    The approach to total patient care in a teenager with malignant disease represents one of the greatest challenges to any physician. To bridge the generation gap and communicate in an area of medicine where we cannot always cure, but often only palliate, guide, comfort and listen, is often difficult for we physicians who are so often taught only how to win therapeutic battles. Malignancy is the leading medical cause of death in the 10-21 year age group in the United States. Leukemia, Hodgkin\u27s disease, bone and brain tumors are the most commonly encountered forms of cancer during this period (Heald, 1960)

    Trust and trustworthiness

    What is it to trust someone? What is it for someone to be trustworthy? These are the two main questions that this paper addresses. There are various situations that can be described as ones of trust, but this paper considers the issue of trust between individuals. In it, I suggest that trust is distinct from reliance or cases where someone asks for something on the expectation that it will be done due to the different attitude taken by the trustor. I argue that the trustor takes Holton's 'participant stance' and this distinguishes trust from reliance. I argue that trustworthiness is different from reliability and that an account of trustworthiness cannot be successful whilst ignoring the point that aligning trustworthiness with reliability removes the virtue from being trustworthy. On the question of what it is distinguishes trustworthiness from reliability, I argue that the distinction is in the opportunity for the trustee to act against the wishes of the trustor and the trustee's consideration of the value of the trust that has been placed in them by the trustor

    Sporadic inclusion body myositis: an unsolved mystery

    Sporadic inclusion body myositis (sIBM) is considered to be the most common acquired muscle disease associated with aging. It is a disabling disorder still without effective treatment. sIBM causes weakness and atrophy of the distal and proximal muscles. Involvement of quadriceps and deep finger flexors are clues to early diagnosis. Dysphagia in the course of the disease is common. Muscle biopsy shows chronic myopathic features, lymphocytic infiltration invading non-necrotic fibbers, rimmed vacuoles and accumulation of amyloid-related proteins. It remains uncertain whether sIBM is primarily an immune-mediated inflammatory myopathy or a degenerative myopathy with an associated inflammatory component. This review describes the epidemiology and clinical features of the disease as well as the current genetic and pathogenic concepts and therapeutic approaches. Despite recent clues, in many respects sIBM remains an unsolved mystery

    Learning the rules of the ‘student game’: Transforming the ‘student habitus’ through [im]mobility

    In recent years, a growing body of literature has emerged concerning the mobilities of students, specifically relating to the interactions between local and non-local students, which can accentuate unequal access to education; social interactions and learner outcomes. Central to much of this literature is a sense that being mobile in institutional choice is the most appropriate and expected approach to successful university life. Conversely, local students, disadvantaged by their age, history, external commitments and immobility, are thought to be unlikely to share the same ‘student experiences’ as their traditional counterparts, leading to feelings of alienation within the student community. This paper will seek to problematise this binary by examining the experiences of a group of local and non-local students studying at the University of Portsmouth using Bourdieu’s reading of habitus and capital. This is useful as it provides a more critical insight into how students’ [dis]advantaged learner identities are [re]produced through their everyday sociability. Moreover, these findings extend previous discussions of first year transitions by questioning the influence of accommodation upon the formation of identities and the initial experiences of ‘being’, or ‘becoming’ students.     A new model for mixing by double-diffusive convection (semi-convection): I. The conditions for layer formation

    The process referred to as "semi-convection" in astrophysics and "double-diffusive convection in the diffusive regime" in Earth and planetary sciences, occurs in stellar and planetary interiors in regions which are stable according to the Ledoux criterion but unstable according to the Schwarzschild criterion. In this series of papers, we analyze the results of an extensive suite of 3D numerical simulations of the process, and ultimately propose a new 1D prescription for heat and compositional transport in this regime which can be used in stellar or planetary structure and evolution models. In a preliminary study of the phenomenon, Rosenblum et al. (2011) showed that, after saturation of the primary instability, a system can evolve in one of two possible ways: the induced turbulence either remains homogeneous, with very weak transport properties, or transitions into a thermo-compositional staircase where the transport rate is much larger (albeit still smaller than in standard convection). In this paper, we show that this dichotomous behavior is a robust property of semi-convection across a wide region of parameter space. We propose a simple semi-analytical criterion to determine whether layer formation is expected or not, and at what rate it proceeds, as a function of the background stratification and of the diffusion parameters (viscosity, thermal diffusivity and compositional diffusivity) only. The theoretical criterion matches the outcome of our numerical simulations very adequately in the numerically accessible "planetary" parameter regime, and can easily be extrapolated to the stellar parameter regime.     The role of 1,25-dihydroxyvitamin D in the inhibition of bone formation induced by skeletal unloading

