    A Study on the Feasibility of Subject Authority Control of Web-based Persian Medical Databases: An Iranian Experience

    One of the most important factors challenging the issue of "information storage and retrieval" in the Internet environment is the lack of control on authorities, i.e. subject authority control. The present research aims at examining the feasibility of subject authority control of Persian medical databases available on the Net. Based on research methodology, we have randomly chosen 50 keywords utilized by users searching databases for articles. In the pre-test stage, these keywords were searched through Iranmedex, a database for Persian medical articles. Comparing them with Persian medical thesaurus, those keywords exactly matched to the thesaurus words were entered in a designed database using Microsoft Access software. Then, we entered these authorized keywords in Iranmedex. Findings of new search sessions revealed that control of authorities on the one hand, makes information retrieval more precise and accurate and, on the other prevents false drops. The research findings can be used for modifying the process of information storage and retrieval on the Internet. The research concludes with a model for applying thesauruses as authority control tools for other databases available at Internet

    Biomass estimation of demersal resources in the Persian Gulf and Oman Sea by swept area method

    Regarding to monitor of demersal resources in the Persian Gulf and Oman Sea, and also biomass and CPUA estimation of them, ten research cruises were carried out by using R/V Ferdows-1 equipped with bottom trawl, covering the area from 49º 00´ E in the west (north-west Persian Gulf) to 61º 25´ E in the east (borderline with Pakistan) from 2012 to 2014 The study area was stratified into 17 strata (A to Q) of which 10 strata (A to J) were in the Persian Gulf and 7 strata (K to Q) were in the Oman Sea, covering the depths of 10-50 m in the Persian Gulf and 10-100 m in the Oman Sea. A total of 316 stations were randomly selected and the biomass and CPUA were estimated by swept area method during this three years period, the strat A and B weren’t covered. The comparison between two regions indicated that the percentage of density of demersal fishes in the Persian Gulf during years 2012, 2013 and 2014 were 1.0, 1.4 and 1.6 times more than the Oman Sea and totally 50-60% of total biomass was found for the Persian Gulf. Also a comparison among 17 strata the highest biomass was found for K region (Sirik to Jask) in the Oman Sea in 2012 & 2014; and C region (Genaveh to Bordkhoon) in 2013 in the Persian Gulf. The same comparison was done for CPUA of commercial, non-commercial and total in both water bodies and it was found that in years 2012 to 2014 the region K (Sirik to Jask) in the Oman Sea and Stratum Q (Bersi to Gwatr) had the highest value of CPUA. On the contrary, the stratum M (Biahi to Galak estuary) showed the lowest value of biomass for both commercial and non-commercial fishes. With review the mean CPUA in different depth layers for years 2012, 2013 and 2014, it was concluded that in the Oman Sea with increasing the depth, the mean CPUA is decreased and the lowest CPUA belongs to depths of 30-50 m The comparison between commercial and non-commercial groups in both ecosystems, it concluded that the density of commercial species were higher than non-commercial ones; and for years 2009, 2010 and 2011 the commercial species consist of 63.4, 65.0 and 59.7 % of total biomass. In all years the Persian Gulf indicated higher values than the Oman Sea. The most abundant fishes were Rays, Ribbon fishes, Carangids, Grunts, Japanese threadfin bream, Lizardfish and Barracuda for both Persian Gulf and Oman Sea

    Monogenic Primary Immunodeficiency Disorder Associated with Common Variable Immunodeficiency and Autoimmunity

    Background: Common variable immunodeficiency (CVID) is the most frequent primary immunodeficiency disorder mainly characterized by recurrent bacterial infections besides other immunological defects including loss of or dysfunction of B cells and decreased immunoglobulin levels. In this study, our aim is to evaluate clinical, immunological, and molecular data of patients with a primary clinical diagnosis of CVID and autoimmune phenotype with a confirmed genetic diagnosis. Methods: Among 297 patients with CVID, who were registered in the Iranian Primary Immunodeficiency Registry at Children's Medical Center Hospital in Iran, 83 patients have been genetically examined and 27 patients with autoimmunity and confirmed genetic mutations were selected for analysis. Whole-exome sequencing and confirmatory Sanger sequencing methods were used for the study population. A questionnaire was retrospectively filled for all patients to evaluate demographic, laboratory, clinical, and genetic data. Results: In the 27 studied patients, 11 different genetic defects were identified, and the most common mutated gene was LRBA, reported in 17 (63.0) patients. Two patients (7.7) showed autoimmune complications as the first presentation of immunodeficiency. Eleven patients (40.7) developed one type of autoimmunity, and 16 patients (59.3) progressed to poly-autoimmunity. Most of the patients with mono-autoimmunity (n = 9, 90.0) primarily developed infectious complications, while in patients with poly-autoimmunity, the most common first presentation was enteropathy (n = 6, 37.6). In 13 patients (61.9), the diagnosis of autoimmune disorders preceded the diagnosis of primary immunodeficiency. The most frequent autoimmune manifestations were hematologic (40.7), gastrointestinal (48.1), rheumatologic (25.9), and dermatologic (22.2) disorders. Patients with poly-autoimmunity had lower regulatory T cells than patients with mono-autoimmunity. Conclusion: In our cohort, the diagnosis of autoimmune disorders preceded the diagnosis of primary immunodeficiency in most patients. This association highlights the fact that patients referring with autoimmune manifestations should be evaluated for humoral immunity. © 2020 Georg Thieme Verlag. All rights reserved

