The status of Iran measles surveillance system in 2014
The results showed success of the health system to detect suspected cases of the measles and appropriate sampling from suspected cases that were more than expected. In addition, per every one million population of Iran in 2014, 1.27 of cases of measles were reported; that a numbers of confirmed cases observed in Afghan refugees. Low levels of immunization coverage in eastern neighboring countries and high immigration to Iran, are the main challenge of measles elimination in Iran. The gradual accumulation of susceptible individuals, due to lack of proper vaccination or failure to respond to immunization, may underlie the development of outbreaks of measles in Iran. © 2015, Isfahan University of Medical Sciences(IUMS). Rotation Symmetry Spontaneous Breaking of Edge States in Zigzag Carbon Nanotubes
Analytical solutions of the edge states were obtained for the (N, 0) type
carbon nanotubes with distorted ending bonds. It was found that the edge states
are mixed via the distortion. The total energies for N=5 and N>=7 are lower in
the asymmetric configurations of ending bonds than those having axial rotation
symmetry. Thereby the symmetry is breaking spontaneously. The results imply
that the symmetry of electronic states at the apex depends on the occupation;
the electron density pattern at the apex could change dramatically and could be

Cube Testers and Key Recovery Attacks On Reduced-Round MD6 and Trivium
CRYPTO 2008 saw the introduction of the hash function
MD6 and of cube attacks, a type of algebraic attack applicable to cryptographic
functions having a low-degree algebraic normal form over GF(2).
This paper applies cube attacks to reduced round MD6, finding the full
128-bit key of a 14-round MD6 with complexity 2^22 (which takes less
than a minute on a single PC). This is the best key recovery attack announced
so far for MD6. We then introduce a new class of attacks called
cube testers, based on efficient property-testing algorithms, and apply
them to MD6 and to the stream cipher Trivium. Unlike the standard
cube attacks, cube testers detect nonrandom behavior rather than performing
key extraction, but they can also attack cryptographic schemes
described by nonrandom polynomials of relatively high degree. Applied
to MD6, cube testers detect nonrandomness over 18 rounds in 2^17 complexity;
applied to a slightly modified version of the MD6 compression
function, they can distinguish 66 rounds from random in 2^24 complexity.
Cube testers give distinguishers on Trivium reduced to 790 rounds from
random with 2^30 complexity and detect nonrandomness over 885 rounds
in 2^27, improving on the original 767-round cube attack
Evolving evidence on a link between the ZMYM3 exceptionally long GA-STR and human cognition
The human X-linked zinc finger MYM-type protein 3 (ZMYM3) contains the longest GA-STR identified across protein-coding gene 5� UTR sequences, at 32-repeats. This exceptionally long GA-STR is located at a complex string of GA-STRs with a human-specific formula across the complex as follows: (GA)8-(GA)4-(GA)6-(GA)32 (ZMYM3-207 ENST00000373998.5). ZMYM3 was previously reported among the top three genes involved in the progression of late-onset Alzheimer�s disease. Here we sequenced the ZMYM3 GA-STR complex in 750 human male subjects, consisting of late-onset neurocognitive disorder (NCD) as a clinical entity (n = 268) and matched controls (n = 482). We detected strict monomorphism of the GA-STR complex, except of the exceptionally long STR, which was architecturally skewed in respect of allele distribution between the NCD cases and controls F (1, 50) = 12.283; p = 0.001. Moreover, extreme alleles of this STR at 17, 20, 42, and 43 repeats were detected in seven NCD patients and not in the control group (Mid-P exact = 0.0003). A number of these alleles overlapped with alleles previously found in schizophrenia and bipolar disorder patients. In conclusion, we propose selective advantage for the exceptional length of the ZMYM3 GA-STR in human, and its link to a spectrum of diseases in which major cognition impairment is a predominant phenotype. © 2020, The Author(s)
Early-infantile onset epilepsy and developmental delay caused by bi-allelic GAD1 variants
