    The epidemiology and outcomes of central nervous system infections in Far North Queensland, tropical Australia; 2000-2019

    Background: The epidemiology of central nervous system (CNS) infections in tropical Australia is incompletely defined. Methods: A retrospective study of all individuals in Far North Queensland, tropical Australia, who were diagnosed with a CNS infection between January 1, 2000, and December 31, 2019. The microbiological aetiology of the infection was correlated with patients' demographic characteristics and their clinical course. Results: There were 725 cases of CNS infection during the study period, meningitis (77.4%) was the most common, followed by brain abscess (11.6%), encephalitis (9.9%) and spinal infection (1.1%). Infants (24.3%, p<0.0001) and Aboriginal and Torres Strait Islander Australians (175/666 local residents, 26.3%, p<0.0001) were over-represented in the cohort. A pathogen was identified in 513 cases (70.8%); this was viral in 299 (41.2%), bacterial in 175 (24.1%) and fungal in 35 (4.8%). Cryptococcal meningitis (24 cases) was diagnosed as frequently as pneumococcal meningitis (24 cases). There were only 2 CNS infections with a S. pneumoniae serotype in the 13-valent pneumococcal vaccine after its addition to the National Immunisation schedule in 2011. Tropical pathogens-including Cryptococcus species (9/84, 11%), Mycobacterium tuberculosis (7/84, 8%) and Burkholderia pseudomallei (5/ 84, 6%)-were among the most common causes of brain abscess. However, arboviral CNS infections were rare, with only one locally acquired case-a dengue infection in 2009-diagnosed in the entire study period. Intensive Care Unit admission was necessary in 14.3%; the overall case fatality rate was 4.4%. Conclusion: Tropical pathogens cause CNS infections as commonly as traditional bacterial pathogens in this region of tropical Australia. However, despite being highlighted in the national consensus guidelines, arboviruses were identified very rarely. Prompt access to sophisticated diagnostic and supportive care in Australia's well-resourced public health system is likely to have contributed to the cohort's low case-fatality rate

    Golf and Skin Health: A Narrative Review

    OBJECTIVES Recent studies have reported that playing golf has overall physical and mental health benefits. Through being placed in the sun for extended periods of time, the golfing population faces an increased risk of ultraviolet radiation (UVR) associated skin problems, most importantly skin cancer. Our aim was to collate and summarise current literature on the relationship between golf and skin cancer. METHODS We searched multiple web-based, health-focused databases (Medline, Embase, Cinahl, Web of science, Scopus, Sportdiscus, UK clinical trials, Current controlled trials and Prospero) to identify records. Relevant papers were critically appraised and reported using a descriptive analysis. RESULTS A total of 11 studies are included in the review. Golf is associated with relatively more UVR exposure than other outdoor activities such as fishing, tennis, pool swimming, cricket, gardening, ‘sun-worshipping’ and sailing. Calculated relative risk of non- melanoma skin cancer, based on cumulative UV exposure, is higher in golfers than non-golfers. Mid morning tee-times or playing golf between 11am -3pm associated with highest UVR exposures. Golfers can be exposed to potentially harmful levels of UVR during play in winter at some latitudes. Vertex (scalp), shoulders, back, back of neck and posterior arms exposed to more UVR than front of body during play. Common golf attire is of limited value in protecting the neck. Targeted skin cancer skin cancer prevention and early detection campaigns are well received among the golfing population. CONCLUSIONS Overall, playing golf is associated with longevity and both physical and mental health benefits. Studies to date indicate that golfers (professionals > recreational players) are exposed to potentially harmful levels of UVR during play, but the true morbidity/mortality associated with this exposure is not known. Playing golf should be encouraged in all age groups, though golfers, the golf industry and policy makers should act to minimize the harmful effects of UVR exposure, and have they have ability to also be hugely influential on an important public health message

    IMPROVING ACCESS TO FRESH PRODUCE FOR PREDOMINANTLY NON-HISPANIC BLACK NEIGHBORHOODS IN DURHAM COUNTY, NC THROUGH THE MOBILE FARMER’S MARKET COLLABORATIVE PROGRAM

    The infrastructure of a community’s physical surroundings and environment dictate their access to fresh, healthy foods. Compared to the North Carolina average of 24%, more than 30% of Durham County residents are burdened by poor grocery store access (Food Security and Nutrition, n.d.). This disparity is reflected in the higher rates of food insecurity and diet-modifiable chronic disease among racial and ethnic minority groups (Durham County, 2021; Osei & Gaillard, 2017). This proposal aims to increase access to fresh produce for residents of primarily non-Hispanic Black neighborhoods through the creation and maintenance of a Mobile Farmer’s Market Collaborative, rotating weekly between predominantly non-Hispanic Black neighborhoods, churches, hospitals, and other community locations. The cost of produce available through the Collaborative will be subsidized to address the financial barriers of healthy eating, and cooking demonstrations in conjunction with nutrition education will be provided to foster long-term health maintenance for beneficiaries.     Capture and inactivation of viral particles from bioaerosols by electrostatic precipitation.

    Infectious viral particles in bioaerosols generated during laparoscopic surgery place staff and patients at significant risk of infection and contributed to the postponement of countless surgical procedures during the COVID-19 pandemic causing excess deaths. The implementation of devices that inactivate viral particles from bioaerosols aid in preventing nosocomial viral spread. We evaluated whether electrostatic precipitation (EP) is effective in capturing and inactivating aerosolised enveloped and non-enveloped viruses. Using a closed-system model mimicking release of bioaerosols during laparoscopic surgery, known concentrations of each virus were aerosolised, exposed to EP and collected for analysis. We demonstrate that both enveloped and non-enveloped viral particles were efficiently captured and inactivated by EP, which was enhanced by increasing the voltage to 10kV or using two discharge electrodes together at 8kV. This study highlights EP as an effective means for capturing and inactivating viral particles in bioaerosols, which may enable continued surgical procedures during future pandemics

    Parents’ experiences and perceptions of the acceptability of a whole-hospital, pro-active electronic pediatric early warning system (the DETECT study): A qualitative interview study

    BackgroundFailure to recognize and respond to clinical deterioration in a timely and effective manner is an urgent safety concern, driving the need for early identification systems to be embedded in the care of children in hospital. Pediatric early warning systems (PEWS) or PEW scores alert health professionals (HPs) to signs of deterioration, trigger a review and escalate care as needed. PEW scoring allows HPs to record a child's vital signs and other key data including parent concern.AimThis study aimed to explore the experiences and perceptions of parents about the acceptability of a newly implemented electronic surveillance system (the DETECT surveillance system), and factors that influenced acceptability and their awareness around signs of clinical deterioration and raising concern.MethodsDescriptive, qualitative semi-structured telephone interviews were undertaken with parents of children who had experienced a critical deterioration event (CDE) (n = 19) and parents of those who had not experienced a CDE (non-CDE parents) (n = 17). Data were collected between February 2020 and February 2021.ResultsQualitative data were analyzed using generic thematic analysis. Analysis revealed an overarching theme of trust as a key factor that underpinned all aspects of children's vital signs being recorded and monitored. The main themes reflect three domains of parents' trust: trust in themselves, trust in the HPs, and trust in the technology.ConclusionParents' experiences and perceptions of the acceptability of a whole-hospital, pro-active electronic pediatric early warning system (The DETECT system) were positive; they found it acceptable and welcomed the use of new technology to support the care of their child
