Evaluating the effectiveness of the Emergency Neurological Life Support educational framework in low-income countries.
BackgroundThe Emergency Neurological Life Support (ENLS) is an educational initiative designed to improve the acute management of neurological injuries. However, the applicability of the course in low-income countries in unknown. We evaluated the impact of the course on knowledge, decision-making skills and preparedness to manage neurological emergencies in a resource-limited country.MethodsA prospective cohort study design was implemented for the first ENLS course held in Asia. Knowledge and decision-making skills for neurological emergencies were assessed at baseline, post-course and at 6 months following course completion. To determine perceived knowledge and preparedness, data were collected using surveys administered immediately post-course and 6 months later.ResultsA total of 34 acute care physicians from across Nepal attended the course. Knowledge and decision-making skills significantly improved following the course (p=0.0008). Knowledge and decision-making skills remained significantly improved after 6 months, compared with before the course (p=0.02), with no significant loss of skills immediately following the course to the 6-month follow-up (p=0.16). At 6 months, the willingness to participate in continuing medical education activities remained evident, with 77% (10/13) of participants reporting a change in their clinical practice and decision-making, with the repeated use of ENLS protocols as the main driver of change.ConclusionsUsing the ENLS framework, neurocritical care education can be delivered in low-income countries to improve knowledge uptake, with evidence of knowledge retention up to 6 months
Surgeonsâ and Emergency Physiciansâ Perceptions of Trauma Management and Training
Objective: The study objective was to determine whether surgeons and emergency medicine physicians (EMPs) have differing opinions on trauma residency training and trauma management in clinical practice.Methods: A survey was mailed to 250 EMPs and 250 surgeons randomly selected.Results: Fifty percent of surgeons perceived that surgery exclusively managed trauma compared to 27% of EMPs. Surgeons were more likely to feel that only surgeons should manage trauma on presentation to the ED. However, only 60% of surgeons currently felt comfortable with caring for the trauma patient, compared to 84% of EMPs. Compared to EMPs, surgeons are less likely to feel that EMPs can initially manage the trauma patient (71% of surgeons vs. 92% of EMPs).Conclusion: EMPs are comfortable managing trauma while many surgeons do not feel comfortable with the complex trauma patient although the majority of surgeons responded that surgeons should manage the trauma.[WestJEM. 2009;10:144-149.
A possible phase dependent absorption feature in the transient X-ray pulsar SAX J2103.5+4545
We present an X-ray spectral and timing analysis of two observations
of the transient Be X-ray binary SAX J2103.5+4545 during its April 2016
outburst, which was characterized by the highest flux since 's launch.
These observations provide detailed hard X-ray spectra of this source during
its bright precursor flare and subsequent fainter regular outburst for the
first time. In this work, we model the phase-averaged spectra for these
observations with a negative and positive power law with an exponential cut-off
(NPEX) model and compare the pulse profiles at different flux states. We found
that the broad-band pulse profile changes from a three peaked pulse in the
first observation to a two peaked pulse in the second observation, and that
each of the pulse peaks has some energy dependence. We also perform pulse-phase
spectroscopy and fit phase-resolved spectra with NPEX to evaluate how spectral
parameters change with pulse phase. We find that while the continuum parameters
are mostly constant with pulse phase, a weak absorption feature at ~12 keV that
might, with further study, be classified as a cyclotron line, does show strong

Those Who Can, Do and They Teach Too: Faculty Clinical Productivity and Teaching
Objective: Academic emergency physicians (EPs) often feel that the demands of clinical productivity, income generation, and patient satisfaction conflict with educational objectives. The objective of this study was to explore whether the quality of faculty bedside teaching of residents correlated with high clinical productivity, measured by relative value units (RVUs). We also explored the strategies of high-performing faculty for optimal RVU generation and teaching performance.Methods: We performed a mixed method study using quantitative and qualitative methods to analyze the relationship between RVUs, patient satisfaction, and teaching performance. We examined the relationship between teaching performance ratings, patient satisfaction, and RVUs per hour using correlations. Following this initial analysis, we conducted semi-structured interviews with the eight faculty members who have the highest clinical (RVU) and educational productivity ratings to learn more about their strategies for success. Our Institutional Review Board approved this study.Results: We correlated resident evaluations of faculty with RVUs billed per hour. We conducted semi-structured interviews of faculty who led in both RVU productivity and resident evaluations. From these interviews, several themes emerged. When asked about how they excel in billing, most said that they pay attention to dictating a thorough chart on every patient and try to âstay busyâ throughout their entire shift. When asked how they excel at resident education, most leading faculty said that they try to find a âteaching momentâ and find small âclinical pearlsâ to pass along. Nevertheless, all eight leading faculty members believe that as the emphasis on billing productivity increases, resident and student education will suffer.Conclusion: Contrary to the opinion of some physicians, faculty can excel at both clinical productivity and resident education. This study found that highly efficient clinical productivity correlated with excellent resident teaching. This high level of performance did not appear to be at the expense of other important measures such as patient satisfaction or student teaching. [West J Emerg Med. 2011;12(2):254-257.
Ab-initio design of perovskite alloys with predetermined properties: The case of Pb(Sc_{0.5} Nb_{0.5})O_{3}
A first-principles derived approach is combined with the inverse Monte Carlo
technique to determine the atomic orderings leading to prefixed properties in
Pb(Sc_{0.5}Nb_{0.5})O_{3} perovskite alloy. We find that some arrangements
between Sc and Nb atoms result in drastic changes with respect to the
disordered material, including ground states of new symmetries, large
enhancement of electromechanical responses, and considerable shift of the Curie
temperature. We discuss the microscopic mechanisms responsible for these

