    Within the last decade, numerous new challenges have appeared in the UNHS arena, such as (i) the need to validate the automated OAE/ABR screeners; (ii) the need to qualify the responses from the automated devices; (iii) the need to obtain additional information (i.e., hearing threshold) for the subject under assessment, in a short period of time; (iv) and the need to integrate numerous measurements in a single portable automated device. To respond to these clinical demands, several new methodologies have been introduced to the UNHS clinical practice. In this context, the aim of this chapter is to provide information on these new technological trends

    OAEs and Meniere Disease

    Otoacoustic emissions (OAEs) are responses originating from the inner ear. Clinically they are evoked by different families of acoustic stimuli, such as transient acoustic clicks, tone pips, and pure tones. Upon stimulation, the acoustic energy is transformed in the middle ear at acoustic pressure acting upon the stapes footplate. The pressure wave inside the cochlea stimulates the OAE generators and a reverse acoustic energy (the OAE response) propagates from the inner ear, through the stapes and the middle ear structures, to the tympanic membrane. Considering that the acoustic energy has to cross the middle ear structures twice, the functional status of the middle ear can influence or attenuate considerably the OAE response. In this context, any vestibular alteration can influence the middle ear mechanics (mainly the middle ear impedance) and consequently the OAE response characteristics. The data in the literature indicate that OAEs are very sensitive to changes in the intracranial pressure. These pressure alterations during the Meniere’s hydrops phase are expressed as changes in the intralabyrinthine pressure. Other studies have presented data supporting the assumption that OAEs can adequately monitor middle ear changes induced by the presentation of the glycerol test. The data in the literature suggest that OAEs can monitor the progress of Meniere’s disease using reliable indices

    Aim of this paper is to remind the risk of ototoxicity when using chloroquine and hydroxychloroquine, in particular as prophylactic agents against SARS-CoV-2, during the pandemic. Healthy subjects taking chloroquine and hydroxychloroquine as prophylactic agents against SARS-CoV-2, during the pandemic, should be screened periodically, at least by Otoacoustic Emissions (OAEs) in order to detect early manifestations of possible cochlear ototoxic damages

    Genetic Polymorphisms in Sudden Sensorineural Hearing Loss: An Update

    Objective: To investigate the association between genetic polymorphisms and sudden sensorineural hearing loss (SSNHL). Most of the SSNHL cases still remain idiopathic, and several etiopathogenetic hypotheses, including a genetic predisposition, have been proposed. Methods: A literature review was conducted using different databases: Medline/PubMed, EMBASE, and CINAHL, according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. All databases have been searched from May 2016 to April 2020. Results: Genetic susceptibility could represent a key element in the pathogenesis of SSNHL. A number of genetic polymorphisms related to (1) inner ear microvascular disease and endothelial dysfunction and (2) to inner ear oxidative stress and inflammation have been addressed in the current literature. Conclusions: The potential identification of a genetic profile related to SSNHL could provide a more accurate prognostic evidence of idiopathic SSNHL (ISSNHL), offering to the patients not only early-prevention strategies but eventually information on various inheritance modalities. © The Author(s) 2020

    Wideband Tympanometry

    The wideband tympanometry (WBT) assesses the middle ear function with a transient wideband stimulus in order to capture the middle ear behavior at a wide range of frequencies. Data in the literature suggest that the WBT has more sensibility to detect middle ear disorders than the traditional tympanometry. In this context, pathologies, which might be more easily identified/monitored by WBT, include otosclerosis, flaccid eardrums, ossicular chain discontinuity with semicircular canal dehiscence, and negative middle ear pressure with middle ear effusion. The chapter presents information on classical tympanometry, the multifrequency tympanometry equivalent coded as WBT, clarification of terms used in WBT measurements, and a short overview of clinical applications in infants and adults

    Aim of this communication is to remind clinical professionals to be aware of ototoxic side effects of several specific drugs proposed for the treatment of the new virus SARS-CoV-2 (Covid-19). In particular, chloroquine and hydroxychloroquine, azithromycin, as well as antiviral drugs such as remdesivir, favipiravir and lopinavir can all present potential ototoxic side effects. The data in the literature do not offer specific information on their potential synergetic effects nor on their interactions

    Rehabilitation of Severe to Profound Sensorineural Hearing Loss in Adults: Audiological Outcomes:

    The aim of this article is to describe the audiological patterns of 71 adult patients presenting severe to profound sensorineural hearing loss, who were rehabilitated by cochlear implants (CIs) and hearing aids. This is a retrospective study in a university setting, where the clinical records of 71 adult patients were reviewed and processed. Speech intelligibility was evaluated at one aided ear (CI) or at both aided ears (double CI or a combination of CI and hearing aid [HA]). Patients with a bilateral CI or with a bimodal hearing setup (CI and HA) performed better than those with a single CI; data from the phonetic matrices test showed that there was a statistically significant difference among patients aided by a single CI versus binaural setup (double CI or CI + HA). In particular, patients aided by a bilateral CI, or by a CI and HA, showed an improvement in the functional results of the speech tests, compared to patients using a single CI. Binaural hearing (either with a bilateral CI or bimodal) allows an improvement in the functional results at the speech tests, compared to the use of a CI only
