    Integration of sexual and reproductive health services in the provision of primary health care in the Arab States: status and a way forward.

    Different approaches are used for integration of sexual and reproductive health (SRH) services at the primary health care (PHC) level, aiming at providing comprehensive services leaving no one behind. This paper aims to assess gaps in the delivery of SRH in PHC services, identifying challenges and proposing action towards universal health coverage in Arab countries. The United Nations Population Fund - Arab States Regional Office (UNFPA/ASRO), in partnership with Middle East and North Africa Health Policy Forum (HPF), launched an assessment of integration of SRH into PHC in 11 Arab countries in 2017-2018. Desk reviews were conducted, using published programme reports and national statistics. Data from country reports were compiled to present a regional assessment, challenges and recommendations. SRH services are partially integrated in PHC. Family planning is part of PHC in all countries except Libya, where only counselling is provided. Only Morocco, Tunisia and Oman provide comprehensive HIV services at PHC level. Jordan, Libya and Saudi Arabia rely mainly on referral to other facilities, while most of the integrated family planning or HIV services in Sudan, Morocco and Oman are provided within the same facilities. Action is required at the policy, organisational and operational levels. Prioritisation of services can guide the development of essential packages of SRH care. Developing the skills of the PHC workforce in SRH services and the adoption of the family medicine/general practice model can ensure proper allocation of resources.     HIV-1 nef suppression by virally encoded microRNA

    BACKGROUND: MicroRNAs (miRNAs) are 21~25-nucleotides (nt) long and interact with mRNAs to trigger either translational repression or RNA cleavage through RNA interference (RNAi), depending on the degree of complementarity with the target mRNAs. Our recent study has shown that HIV-1 nef dsRNA from AIDS patients who are long-term non-progressors (LTNPs) inhibited the transcription of HIV-1. RESULTS: Here, we show the possibility that nef-derived miRNAs are produced in HIV-1 persistently infected cells. Furthermore, nef short hairpin RNA (shRNA) that corresponded to a predicted nef miRNA (~25 nt, miR-N367) can block HIV-1 Nef expression in vitro and the suppression by shRNA/miR-N367 would be related with low viremia in an LTNP (15-2-2). In the 15-2-2 model mice, the weight loss, which may be rendered by nef was also inhibited by shRNA/miR-N367 corresponding to suppression of nef expression in vivo. CONCLUSIONS: These data suggest that nef/U3 miRNAs produced in HIV-1-infected cells may suppress both Nef function and HIV-1 virulence through the RNAi pathway

    Magnetization plateau in a two-dimensional multiple-spin exchange model

    We study a multiple-spin exchange model on a triangular lattice, which is a possible model for low-density solid 3He films. Due to strong competitions between ferromagnetic three-spin exchange and antiferromagnetic four-spin one, the ground states are highly degenerate in the classical limit. At least 2^{L/2}-fold degeneracy exists on the L*L triangular lattice except for the SO(3) symmetry. In the magnetization process, we found a plateau at m/m_{sat}=1/2, in which the ground state is "uuud state" (a collinear state with four sublattices). The 1/2-plateau appears due to the strong four-spin exchange interaction. This plateau survives against both quantum and thermal fluctuations. Under a magnetic field which realizes the "uuud" ordered state, a phase transition occurs at a finite temperature. We predict that low-density solid 3He thin films may show the 1/2-plateau in the magnetization process. Experimental observation of the plateau will verify strength of the four-spin exchange.     Ferromagnetism in the one-dimensional Hubbard model with orbital degeneracy: From low to high electron density

