496 cases and 964 controls from 73 Spanish centres were included. No differences were found in the basal characteristics between cases and controls. Cases had higher comorbidity Charlson scores (24% vs. 19%; p = 0.02) and occupational risk (28% vs. 10.5%; p < 0.0001) more frequently than did controls. Lockdown was the only protective measure against COVID-19 (50% vs. 70%; p < 0.0001). No differences were found in the use of systemic steroids, immunosuppressants or biologics between cases and controls. Cases were more often treated with 5-aminosalicylates (42% vs. 34%; p = 0.003). Having a moderate Charlson score (OR: 2.7; 95%CI: 1.3-5.9), occupational risk (OR: 2.9; 95%CI: 1.8-4.4) and the use of 5-aminosalicylates (OR: 1.7; 95%CI: 1.2-2.5) were factors for COVID-19. The strict lockdown was the only protective factor (OR: 0.1; 95%CI: 0.09-0.2). (4) Comorbidities and occupational exposure are the most relevant factors for COVID-19 in patients with IBD. The risk of COVID-19 seems not to be increased by immunosuppressants or biologics, with a potential effect of 5-aminosalicylates, which should be investigated further and interpreted with caution

    Diverse Large HIV-1 Non-subtype B Clusters Are Spreading Among Men Who Have Sex With Men in Spain

    In Western Europe, the HIV-1 epidemic among men who have sex with men (MSM) is dominated by subtype B. However, recently, other genetic forms have been reported to circulate in this population, as evidenced by their grouping in clusters predominantly comprising European individuals. Here we describe four large HIV-1 non-subtype B clusters spreading among MSM in Spain. Samples were collected in 9 regions. A pol fragment was amplified from plasma RNA or blood-extracted DNA. Phylogenetic analyses were performed via maximum likelihood, including database sequences of the same genetic forms as the identified clusters. Times and locations of the most recent common ancestors (MRCA) of clusters were estimated with a Bayesian method. Five large non-subtype B clusters associated with MSM were identified. The largest one, of F1 subtype, was reported previously. The other four were of CRF02_AG (CRF02_1; n = 115) and subtypes A1 (A1_1; n = 66), F1 (F1_3; n = 36), and C (C_7; n = 17). Most individuals belonging to them had been diagnosed of HIV-1 infection in the last 10 years. Each cluster comprised viruses from 3 to 8 Spanish regions and also comprised or was related to viruses from other countries: CRF02_1 comprised a Japanese subcluster and viruses from 8 other countries from Western Europe, Asia, and South America; A1_1 comprised viruses from Portugal, United Kingom, and United States, and was related to the A1 strain circulating in Greece, Albania and Cyprus; F1_3 was related to viruses from Romania; and C_7 comprised viruses from Portugal and was related to a virus from Mozambique. A subcluster within CRF02_1 was associated with heterosexual transmission. Near full-length genomes of each cluster were of uniform genetic form. Times of MRCAs of CRF02_1, A1_1, F1_3, and C_7 were estimated around 1986, 1989, 2013, and 1983, respectively. MRCA locations for CRF02_1 and A1_1 were uncertain (however initial expansions in Spain in Madrid and Vigo, respectively, were estimated) and were most probable in Bilbao, Spain, for F1_3 and Portugal for C_7. These results show that the HIV-1 epidemic among MSM in Spain is becoming increasingly diverse through the expansion of diverse non-subtype B clusters, comprising or related to viruses circulating in other countries
