Treatment of keloid scars with intralesional triamcinolone and 5-fluorouracil injections - a randomized controlled trial
Keloids have high recurrence rates. Current first-line therapy is triamcinolone (TAC) injection, but it has been suggested that approximately 50% of keloids are steroid resistant. We compared the efficacy of intralesional 5-fluorouracil (5-FU) and triamcinalone injections in a double-blind randomized controlled trial. Forty-three patients with 50 keloid scars were treated with either intralesional TAC or 5-FU-injections over 6 months. There was no statistically significant difference in the remission rate at 6 months between the 5-FU and TAC groups (46% vs 60%, respectively). Local adverse effects were higher in the TAC group compared to the 5-FU group. Occurrence of skin atrophy in TAC group was 44% and in the 5-FU group 8% (p <0.05). Also the occurrence of telangiectasia in the TAC group was 50% and in the 5-FU 21% (p <0.05). Vascularity of the keloids, assessed by spectral imaging and immunohistochemical staining for blood vessels, after treatment decreased in the TAC group, but not in the 5-FU group (p <0.05). Fibroblast proliferation evaluated by Ki-67 staining significantly decreased in the TAC group (p <0.05) but increased in the 5-FU group (p <0.05). TAC and 5-FU injections did not differ in their clinical effectivity in this randomized study, but 5-FU injections lead to increased proliferation rate and did not affect vascular density in histological assessment. Due to the greater number of adverse effects observed after TAC treatment, 5-FU injections may be preferable for cosmetically sensitive skin areas. On the Isomorphic Description of Chiral Symmetry Breaking by Non-Unitary Lie Groups
It is well-known that chiral symmetry breaking (SB) in QCD with
light quark flavours can be described by orthogonal groups as , due to local isomorphisms. Here we discuss the question how specific
this property is. We consider generalised forms of SB involving an
arbitrary number of light flavours of continuum or lattice fermions, in various
representations. We search systematically for isomorphic descriptions by
non-unitary, compact Lie groups. It turns out that there are a few alternative
options in terms of orthogonal groups, while we did not find any description
entirely based on symplectic or exceptional Lie groups. If we adapt such an
alternative as the symmetry breaking pattern for a generalised Higgs mechanism,
we may consider a Higgs particle composed of bound fermions and trace back the
mass generation to SB. In fact, some of the patterns that we encounter
appear in technicolour models. In particular if one observes a Higgs mechanism
that can be expressed in terms of orthogonal groups, we specify in which cases

Plaquette expectation value and gluon condensate in three dimensions
In three dimensions, the gluon condensate of pure SU(3) gauge theory has
ultraviolet divergences up to 4-loop level only. By subtracting the
corresponding terms from lattice measurements of the plaquette expectation
value and extrapolating to the continuum limit, we extract the finite part of
the gluon condensate in lattice regularization. Through a change of
regularization scheme to MSbar and (inverse) dimensional reduction, this result
would determine the first non-perturbative coefficient in the weak-coupling
expansion of hot QCD pressure.Comment: 11 page
Local treatment of pressure ulcers in long-term care: a correlational cross-sectional study
Objective:To analyse the treatment of pressure ulcers (PU) in long-term care.Method:In this correlational cross-sectional study, data was collected between November 2015 and January 2016 from older people with PUs in private and public long-term care facilities in Finland. Data collection was conducted by trained nurses using the Pressure Ulcer Patient Instrument (PUP-Ins). Outcomes measured were: prevalence and localisation of PU, local PU treatment, frequency (how often/week/day) and duration (minutes/week or day) of PU treatment.Results:In total, 112 patients with 158 PUs were identified (a prevalence rate of 5%). PUs were located most often on the heel (38%), hip (13%), buttocks (10%) and lateral malleolus (9.5%). The most frequently used PU treatment was skin protecting agents and local wound care products. The most typical treatment in category I, II and III PUs were foam dressings. In category III PUs, ribbon gauze dressings were also used. The most typical products for category IV PUs were complex dressings. Category I PUs received more treatment per day or week than other categories of PUs.Conclusion:PU treatment is inconsistent and often conducted with varying methods and products. Holistic patient care must be the focus. Nurses in long-term care settings might benefit from in-depth in-service education focusing on the treatment of PUs. More research is needed about nurses' competence in PU treatment.</p
A novel techno-economical layout optimization tool for floating wind farm design