    We studied ferromagnetism in the one-dimensional Hubbard model with doubly degenerate atomic orbitals by means of the density-matrix renormalization-group method and obtained the ground-state phase diagrams. It was found that ferromagnetism is stable from low to high (0< n < 1.75) electron density when the interactions are sufficiently strong. Quasi-long-range order of triplet superconductivity coexists with the ferromagnetic order for a strong Hund coupling region, where the inter-orbital interaction U'-J is attractive. At quarter-filling (n=1), the insulating ferromagnetic state appears accompanying orbital quasi-long-range order. For low densities (n<1), ferromagnetism occurs owing to the ferromagnetic exchange interaction caused by virtual hoppings of electrons, the same as in the quarter-filled system. This comes from separation of the charge and spin-orbital degrees of freedom in the strong coupling limit. This ferromagnetism is fragile against variation of band structure. For high densities (n>1), the phase diagram of the ferromagnetic phase is similar to that obtained in infinite dimensions. In this case, the double exchange mechanism is operative to stabilize the ferromagnetic order and this long-range order is robust against variation of the band-dispersion.     A qualidade microbiológica em lactários é um item que merece toda a atenção e deve ser mantida de forma rigorosa. Considerando a população alvo desses serviços e o ambiente em que estão inseridos, sabe-se que são locais de alto risco para contaminações microbiológicas e as consequências destas podem ser fatais. Assim, o objetivo deste estudo foi verificar a qualidade microbiológica de fórmulas infantis utilizadas para alimentação de crianças internadas na unidade de pediatria de um hospital público do município de Campinas – SP. Foram realizadas análises microbiológicas de bactérias aeróbias mesófilas totais, coliformes totais e termotolerantes e Estafilococos coagulase positiva das fórmulas infantis, comparando-se resultados com os valores especificados na Resolução da Diretoria Colegiada no 12, de 02 de janeiro de 2001 (RDC 12/2001), da Agência de Vigilância Sanitária. Das 26 amostras de fórmulas infantis, 88,5% atenderam aos parâmetros para oferecimento para menores de 1 ano de vida, e 92,3% atenderam para as exigências para pacientes com mais de 1 ano de vida, segundo a RDC 12/2001. Porém, quando observados paralelamente adequação a parâmetros desta legislação e a recomendação da Food and Drug Administration (2006) para microrganismos aeróbios mesófilos, apenas 46,2% apresentavam contagens dos microrganismos avaliados dentro dos limites tolerados

    Effects of anti-malarial drugs on the electrocardiographic QT interval modelled in the isolated perfused guinea pig heart system

    <p>Abstract</p> <p>Background</p> <p>Concern over the potential cardiotoxicity of anti-malarial drugs inducing a prolonged electrocardiographic QT interval has resulted in the almost complete withdrawal from the market of one anti-malarial drug - halofantrine. The effects on the QT interval of four anti-malarial drugs were examined, using the guinea pig heart.</p> <p>Methods</p> <p>The guinea pig heart was isolated, mounted on a Langendorff apparatus, and was then perfused with pyruvate-added Klebs-Henseleit solutions containing graded concentrations of the four agents such as quinidine (0.15 - 1.2 μM), quinine (0.3 - 2.4 μM), halofantrine (0.1 - 2.0 μM) and mefloquine (0.1 - 2.0 μM). The heart rate-corrected QaTc intervals were measured to evaluate drug-induced QT prolongation effects.</p> <p>Results</p> <p>Quinidine, quinine, and halofantrine prolonged the QaTc interval in a dose-dependent manner, whereas no such effect was found with mefloquine. The EC<sub>50 </sub>values for the QaTc prolongation effects, the concentration that gives a half-maximum effect, were quinidine < quinine ≈ halofantrine.</p> <p>Conclusions</p> <p>In this study, an isolated, perfused guinea pig heart system was constructed to assess the cardiotoxic potential of anti-malarial drugs. This isolated perfused guinea pig heart system could be used to test newly developed anti-malarial drugs for their inherent QT lengthening potential. More information is required on the potential variation in unbound drug concentrations in humans, and their role in cardiotoxicity.</p

    Knowledge, experience, and potential risks of dating violence among Japanese university students: a cross-sectional study

    <p>Abstract</p> <p>Background</p> <p>The Domestic Violence Prevention Act came into effect in Japan in 2001, but covers only marriage partner violence and post-divorce partner violence, and does not recognize intimate partner violence (IPV). The present study was performed to determine the experience of harassment, both toward and from an intimate partner, and recognition of harassment as IPV among Japanese university students.</p> <p>Methods</p> <p>A self-administered questionnaire survey regarding the experience of harassment involving an intimate partner was conducted as a cross-sectional study among freshman students in a prefectural capital city in Japan.</p> <p>Results</p> <p>A total of 274 students participated in the present study. About half of the subjects (both male and female students) had experience of at least one episode of harassment toward or had been the recipient of harassment from an intimate partner. However, the study participants did not recognize verbal harassment, controlling activities of an intimate partner, and unprotected sexual intercourse as violence. Experience of attending a lecture/seminar about domestic violence and dating violence did not contribute to appropriate help-seeking behavior.</p> <p>Conclusions</p> <p>An educational program regarding harassment and violence prevention and appropriate help-seeking behavior should be provided in early adolescence to avoid IPV among youth.</p

    Abstract Background CRISPR/Cas9 enables the targeting of genes in zygotes; however, efficient approaches to create loxP-flanked (floxed) alleles remain elusive. Results Here, we show that the electroporation of Cas9, two gRNAs, and long single-stranded DNA (lssDNA) into zygotes, termed CLICK (CRISPR with lssDNA inducing conditional knockout alleles), enables the quick generation of floxed alleles in mice and rats. Conclusions The high efficiency of CLICK provides homozygous knock-ins in oocytes carrying tissue-specific Cre, which allows the one-step generation of conditional knockouts in founder (F0) mice
