    Clinical significance of Meda and Medovaha Srotas in present scenario - A Comprehensive Study

    The explanation of Sharir in Ayurvedic texts states that the existence of Dosha, Dhatu and Mala together is Sharir. Basically, there are Tridosha, Saptdhatu and Trimala. Meda is one among the Saptdhatu. The internal transport system which is used to carry these Dhatus (Parinampradh) is Srotas. Vitiation and depletion of Srotas can lead to many pathological conditions in body. If Medovaha Srotas is vitiated it will cause Medo Dushti which may be expressed as either Prameha Poorvaroopa or the Sthoulya Vyadhi. And in present era they are compared with diabetes and obesity respectively. The epidemiology of obesity and diabetes is increasing day by day and having serious effect on our current as well as upcoming generations. Obesity has become a worldwide epidemic, and its prevalence has been projected to grow by 40% in the next decade. This increasing prevalence has implications for the risk of diabetes, cardiovascular diseases and also for chronic kidney diseases (CKD) – CKJ OXFORD. In 2017, about 72 million people and 20% of the urban population were diabetic in India - international diabetes federation (IDF). Health service use and medical cost associated with obesity, diabetes and related diseases have risen dramatically and are expected to continue to grow. The above mention data necessitates throw knowledge of the basic concept of the Meda Dhatu, Medovaha Srotas. Thus the present study deals with the understanding of Meda Dhatu, Medovaha Srotas along with its Moola Sthana and the disease that may occur from its vitiation.     Context: Functional constipation which has no underlying organic causes is difficult to be allopathic treatment for long term due to its side effects and undeniable effect, thus a natural balanced and compatible formulation needs to be validated. Objectives: Current study aimed to assess a poly-herbal formulation in management of functional constipation. Material and Methods: This study was 28 days, two armed, randomized, open labeled, prospective clinical study. 60 clinically confirmed cases of functional constipation patients randomized to receive orally either 2 teaspoonful of PP/JLN/107/09-10 Syrup or 1 teaspoonful of 5- 6 gm of Isabgol powder. Results were analyzed as per Rome II criteria and other associated symptoms like headache, acidity, belching, barborgysmy, flatulence and abdominal distension or bloating which are recorded on VAS score. Results: PP/JLN/107/09-10 scored over Isabgol on four out of six parameters of Rome II Criteria viz., frequency of bowel movement, straining at defecation, lumpy I hardstool formation, feeling of incomplete evacuation, feeling of ano-rectal blockage and manual maneuvers (p < 0.001). Trial drugs showed comparable effects (p > 0.05) in reducing the mean scores of associated symptoms like headache, acidity, belching, borgorgysmy, flatulence and abdominal distension. However, trial drug was found to perform statistically significant result in more number of parameters in comparison to Isabgol. Conclusion: PP/JLN/107/09-10 was found to be effective and safe in reliving functional constipation

    The number of outbreaks have  progressively increased since many years in India. In this era of globalization and rapid international travel, any infectious disease in one country can become a potential threat to the entire globe. Outbreaks of Nipah, Zika, Crimean- Congo Haemorrhagic Fever and Kyasanur Forest Disease and have been reported since a decade and now we are facing COVID 19 pandemic. One of the challenges in the prevention of these outbreaks is that as the cases decrease, the felt need declines, the public demand decreases and the mitigation responses get overshadowed by the need of emergency responses elsewhere. The One Health approach is a movement to promote alliance between medicine field, veterinary medicine and environmental sciences to upgrade the health of humans, animals, and ecosystem. The data in this article is compiled from different websites and publications of World Health Organization (WHO), Centre for Disease Control and Prevention (CDC), Integrated Disease Surveillance Programme (IDSP), grey literature and media. There is an urgent need for better surveillance and disease burden assessments in the country and to gain detailed insights into vector biology, factors of environment influencing the diseases, mapping of endemic areas, strengthen intersectoral coordination, infection control practices, and ensure use of Personal Protective Equipment’s (PPE) and availability of drugs and vaccines to handle the outbreaks in a better way

    Carpal alignment in distal radial fractures

    BACKGROUND: Carpal malalignment following the malunited distal radial fracture is described to develop as an adaptation to realign the hand to the malunion. It worsens gradually after healing of the fracture due to continued loading of the wrist. It is also reported to develop during the immobilization itself rather than after fracture healing. The present work was aimed to study the natural course and the quantitative assessment of such adaptive carpal realignment following distal radial fracture. METHODS: In a prospective study, 118 distal radial fractures treated with different modalities were followed-up with serial radiographs for a year for assessment of various radiological parameters. RESULTS: Two patterns of carpal malalignment were identified depending upon the effective radio-lunate flexion (ERLF) measured on pre-reduction radiographs. The midcarpal malalignment was seen in 98 radial fractures (83%) with the lunate following the dorsiflexed fracture fragment and a measured ERLF of less than 25°. The second pattern of radio-carpal malalignment showed the fracture fragment to dorsiflex without taking the lunate with a measured ERLF of more than 25°. The scaphoid did not follow the fracture fragment in both the patterns of malalignment. CONCLUSION: It is better to assess distal radial fractures for any wrist ligamentous injury on the post-reduction film with the restored radial anatomy than on the pre-reduction film since most carpal malalignments get corrected with the reduction of the fracture. Similar carpal malalignment reappear with the redisplacement of the fracture as seen in pre-reduction radiographs and develops during the immobilization rather than as a later compensatory mechanism for the malunion
