    La semiótica de la historia no contada en la novela «The Crow’s Leg» de Yahya Amqasem

    Through our study of the subject of the semiotics of the untold story in the novel “The Crow’s Leg”, we aimed to show the image of the religious, moral and political prohibitions through the intersection of discourses with the social and cultural levels highlighted by the semantic context in the discourse. The study used the semiotic approach that highlights the mechanisms of the narrative discourse operating on the semiotic dimensions and the significance of the untold story through the strategy of the raised issues (religion–politics–ethics). At the end of our study we get to the importance of the semiotic dimensions that extend from the signifier to the signified by devoting the untold story theme more deeply through the significance of the religious prohibition , in which we focus on the centrality of women and the mixing with men in many domains that are considered by the principality as forbidden by Sharia, which we dealt with as a religious prohibition. Also in this study we came to the essence of the semiotic concepts and what is content through methodological tools that study the semantic world of the novel and its basic minor moral units that are only achieved in its relation with the other elements and which are based on a common semantic axis in the untold story and its interpretations in the body of the novel.A través de nuestro estudio sobre el argumento de la semiótica de la historia no contada en la novela «The Crow’s Leg», pretendíamos mostrar la imagen de las prohibiciones religiosas, morales y políticas mediante la interacción de discursos con los niveles sociales y culturales destacados por el contexto semántico en el discurso. El estudio utilizó el planteamiento semiótico que señala los mecanismos del discurso narrativo que intervienen en las dimensiones semióticas y el significado de la historia no contada a través de la estrategia de los temas planteados (religión–política–ética). Al finalizar nuestro estudio concluimos la importancia que tienen las dimensiones semióticas que se extienden desde el significante al significado centrándonos de manera más profunda en el tema de la historia no contada a través de la significación de la prohibición religiosa, en la que enfocamos la centralidad de la mujer y la interactuación con hombres en muchos dominios que son considerados por el principado como vetados por la Sharía, que abordamos como una prohibición religiosa. En este estudio también nos referimos a la esencia de los conceptos semióticos y a la definición de contenido mediante herramientas metodológicas que estudian el mundo semántico de la novela y sus unidades morales menores y fundamentales que solo se alcanzan en su relación con los demás elementos y que se basan en un eje semántico común en la historia no contada y sus interpretaciones en el cuerpo de la novela

    Knowledge, Attitude and Practice of Hepatitis B Virus Infection among Dental Students and Interns in Saudi Arabia

    Hepatitis B virus (HBV) is a worldwide healthcare problem. Dental health care professionals are at a high risk of infection by HBV. The present study investigated the knowledge, attitude and practice of HBV infection among Saudi dental students and Interns in Saudi Arabia. This was a questionnaire-based cross-sectional study. A self-administered questionnaire consisting of questions on students? knowledge, attitudes, and practices regarding HBV was used. Data of 420 participants were analyzed using SPSS (Statistical Package for Social Studies) version 22.0. The response rate was 84%. Overall, the participants showed fair level of knowledge about HBV, with significant differences between final year students and the interns. Also, the subjects showed negative attitude toward HBV patients. The vast majority reported always wearing gloves (97.9%), gowns (92.1%), face masks (89.2%), disposable caps (87.1%) and protective eye wear (80.9%). The majority of participants (91.4%) had been vaccinated against HBV. However, only 41% completed the recommended 3 doses of the vaccine. These unsatisfactory findings emphasize the necessity of continued education about HBV in order to improve knowledge, attitudes, and practices of dental students and Interns regarding HBV

    Autosomal Short Tandem Repeat (STR) Variation Based on 15 Loci in a Population from the Central Region (Riyadh Province) of Saudi Arabia

