Nurse-Midwivesâ Knowledge and Promotion of Lactational Amenorrhea and Other Natural Family-Planning Methods for Child Spacing
The purpose of this study was to describe and assess certified nurse-midwivesâ (CNMs) knowledge and promotion of two modalities for child spacing, natural family-planning (NFP) and the lactational amenorrhea method (LAM). One thousand two hundred CNMs were randomly selected from a national membership list and mailed a 24-item questionnaire on NFP and LAM. Of the 514 respondents (42.8% return rate), 450 (87.5%) were currently practicing as CNMs. Respondents had an average age of 46 years, with an average of 10 years of practice. CNMs ranked NFP as the ninth most used and the eighth most effective family-planning method in their practice, with an average perceived method-effectiveness of 88% and use-effectiveness of 70%. Although most respondents felt somewhat prepared during their education program to provide NFP, only 22% would offer NFP as a family-planning option for child spacing
The Leader programme 2007-2013: Enabling or disabling social innovation and neo-endogenous development? Insights from Austria and Ireland
Since the beginning of the 1990s, the Leader programme has been hailed as the instrument of rural policy that most explicitly takes account of the territorial dimension. This culminated in the mainstreaming of its underlying concept into the Rural Development Programmes of the current period (2007â2013), with the aim of having more effective policy implementation that considers the diversified needs of rural regions. Starting from analysis of the application and delivery of Leader under the present Rural Development Programme in two EU countries, Austria and Ireland, this paper presents an assessment of the effects of this programme change. In addition, it includes the EU-wide discussion on the (limited) effectiveness of the current implementation of Leader and the search for a reorientation towards local development activities in the EUâs reform proposals. The paper frames the analysis around the notion of social innovation, a concept of central importance to the aims of Leader. It is argued that the implementation of Leader in this period falls far behind its potential to beneficially impact rural regions; hence it should be an object of critical debate in the reform of the Common Agricultural Policy and rural development measures, as well as coherence analyses with other policies, beyond 2013
Unified framework for generalized and transverse-momentum dependent parton distributions within a 3Q light-cone picture of the nucleon
We present a systematic study of generalized transverse-momentum dependent
parton distributions (GTMDs). By taking specific limits or projections, these
GTMDs yield various transverse-momentum dependent and generalized parton
distributions, thus providing a unified framework to simultaneously model
different observables. We present such simultaneous modeling by considering a
light-cone wave function overlap representation of the GTMDs. We construct the
different quark-quark correlation functions from the 3-quark Fock components
within both the light-front constituent quark model as well as within the
chiral quark-soliton model. We provide a comparison with available data and
make predictions for different observables.Comment: version to appear in JHE

Transversity in hard exclusive electroproduction of pseudoscalar mesons
Estimates for electroproduction of pseudoscalar mesons at small values of
skewness are presented. Cross sections and asymmetries for these processes are
calculated within the handbag approach which is based on factorization in hard
parton subprocesses and soft generalized parton distributions (GPDs). The
latter are constructed from double distributions. Transversity GPDs are taken
into account; they are accompanied by twist-3 meson wave functions. For most
pseudoscalar-meson channels a combination of H-tilde_T plays a
particularly prominent role. This combination of GPDs which we constrain by
moments obtained from lattice QCD, leads with the exception of the pi+ and eta'
channels, to large transverse cross sections.Comment: 36 pages and 27 figure
Association Between Advanced Maternal Age and Maternal and Neonatal Morbidity: A Cross-Sectional Study on a Spanish Population
Background and objective: Over recent decades, a progressive increase in the maternal age at childbirth has been observed in developed countries, posing a health risk for both women and infants. The aim of this study was to analyze the association between advanced maternal age (AMA) and maternal and neonatal morbidity.
Material and methods: A cross-sectional study of 3,315 births was conducted in the north of Spain in 2014. We compared childbirth between women aged 35 years or older, with a reference group of women aged between 24 and 27 years. AMA was categorized based on ordinal ranking into 35-38 years, 39-42 years, and >42 years to estimate a dose-response pattern (the older the age, the greater the risk). As an association measure, crude and adjusted Odds Ratios (OR) were estimated by non-conditional logistic regression and 95% Confidence Intervals (95%CI) were calculated.
Results: Repeated abortions were more common among women of AMA in comparison to pregnant women aged 24-27 years (reference group): adjusted OR = 2.68; 95%CI (1.52-4.73). A higher prevalence of gestational diabetes was also observed among women of AMA, reaching statistical significance when restricted to first time mothers: adjusted OR = 8.55; 95%CI (1.12-65.43). In addition, the possibility of an instrumental delivery was multiplied by 1.6 and the possibility of a cesarean by 1.5 among women of AMA, with these results reaching statistical significance, and observing a dose-response pattern. Lastly, there were associations between preeclampsia, preterm birth (<37 weeks) and low birthweight, however without reaching statistical significance.
Conclusion: Our results support the association between AMA and suffering repeated abortions. Likewise, being of AMA was associated with a greater risk of suffering from gestational diabetes, especially among primiparous women, as well as being associated with both instrumental deliveries and cesareans among both primiparous and multiparous women