Over the past few years, the offshore wind sector has been subject to renewed yet growing interest from the industry and from the research sphere, with a particular focus on a recently developed concept, the floating offshore wind (FOW). Because of its novelty, floating research material is found in limited quantity. This paper focuses on the layout optimization of a floating offshore wind farm (FOWF) considering multiple parameters and engineering constraints, combining floating-specific parameters together with economic indicators. Todayâs common wind farm layout optimization codes do not take into account either floating-specific technical parameters (anchors, mooring lines, inter-array cables (IACs), etc.) or non-technical parameters (operational expenditure, OPEX; capital expenditure, CAPEX; and other techno-economic project parameters). In this paper, a multi-parametric objective function is used in the optimization of the layout of a FOWF, combining the annual energy production (AEP) together with the costs that depend on the layout. The mooring system and the collection system including the inter-array cables and the offshore substation are identified as layout-dependent and therefore modeled in the optimization loop. Using ScotWind site 10 as a study case, it was found with the predefined technical and economic assumptions that the profit was increased by EURâ34.5 million compared to a grid-based layout. The main drivers were identified to be the AEP, followed by the anchors and the availability associated with the failures of inter-array cables.</p
Carcinogen metabolism in human lung tissues and the effect of tobacco smoking: results from a case--control multicenter study on lung cancer patients.
Cigarette smoking is the strongest risk factor for lung cancer, but genetically determined variations in the activities of pulmonary enzyme that metabolize tobacco-derived carcinogens may affect individual risk. To investigate whether these enzymes (e.g., CYP1A-related) can serve as markers for carcinogen-DNA damage, lung tissue specimens were taken during surgery from middle-aged men with either lung cancer or non-neoplastic lung disease. Phase I [aryl hydrocarbon hydroxylase (AHH), ethoxycoumarin O-deethylase (ECOD)] and phase II (epoxide hydrolase, UDP-glucuronosyltransferase, glutathione S-transferase) enzyme activities, glutathione and malondialdehyde contents were determined in lung parenchyma and/or bronchial tissues; some samples were also analyzed for DNA adducts, using 32P-postlabeling. The data were then analyzed for the following: a) differences in metabolic profiles between bronchial and parenchymal lung tissue; b) the effect of recent exposure to tobacco smoke on enzyme inducibility and benzo[a]pyrene metabolism; c) differences in enzyme inducibility between lung cancer and non-lung cancer patients; d) the effect of smoking on metabolism of mutagens in vitro; e) pulmonary DNA adduct levels and AHH activity in lung parenchyma of smokers and ex-smokers; f) lipid peroxidation products in lung tissue from lung cancer and non-lung cancer patients, as related to smoking habits and degree of airway obstruction; and g) prognostic value of AHH pulmonary activity in lung cancer patients. The results demonstrate a pronounced effect of tobacco smoke on pulmonary metabolism of xenobiotics and prooxidant state and suggest the existence of a metabolic phenotype at higher risk for tobacco-associated lung cancer
On the gauge boson's properties in a candidate technicolor theory
The technicolor scenario replaces the Higgs sector of the standard model with
a strongly interacting sector. One candidate for a realization of such a sector
is two-technicolor Yang-Mills theory coupled to two degenerate flavors of
adjoint, massless techniquarks. Using lattice gauge theory the properties of
the technigluons in this scenario are investigated as a function of the
techniquark mass towards the massless limit. For that purpose the minimal
Landau gauge two-point and three-point correlation functions are determined,
including a detailed systematic error analysis. The results are, within the
relatively large systematic uncertainties, compatible with a behavior very
similar to QCD at finite techniquark mass. However, the limit of massless
techniquarks exhibits features which could be compatible with a

A non-perturbative contribution to jet quenching
It has been argued by Caron-Huot that infrared contributions to the jet
quenching parameter in hot QCD, denoted by qhat, can be extracted from an
analysis of a certain static-potential related observable within the
dimensionally reduced effective field theory. Following this philosophy, the
order of magnitude of a non-perturbative contribution to qhat from the
colour-magnetic scale, g^2T/pi, is estimated. The result is small; it is
probably below the parametrically perturbative but in practice slowly
convergent contributions from the colour-electric scale, whose all-orders
