    Throughputs of two cohorts of dental students at Sefako Makgatho Health Sciences University: A comparison.

    INTRODUCTION: The numbers of student dentists enrolled at dental schools across the country do not give an indication of the students' progress to degree. AIMS AND OBJECTIVES: To describe and compare the throughputs of dentistry course for two cohorts of students at Sefako Makgatho Health Science University. The progress to degree of the 2005 and 2010 cohorts of first year dental students was tracked and compared. DESIGN: A comparative cross-sectional study. METHODS: Academic records of the 2005 and 2010 cohorts of first year dental students were followed up over a five-year period. Data related to the demographic characteristics, numbers enrolled, numbers who dropped out, and the numbers who graduated were acquired and then captured in Microsoft Excel software. RESULTS: Female students constituted the majority of enrolees in both cohorts (53.8% vs. 51.3%). The proportions of students who started the course, completed the degree and graduated within the regulation time among the 2005 and 2010 cohorts were similar (42.1% vs. 41.2%. A lone student among the 2010 cohort dropped out of the course. The majority of students (57.9% vs. 55.9%) in both cohorts took longer to qualify. CONCLUSIONS: The throughputs of dentistry course for the two cohorts hovered around 40%

    Baseline adjustment methodology in a shared water savings contract during severe water restrictions – a case study in the Western Cape, South Africa

    A novel method for baseline adjustment in a shared water savings contract under serious drought conditions was presented in a companion paper. The newly developed baseline adjustment method was subsequently applied to a case study, as discussed in this manuscript. The case study involved application of the method to 24 relatively complex sites, spread over the Western Cape Province in South Africa. The sites included, for example, military bases, naval dockyards, an airforce base, prison facilities, large multi-storey blocks of flats and administrative office buildings. Baseline adjustment became essential mid-contract during the serious water restrictions in Cape Town at the time. The restrictions were linked to the ‘Day Zero’ scenario in 2018 when water supply would potentially run out, and resulted in water savings at baseline sites that were ascribed to external factors. The study incorporated a comprehensive review of the approved baseline reports with site visits to 12 of the properties. The baseline adjustment method provided a robust means to obtain adjustments for sites with relatively limited data. The minimum data requirement was a record of monthly water consumption per site. The adjustments varied between 0% and 64% of the original baseline value for the different sites in the study sample. The relatively higher adjustments were linked to sites where outdoor irrigation and pool water use was prevalent during the baseline-setting period, but was banned during the drought. Zero adjustments were found for sites with exceptionally high leakage flows that had subsequently been repaired; leaks dwarfed actual use in these cases. The fundamental principle underpinning the baseline adjustment methodology presented in this paper involved segregating real water losses from the actual consumption of end-users, preferably by analysing the minimum night flow. In the absence of recorded night flows, an alternative procedure involving the minimum monthly consumption pre- and post-baseline was employed. The baseline adjustment method was subsequently applied in a South African case study, reported on separately. This technique is helpful because adjustments could be determined without adding unnecessary complexity or cost, and provides a means to resolve disputes in cases where unexpected savings occur mid-contract.Publisher's versio

    Utilization of the ART approach in a group of public oral health operators in South Africa: a 5-year longitudinal study

    Contains fulltext : 80990.pdf (publisher's version ) (Open Access)BACKGROUND: A significant increase in the proportion of restorations to the number of tooth extractions was reported after the introduction of ART in an academic mobile dental service in South Africa. The changes were ascribed to its less threatening procedure. Based on these findings, ART was subsequently introduced into the public oral health service of Ekurhuleni district in the South African province of Gauteng. This article reports on the 5-year restorative treatment pattern of operators in the Ekurhuleni district, who adopted the ART approach into their daily dental practice. METHODS: Of the 21 trained operators, 11 had placed more than 10% of restorations using ART at year 1 and were evaluated after 5 years. Data, including number of restored and extracted teeth and type of restoration, were drawn from clinical records 4 months before, and up to 5 years after training. The restoration/extraction ratio (REX score) and the proportion of ART restorations to the total number of restorations were calculated. The paired sample t-test and linear regression analysis were applied. RESULTS: The mean percentage of ART restorations after 1 year was 24.0% (SE 7.2) and significantly increased annually to 42.7% (SE 9.2) after 5 years in permanent dentitions. In primary dentitions the mean percentage of ART restorations after 1 year was 80.6% (SE 4.9) and 72.6% (SE 8.8) after 5 years. The mean REX score before ART training was 0.08 (SE 0.03) and 0.07 (SE 0.04) for permanent and primary teeth, respectively and 0.11 (SE 0.03) and 0.17 (SE 0.05) after 5 years. CONCLUSION: Five years after training, ART had been used consistently in this selected group of operators as the predominant restorative treatment used for primary teeth and showed a significant annual increase in permanent teeth. However, this change had not resulted in an increase in the REX score in both dentitions

