Low molecular weight heparin (dalteparin) as adjuvant treatment to thrombolysis in acute myocardial infarctionâa pilot study: Biochemical Markers in Acute Coronary Syndromes (BIOMACS II)
AbstractOBJECTIVESThis randomized, double blind, placebo-controlled pilot trial evaluated the effect of dalteparin as an adjuvant to thrombolysis in patients with acute myocardial infarction regarding early reperfusion, recurrent ischemia and patency at 24 h.BACKGROUNDLow-molecular-weight heparin, given subcutaneously twice daily without monitoring, might be an attractive alternative to conventional intravenous heparin in the treatment of acute myocardial infarction.METHODSIn 101 patients dalteparin/placebo 100 IU/kg was given just before streptokinase and a second injection 120 IU/kg after 12 h. Monitoring with continuous vector-ECG was done to obtain signs of early reperfusion and later ischemic episodes. Blood samples for myoglobin were obtained at start and after 90 min to evaluate signs of reperfusion. Coronary angiography was performed after 20â28 h to evaluate TIMI-flow in the infarct-related artery.RESULTSDalteparin added to streptokinase tended to provide a higher rate of TIMI grade 3 flow in infarct-related artery compared to placebo, 68% versus 51% (p = 0.10). Dalteparin had no effects on noninvasive signs of early reperfusion. In patients with signs of early reperfusion, there seemed to be a higher rate of TIMI grade 3 flow, 74% versus 46% (myoglobin) (p = 0.04) and 73% versus 52% (vector-ECG) (p = 0.11). Ischemic episodes 6â24 h. after start of treatment were fewer in the dalteparin group, 16% versus 38% (p = 0.04).CONCLUSIONSWhen dalteparin was added as an adjuvant to streptokinase and aspirin, there were tendencies for less ECG monitoring evidence of recurrent ischemia and better patency at 24 h, warranting further study

Exploring childhood cancer survivors' views about sex and sexual experiences -findings from online focus group discussions
PURPOSE: To explore childhood cancer survivors' views about sex and sexual experiences and, as an additional aim, their possible needs for care and support from health care professionals regarding sexual life.
METHODS: Written online focus group discussions were performed with survivors of childhood cancer, identified through the Swedish Childhood Cancer Registry; 133 (36%; aged 16-25) participated. Written text was analysed using qualitative content analysis.
RESULTS: The analysis resulted in one main category: Could my cancer experience have an impact on my sexual life? with four generic categories: Sex considered to be good, Feeling insecure and falling behind, Relating sex to a stable relationship and Concerns related to the physical body. In general, participants had not reflected on the possibility that their cancer experience could impact on sexual life. Sex was often considered to be something natural, important and taken for granted. However, thoughts and worries were expressed including being shy, feeling insecure and falling behind peers. Physical concerns included vaginal dryness and difficulties related to erection and reaching orgasm. Many participants stated that sexual issues had not been discussed with health care professionals, however, the need for such support differed.
