    Cervical cancer in women diagnosed at the National Health Laboratory, Sudan: A call for screening

    Background: Cancer of the cervix is the second most prevalent cancer of women to date in the Sudan, in a concerted review of the records of the hospital-based cancer registry of the Radiation & Isotope Centre of Khartoum (RICK). However, in spite of a wealth of data, this is the first study to date describing the histopathologic prevalence of cervical cancer in the Sudan.Objectives: To identify the percentage and clinicopathological pattern of cervical cancer cases diagnosed at Histopathology Department, National Health Laboratory (NHL) in Khartoum, Sudan.Material and Methods: This is a cross-sectional, descriptive study  conducted at the NHL. All cases with histopathological diagnosis of cervical neoplasm in the period from 2004-2009 were reviewed. Patients’ clinical data were obtained from clinical records. Exclusion criteria included inadequate clinical information and unavailability of both Hematoxylin and Eosin stained (H&E) sections and formalin-fixed paraffin-embedded (FFPE) blocks. The WHO classification of cancer of the cervix (2003) was used to describe disease type. SPSS data analysis was applied.Results: A total of 287 cases were reviewed and 195 cases were included in the study. The mean of cervical cancer cases diagnosed per year at NHL is 7.9%. The commonest age group affected was patients grouped between 41- 60 years (52%) followed by 61-80 years (26.3%). Histologically, 95.9% of the cases were carcinomas. Squamous cell carcinomas were 90.9%, Adenocarcinomas 4.8%, and other epithelial tumours were 4.3%. Of the Squamous carcinomas, 98.8% were invasive and 1.2% intraepithelial (cervical intraepithelial neoplasia). The majority of case presentations were that of a protruding cervical mass. We noticed the commonest symptom being bleeding per vagina.Conclusion: To determine the incidence of cervical cancer in the Sudan a national populationbased registry is necessary. The mean age of patients presenting with cervical cancer to NHL is 53.25 years. This is in keeping with the natural history of the human papilloma virus (HPV). The late presentation of patients with aggressive disease necessitates health  education and cervical cancer screening as well as strict guidelines for medical record keeping in line with good medical practices, enabling good data collection for the newly established population-based cancer registry.Keywords: HPV, invasive squamous cell carcinoma

    Activation of MAPK signalling results in resistance to saracatinib (AZD0530) in ovarian cancer

    SRC tyrosine kinase is frequently overexpressed and activated in late-stage, poor prognosis ovarian tumours, and preclinical studies have supported the use of targeted SRC inhibitors in the treatment of this disease. The SAPPROC trial investigated the addition of the SRC inhibitor saracatinib (AZD0530) to weekly paclitaxel for the treatment of platinum resistant ovarian cancer; however, this drug combination did not provide any benefit to progression free survival (PFS) of women with platinum resistant disease. In this study we aimed to identify mechanisms of resistance to SRC inhibitors in ovarian cancer cells. Using two complementary strategies; a targeted tumour suppressor gene siRNA screen, and a phospho-receptor tyrosine kinase array, we demonstrate that activation of MAPK signalling, via a reduction in NF1 (neurofibromin) expression or overexpression of HER2 and the insulin receptor, can drive resistance to AZD0530. Knockdown of NF1 in two ovarian cancer cell lines resulted in resistance to AZD0530, and was accompanied with activated MEK and ERK signalling. We also show that silencing of HER2 and the insulin receptor can partially resensitize AZD0530 resistant cells, which was associated with decreased phosphorylation of MEK and ERK. Furthermore, we demonstrate a synergistic effect of combining SRC and MEK inhibitors in both AZD0530 sensitive and resistant cells, and that MEK inhibition is sufficient to completely resensitize AZD0530 resistant cells. This work provides a preclinical rationale for the combination of SRC and MEK inhibitors in the treatment of ovarian cancer, and also highlights the need for biomarker driven patient selection for clinical trials

    Ashghal Guidelines for the Evaluation and Repair of Sinkholes

    Sinkholes are a common and recurring problem with potential safety risks to road users and authorities. A sinkhole is a void in the ground caused by natural and/or manmade activities, and mainly associated with carbonate rocks and underground water movement. It occurs suddenly and manifests as a hole in the ground, with the potential to cause significant problems of road closure and interruption to road users and construction activities. With the vast infrastructure, development over the last two decades and the majority of soil formation of Qatar exposes limestone and dolomites, the number of reported sinkholes has increased dramatically. This paper presents the development and implementation of new guidelines for the evaluation and repair of sinkholes for the Roads Operation and Maintenance Department (ROMD) at the Public Works Authority (Ashghal). It reviews the methodology adopted for the classification of sinkholes, application of Ground Penetrating Radar (GPR) technique for the early detection of subsurface voids and sinkholes before they become major problems. It also covers the approach adopted by ROMD and its Framework Partners for the repair of sinkholes. A case study is presented on the implementation of the new guidelines for the effective evaluation and repair of sinkholes, with performance monitoring in service. Successful implementation of the new guidelines contributes to enhanced management of road network in Qatar by maximizing asset performance and minimizing accidents and service disruption

    Mutual Relationships: Saudi Universities and The Private Sector for Economic Development

