    Psychological factors and cardiac repolarization instability during anger in implantable cardioverter defibrillator patients

    BACKGROUND: Evidence indicates that emotions such as anger are associated with increased incidence of sudden cardiac death, but the biological mechanisms remain unclear. We tested the hypothesis that, in patients with sudden death vulnerability, anger would be associated with arrhythmic vulnerability, indexed by cardiac repolarization instability. METHODS: Patients with coronary artery disease (CAD) and an implantable cardioverter defibrillator (ICD; n = 41) and healthy controls (n = 26) gave an anger‐inducing speech (anger recall), rated their current (state) anger, and completed measures of trait (chronic) levels of Anger and Hostility. Repolarization instability was measured using QT Variability Index (QTVI) at resting baseline and during anger recall using continuous ECG. RESULTS: ICD patients had significantly higher QTVI at baseline and during anger recall compared with controls, indicating greater arrhythmic vulnerability overall. QTVI increased from baseline to anger recall to a similar extent in both groups. In ICD patients but not controls, during anger recall, self‐rated anger was related to QTVI (r = .44, p = .007). Trait (chronic) Anger Expression (r = .26, p = .04), Anger Control (r = −.26, p = .04), and Hostility (r = .25, p = .05) were each associated with the change in QTVI from baseline to anger recall (ΔQTVI). Moderation analyses evaluated whether psychological trait associations with ΔQTVI were specific to the ICD group. Results indicated that Hostility scores predicted ΔQTVI from baseline to anger recall in ICD patients (ÎČ = 0.07, p = .01), but not in controls. CONCLUSIONS: Anger increases repolarization lability, but in patients with CAD and arrhythmic vulnerability, chronic and acute anger interact to trigger cardiac repolarization lability associated with susceptibility to malignant arrhythmias

    Enabling Long-Duration Lunar Equatorial Operations With Thermal Wadi Infrastructure

    Long duration missions on the Moon s equator must survive lunar nights. With 350 hr of cryogenic temperatures, lunar nights present a challenge to robotic survival. Insulation is imperfect, so it is not possible to passively contain enough heat to stay warm through the night. Components that enable mobility, environmental sensing and solar power generation must be exposed, and they leak heat. Small, lightweight rovers cannot store enough energy to warm components throughout the night without some external source of heat or power. Thermal wadis, however, can act as external heat sources to keep robots warm through the lunar night. Electrical power can also be provided to rovers during the night from batteries stored in the ground beside wadis. Buried batteries can be warmed by the wadi s heat. Results from analysis of the interaction between a rover and a wadi are presented. A detailed three-dimensional (3D) thermal model and an easily configurable two-dimensional (2D) thermal model are used for analysis

    The Parents under Pressure parenting programme for families with fathers receiving treatment for opioid dependency: the PuP4Dads feasibility study

    Background: The impact of parental drug use on children is a major public health problem. However, opioid-dependent fathers have been largely ignored in parenting research. Objective: Implement and test the feasibility and acceptability of the ‘Parents under Pressure’ parenting programme for opioid-dependent fathers and their families (PuP4Dads) and determine whether a full scale evaluation could be conducted. Design: Mixed methods feasibility study. Setting: Two non-NHS family support services for parents who use drugs in Scotland. Participants: Fathers prescribed Opioid Substitution Therapy (n=25), their partners (n=17) and children; practitioners; supervisors, service managers; referrers. Intervention: Home-visiting programme, including an integrated theoretical framework, case formulation, collaborative goal setting, and modules designed to improve parenting, the caregiving environment and child welfare. Delivered flexibly over six months by accredited practitioners. Main outcome measures: Feasibility progression criteria: recruitment target (n=24 fathers); acceptability of PUP; father engagement in the study (66% complete programme; minimum 10 complete baseline and post-treatment interviews); engagement in qualitative interviews (fathers n=10 minimum; practitioners 90% uptake; managers 80% uptake); focus groups (referrers 80% uptake); adequate fidelity; no adverse events. Data sources: Researcher administered validated questionnaires: Brief Child Abuse Potential; Parenting Sense of Competence; Difficulties in Emotion Regulation; Paternal/Maternal Antenatal Attachment; Emotional Availability (video); Infant Toddler Social Emotional Assessment/Strengths and Difficulties; Conflict Tactics Scale; Treatment Outcomes Profile; EQ-5D-5L. Other sources: Parent-completed service use (economic measure); Social work child protection data; NHS opioid substitution therapy prescription data. Practitioner reported attendance data. Interviews with fathers, mothers, practitioners (n=8), supervisors (n=2), service managers (n=7); focus groups with referrers (n=28); ‘expert event’ with stakeholders (n=39). Results: PuP was successfully delivered within non-NHS settings and acceptable and suitable for the study population. Referrals (n=44) resulted in 38 (86%) eligible fathers, of whom 25 (66%) fathers and 17 partners/mothers consented to participate. Most fathers reported no previous parenting support. Intervention engagement: 248 sessions delivered to 20 fathers and 14 mothers who started the intervention; 14 fathers (10 mothers) completed ≄ six sessions; six fathers (4 mothers) completed ≀ five sessions. Father and mother attendance rates were equal (mean: 71%). Median length of engagement: fathers 26 weeks, mothers 30 weeks. Research interview completion rates for fathers: 23 at baseline, 16 follow-up one, 13 follow-up two. Measures: well tolerated; suitability of some measures dependent on family circumstances; researcher administered questionnaires resulted in little missing data. Perceived benefits of PuP4Dads from parent, practitioner and manager perspectives: therapeutic focus on fathers, improved parental emotion regulation; understanding and responding to child’s needs; better multi-agency working; programme a good fit with practice ‘ethos’ and policy agenda. Learning highlighted importance of: service-wide adoption and implementation support; strategies to improve recruitment and retention of fathers; managing complex needs of both parents concurrently; understanding contextual factors affecting programme delivery and variables affecting intervention engagement and outcomes. Limitations: Lack of emotional availability and economic (service use) data. Conclusions: A larger evaluation of PuP4Dads is feasible. Future work: Demonstrating the effectiveness of PuP4Dads and the cost implications. Better understanding of how the intervention works, for whom, under what circumstances, and why

    A standardised protocol for assessment of relative SARS-CoV-2 variant severity, with application to severity risk for COVID-19 cases infected with Omicron BA.1 compared to Delta variants in six European countries

    Several SARS-CoV-2 variants that evolved during the COVID-19 pandemic have appeared to differ in severity, based on analyses of single-country datasets. With decreased SARS-CoV-2 testing and sequencing, international collaborative studies will become increasingly important for timely assessment of the severity of newly emerged variants. The Joint WHO Regional Office for Europe and ECDC Infection Severity Working Group was formed to produce and pilot a standardised study protocol to estimate relative variant case-severity in settings with individual-level SARS-CoV-2 testing and COVID-19 outcome data during periods when two variants were co-circulating. To assess feasibility, the study protocol and its associated statistical analysis code was applied by local investigators in Denmark, England, Luxembourg, Norway, Portugal and Scotland to assess the case-severity of Omicron BA.1 relative to Delta cases. After pooling estimates using meta-analysis methods (random effects estimates), the risk of hospital admission (adjusted hazard ratio [aHR]=0.41, 95% CI 0.31-0.54), ICU admission (aHR=0.12, 95% CI 0.05-0.27), and death (aHR=0.31, 95% CI 0.28-0.35) was lower for Omicron BA.1 compared to Delta cases. The aHRs varied by age group and vaccination status. In conclusion, this study has demonstrated the feasibility of conducting variant severity analyses in a multinational collaborative framework.