    Skeletal unloading results in osteopenia. To examine the involvement of vitamin D in this process, the rear limbs of growing rats were unloaded and alterations in bone calcium and bone histology were related to changes in serum calcium (Ca), inorganic phosphorus (P sub i), 25-hydroxyvitamin D (25-OH-D), 24,25-dihydroxyvitamin D (24,25(OH)2D and 1,25-dihydroxyvitamin D (1,25(OH)2D. Acute skeletal unloading induced a transitory inhibition of Ca accumulation in unloaded bones. This was accompanied by a transitory rise in serum Ca, a 21% decrease in longitudinal bone growth (P 0.01), a 32% decrease in bone surface lined with osteoblasts (P .05), no change in bone surface lined with osteoclasts and a decrease in circulating (1,25(OH)2D. No significant changes in the serum concentrations of P sub i, 25-OH-D or 24,25(OH)2D were observed. After 2 weeks of unloading, bone Ca stabilized at approximately 70% of control and serum Ca and 1,25(OH)2D returned to control values. Maintenance of a constant serum 1,25(OH)2D concentration by chronic infusion of 1,25(OH)2D (Alza osmotic minipump) throughout the study period did not prevent the bone changes induced by acute unloading. These results suggest that acute skeletal unloading in the growing rat produces a transitory inhibition of bone formation which in turn produces a transitory hypercalcemia

    Biweekly reports generated for the Weber Student Shuttle Involvement Project (SSIP) are discussed. The reports document the evolution of science, hardware, and logistics for this Shuttle project aboard the eleventh flight of the Space Transportation System (STS-41B), launched from Kennedy Space Center on February 3, 1984, and returned to KSC 8 days later. The reports were intended to keep all members of the team aware of progress in the project and to avoid redundancy and misunderstanding. Since the Weber SSIP was NASA's first orbital rat project, documentation of all actions was essential to assure the success of this complex project. Eleven reports were generated: October 3, 17 and 31; November 14 and 28; and December 12 and 17, 1983; and January 3, 16, and 23; and May 1, 1984. A subject index of the reports is included.     We show how Geophysics may illustrate and thus improve classical Mechanics lectures concerning the study of Coriolis force effects. We are then interested in atmospheric as well as oceanic phenomena we are familiar with, and are for that reason of pedagogical and practical interest.     Implementation and performance of SIBYLS: a dual endstation small-angle X-ray scattering and macromolecular crystallography beamline at the Advanced Light Source.

    The SIBYLS beamline (12.3.1) of the Advanced Light Source at Lawrence Berkeley National Laboratory, supported by the US Department of Energy and the National Institutes of Health, is optimized for both small-angle X-ray scattering (SAXS) and macromolecular crystallography (MX), making it unique among the world's mostly SAXS or MX dedicated beamlines. Since SIBYLS was commissioned, assessments of the limitations and advantages of a combined SAXS and MX beamline have suggested new strategies for integration and optimal data collection methods and have led to additional hardware and software enhancements. Features described include a dual mode monochromator [containing both Si(111) crystals and Mo/B(4)C multilayer elements], rapid beamline optics conversion between SAXS and MX modes, active beam stabilization, sample-loading robotics, and mail-in and remote data collection. These features allow users to gain valuable insights from both dynamic solution scattering and high-resolution atomic diffraction experiments performed at a single synchrotron beamline. Key practical issues considered for data collection and analysis include radiation damage, structural ensembles, alternative conformers and flexibility. SIBYLS develops and applies efficient combined MX and SAXS methods that deliver high-impact results by providing robust cost-effective routes to connect structures to biology and by performing experiments that aid beamline designs for next generation light sources
