    The Lazy Bureaucrat Scheduling Problem

    We introduce a new class of scheduling problems in which the optimization is performed by the worker (single ``machine'') who performs the tasks. A typical worker's objective is to minimize the amount of work he does (he is ``lazy''), or more generally, to schedule as inefficiently (in some sense) as possible. The worker is subject to the constraint that he must be busy when there is work that he can do; we make this notion precise both in the preemptive and nonpreemptive settings. The resulting class of ``perverse'' scheduling problems, which we denote ``Lazy Bureaucrat Problems,'' gives rise to a rich set of new questions that explore the distinction between maximization and minimization in computing optimal schedules.Comment: 19 pages, 2 figures, Latex.     Development of a Clinician Report Measure to Assess Psychotherapy for Depression in Usual Care Settings

    Although mental health policy initiatives have called for quality improvement in depression care, practical tools to describe the quality of psychotherapy for depression are not available. We developed a clinician-report measure of adherence to three types of psychotherapy for depression—cognitive behavioral therapy, interpersonal therapy, and psychodynamic therapy. A total of 727 clinicians from a large, national managed behavioral health care organization responded to a mail survey. The measure demonstrated good psychometric properties, including appropriate item-scale correlations, internal consistency reliability, and a three-factor structure. Our results suggest that this questionnaire may be a promising approach to describing psychotherapy for depression in usual care

    Serum Neurofilament Light, Glial Fibrillary Acidic Protein and Tau Are Possible Serum Biomarkers for Activity of Brain Metastases and Gliomas

    BACKGROUND: Primary central nervous system (CNS) tumors and brain metastases (BMs) are major causes of morbidity and mortality, accompanied by low survival rates. Efforts to early discovery of CNS malignancies are critical. However, to date, there are no biomarkers approved for detection of cancer activity in the brain. Blood levels of neurofilament light (NfL) and tau, as well as glial fibrillary acidic protein (GFAp), show promise as biomarkers for brain injury in previous studies. Therefore, we performed a cross-sectional study to investigate correlations of those biomarkers with CNS activity of gliomas and BMs. METHODS: Serum samples of 36 participants of a single centered institution were tested for NfL, GFAp and tau with Simoa immunoassay, and correlated with clinical and radiological data. RESULTS: fL and GFAp levels were significantly associated with the state of intracranial disease (analysis of variance (ANOVA), P_{sNfL} = 0.03; ANOVA, P_{GFAp} = 0.03). Although statistically significant (P = 0.04), differences in concentrations were not clinically meaningful for tau levels. Serum NfL (sNfL) and GFAp concentrations were higher in the group of patients with CNS tumors with disease in progression versus CNS with stable disease (P = 0.03 and P = 0.01, respectively). In addition, sNfL were higher in patients with metastatic solid tumors with known BMs than in those with metastatic tumors with no BM (P = 0.0004). CONCLUSION: sNfL and GFAp both apparently vary closely with presence and activity of gliomas and BMs. Further studies in larger populations are needed to expand these findings

    Scheduling Algorithms for Procrastinators

    This paper presents scheduling algorithms for procrastinators, where the speed that a procrastinator executes a job increases as the due date approaches. We give optimal off-line scheduling policies for linearly increasing speed functions. We then explain the computational/numerical issues involved in implementing this policy. We next explore the online setting, showing that there exist adversaries that force any online scheduling policy to miss due dates. This impossibility result motivates the problem of minimizing the maximum interval stretch of any job; the interval stretch of a job is the job's flow time divided by the job's due date minus release time. We show that several common scheduling strategies, including the "hit-the-highest-nail" strategy beloved by procrastinators, have arbitrarily large maximum interval stretch. Then we give the "thrashing" scheduling policy and show that it is a \Theta(1) approximation algorithm for the maximum interval stretch.Comment: 12 pages, 3 figure

    Dobutamine stress echocardiography: Prevalence of a nonischemic response in a low-risk population

    The problems of population referral bias in the calculation of specificity in diagnostic testing for coronary artery disease have been previously described. Previous studies investigating the sensitivity and specificity of dobutamine stress echocardiography (DSE) have been subject to pretest and posttest referral biases, largely as a result of the requirement for coronary arteriography. This study determines the normalcy rate for DSE by examining a population at statistically low risk for coronary artery disease. The probability of significant coronary artery disease was determined for 828 consecutive patients referred for DSE at the University of Michigan, and groups were identified with <10% and <5% probability of disease. Four of 72 patients (5.6%) with a normal baseline echocardiogram and a probability of coronary artery disease of <10%, and three of 38 (7.9%) with a probability of <5% were found to have an abnormal DSE, yielding normalcy rates of 94.4% and 92.1%, respectively. The area of abnormality involved the posterior circulation in three of four patients (75%). This study demonstrates that DSE has a normalcy rate of 92% to 94% and is an accurate test for excluding the presence of significant coronary artery disease.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/30826/1/0000488.pd