    INTRODUCTION: The small size of Short Tandem Repeats (STRs), their ubiquitous genome-wide distribution and polymorphic nature enhances their value in human forensic/population genetics applications. OBJECTIVES: This study aims to investigate the short tandem repeat variation based on 15 loci in a population from the central region of Saudi Arabia. METHODS: Allele frequency variation for 15 Short Tandem Repeat (STR) loci was examined in 190 unrelated Saudi volunteers. Results: This study summarizes the allele distribution in the Saudi population and compares them to other populations located in Asia, Africa, the Middle East and Europe. The standard forensic parameters of Observed Hetrozygosity (Ho), Expected Heterozygosity (He) and Gene Diversity Index (GD) were determined for the following 15 STR loci: D8S1179, D21S1, D7S820, CSF1PO, D3S1358, TH0, D13S317, D16S539, D2S1338, D19S433, vWA, TPOX, D18S5, D5S818 and FGA. The most frequent alleles in the Saudi population were: 8 repeats (0.558) at TPOX, 12 (0.411) at D13S317, 12 (0.385) at CSF1PO, 11 (0.382) at D16D539 and 10 (0.358) at D7S820. The 15 markers utilized in this study are highly informative as evidenced by their high power of discrimination (PD) values with D2S1338, D19S433 and FGA having the highest PD values. The relationship between the Saudi population and other geographically distributed populations, assessed by a Multidimensional Scaling (MDS) plot, showed that the Saudi population clustered with groups from Yemen, Iraq, Qatar, Oman and Bahrain. CONCLUSION: TPOX, D13S317, CSF1PO, D16D539 and D7S820 markers were found suitable for forensic analysis, paternity testing and can also be used for chimerism study after allogenic bone marrow transplantation for Saudi population. On the other hand, the population admixture with other ethnic origins might explain the variable degree of genetic distances of this population and other Arab-related groups

    Role of L- glutamine and crizanlizumab in sickle cell anaemia painful crisis reduction

    BackgroundPatients with sickle cell disease, frequently ‎ suffer from intense painful episodes. Till recently hydroxyurea was the only available medical therapy that approved for reduction of painful episodes.AimsTo summarize the available data from randomized controlled trials that aim to evaluate the efficacy of newly approved L-‎glutamine‎ (alters redox state of red blood cells ‎‎[RBCs]) ‎and ‎crizanlizumab (‎(anti-P-selectin)‎)‎ ‎on vaso-occlusive episodes in Sickle cell disease ‎ patients.Methods PubMed, ‎Google Scholar, and EBSCO ‎ databases were ‎‎systematically search for relevant articles. The terms ‎ ‎ ‎ L-glutamine, sickle cell disease, sickle cell ‎anaemia,‎ ‎‎crizanlizumab ‎and vaso-occlusive episodes‎ were used.Results Out of Four-hundred seventy-two records, only three fulfilled the inclusion criteria. Two trials were aimed to evaluate the efficacy of L-glutamine therapy on the frequency of painful crises in sickle cell anaemia patients. Both studies showed that L-glutamine therapy significantly reduce the frequency of VOEs. Only one trial examined the ability of crizanlizumab on VOEs reduction, and showed crizanlizumab successful reduce the occurrence of VOEs.‎ConclusionNewer agent ‎with different mechanism of action, such as ‎L-glutamine, ‎and crizanlizumab may consider if ‎hydroxyurea not effective or not ‎tolerable

    A High Dose, Not Low Dose, of Vitamin D Ameliorates Insulin Resistance in Saudi Women

    Vitamin D has been traditionally seen to be mainly involved in the regulation of bone homeostasis. However, vitamin D has also been clinically linked to various diseases, including metabolic syndrome. The aim of this study was to examine the effect of low and high doses of a vitamin D supplement on the serum levels of 25(OH)D3 and insulin resistance. A total of 120 females were recruited in this study and supplemented weekly with 25,000 IU vitamin D or 50,000 IU vitamin D for three months. Anthropometric measurements were taken at the beginning of the study. Blood samples were collected at the beginning of the study to determine the baseline of the clinical variables and collected again after three months. Insulin resistance was measured using Homeostatic Model Assessment for Insulin Resistance (HOMA-IR). After vitamin D supplementation, a non-significant increase was observed in the serum levels of 25(OH)D3 in the group treated with a low dose of vitamin D (LDVD) and a highly significant increase was seen in the group treated with a high dose of vitamin D (HDVD). In the group treated with a higher dose (HDVD), a significant improvement in insulin sensitivity was observed. The high dose of vitamin D (50,000 IU) supplementation was more effective in both correcting the blood levels of vitamin D and improving the sensitivity of insulin

    Genetic susceptibility for celiac disease is highly prevalent in the Saudi population

