Surgical antibiotic prophylaxis compliance in a university hospital.

International audienceTo assess surgical antibiotic prophylaxis (SAP) practices in a university hospital in order to identify risk factors associated with non-compliance. Retrospective monocentric study conducted over a 4-month period. Data were collected from the software used in the operating theatre. Practice non-compliance was evaluated in comparison with the 2010 version of the French national recommendations. We only took in account the interventions identified as priority surveillance interventions according to the surgical site infections national surveillance. The risk factors associated with SAP non-compliance were identified with a multivariate statistical analysis. We evaluated 1312 SAPs. Among the 1298 indicated SAPs, 44.4% were not compliant. The most frequent inappropriate criterion was the timing of injection (34.8% non-compliance), which was, in the majority of cases, too close to the time of incision. Other inappropriate criteria were identified: antibiotic choice for patients allergic to β-lactams (inappropriate among 45% of allergic patients), and antibiotic dosing for obese patients (96% of non-compliance). Obesity (OR=84.32), allergy to β-lactams (OR=17.11) and certain types of surgery (digestive, OR=4.56; gynaecological and obstetrical, OR=7.10; urological, OR=3.95) were independently associated with the non-compliance of SAP practices. Dépôts, panoplies et accessoires dans les sépultures du 2e âge du Fer en Picardie.

Anhand der Untersuchung von an die 700 Bestattungsensembles mit 4 300 Fundstücken aus der Zeit von Latène A1 bis Latène D2 aus der Picardie und den angrenzenden Gebieten konnte eine Zuordnungsliste erstellt werden, die neben einer hierarchisierten Darstellung der Daten und der unterschiedlichen im Grab angetroffenen funktionellen Kategorien eine synthetische Betrachtungsweise ermöglicht. Der persönliche Besitz, Kleidung oder Grabbeigaben sind durch Schmuck, Waffen, Trachtbestandteile und Toilettgegenstände oder Werkzeug vertreten. Selbst, wenn der Status des Verstorbenen in einigen chronologisch-kulturellen Horizonten an der Ausstattung erkennbar ist, haben wir keine materiellen Spuren gefunden, die uns über die Funktion des Individuums innerhalb der Gruppe Auskunft gegeben hätten. Allein das Vorhandensein von Waffen kann zugleich von einer soziale Stellung und einer militärische Funktion zeugen. Nahrungsreste sowie Tisch-und Kochgeschirr und Vorratsgefäße erlauben Rückschlüsse auf die Ernährungsgewohnheiten. Die Unterschiede hinsichtlich der Form und der Anzahl der Gefäße sind bezeichnend für die Auswahlkriterien der als Grabbeigaben dienenden Keramik und die Existenz unterschiedlicher Typen von „ Geschirrsätzen“, die ebenso die Entwicklung der Ernährungsgewohnheiten widerspiegeln wie die jeweils hervorgehobenen Funktionen. Die unterschiedlichen, innerhalb unseres Corpus’ von Latène A1 bis Latène D2 in der Picardie angetroffenen materiellen, funktionalen, und symbolischen Konfigurationen veranschaulichen die Unterschiede in der „ Totenkleidung“ und den Grabbeigaben. Sie zeugen auch von einer großen Bandbreite der Bestattungsrituale, die der Deponierung vorausgingen.The survey of nearly 700 sets of burial deposits, that is 4300 items, dating from La Tène A1 to La Tène D2 , in Picardy and on its fringes, has enabled us to establish a model allowing a hierarchical presentation of the data and of the various functional groups which appear in the graves, as well as facilitating a global approach. The personal possessions worn by or deposited alongside the deceased are represented by jewels, weapons, dress accessories and toilet or tool bags. Even if the social status of the deceased may be determined, in certain chrono-cultural levels, thanks to the type of accessories displayed, we cannot rely on any specific item which would indicate the position of the person in the group. Only the presence of weapons may attest both a social status and a military position. Diet is suggested both by edible goods and recipients belonging to table sets, or related to cooking and sometimes storage. Variations in shapes and numbers are typical here of changes in the way the pottery deposited in the burials was selected, and of the existence of different sorts of “ sets”, showing not only an evolution in dietary practices, but also a deliberate exercise of choice in the functions highlighted. The varied practical, functional and symbolic aspects observed in our corpus, in the periods from La Tène A1 to La Tène D2 in Picardy, are examples of the wide range of choice in the way the deceased were “ dressed up”, and in the offerings that accompany them. They also highlight some deep differences in the funerary rites which governed these offerings.L’étude de près de 700 ensembles funéraires, soit 4 300 objets, répartis de La Tène A1 à La Tène D2 en Picardie et ses marges, a permis la mise en place d’une grille de lecture autorisant une présentation hiérarchisée des données et des différentes catégories fonctionnelles figurées dans la tombe, ainsi qu’une approche synthétique. Les effets personnels du défunt portés ou déposés à ses côtés sont évoqués par des éléments de parure, d’armement, d’accessoires vestimentaires et des trousses (de toilette ou d’outils). Même si le statut du défunt est perceptible à certains horizons chrono-culturels, à travers le type de «panoplies » , nous ne disposons d’aucun objet pour définir la fonction de l’individu au sein du groupe. Seule la présence d’armes peut révéler à la fois un statut social et une fonction de type militaire. L’alimentation est suggérée à la fois par des consommables et des vases liés au service de la table, à la préparation et parfois au stockage. La variation des formes et des effectifs caractérise ici des changements dans le mode de sélection des récipients déposés dans les sépultures et l’existence de différents types de «service » , qui reflètent l’évolution des pratiques alimentaires mais aussi des orientations dans les fonctions mises en avant. Les différentes configurations matérielles, fonctionnelles et symboliques enregistrées au sein de notre corpus de La Tène A1 à La Tène D2 en Picardie, illustrent l’éventail des choix de «l’habillement » des défunts et des dépôts qui les accompagnent. Outcomes associated with routine systemic antifungal therapy in critically ill patients with Candida colonization.