CONCLUSIONS: Many of the childhood cancer survivors' did not relate their sexual experiences to previous cancer treatment. However, problems were expressed, both of emotional and physical nature. Many participants stated that they had not received any information or support regarding sexual issues from health care professionals, why it is recommended to be regularly addressed in follow-up care
Perspectives of Portuguese People with Physical Disabilities Regarding Their Sexual Health: A Focus Group Study
The World Health Organization has considered sexual health as a major dimension of global health and a sexual right. However, the sexual health of people with physical disabilities is still poorly addressed by health and social care professionals, and it is very stigmatized by society. This study aimed to assess the perspectives of Portuguese people living with physical disabilities regarding issues affecting their sexual health. Nine women and 17 men with different physical disabilities participated in the study. Participants were recruited from a professional rehabilitation facility located in the North of Portugal and were assigned to four groups in one-hour sessions. Three main categories emerged from the content analysis: (1) meanings and beliefs regarding sexuality; (2) experiences of sexuality; (3) necessary changes. Despite the positive social changes towards sexuality, participants expressed that their sexual rights are still unfulfilled, as they live in a context that perpetuates their dependency. They pointed out low self-esteem, prejudice and social isolation, poor architectural accessibility and scarcity of financial support as some of the barriers to their lives and their sexual health. Finally, participants identified the main needs regarding their sexual health, such as: access to specialized information; training for health professionals. This study gives voice to people with physical disabilities and sheds light into both individual and contextual factors affecting their sexual health. Disability, sexuality and sociosexual relationships in women's everyday life
This article draws on a qualitative study in Sweden in which 21 women from three generations, with congenital physical disabilities, were interviewed about their daily life. Using a social constructionist perspective on both disability and sexuality, this paper focuses on sexuality and sociosexual relationships as part of these women's everyday life. Data indicate that disabled women in younger generations might accumulate experiences of sexuality differently from women in older generations. But regardless of generation, the most common pattern is that these women recount their teenage years as an unhappy period in terms of exclusion from sociosexual relationships. One conclusion drawn is that issues of autonomy, independence and personal assistants in these women's everyday life are brought to a head when they are related to private sexual life. Also, there is still a way to go as regards integration and enabling interventions in the field of women, disability and sexuality
Fashion and the household : The formation of media and gender in 18th Century Swedish periodicals and pamphlets
This thesis, Fashion and the household: The formation of media and gender in 18th Century Swedish periodicals and pamphlets, deals with the formation of media and gender in 18th Century Sweden using fashion and the household as themes of discussion and gender as an analytical category. The material used consists of periodicals, small prints and series of pamphlets published from the 1730s to the 1770s in Sweden, and one engraving showing five housewives beating a husband on his bare buttocks printed in 1755. According to both writers of that time and to earlier research these periodicals and pamphlets, printed in small editions and circulated mostly in the capital of Stockholm, meant a breakthrough for a new public discourse. One aim is to study how gender was used, discussed and constructed in this new media. Another aim is to study how gender was used to create modern forward looking identities. Of particular interest in this new media is the debate on modernity and how it brought about new views on modern society including both gender relations and the construction of public and private spheres. The public discourse fostered a debate on the privatization of the household. The perspective chosen is meant to show how gender was used differently, on different levels by both male and female writers depending on purpose or agenda and sometimes in opposition to the described changes taking place. Gender could be used both benevolently and with hostility. By using gender as analytical category the public discourse studied in this thesis is decoded showing how both society and the public discourse, simultaneously, were gendered in new ways. It presents a debate of modernity on the threshold to modernity, and a debate on the exploitation of private vices and the privatization or marginalization of the household, later to be described as a division of private and public spheres
Between Neighbours and Nature : Resposibility and Risk for Spruce Bark Beetle Infestations in Forests
This thesis discusses the responsibility of and risk for infestations of spruce bark beetle. The spruce bark beetle, Ips typographus, is a natural part of the Swedish forest ecosystem but causes severe financial damage in forestry. Since 2018, the spruce bark beetle infestations have been substantial in Sweden. The first task of the thesis is to evaluate the balance between the forest production interests and the environmental interests, based on the legal regulation of spruce bark beetle control. Production and environment are the equal aims of the Swedish Forestry Act. The regulation of spruce bark beetle control differs depending on whether the forest is in a conservation area or not. For example, owners of forests outside conservation areas are obliged to take care of damaged, fresh wood of spruce and pine, while such an obligation in general does not exist in conservation areas. In this evaluation, I conclude that the balance between the two interests in the spruce bark beetle regulation is good as a whole. The second question of the thesis is the legal allocation of risk for spruce bark beetle infestations. In this investigation, it is examined whether established law presents a possibility to claim damages for spruce bark beetle infestations that may derive from an adjacent forest. The conclusion drawn is that the regulation of compensation for certain kinds of environmental damage in the Swedish Environmental Code probably would not be applicable as a legal basis for such a claim. By contrast, neighbour law and the Swedish Torts Act could be used as the basis of an action for damages. However, to prove causality regarding the origins of a specific infestation might bring problems, which altogether means that an action for damages faces considerable difficulties. Therefore lastly, some alternative ways of allocating the risk for spruce bark beetle infestations are presented. I denna uppsats diskuteras ansvar och risk för granbarkborreangrepp. Granbarkborren, Ips typographus, Àr en naturlig del av skogens ekosystem men orsakar stora ekonomiska skador i skogsbruket.