Gamma-aminobutyric acid (GABA) and glutamate are the most abundant amino acid neurotransmitters in the brain. GABA, an inhibitory neurotransmitter, is synthesized by glutamic acid decarboxylase (GAD). Its predominant isoform GAD67, contributes up to ∼90% of base-level GABA in the CNS, and is encoded by the GAD1 gene. Disruption of GAD1 results in an imbalance of inhibitory and excitatory neurotransmitters, and as Gad1−/− mice die neonatally of severe cleft palate, it has not been possible to determine any potential neurological dysfunction. Furthermore, little is known about the consequence of GAD1 disruption in humans. Here we present six affected individuals from six unrelated families, carrying bi-allelic GAD1 variants, presenting with developmental and epileptic encephalopathy, characterized by early-infantile onset epilepsy and hypotonia with additional variable non-CNS manifestations such as skeletal abnormalities, dysmorphic features and cleft palate. Our findings highlight an important role for GAD1 in seizure induction, neuronal and extraneuronal development, and introduce GAD1 as a new gene associated with developmental and epileptic encephalopathy
Primary antibody deficiency in a tertiary referral hospital: A 30-year experiment
Background: Primary antibody deficiency (PAD) is the most common group of primary immunodeficiency disorders (PID), with a broad spectrum of clinical features ranging from severe and recurrent infections to asymptomatic disease. Objectives: The current study was performed to evaluate and compare demographic and clinical data in the most common types of PAD. Materials and Methods: We performed a retrospective review of the medical records of all PAD patients with a confirmed diagnosis of common variable immunodeficiency (CVID), hyper IgM syndrome (HIgM), selective IgA deficiency (SIgAD), and X-linked agammaglobulinemia (XLA) who were diagnosed during the last 30 years at the Children�s Medical Center, Tehran, Iran. Results: A total number of 280 cases of PAD (125 CVID, 32 HIgM, 63 SIgAD, and 60 XLA) were enrolled in the study. The median (range) age at the onset of disease in CVID, HIgM, SIgAD, and XLA was 2 (0-46), 0.91 (0-9), 1 (0-26), and 1 (0-10) years, respectively. Gastrointestinal infections were more prevalent in CVID patients, as were central nervous system infections in XLA patients. Autoimmune complications were more prevalent in HIgM patients, malignancies in CVID patients, and allergies in SIgAD patients. The mortality rate for CVID, HIgM, and XLA was 27.2, 28.1, and 25, respectively. No deaths were reported in SIgAD patients. Conclusions: SIgAD patients had the best prognosis. While all PAD patients should be monitored for infectious complications, special attention should be paid to the finding of malignancy and autoimmune disorders in CVID and HIgM patients, respectively. © 2015 Esmon Publicidad
Degrees of tenant isolation for cloud-hosted software services : a cross-case analysis
A challenge, when implementing multi-tenancy
in a cloud-hosted software service, is how to ensure that the
performance and resource consumption of one tenant does
not adversely affect other tenants. Software designers and
architects must achieve an optimal degree of tenant isolation
for their chosen application requirements. The objective
of this research is to reveal the trade-offs, commonalities,
and differences to be considered when implementing
the required degree of tenant isolation. This research uses
a cross-case analysis of selected open source cloud-hosted
software engineering tools to empirically evaluate varying
degrees of isolation between tenants. Our research reveals
five commonalities across the case studies: disk space reduction,
use of locking, low cloud resource consumption,
customization and use of plug-in architecture, and choice of
multi-tenancy pattern. Two of these common factors compromise
tenant isolation. The degree of isolation is reduced
when there is no strategy to reduce disk space and customization
and plug-in architecture is not adopted. In contrast,
the degree of isolation improves when careful consideration
is given to how to handle a high workload, locking of
data and processes is used to prevent clashes between multiple
tenants and selection of appropriate multi-tenancy pattern. The research also revealed five case study differences:
size of generated data, cloud resource consumption, sensitivity
to workload changes, the effect of the software process,
client latency and bandwidth, and type of software process.
The degree of isolation is impaired, in our results, by
the large size of generated data, high resource consumption
by certain software processes, high or fluctuating workload,
low client latency, and bandwidth when transferring multiple
files between repositories. Additionally, this research
provides a novel explanatory framework for (i) mapping tenant
isolation to different software development processes,
cloud resources and layers of the cloud stack; and (ii) explaining
the different trade-offs to consider affecting tenant
isolation (i.e. resource sharing, the number of users/requests,
customizability, the size of generated data, the scope of control
of the cloud application stack and business constraints)
when implementing multi-tenant cloud-hosted software services.