Negotiating sexuality and masculinity in school sport: An autoethnography
This autoethnography explores challenging and ethically sensitive issues around sexual orientation, sexual identity and masculinity in the context of school sport. Through storytelling, I aim to show how sometimes ambiguous encounters with heterosexism, homophobia and hegemonic masculinity through sport problematise identity development for young same-sex attracted males. By foregrounding personal embodied experience, I respond to an absence of stories of gay and bisexual experiences among males in physical education and school sport, in an effort to reduce a continuing sense of Otherness and difference regarding same-sex attracted males. I rely on the story itself to express the embodied forms of knowing that inhabit the experiences I describe, and resist a finalising interpretation of the story. Instead, I offer personal reflections on particular theoretical and methodological issues which relate to both the form and content of the story

Echinococcus metacestodes as laboratory models for the screening of drugs against cestodes and trematodes
Among the cestodes, Echinococcus granulosus, Echinococcus multilocularis and Taenia solium represent the most dangerous parasites. Their larval stages cause the diseases cystic echinococcosis (CE), alveolar echincoccosis (AE) and cysticercosis, respectively, which exhibit considerable medical and veterinary health concerns with a profound economic impact. Others caused by other cestodes, such as species of the genera Mesocestoides and Hymenolepis, are relatively rare in humans. In this review, we will focus on E. granulosus and E. multilocularis metacestode laboratory models and will review the use of these models in the search for novel drugs that could be employed for chemotherapeutic treatment of echinococcosis. Clearly, improved therapeutic drugs are needed for the treatment of AE and CE, and this can only be achieved through the development of medium-to-high throughput screening approaches. The most recent achievements in the in vitro culture and genetic manipulation of E. multilocularis cells and metacestodes, and the accessability of the E. multilocularis genome and EST sequence information, have rendered the E. multilocularis model uniquely suited for studies on drug-efficacy and drug target identification. This could lead to the development of novel compounds for the use in chemotherapy against echinococcosis, and possibly against diseases caused by other cestodes, and potentially also trematode
Power in a Pandemic: Teachersâ Unions and Their Responses to School Reopening
Drawing on Bachrach and Baratzâs first and second faces of interest group power, we explore the relationship between teachersâ union power and reopening decisions during the fall 2020 semester in 250 large districts around the United States. We leverage a self-collected panel data set of reopening decisions coupled with measures of teachersâ union first face power (drawn from social media postings on teachersâ unionsâ Facebook pages) and second face power (operationalized as district size, whether the school district negotiates a collective bargaining agreement with the teachersâ union, the length of the collective bargaining agreement, and the amount of revenue raised by the union). We found that school districts where teachersâ unions exhibit strong second face power (but not first face power) were less likely to start the school year with in-person instruction, were less likely to ever open during fall semester with in-person instruction and spent fewer weeks in in-person learning
Cyclotron resonant scattering feature simulations. I. Thermally averaged cyclotron scattering cross sections, mean free photon-path tables, and electron momentum sampling
Electron cyclotron resonant scattering features (CRSFs) are observed as
absorption-like lines in the spectra of X-ray pulsars. A significant fraction
of the computing time for Monte Carlo simulations of these quantum mechanical
features is spent on the calculation of the mean free path for each individual
photon before scattering, since it involves a complex numerical integration
over the scattering cross section and the (thermal) velocity distribution of
the scattering electrons.
We aim to numerically calculate interpolation tables which can be used in
CRSF simulations to sample the mean free path of the scattering photon and the
momentum of the scattering electron. The tables also contain all the
information required for sampling the scattering electron's final spin.
The tables were calculated using an adaptive Simpson integration scheme. The
energy and angle grids were refined until a prescribed accuracy is reached. The
tables are used by our simulation code to produce artificial CRSF spectra. The
electron momenta sampled during these simulations were analyzed and justified
using theoretically determined boundaries.
We present a complete set of tables suited for mean free path calculations of
Monte Carlo simulations of the cyclotron scattering process for conditions
expected in typical X-ray pulsar accretion columns (0.01<B/B_{crit}<=0.12,
where B_{crit}=4.413x10^{13} G and 3keV<=kT<15keV). The sampling of the tables
is chosen such that the results have an estimated relative error of at most
1/15 for all points in the grid. The tables are available online at
