    A study on the ethical components of nursing practice (moral distress, ethical sensitivity, ethical decision)

    This paper is an applied research in terms of objective and a descriptive research in terms of method. Having prepared the research plan, a questionnaire was designed based on goals and hypotheses of the research and was sent to the statistical universe. Also this paper is a field research in terms of data collection. As regards theoretical bases of the research, library data collection method has been applied. So, the required data has been gathered by referring to the related references, books, libraries and so on. To design a questionnaire and gather the opinions of the statistical universe members, field study method and researcher-made questionnaire have been used. The statistical universe comprises nurses and head of ICU and head nurses of Najmieh Hospital in Tehran. The respondents were selected by random sampling method. Also to estimate sample size, Morgan table was applied. The statistical universe consists of 65 members and according to the table, 56 questionnaires were determined for the research. So 60 questionnaires were sent and 58 ones were returned. Face and content validity of the research tool were approved by experts. The test reliability was estimated 0.777 by calculating Cronbach's alpha coefficient. In this paper, factor analysis based on partial least squares structural equations method has been applied to analyze more important factors and coefficients, estimate independent variables coefficients and even determine effectiveness of each independent variable on each other and determine appropriateness of the questions and their coefficients in explaining the related index. The main result of this paper presents a proper model for the relation of effective variables on nurse performance by using regression model. © IDOSI Publications, 2014

    Time Series Imputation via L1 Norm-Based Singular Spectrum Analysis

    Missing values in time series data is a well-known and important problem which many researchers have studied extensively in various fields. In this paper, a new nonparametric approach for missing value imputation in time series is proposed. The main novelty of this research is applying the L1 norm-based version of Singular Spectrum Analysis (SSA), namely L1-SSA which is robust against outliers. The performance of the new imputation method has been compared with many other established methods. The comparison is done by applying them to various real and simulated time series. The obtained results confirm that the SSA-based methods, especially L1-SSA can provide better imputation in comparison to other methods

    Weighted Linear Recurrent Forecasting in Singular Spectrum Analysis

    Singular Spectrum Analysis (SSA) is an increasingly popular time series filtering and forecasting technique. Owing to its widespread applications in a variety of fields, there is a growing interest towards improving its forecasting capabilities. As such, this paper takes into consideration the Recurrent forecasting approach in SSA (SSA-R) and presents a new mechanism for improving the accuracy of forecasts attainable via this method. The proposed Recurrent SSA-R approach is referred to as Weighted SSA-R (W:SSA-R), and we propose using a weighting algorithm for weigthing the coefficients of the Linear Recurrent Relation (LRR). The performance of forecasts from the W:SSA-R approach are compared with forecasts from the established SSA-R approach. We exploit real data and various simulated time series for the comparison, so as to provide the reader with more conclusive findings. Our results confirm that the W:SSA-R approach can provide comparatively more accurate forecasts and is indeed a viable solution for improving forecasts by SSA

    Chromogenic in situ hybridisation test for breast cancer patients with equivocal IHC results - A study from Iran

    Background: HER2/neu overexpression on cell membranes of breast cancer cells is due to HER2/neu gene amplification and it is important to identify potential candidates for anti HER2 therapy with trastuzumab. IHC, FISH and CISH are standard FDA approved assays currently used to determine HER2 status in routine practice. The aim of this study was to determine HER2 gene amplification, using the CISH method in breast carcinoma samples which had IHC +2 reactions. Materials and Methods: This study was conducted from 2008-2010 using 334 consecutive breast carcinoma samples referred from local laboratories to Mehr Hospital. CISH assays were performed for all cases, and IHC tests were also done for determining efficacy and accuracy of local labs. HER2 status in local IHC tests was compared with central IHC and CISH results. Results: Of 334 breast cancer patients, 16 were negative for HER2 IHC (0, +1), 201 cases were equivocal (+2), and 31 positive (+3). Of 334 referral cases, 88 were CISH positive (26.3) and 246 were CISH negative (73.7). Of 201 IHC +2 cases, HER2 gene amplification was observed in 42 cases (kappa: 0.42). A 29.9 concordance was found between local IHC and central IHC. Sensitivity and specificity of local IHC were 90 and 53.8, respectively. Conclusions: Low accuracy of IHC results in local labs was associated with the following factors: using former FDA-approved criteria for HER2 interpretation, utilizing non-validated kits, and lack of any quality assurance program. Therefore, following the new 2014 ASCO/CAP guideline and comprehensive quality assurance should be implemented to ensure accuracy of HER2 testing

