    Thermo-mechanical large deformation responses of Hydrogenated Nitrile Butadiene Rubber (HNBR): Experimental results

    AbstractA comprehensive study on the large uniaxial deformation responses of two elastomers, Hydrogenated Nitrile Butadiene Rubbers (HNBR A and HNBR B), over wide ranges of strain rates (10-4⩽ε˙⩽5×103s-1) and temperatures (348⩽T⩽450K) are presented. The material is found to be non-linearly dependent on strain-rate and temperature. The large deformation responses of HNBR are determined to be almost purely viscoelastic. It is observed that, the instantaneous stress drop during the stress relaxation is dependent on the strain-rate. The relaxation and creep responses during the equilibrium state (a state where there is no drop in stress during relaxation and no increase in strain during creep) are dependent on strain/stress level at which the phenomenon is started

    Yield of esophagogastroduodenoscopy and colonoscopy in cancer of unknown primary

    Objectives: Carcinoma of unknown primary origin (CUP) is heterogeneous group of cancers. Role of gastrointestinal (GI) endoscopy in this entity is under investigated. Aim of this study was to evaluate yield of Colonoscopy and Esophagogastroduodenoscopy (EGD) in localizing primary tumor in patients with CUP. METHODOLOGY: Patients with histopathologically proven CUP who underwent colonoscopy / EGD to find the primary tumor from December 2009 to December 2011 were included in the study. Abdominal symptoms and cytokeratin (CK) 7 and 20 markers were correlated with presence of primary in GI tract. Results: After giving informed consent 86 patients were included in final analysis. All patients underwent colonoscopy while 60(70%) got EGD along with colonoscopy. Mean age was 55.10 +/-11.94 years with 52(60%) male. Abdominal symptoms were present in 50%. CK7+/CK20- in 34(40%); CK7-/CK20+ in 2(2%) while CK7+/20+ in 7(8%) of metastatic tumor samples. Liver was metastatic site in 47(55%), Lymph node 12(14%) and Ascites in 8(9%). Endoscopy detected primary in 6 (7%) patients with 3 each in stomach and colon. No association of abdominal symptoms and cytokeratin markers was found with presence of GI primary site. CONCLUSION: Yield of localizing primary lesion in the GI tract by pan-endoscopy was limited. Abdominal symptoms and cytokeratin markers do not predict presence of gastrointestinal malignancies

    Hepatocellular carcinoma is a serious human disease with fatal consequences. The most distressing aspect of hepatocellular carcinoma is the limited improvement in mortality (mortality rate of more than 90%). At present, the underlying molecular mechanisms are not well understood and treatment options are often of limited efficacy. This review presents our current understanding of the burden of hepatocellular carcinoma on human health, pathogenesis and pathophysiology, and molecular mechanisms associated with the disease, as well as our knowledge of the physical barriers, cellular mechanisms and molecular elements that may be targets for therapeutic interventions and/ or the development of preventative measures. As the proposed findings present a major risk to public health, it is hoped that robust intervention measures will be introduced for aflatoxins monitoring and reduction in diet

    Ruptured testicular abscess: a rare complication of epididymo-orchitis

    Epididymo-orchitis is a medical condition characterised by inflammation of epididymis and testis which is a common urological diagnosis encountered by healthcare professionals. Misdiagnosis and under treatment can lead to poor outcome such as infarction and abscess formation. We presented a 33-year-old man, with no known medical illness, presented with one-week history of left testicular pain, swelling and fever. He was commenced on Amoxicillin by his general practitioner. Physical examination showed left swollen and tender testis and normal right testis. Investigations showed an elevated white cell count (WCC) 41.3x103 /L and colour doppler ultrasound (USG) testes showed left epididymo-orhitis. He was admitted and treated with IV Unasyn® (Ampicillin+Sulbactam) for 5 days. His symptoms improved, and he discharged home well with one week oral Unasyn®. He represented 2 weeks later with worsening left scrotal swelling and pain. Clinical examination found a swollen, tender and erythematous scrotum with 1x1cm defect in the left scrotum with purulent discharge. He underwent operation incision and drainage of left scrotum and left orchidectomy. Post-operatively he recovered well. Histopathology reported as epididimoorchitis with abscess, necrosis and perforation. As a conclusion, testicular rupture and abscess formation secondary to epididymo-orchitis is an uncommon reported complication. Immediate recognition and treatment is crucial as a delayed diagnosis can lead to sepsis and infertility

    In recent years, there is an increased in floor vibration problems of structures like residential and commercial building. Vibration is defined as a serviceability issue related to the comfort of the occupant or damage equipment. Human activities are the main source of vibration in the building and it could affect the human comfort and annoyance of residents in the building when the vibration exceed the recommend level. A new building, Madrasah Tahfiz located at Yong Peng have vibration problem when load subjected on the first floor of the building. However, the limitation of vibration occurs on building is unknown. Therefore, testing is needed to determine the vibration behaviour (frequency, damping ratio and mode shape) of the building. Heel-drop with pace 2Hz was used in field measurement to obtain the vibration response. Since, the heel-drop test results would vary in light of person performance, test are carried out three time to reduce uncertainty. Natural frequency from Frequency Response Function analysis (FRF) is 17.4Hz, 16.8, 17.4Hz respectively for each test

In reality, recent advances in the Internet of Things (IoT) have created new opportunities by solving a number of critical issues that are allowing innovations for smart cities as well as the creation and computerization of cutting-edge services and applications for the many city partners. In order to further the development of smart cities toward compelling sharing and connection, this study will explore the information innovation in smart cities in light of the Internet of Things (IoT) and cloud computing (CC). IoT data is first collected in the context of smart cities. The data that is gathered is uniform. The Internet of Things, which enables gadgets to connect with one another mostly without human involvement, is made possible by AI. In line with this, The Ad Hoc Routing Function (ARF) AI computation is used for multi-rule simplification, the use of Adaptive Cloud Computing Virtual Machine Asset Allotment Technique (ACC-VMRA) is advised. To confirm its viability, the applied developments of IoT and CC in smart cities is examined and duplicated. The experiment results show that the recommended enhancement calculation is more productive than other currently used methods

    Correlation between maternal and neonatal blood Vitamin D level; A cross sectional study of 416 participants visiting a tertiary care hospital in Pakistan

    In Pakistan there is limited evidence for the levels and relationship of 25 (OH) Vitamin D [25(OH)D] status in pregnant women and their newborns while the association between maternal 25(OH)D and newborn anthropometric measurements remains unexplored. Sociodemographic data was collected from 213 pregnant mothers during their visit to a tertiary care hospital at the time of childbirth. Anthropometric measurements were performed on all mothers and their newborns and blood samples collected from both for 25(OH)D levels. Participants were classified into two groups according to their 25(OH)D status: Sufficient (25(OH)D ≥50nmol/l) and Deficient (25(OH)D 0.05). Our study shows a high prevalence of 25(OH)D deficiency in pregnant women and their newborns and a strong positive association between maternal and newborn 25(OH)D levels. Findings of this study indicate the importance of maintaining sufficient 25(OH)D levels during pregnancy