    Tobacco use and oral sex practice among dental clinic attendees

    Tobacco use and oral sex (OS) are important risk factors for oral and oropharyngeal Human papillomavirus (HPV) infection. Little is known about the prevalence of OS practice in South Africa. This study aimed to determine the prevalence of OS practice and tobacco use in a South African patient population. This cross-sectional study used a structured questionnaire to collect socio-demographic characteristics, tobacco use, betel nut use and OS practice data from consenting adults (≥18 years; n = 850). Oral sex practices were recorded for patients 18–45 years-old (n = 514). Data analysis included chi-square and multiple logistic regression analyses. Of the study population, 55.2% (n = 468) were female, 88% (n = 748) self-identified as black Africans and 45.1% (n = 383) were unemployed. Furthermore, 19.7% (n = 167), 6.4% (n = 54) and 2.1% (n = 18) were current smokers, snuff users and betel nut users, respectively. Out of the 514 who answered the questionnaire in relation to OS, 22.8% (n = 115) reported to practice it. Oral sex practice in the age group 18–45 years was most common among the self-identified white participants (41.9%); and among tobacco users than among non-tobacco users (30.9% vs. 20.5%; p = 0.022). A multivariable-adjusted regression model showed that white South Africans were more likely to use tobacco than black Africans (OR = 5.25; 95% CI = 2.21–12.47). The practice of OS was more likely among those 18–35 years-old (OR = 1.67; 95% CI = 1.01–2.74), but had no significant association with tobacco use (OR = 1.06; 95% CI = 0.62–1.83). The observed age and ethnic differences in both risk behaviours suggest a need for targeted population intervention in order to reduce the risk for oral HPV infection.</div

    Atraumatic restorative treatment and dental anxiety in outpatients attending public oral health clinics in South Africa.

    Contains fulltext : 53216.pdf (publisher's version ) (Closed access)OBJECTIVES: This study was undertaken to test the hypotheses that using the atraumatic restorative treatment (ART) approach results in lower patient anxiety and that lower anxiety leads to higher restoration/extraction ratios. METHODS: The test group of dental operators (n = 9) was trained in ART The control group (n = 11) was not, and did not apply ART The Short Form of the Dental Subscale of the Children's Fear Survey Schedule (CFSS-SF) and Corah's Dental Anxiety Scale (DAS) were used to assess patient anxiety after ART (test group) and after traditional restorations (control group). The restoration/extraction ratio calculated for primary (children) and permanent dentitions (adults) per operator was based on 12-month treatment statistics. Dental anxiety assessments were analysed using ANOVA. Differences were compared using the t-test and corrected for confounding factors (ANCOVA). The Pearson correlation coefficient was used to measure the correlation between dental anxiety levels and restoration/extraction ratios. RESULTS: The mean CFSS-SF score for test-group children was statistically significantly lower than for the control-group children. The mean DAS score for test-group adults was statistically significant lower than the control. No significant correlation was observed between dental anxiety level and restoration/extraction ratio per operator for both dentitions in both groups. Conclusion: The first hypothesis was accepted; the second, rejected. Although dental anxiety scores were lower both in child and in adult patients treated by ART than in those who received traditional restorative treatments, this positive effect had not resulted in higher restoration/extraction ratios