International audienceTo assess the evolution of patient deep colonization by Candida spp. in a surgical ICU over an 8-year period. This retrospective, observational study included all patients hospitalized for more than 2 days in a surgical and trauma ICU of a university hospital, from 2005 to 2012. Mycological samples were monitored weekly from five sites (oropharyngeal, rectal, gastric, tracheal and urinary). Preemptive fluconazole therapy was started in patients highly colonized with Candida albicans. The evolution in Candida spp. involved in the deep colonization sites distribution over the study period (main outcome measure, trend chi-square and time-series analysis), antifungal consumption, ICU-acquired candidemia and mortality were determined. Among the 3029 patients with ICU stay >48 h, 2651 had at least one set of mycological sampling. Thirty percent of the 31,171 samples were positive to Candida spp. Caspofungin consumption increased over the years, whereas fluconazole consumption decreased. No trend in C. albicans colonization was observed, after adjusting on colonization risk-factors. A significant increase of acquired C. glabrata colonization was observed, whereas the clearing of C. parapsilosis colonization significantly decreased. No significant shift of colonization to other Candida spp. and mortality was observed. Preemptive strategy of antifungal drug prescriptions in highly colonized ICU patients induced an increase in C. glabrata colonization without significant shift of colonization to other Candida spp. in surgical ICU patients. However, the potential detrimental impact of fluconazole on Candida ecology in ICU and/or on Candida susceptibility to antifungal drugs should be considered, and deserves further studies

Proportion of single-chain recombinant tissue plasminogen activator and outcome after stroke

International audienceTo determine whether the ratio single chain (sc)/(sc + 2 chain [tc]) recombinant tissue plasminogen activator (rtPA) influences outcomes in patients with cerebral ischemia.METHODS:We prospectively included consecutive patients treated with IV rtPA for cerebral ischemia in 13 stroke centers and determined the sc/(sc + tc) ratio in the treatment administered to each patient. We evaluated the outcome with the modified Rankin Scale (mRS) at 3 months (prespecified analysis) and occurrence of epileptic seizures (post hoc analysis). We registered Outcome of Patients Treated by IV Rt-PA for Cerebral Ischaemia According to the Ratio Sc-tPA/Tc-tPA (OPHELIE) under ClinicalTrials.gov identifier no. NCT01614080.RESULTS:We recruited 1,004 patients (515 men, median age 75 years, median onset-to-needle time 170 minutes, median NIH Stroke Scale score 10). We found no statistical association between sc/(sc + tc) ratios and handicap (mRS > 1), dependency (mRS > 2), or death at 3 months. Patients with symptomatic intracerebral hemorrhages had lower ratios (median 69% vs 72%, adjusted p = 0.003). The sc/(sc + tc) rtPA ratio did not differ between patients with and without seizures, but patients with early seizures were more likely to have received a sc/(sc + tc) rtPA ratio >80.5% (odds ratio 3.61; 95% confidence interval 1.26-10.34).CONCLUSIONS:The sc/(sc + tc) rtPA ratio does not influence outcomes in patients with cerebral ischemia. The capacity of rtPA to modulate NMDA receptor signaling might be associated with early seizures, but we observed this effect only in patients with a ratio of sc/(sc + tc) rtPA >80.5% in a post hoc analysis

Économie et société de la fin de la Préhistoire : Actualité de la recherche

Les 7e Rencontres méridionales de Préhistoire récente, tenues à Bron, sur le campus de l'Université Lumière-Lyon 2, les 3 et 4 novembre 2006, ont réuni plus de 150 préhistoriens venus du Sud de la France et des régions alentour. Ces Rencontres biennales représentent le lien majeur de la communauté des chercheurs pour les périodes allant du Mésolithique à l'Âge du Bronze dans la moitié sud de la France. Les Actes présentés ici regroupent 24 contributions réparties en deux sections : l'actualité de la recherche, présentant les principaux résultats des fouilles et programmes récents, et le thème spécifique choisi pour cette session « Économie et société à la fin de la Préhistoire ». Cette thématique large est abordée au fil de 9 contributions très ciblées portant sur les économies de subsistance, de production et d'échange ainsi que sur l'analyse de structures immobilières qui en témoignent, où l'on voit que l'idéel et le matériel ne sont jamais très éloignés dans ces sociétés du passé