    Optimized energy window configuration for201Tl imaging

    A poor signal-to-noise ratio attributable to a low injected dose of thallium and the presence of scattered photons are the major impediments in the use of thallium as an imaging agent. Thallium decays in a complicated way and emits photons in a wide range of energies (68-82 keV). To increase the ratios of primary photons to scatter photons (primary-to-scatter ratios) and possibly increase system sensitivity, a new energy window for thallium was investigated. Methods: The NCAT phantom was used to simulate the distribution of activity and the attenuation coefficient in a typical patient torso. The phantom was imaged with a SPECT simulator in different energy window configurations. The energy spectra for primary photons and scatter photons were generated, and the most suitable energy windows were investigated. To evaluate the results of the simulation study, a physical phantom was imaged in different energy windows with a SPECT system. The images of the physical phantom were analyzed for the best-quality image and the corresponding window setting. To evaluate the windows determined in the simulation and phantom studies, SPECT images of 7 patients who had angiographically confirmed myocardial defects were acquired in different energy windows. The images were quantitatively compared on the basis of the calculated contrast, scatter-to-noise ratio, and sensitivity. The images were also qualitatively evaluated independently by 4 nuclear medicine specialists. Results: The simulation study showed that the conventional window setting (68 ± 10 keV) is not the most suitable window configuration for 201Tl imaging and that the optimum energy window is 77 ± 15 keV. The images acquired in the latter window configuration yielded higher primary-to-scatter ratios, higher sensitivity (total counts), and better contrast than the images acquired in the conventional window configuration. The phantom study confirmed the results of the simulation study. In the clinical study, the images acquired in the suggested window showed a considerable increase in myocardium-to-defect contrast (1.541 ± 0.368) and myocardium-to-cavity contrast (1.171 ± 0.099) than those acquired in the conventional window configuration. Conclusion: The energy window configuration of 77 ± 15 keV yields higher-quality images than the conventional window configuration. COPYRIGHT © 2008 by the Society of Nuclear Medicine, Inc

    Exposure to nanoscale diesel exhaust particles: Oxidative stress, neuroinflammation, anxiety and depression on adult male mice

    ABSTRACT Exposure to nanoscale diesel engines exhausted particles (DEPs) is a well-recognized risk factor for respiratory and cardiovascular diseases. Rodents as commonly used models for urban air pollution in health effect studies demonstrate constant stimulation of inflammatory responses in the main areas of the brain. Nevertheless, the primary effect of diesel exhaust particulate matter on some of the brain regions and relation by behavioral alterations still remains untouched. We evaluated the brain regional in flammatory responses to a nanosized subfraction of diesel engines exhaust particulate matter (DEPs < 200 nm) in an adult male mice brain. Adult male mice were exposed to DEPs for 3, 6, and 8 h per day, 12 weeks and five days per week. Degree of anxiety and the depression by elevated plus maze and Forced Swimming Test respectively (FST) did measurement. After behavior tests, the plasma and some of the brain regions such as olfactory bulb (OB) and hippocampus (HI) were analyzed for oxidative stress and in flammatory responses. The inflammation and oxidative stress changes in OB and HI, markedly coincides with the results of behavioral alterations. These responses corresponded with rapid induction of MDA and nitrite oxide (NO) in brain regions and neuronal nitric oxide synthase (nNOS) mRNA followed by IL6, IL1 α , and TNF α in OB and HI. The different times of DEPs exposure, leads to oxidative stress and inflammatory in plasma and brain regions. That this cumulative transport of inhaled nanoscale DEPs into the brain and creating to inflammation responses of brain regions may cause problems of brain function and anxiety and depression.     Measurements of scattering observables for the pdpd break-up reaction

    High-precision measurements of the scattering observables such as cross sections and analyzing powers for the proton-deuteron elastic and break-up reactions have been performed at KVI in the last two decades and elsewhere to investigate various aspects of the three-nucleon force (3NF) effects simultaneously. In 2006 an experiment was performed to study these effects in p+d\vec{p}+d break-up reaction at 135 MeV with the detection system, Big Instrument for Nuclear polarization Analysis, BINA. BINA covers almost the entire kinematical phase space of the break-up reaction.