    The purpose of this research is to determine the level of the mutual relationship between Saudi universities and the private sector to achieve Economic development in light of the Kingdoms 2030 vision. The researcher relied heavily on the questionnaire as the primary instrument for data collection for the study, employing a descriptive research design and a quantitative methodology in order to accomplish the goals of the study. The investigation was carried out with a sample size of 180 universities leaders in the southern region of Saudi Arabia, all of them were chosen at random. The findings of this study indicated that leaders had high expectations for a mutual relationship between Saudi universities and the private sector. In addition, the findings demonstrated that there are no statistically significant variations in the mutual relationship between Saudi universities and the private sector with regard to the variables of academic rank, and years of experience

    The effects of microstructural features on the performance gap in corrosion resistance between bulk and HVOF sprayed Inconel 625

    It is commonly observed that there is a performance gap between the corrosion resistance of thermally sprayed coatings and the equivalent bulk material. This is attributed to the significantly modified microstructure of the sprayed coatings. However, currently there is no detailed understanding of which aspects of microstructural modification are primarily responsible for this performance gap. In this work several deliberately microstructurally modified versions of the Ni-based superalloy Inconel 625 were produced. These were subjected to potentiodynamic electrochemical testing in 0.5M H2SO4 to investigate the links between specific microstructural features and electrochemical behaviour. Samples were prepared by high velocity oxy-fuel (HVOF) thermal spraying, laser surface remelting using a high power diode laser and conventional powder sintering. Microstructural features were examined by optical and scanning electron microscopy and X-ray diffraction. Potentiodynamic testing was carried out on the following forms of Inconel 625: wrought sheet; HVOF sprayed coatings; sintered powder compacts; laser melted wrought sheet and HVOF sprayed coatings. Using the corrosion behaviour, i.e. passive current density, of the wrought sheet as a baseline, the performance of different forms of Inconel 625 were compared. It is found that a fine dendritic structure (with associated microsegregation) produced by laser remelting wrought sheet has no significant effect on corrosion performance. Up to 12% porosity in sintered powder samples increases the passive current density by a factor of only around 2. As observed previously, the passive current density of HVOF sprayed coatings is 20 - 40 times greater. However, HVOF coatings subjected to laser surface remelting are found to have a passive current density close to that of wrought material. It is concluded that, whilst porosity in coatings produces some decrease in corrosion resistance, the main contributing factor is the galvanic corrosion of localised Cr-depleted regions which are associated with oxide inclusions within HVOF sprayed samples

    Revolutionizing Clinical Microbiology Laboratory Organization in Hospitals with In Situ Point-of-Care

    BACKGROUND: Clinical microbiology may direct decisions regarding hospitalization, isolation and anti-infective therapy, but it is not effective at the time of early care. Point-of-care (POC) tests have been developed for this purpose. METHODS AND FINDINGS: One pilot POC-lab was located close to the core laboratory and emergency ward to test the proof of concept. A second POC-lab was located inside the emergency ward of a distant hospital without a microbiology laboratory. Twenty-three molecular and immuno-detection tests, which were technically undemanding, were progressively implemented, with results obtained in less than four hours. From 2008 to 2010, 51,179 tests yielded 6,244 diagnoses. The second POC-lab detected contagious pathogens in 982 patients who benefited from targeted isolation measures, including those undertaken during the influenza outbreak. POC tests prevented unnecessary treatment of patients with non-streptococcal tonsillitis (n = 1,844) and pregnant women negative for Streptococcus agalactiae carriage (n = 763). The cerebrospinal fluid culture remained sterile in 50% of the 49 patients with bacterial meningitis, therefore antibiotic treatment was guided by the molecular tests performed in the POC-labs. With regard to enterovirus meningitis, the mean length-of-stay of infected patients over 15 years old significantly decreased from 2008 to 2010 compared with 2005 when the POC was not in place (1.43±1.09 versus 2.91±2.31 days; p = 0.0009). Altogether, patients who received POC tests were immediately discharged nearly thrice as often as patients who underwent a conventional diagnostic procedure. CONCLUSIONS: The on-site POC-lab met physicians' needs and influenced the management of 8% of the patients that presented to emergency wards. This strategy might represent a major evolution of decision-making regarding the management of infectious diseases and patient care

    We have accelerated the ageing of CuAlS2 by the application of a static electrical field for different degradation times. We have investigated the admittance spectroscopy and the scanning electron microscopy to follow and understand the (mass-charge) coupled transport processes produced in the volume and on the surface of these films. The electrical constraint induces, after an incubation phase, an activated decrease of the resistance, followed by a susbstantial increase correlated to the formation of an open circuit. This degradation occurs more rapidly for the films having initially a lower resistance, due to the thermal dissipation which increases considerably the temperature to about 140 °C. Admittance spectra reveal, at low frequencies, a capacitive loop related to the formation of a charge space induced by copper diffusion. Such migration develop induces the formation of copper arborescences, spreading from the cathode towards the anode. The effect of these structures on the properties of the degraded films is discussed in relation to electromigration and associated processes (whiskers, fracture, healing, bridge-building, ...). Also, we have noticed their similarity with fractal phenomena such as electrodeposition and dielectric breakdown