    Interpretation: By quantifying levels and trends in exposures to risk factors and the resulting disease burden, this assessment offers insight into where past policy and programme efforts might have been successful and highlights current priorities for public health action. Decreases in behavioural, environmental, and occupational risks have largely offset the effects of population growth and ageing, in relation to trends in absolute burden. Conversely, the combination of increasing metabolic risks and population ageing will probably continue to drive the increasing trends in non-communicable diseases at the global level, which presents both a public health challenge and opportunity. We see considerable spatiotemporal heterogeneity in levels of risk exposure and risk-attributable burden. Although levels of development underlie some of this heterogeneity, O/E ratios show risks for which countries are overperforming or underperforming relative to their level of development. As such, these ratios provide a benchmarking tool to help to focus local decision making. Our findings reinforce the importance of both risk exposure monitoring and epidemiological research to assess causal connections between risks and health outcomes, and they highlight the usefulness of the GBD study in synthesising data to draw comprehensive and robust conclusions that help to inform good policy and strategic health planning

    Jaundice as an unusual presentation of Kawasaki disease: A case report

    Introduction: Atypical Kawasaki disease (KD) is a diagnostic dilemma. Acute acalculous cholestasis and liver involvement sometimes indicate atypical presentations of KD. Case Presentation: We report a four-year-old boy with fever, abdominal pain, vomiting, jaundice and mild liver enlargement who gradually developed other signs of KD. He was treated by aspirin, intravenous immunoglobulin and finally methylprednisolone pulse therapy with no sequela. Conclusions: KD should be kept in mind as one of the causes of childhood acalculous cholestasis, abdominal discomfort and prolonged fever. © 2015 Pediartric Infections Research Center

    Effect of Salinity on Germination and Its Relationship with Vegetative growth in Bromus danthoniae Genotypes from Saline and Non-Saline Areas of Iran

    Bromus danthoniae Trin. is an annual grass species that is well adapted to harsh climates and could be considered as an important genetic resources for tolerance to environmental stresses such as salinity. In this study, 24 genotypes collected from Ilam, Kurdistan, Kermanshah (non-saline areas) and West Azerbaijan (saline area: shores of Uremia Salt Lake) provinces of Iran were investigated at the germination stage under salt treatments with concentrations of 0, 60, 120, 180, 240 and 300 mM sodium chloride. Germination percentage, germination rate index, seed vigor, root length, shoot length and seedling fresh and dry weights were measured. In addition, the relationship between the percentage of germination in 300 mM sodium chloride and the survival rate (%) after four weeks in 350 mM sodium chloride at the vegetative stage was evaluated. The results of analysis of variance showed that salinity treatments caused significant reductions in all the studied traits. Genotypic variation and the interaction of genotype × salt treatments were also significant. Genotypes USLN3 and KER4 were found to be the most tolerant and sensitive genotypes to salinity stress, with 13% and 98% reduction in germination percentage at 300 mM NaCl, respectively. Cluster analysis divided the genotypes into three groups, with one group containing only tolerant genotypes from Uremia Salt Lake, another one comprising only sensitive genotypes from non-saline regions, and the third one containing genotypes from both regions. The correlation between the germination percentage and the survival rate at the vegetative stage was not significant, indicating that different mechanisms are, perhaps, responsible for salinity tolerance at the germination and vegetative stages in B. danthoniae

    Seroprevalence of anti-hepatitis a antibody among 1 - 15 year old children in Kashan-Iran

    Background: Worldwide, hepatitis A is a common infection during childhood especially in developing countries. It can cause severe complications in adults and patients with underlying diseases. Objectives: This study was performed to determine the seroprevalence of hepatitis A in 1 - 15 year-old children of Kashan. Patients and Methods: This cross-sectional study was performed on 666 one to fifteen year-old children from health-care centers in Kashan city during 2012. Total antibodies against hepatitis A were measured in sera by enzyme-linked immunosorbent assay (ELISA). Results: Totally, 3.9 of children were seropositive. Mean number of family members was 3.92 ± 0.89. There was no difference in seroprevalence of hepatitis A relative to sex, family size, mean age and age groups. Conclusions: In this city, a great proportion of children are susceptible to hepatitis A and it's complications at an older age. This decrease in seropositivity may be caused by elevated hygien level. According to our results hepatitis A vaccination is recommended at early childhood such as that of other regions where low prevalence of hepatitis A infection is found. © 2013, Kowsar Corp

