    Profil de la néphropathie diabétique à l’Hôpital Général de Référence Nationale de N'Djamena (Tchad)

    Introduction: Le diabète sucré constitue un véritable problème de santé et ses complications touchent plusieurs organes dont les reins. Un diagnostic précoce de la néphropathie diabétique permet de prendre en charge les patients plus efficacement et de façon multidisciplinaire, de retarder sa progression vers l'insuffisance rénale chronique. Peu d'études ont été faites en Afrique dans ce domaine. Au Tchad, nous ne disposons pas de données statistiques sur l'atteinte rénale liée au diabète. C'est pourquoi nous nous proposons d'étudier la néphropathie diabétique avec pour objectifs d'analyser le profil de la néphropathie diabétique des patients de l'hôpital général de référence nationale de N'Djamena. Méthodes: Nous avions mené une étude transversale descriptive à l'Hôpital Générale de Référence Nationale de N'Djamena d'avril à septembre 2012. Etaient inclus dans l'étude tous les patients diabétiques hospitalisés ou suivis dans les services de néphrologie et d'endocrinologie. Résultats: Il y avait 54 cas de néphropathie diabétique sur 181 patients soit 29,80%. L'âge de plus de 50 ans représentait 87%, le sexe masculin dominait avec 67%, la durée d'évolution du diabète au moment de la découverte de la néphropathie était de 10,25 ans, la pression artérielle moyenne était de 138 mm Hg, l'HbA1C était supérieur à 6,5% dans 74,5%, l'insuffisance rénale terminale était retrouvée dans 26,90%, une protéinurie à 2,65 g/24 h était détectée dans 60,30%. 57,4% des patients avaient une rétinopathie diabétique au stade III. Conclusion: Au Tchad, la fréquence de la néphropathie diabétique est de l'ordre de 29,83%. C'est une affection qui touche les hommes avec une moyenne d'âge de 58,7 ans. Le diabète de type 2 est le plus fréquent avec 90,7% des cas. Les facteurs de risque mal contrôlés pouvaient conduire la néphropathie diabétique vers une altération de la fonction rénale notamment l'HTA (70,8%), un déséquilibre glycémique (66,7%) et une protéinurie (62,5%).Pan African Medical Journal 2016; 2

    Leptospirosis and its prevention: knowledge, attitude and practice of urban community in Selangor, Malaysia

    Background: Leptospirosis is still endemic in Malaysia and has been prevalent in Selangor where cases have been underreported. Primarily, this was due to lack of awareness in the urban community in this region. This study determined the knowledge, attitude and preventive practice (KAP) of leptospirosis, and identified the significant predictors influencing KAP among urban community in Hulu Langat, Selangor. Methods: This cross-sectional study was conducted from 2015 to 2017 using validated questionnaire. Sampling methods included multistage cluster sampling, followed by simple random sampling to obtain 315 respondents. Descriptive analysis was performed to determine the KAP while χ 2 and the subsequent logistic regression analysis were carried out to identify associations and predictors between variables. Results: Respondents were mainly Malaysian Bumiputra with a mean (Standard Deviation (SD)) age of 32.5 (13.0) years. Of 315 respondents, 80.3% (n = 253) had poor knowledge, 87.0% (n = 274) had good attitude, and 81.3% (n =256) showed unacceptable practice towards leptospirosis and its prevention. Regression analysis identified age as the sole predictor influencing good knowledge (AOR 2.388; 95% CI = 1.298, 4.396; p = 0.005). Education level (AOR 2. 197; 95% CI = 1.109, 4.352; p = 0.024) was also noted as the significant predictor influencing the overall practice. Conclusions: The urban community in Selangor showed a positive attitude in waste management despite having little knowledge regarding the disease itself. The study also discovered inadequacy in preventive practice, hence marking the importance of the proper integration of knowledge and attitude into forming an acceptable practice to reduce transmission of Leptospira among urban population in Malaysia

    Eradicating poverty and hunger, providing health services and quality education through Islamic social finance: Qatar as a model