This research suggests that software architects have
to pay attention to the trade-offs, commonalities, and differences
we identify to achieve their degree of tenant isolation
requirements
Primary immunodeficiency disorders in Iran: Update and new insights from the third report of the national registry
Background: Primary immunodeficiency disorders (PID) are a group of heterogeneous disorders mainly characterized by severe and recurrent infections and increased susceptibility to malignancies, lymphoproliferative and autoimmune conditions. National registries of PID disorders provide epidemiological data and increase the awareness of medical personnel as well as health care providers. Methods: This study presents the demographic data and clinical manifestations of Iranian PID patients who were diagnosed from March 2006 till the March of 2013 and were registered in Iranian PID Registry (IPIDR) after its second report of 2006. Results: A total number of 731 new PID patients (455 male and 276 female) from 14 medical centers were enrolled in the current study. Predominantly antibody deficiencies were the most common subcategory of PID (32.3 %) and were followed by combined immunodeficiencies (22.3 %), congenital defects of phagocyte number, function, or both (17.4 %), well-defined syndromes with immunodeficiency (17.2 %), autoinflammatory disorders (5.2 %), diseases of immune dysregulation (2.6 %), defects in innate immunity (1.6 %), and complement deficiencies (1.4 %). Severe combined immunodeficiency was the most common disorder (21.1 %). Other prevalent disorders were common variable immunodeficiency (14.9 %), hyper IgE syndrome (7.7 %), and selective IgA deficiency (7.5 %). Conclusions: Registration of Iranian PID patients increased the awareness of medical community of Iran and developed diagnostic and therapeutic techniques across more parts of the country. Further efforts must be taken by increasing the coverage of IPIDR via electronically registration and gradual referral system in order to provide better estimation of PID in Iran and reduce the number of undiagnosed cases. © 2014 Springer Science+Business Media
Role of Stem Cells in Human Uterine Leiomyoma Growth
Uterine leiomyoma is the most common benign tumor in reproductive-age women. Each leiomyoma is thought to be a benign monoclonal tumor arising from a single transformed myometrial smooth muscle cell; however, it is not known what leiomyoma cell type is responsible for tumor growth. Thus, we tested the hypothesis that a distinct stem/reservoir cell-enriched population, designated as the leiomyoma-derived side population (LMSP), is responsible for cell proliferation and tumor growth.LMSP comprised approximately 1% of all leiomyoma and 2% of all myometrium-derived cells. All LMSP and leiomyoma-derived main population (LMMP) but none of the side or main population cells isolated from adjacent myometrium carried a mediator complex subunit 12 mutation, a genetic marker of neoplastic transformation. Messenger RNA levels for estrogen receptor-α, progesterone receptor and smooth muscle cell markers were barely detectable and significantly lower in the LMSP compared with the LMMP. LMSP alone did not attach or survive in monolayer culture in the presence or absence of estradiol and progestin, whereas LMMP readily grew under these conditions. LMSP did attach and survive when directly mixed with unsorted myometrial cells in monolayer culture. After resorting and reculturing, LMSP gained full potential of proliferation. Intriguingly, xenografts comprised of LMSP and unsorted myometrial smooth muscle cells grew into relatively large tumors (3.67 ± 1.07 mm(3)), whereas xenografts comprised of LMMP and unsorted myometrial smooth muscle cells produced smaller tumors (0.54 ± 0.20 mm(3), p<0.05, n = 10 paired patient samples). LMSP xenografts displayed significantly higher proliferative activity compared with LMMP xenografts (p<0.05).Our data suggest that LMSP, which have stem/reservoir cell characteristics, are necessary for in vivo growth of leiomyoma xenograft tumors. Lower estrogen and progesterone receptor levels in LMSP suggests an indirect paracrine effect of steroid hormones on stem cells via the mature neighboring cells
Improving pulse crops as a source of protein, starch and micronutrients
Pulse crops have been known for a long time to have beneficial nutritional profiles for human diets but have been neglected in terms of cultivation, consumption and scientific research in many parts of the world. Broad dietary shifts will be required if anthropogenic climate change is to be mitigated in the future, and pulse crops should be an important component of this change by providing an environmentally sustainable source of protein, resistant starch and micronutrients. Further enhancement of the nutritional composition of pulse crops could benefit human health, helping to alleviate micronutrient deficiencies and reduce risk of chronic diseases such as type 2 diabetes.