    Nasal carriage and resistance pattern of multidrug resistant staphylococcus aureus among healthy children in kashan, Iran

    Background: Nasal carriage of Staphylococcus aureus is a substantial source of human infections. Detection and treatment of nasal carriage in children with methicillin-resistant and multidrug resistant S. aureus (MRSA and MDRSA, respectively) may be an important modality in prevention of infections. Objectives: This study determined the prevalence, antibiotic resistance patterns and risk factors for nasal carriage of MDRSA among healthy children. Patients and Methods: This cross-sectional study was carried out on 350 one-month to 14-year-old healthy children in Kashan city, Iran. From all health-care centers, four were chosen by simple random sampling. Nasal samples were cultured in blood agar medium for S. aureus and antibiotic susceptibility profile was determined by disc diffusion and E-test. Risk factors for nasal carriage of MDRSA were also determined. Results: A total of 92 (26.3) S. aureus isolates were obtained, of which 33 (35.9) were MRSA and 27 (29.3) were MDRSA. Of MRSA strains, 19 (70.4) were MDRSA. S. aureus isolates showed 52.2 resistance to cephalothin, 33.7 to co-trimoxazole, 26.1 to clindamycin, 26.1 to ciprofloxacin, 4.3 to vancomycin, and 35.9 to oxacillin. The risk factors for nasal carriage of MDRSA were antibiotic usage during the last three months (P = 0.006), family size of more than four members (P = 0.044), and parental smoking (P = 0.045). Conclusions: MDRSA was not uncommon among healthy children in Kashan and prevention of its spread in the population is judicious. © 2014, Iranian Red Crescent Medical Journal

    Multidrug-resistant Escherichia coli and Klebsiella pneumoniae isolated from patients in Kashan, Iran

    Background: Escherichia coli and Klebsiella pneumoniae are common human pathogens that cause a wide spectrum of infections. Antimicrobial resistance is a basic obstacle in the management of these infections which has different patterns in various regions. Objectives: In this study, the antibiotic resistance patterns and risk factors for multidrug-resistant (MDR) E. coli and K. pneumoniae were determined. Patients and Methods: In this cross-sectional study, a total of 250 isolates (134 E. coli and 116 K. pneumoniae) were collected and antimicrobial resistances to ampicillin, amoxicillin-clavulanic acid, amikacin, gentamycin, ceftriaxone, ceftazidime, ciprofloxacin and imipenem were evaluated by disc diffusion method and confirmed by E-test. Moreover, risk factors for MDR E. coli and K. pneumoniae were also detected. Results: The mean ages of the culture-positive cases of E. coli and K. pneumoniae were 33.39 ± 24.42 and 36.54 ± 24.66 years, respectively (P = 0.31); 137 (54.8) cases were male and 113 (45.2) were female (P = 0.53). Nineteen (14.2) isolates of E. coli and 12 (10.3) isolates of K. pneumoniae were sensitive to all the evaluated antibiotics. The prevalence of MDR E. coli and MDR K. pneumoniae was 50 and 46.6, respectively (P = 0.59). The highest resistance for both strains was to ampicillin and no imipenem resistance was seen. The risk factors for MDR E. coli were admission history during the recent three months (P = 0.043) and antibiotic use in the previous month (P = 0.03); for MDR K. pneumoniae, they were admission in the pediatric ward (P = 0.016), surgical ward (P = 0.019), or gynecology ward (P = 0.12), admission duration of > seven days, and antibiotic use during the past month (P = 0.04). Conclusions: The prevalence of multidrug resistance was high compared with developed countries, and history of admission, antibiotic use, admission duration and admission wards were the risk factors for multidrug resistance. © 2015, Ahvaz Jundishapur University of Medical Sciences

    Prediction of Neutron Yield of IR-IECF Facility in High Voltages Using Artificial Neural Network

    Artificial neural network (ANN) is applied to predict the number of produced neutrons from IR-IECF device in wide discharge current and voltage ranges. Experimentally, discharge current from 20 to 100 mA had been tuned by deuterium gas pressure and cathode voltage had been changed from −20 to −82 kV (maximum voltage of the used supply). The maximum neutron production rate (NPR) of 1.46 × 107 n/s had occurred when the voltage was −82 kV and the discharge current was 48 mA. The back-propagation algorithm is used for training of the proposed multilayer perceptron (MLP) neural network structure. The obtained results show that the proposed ANN model has achieved good agreement with the experimental data. Results show that NPR of 1.855 × 108 n/s can be achieved in voltage and current of 125 kV and 45 mA, respectively. This prediction shows 52% increment in maximum voltage of power supply. Also, the optimum discharge current can increase 1270% NPR