    This paper aimed to address four important goals of the sustainable development goals, which are the eradication of poverty and hunger, the provision of health services and quality education. It also signifies the importance of Islamic social finance and its potential in achieving these goals with different examples in Qatar. The inductive approach was relied on, and the study showed that the efforts of Qatar to eradicate poverty and hunger since the past decade have borne fruit through the cooperation of all sectors and several policies and institutions that included Islamic social finance institutions, and that Islamic financial institutions actively participate in efforts to provide health services. This paper also found that Qatar provides quality education to all citizens with the aim of building generations aware of a culture of peace ،and the country's government is also promoting means of benefiting from science and technology. However, Islamic social finance institutions still need to expand the horizons of their support for education and health services through the dissemination of public knowledge on tools such as Sukuk (Islamic bonds), in addition to the fact that challenges still exist regarding the implementation of some national policies to achieve integration between different sectors and Islamic social finance institutions

    The effects of self-polymerized polydopamine coating on mechanical properties of polylactic acid (PLA)–kenaf fiber (KF) in fused deposition modeling (FDM)

    This research examines the impact of self-polymerized polydopamine (PDA) coating on the mechanical properties and microstructural behavior of polylactic acid (PLA)/kenaf fiber (KF) composites in fused deposition modeling (FDM). A biodegradable FDM model of natural fiber-reinforced composite (NFRC) filaments, coated with dopamine and reinforced with 5 to 20 wt.% bast kenaf fibers, was developed for 3D printing applications. Tensile, compression, and flexural test specimens were 3D printed, and the influence of kenaf fiber content on their mechanical properties was assessed. A comprehensive characterization of the blended pellets and printed composite materials was performed, encompassing chemical, physical, and microscopic analyses. The results demonstrate that the self-polymerized polydopamine coating acted as a coupling agent, enhancing the interfacial adhesion between kenaf fibers and the PLA matrix and leading to improved mechanical properties. An increase in density and porosity was observed in the FDM specimens of the PLA–PDA–KF composites, proportional to their kenaf fiber content. The enhanced bonding between kenaf fiber particles and the PLA matrix contributed to an increase of up to 13.4% for tensile and 15.3% for flexural in the Young’s modulus of PLA–PDA–KF composites and an increase of up to 30% in compressive stress. The incorporation of polydopamine as a coupling agent in the FDM filament composite led to an improvement in tensile, compressive, and flexural stresses and strain at break, surpassing that of pure PLA, while the reinforcement provided by kenaf fibers was enhanced more by delayed crack growth, resulting in a higher strain at break. The self-polymerized polydopamine coatings exhibit remarkable mechanical properties, suggesting their potential as a sustainable material for diverse applications in FDM

    A comprehensive review of barriers to a functional Zakat system in Nigeria: what needs to be done?

    Purpose: Muslims in Nigeria, like in many Muslim-majority countries, are striving to have a functional Zakat system in search of solutions to the perennial problem of poverty and its damning consequences. Nevertheless, there are still unsettled concerns arising from the current and widespread implementation of dissimilar (diverse) approaches to the Zakat system in various parts of the country. The purpose of this paper is to review comprehensively what are the hindrances of a vibrant Zakat system and how far the identified impediments may affect the system in the Nigerian context.Design/methodology/approach:The methodology adopted is the review of extant relevant literature in the field of scholarly publications.Findings: The findings of this study revealed that the fragmented implementation of the Zakat system within the context of the Nigerian democratic system of government lead to the weak governance with respect to law, administration and management; lack of a generally accepted fatwa from the Muslims scholars (Ulamas); absence of Zakat accounting standard; and low compliance behavior are the major barriers that require the attention of government and other stakeholders such as the traditional leaders, the accounting regulatory bodies, the Ulamas, as well as the economic and accounting researchers.Practical implications: It was recommended that the stakeholders should make concerted efforts toward ensuring success of the Zakat system for attaining salvation in the hereafter and for social security, as well as economic prosperity. Originality/value: The paper is the first paper that comprehensively reviews previous literature in the Zakat environment on factors that become barriers to implement a comprehensive Zakat system in Nigeria