    Background/Aim: To determine the frequency of celiac disease (CD)-predisposing human leukocyte antigen (HLA)-DQ genotypes in the Saudi population, where the prevalence of CD is 1.5% as recently reported in a mass screening study. Patients and Methods: In a cross-sectional population-based study, a total of 192 randomly selected healthy school children (97 females, mean age 10.5 ± 2.2 years, all negative for tissue transglutaminase-IgA) were typed for D QA1 and D QB1 genes by polymerase chain reaction sequence–specific oligonucleotide probes. Results: Of the 192 children, 52.7% carried the high-risk CD-associated HLA-DQ molecules: homozygous DQ2.5 ( 2.6%), DQ2.5/DQ2.2 ( 4.7%), heterozygous DQ2.5 ( 28.15%), homozygous DQ8 ( 4.2%), DQ8/DQ2.2 ( 3.6%), and double dose DQ2.2 ( 9.4%). Low-risk CD-associated HLA-DQ molecules (single dose DQ2.2 and heterozygous DQ8) constituted 3.6% and 9.4%, respectively. Among the very low–risk groups, individuals lacking alleles that contribute to DQ2/DQ8 variants (33.5%), 13.5% carried only one of the alleles of the high-risk HLA-DQ2.5 heterodimer called “half-heterodimer” (HLA-DQA1*05 in 12% and HLA-DQB1* 02 in 1.5%), and 20.8% lacked all the susceptible alleles (DQX.x). Gender distribution was not significantly different among the CD-risk groups. Conclusion: We report one of the highest frequencies of CD-predisposing HLA-DQ genotypes among healthy general populations (52.7%) worldwide, which might partly explain the high prevalence of CD in the Saudi community

    Oxidized Natural Biopolymer for Enhanced Surface, Physical and Mechanical Properties of Glass Ionomer Luting Cement

    This laboratory investigation aimed to synthesize and characterize micron-sized Gum Arabic (GA) powder and incorporate it in commercially available GIC luting formulation for enhanced physical and mechanical properties of GIC composite. Oxidation of GA was performed and GA-reinforced GIC in 0.5, 1.0, 2.0, 4.0 & 8.0 wt.% formulations were prepared in disc-shaped using two commercially available GIC luting materials (Medicem and Ketac Cem Radiopaque). While the control groups of both materials were prepared as such. The effect of reinforcement was evaluated in terms of nano hardness, elastic modulus, diametral tensile strength (DTS), compressive strength (CS), water solubility and sorption. Two-way ANOVA and post hoc tests were used to analyze data for statistical significance (p < 0.05). FTIR spectrum confirmed the formation of acid groups in the backbone of polysaccharide chain of GA while XRD peaks confirmed that crystallinity of oxidized GA. The experimental group with 0.5 wt.% GA in GIC enhanced the nano hardness while 0.5 wt.% and 1.0 wt.% GA in GIC increased the elastic modulus compared to the control. The CS of 0.5 wt.% GA in GIC and DTS of 0.5 wt.% and 1.0 wt.% GA in GIC demonstrated elevation. In contrast, the water solubility and sorption of all the experimental groups increased compared to the control groups. The incorporation of lower weight ratios of oxidized GA powder in GIC formulation helps in enhancing the mechanical properties with a slight increase in water solubility and sorption parameters. The addition of micron-sized oxidized GA in GIC formulation is promising and needs further research for improved performance of GIC luting composition

    Lessons Learned from the COVID-19 Outbreak as Experienced by ICU Nurses: Manifest Qualitative Content Analysis

    From the moment the World Health Organization (WHO) declared the coronavirus (COVID-19) a global pandemic, intensive care unit (ICU) nurses struggled to care for the COVID-19 patients. As the pandemic became increasingly critical, the task of daily care for critically ill patients fell upon the shoulders of ICU nurses. Understanding the lived experience of ICU nurses, as well as their perception of the experience, is important to identify key lessons to further improve ICU nurses’ psychological well-being and resilience. This study explored the lived experiences of ICU nurses who cared for COVID-19 patients using exploratory manifest qualitative content analysis. The findings of this study revealed that these ICU nurses offer important information concerning the crucial role of nurses’ family support and religious–spiritual practices in helping them to maintain well-being and cope with the intense situations caused by the pandemic. The lessons emerging from the study findings show that family support and religious–spiritual practices are resources for coping and resilience in times of future pandemics. This implies that ICU nurses who provide care during stressful emergency surges require relief by nurses working in other areas to provide them the opportunity to rest, reconnect with family and become emotionally re-energized