    Right atrial spontaneous contrast: echocardiographic and clinical features

    We describe the clinical and echocardiographic findings in eight patients with right atrial spontaneous echo contrast who were identified from 648 consecutive patients undergoing transesophageal echocardiography. Common findings in these patients were right atrial enlargement (8 patients), tricuspid regurgitation (7 patients), atrial fibrillation or flutter (6 patients), elevated right ventricular pressure (5 patients), moderate or severe mitral valve disease (5 patients), and right to left interatrial shunts (3 patients). Right heart catheterization in three patients showed markedly elevated right atrial, right ventricular, and pulmonary artery pressures. Two patients had thromboembolic events — one patient had recurrent pulmonary emboli, and another patient with an atrial septal aneurysm had recurrent transient ischemic attacks. Right atrial echo contrast is an uncommon finding at echocardiography that is associated with severe right heart dysfunction.     Lost in plasmids: next generation sequencing and the complex genome of the tick-borne pathogen Borrelia burgdorferi

    Background: Borrelia (B.) burgdorferi sensu lato, including the tick-transmitted agents of human Lyme borreliosis, have particularly complex genomes, consisting of a linear main chromosome and numerous linear and circular plasmids. The number and structure of plasmids is variable even in strains within a single genospecies. Genes on these plasmids are known to play essential roles in virulence and pathogenicity as well as host and vector associations. For this reason, it is essential to explore methods for rapid and reliable characterisation of molecular level changes on plasmids. In this study we used three strains: a low passage isolate of B. burgdorferi sensu stricto strain B31(-NRZ) and two closely related strains (PAli and PAbe) that were isolated from human patients. Sequences of these strains were compared to the previously sequenced reference strain B31 (available in GenBank) to obtain proof-of-principle information on the suitability of next generation sequencing (NGS) library construction and sequencing methods on the assembly of bacterial plasmids. We tested the effectiveness of different short read assemblers on Illumina sequences, and of long read generation methods on sequence data from Pacific Bioscience single-molecule real-time (SMRT) and nanopore (Oxford Nanopore Technologies) sequencing technology. Results: Inclusion of mate pair library reads improved the assembly in some plasmids as did prior enrichment of plasmids. While cp32 plasmids remained refractory to assembly using only short reads they were effectively assembled by long read sequencing methods. The long read SMRT and nanopore sequences came, however, at the cost of indels (insertions or deletions) appearing in an unpredictable manner. Using long and short read technologies together allowed us to show that the three B. burgdorferi s.s. strains investigated here, whilst having similar plasmid structures to each other (apart from fusion of cp32 plasmids), differed significantly from the reference strain B31-GB, especially in the case of cp32 plasmids. Conclusion: Short read methods are sufficient to assemble the main chromosome and many of the plasmids in B. burgdorferi. However, a combination of short and long read sequencing methods is essential for proper assembly of all plasmids including cp32 and thus, for gaining an understanding of host- or vector adaptations. An important conclusion from our work is that the evolution of Borrelia plasmids appears to be dynamic. This has important implications for the development of useful research strategies to monitor the risk of Lyme disease occurrence and how to medically manage it

    Kefir consumption does not alter plasma lipid levels or cholesterol fractional synthesis rates relative to milk in hyperlipidemic men: a randomized controlled trial [ISRCTN10820810]

    BACKGROUND: Fermented milk products have been shown to affect serum cholesterol concentrations in humans. Kefir, a fermented milk product, has been traditionally consumed for its potential health benefits but has to date not been studied for its hypocholesterolemic properties. METHODS: Thirteen healthy mildly hypercholesterolemic male subjects consumed a dairy supplement in randomized crossover trial for 2 periods of 4 wk each. Subjects were blinded to the dairy supplement consumed. Blood samples were collected at baseline and after 4 wk of supplementation for measurement of plasma total, low-density lipoprotein, and high-density lipoprotein cholesterol and triglyceride concentrations, as well as fatty acid profile and cholesterol synthesis rate. Fecal samples were collected at baseline and after 2 and 4 wk of supplementation for determination of fecal short chain fatty acid level and bacterial content. RESULTS: Kefir had no effect on total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol or triglyceride concentrations nor on cholesterol fractional synthesis rates after 4 wk of supplementation. No significant change on plasma fatty acid levels was observed with diet. However, both kefir and milk increased (p < 0.05) fecal isobutyric, isovaleric and propionic acids as well as the total amount of fecal short chain fatty acids. Kefir supplementation resulted in increased fecal bacterial content in the majority of the subjects. CONCLUSIONS: Since kefir consumption did not result in lowered plasma lipid concentrations, the results of this study do not support consumption of kefir as a cholesterol-lowering agent
