Chronic Low Back and Pelvic Girdle Pain 10 Months Post-Partum: A Case Study

Background and Purpose Due to the increase in relaxin hormone during pregnancy women often experience increases in joint and ligament laxity. The result of this increase in laxity during or after pregnancy can lead to problems with postural and/or gait abnormalities, diastasis recti (separation between left and right side of rectus abdominus muscle) and pain, specifically, low back and pelvic girdle pain. Activities of daily living are often impacted because of these problems. The purpose of this case study is to highlight some of the principal issues and therapeutic options for postpartum women as presented through the case of one new mother. Case Description The patient isa 35 year old Caucasian female presenting at an outpatient orthopedic clinic ten months postpartum. Her chief complaints include periodic sharp pain down back and left groin, muscle spasms/hitching in left abdomen, locked-in low back, left hip grinding, decreased hip and lumbar range of motion and hip instability. She is limited by pain in many of her ADLs including lifting and carrying her 20lb son. Intervention Therapeutic exercise, manual therapy and therapeutic interventions were employed over the course of 14 weeks and 28 sessions to treat this patient\u27s LE ROM limitations, joint tightness and core muscle weakness. Outcomes Following PT intervention the patient improved in overall lower extremity ROM, core strength and joint motion. Functionally, she was able to return to her normal ADLs with minimal pain symptoms. Discussion The functional gains made by this patient throughout the course of her therapy sessions demonstrate the importance of physical therapy as a useful tool in post-partum recovery. Given the extreme changes to the musculoskeletal system during and after pregnancy it is important for physical therapy to assume a greater role in working with this population

How children’s navigation on digital platforms challenges child welfare assessments

    Get PDF
When a child is placed in public care, digital platforms create an arena for the child and parents to stay updated on each other’s lives while living apart. However, contact through digital platforms can be unpredictable for some children, resulting in unintended adverse consequences and undermining the placement’s stability. In this context, it is not uncommon for children and parents to have contact through social media or other digital platforms, also known as ‘virtual visitation’. To ensure that restrictions on contact will provide the child with the necessary protection, knowing the child’s navigation and access to digital platforms may provide an understanding of potential contact forms and the need for protection. Based on an online questionnaire addressed to Child Welfare Services in Norway, this study reports on 196 child welfare workers’ views as to what extent children’s navigation on digital platforms is included as part of the professional assessment of the regulation of visitation rights between a child and his or her parents after a care order is issued. The findings indicate a minimal consideration of the child’s access to digital media and their navigation on social media when assessing the regulation of contact rights. The findings from this study highlight the need for a new understanding of the possible connection between virtual and physical visitations after a care order is issued. Protecting children involves professionals understanding and assessing new communication and socialization patterns in which children participate. It is not merely a question of learning how to use digital platforms; it is also a question of professional development and new ways of working with children in public care when meetings between people increasingly take place digitally.publishedVersio

How to start a Focus Group: Using cartoons in adult focus groups to discuss consumers feedback expectations in food service settings.

This article tests a novel method for starting focus groups quickly while ensuring that respondents understand the topic of interest for the study. To kick start the focus groups, a cartoon style illustration was developed. The cartoons depicted various food service experience from start to finish. Respondents first task was to indicate when it was natural for them to give feedback on the food service experience. The results show that the cartoons allowed respondents to quickly understand the topic, which in turn gave them confidence to contribute with relevant information from the start. In addition, respondents used the framework of the illustrations to keep the discussion focused on the topic throughout. We generally find that it is getting harder to find people who are willing to commit to 2 h focus groups after work. For us to find participants who are willing to take part, we are having to be much more time conscious, this method allows for shorter groups without losing valuable information. The method has a potential when it comes to testing food products as well as in the development of dishes for gastronomic experiences. It may also be a good tool to use in sensory focus groups. The use of comics to start off the focus group worked well in the context of a non-sensitive topic, in this case food service experiences. Further research could explore using this method for more sensitive topics.publishedVersio

Heidi Hansen describes how people create meaning through stories. An event or episode does not carry any meaning in itself but is assigned meaning through its constructed relations to other events and the way it is put into words. Stories create memory as well as oblivion and thus influence the identity of the organization as well as the identity of its members. Therefore it is a central management discipline to try to influence the dominant stories told in the organization.&nbsp

RNA assemblages orchestrate complex cellular processes

Eukaryotic mRNAs are monocistronic, and therefore mechanisms exist that coordinate the synthesis of multiprotein complexes in order to obtain proper stoichiometry at the appropriate intracellular locations. RNA‐binding proteins containing low‐complexity sequences are prone to generate liquid droplets via liquid‐liquid phase separation, and in this way create cytoplasmic assemblages of functionally related mRNAs. In a recent iCLIP study, we showed that the Drosophila RNA‐binding protein Imp, which exhibits a C‐terminal low‐complexity sequence, increases the formation of F‐actin by binding to 3′ untranslated regions of mRNAs encoding components participating in F‐actin biogenesis. We hypothesize that phase transition is a mechanism the cell employs to increase the local mRNA concentration considerably, and in this way synchronize protein production in cytoplasmic territories, as discussed in the present review
