    Identification of an Unusual Pattern of Global Gene Expression in Group B Streptococcus Grown in Human Blood

    Because passage of the bacterium to blood is a crucial step in the pathogenesis of many group B Streptococcus (GBS) invasive infections, we recently conducted a whole-genome transcriptome analysis during GBS incubation ex vivo with human blood. In the current work, we sought to analyze in detail the difference in GBS gene expression that occurred in one blood sample (donor A) relative to other blood samples. We incubated GBS strain NEM316 with fresh heparinized human blood obtained from healthy volunteers, and analyzed GBS genome expression and cytokine production. Principal component analysis identified extensive clustering of the transcriptome data among all samples at time 0. In striking contrast, the whole bacterial gene expression in the donor A blood sample was significantly different from the gene expression in all other blood samples studied, both after 30 and 90 min of incubation. More genes were up-regulated in donor A blood relative to the other samples, at 30 min and 90 min. Furthermore, there was significant variation in transcript levels between donor A blood and other blood samples. Notably, genes with the highest transcript levels in donor A blood were those involved in carbohydrate metabolism. We also discovered an unusual production of proinflammatory and immunomodulatory cytokines: MIF, tPAI-1 and IL-1β were produced at higher levels in donor A blood relative to the other blood samples, whereas GM-CSF, TNF-α, IFN-γ, IL-7 and IL-10 remained at lower levels in donor A blood. Potential reasons for our observations are that the immune response of donor A significantly influenced the bacterial transcriptome, or both GBS gene expression and immune response were influenced by the metabolic status of donor A

    Epidural Analgesia Decreases Narcotic Requirements in Low Level Spina Bifida Patients Undergoing Urologic Laparotomy for Neurogenic Bladder and Bowel

    Purpose Concern of anatomical anomalies and worsening neurologic symptoms has prevented widespread use of epidural catheters in patients with low level spina bifida (LLSB). We hypothesize that thoracic epidural placement in the T9-T10 interspace is safe and decreases narcotic requirements in LLSB patients following major open lower urinary tract reconstruction (LUTR). Materials and Methods We reviewed consecutive LLSB patients who had LUTR and epidurals for post-operative pain control. Controls were LLSB patients who received single shot transversus abdominis plane (TAP) blocks with similar procedures. Complications from epidural placement, including changes in motor and sensory status were recorded. Opioid consumption was calculated utilizing equivalent IV morphine doses. Mean and maximum pain scores on post-operative day (POD) 0-3 were calculated. Results 10 LLSB patients who had lower urinary tract reconstruction and epidurals were matched to 10 LLSB patients who had lower urinary tract reconstruction and transverse abdominis plane blocks. Groups were demographically similar. All had full abdominal sensation and functional levels at or below L3. No epidural complications or changes in neurological status were noted. The epidural group had decreased opioid consumption on POD 0-3 (0.75 mg/kg vs. 1.29 mg/kg, p=0.04). Pain scores were similar or improved in the epidural group. Conclusions Thoracic epidural analgesia appears to be a safe and effective opioid sparing option to assist with post-operative pain management following lower urinary tract reconstruction in LLSB patients

    Lateral gene transfer of streptococcal ICE element RD2 (region of difference 2) encoding secreted proteins

    Background: The genome of serotype M28 group A Streptococcus (GAS) strain MGAS6180 contains a novel genetic element named Region of Difference 2 (RD2) that encodes seven putative secreted extracellular proteins. RD2 is present in all serotype M28 strains and strains of several other GAS serotypes associated with female urogenital infections. We show here that the GAS RD2 element is present in strain MGAS6180 both as an integrative chromosomal form and a circular extrachromosomal element. RD2-like regions were identified in publicly available genome sequences of strains representing three of the five major group B streptococcal serotypes causing human disease. Ten RD2-encoded proteins have significant similarity to proteins involved in conjugative transfer of Streptococcus thermophilus integrative chromosomal elements (ICEs). Results: We transferred RD2 from GAS strain MGAS6180 (serotype M28) to serotype M1 and M4 GAS strains by filter mating. The copy number of the RD2 element was rapidly and significantly increased following treatment of strain MGAS6180 with mitomycin C, a DNA damaging agent. Using a PCR-based method, we also identified RD2-like regions in multiple group C and G strains of Streptococcus dysgalactiae subsp.equisimilis cultured from invasive human infections. Conclusions: Taken together, the data indicate that the RD2 element has disseminated by lateral gene transfer to genetically diverse strains of human-pathogenic streptococci

    Examining the Delivery Mode of Mental Practice in Reducing Hemiparesis: A Randomized Controlled Trial

    Background: Mental Practice (MP) is an effective intervention to address upper extremity (UE) hemiparesis post-stroke. However, parameters for the delivery mode of MP have not been defined. Therefore, this study\u27s purpose was to define delivery mode parameters by comparing the effectiveness of audio-guided and video-guided MP. Method: Eighteen participants, \u3c 1-month post-stroke, with UE hemiparesis were randomized to a MP, repetitive task practice (RTP) or control group. The MP groups performed audio-guided or video-guided MP, 5x/week. The RTP group physically performed the functional tasks. The control group received traditional stroke rehabilitation. The Fugl-Meyer Assessment (FMA-UE) and Wolf Motor Function Test (WMFT) were used to assess change in UE hemiparesis. Results: Wilcoxon signed-rank test demonstrated audio MP increased FMA-UE scores from pretest (Mdn = 34.0, Mean = 34.0, SD =9.56) to posttest (Mdn = 49.0, Mean = 49.6, SD =7.5), p = .042, r = .64. Similar improvement in FMA-UE scores was found with traditional therapy. Audio MP decreased WMFT time, pretest (Mdn = 10.5, Mean = 49.9, SD = 59.1) to posttest (Mdn = 4.1, Mean = 3.5, SD = 1.4), p =.043, r =.63. Conclusion: Audio MP and traditional therapy appear to decrease impairment and increase the functional abilities of the UE following stroke. Video MP and RTP does not have this effect

    Adaptation of Group A Streptococcus to Human Amniotic Fluid

    BACKGROUND: For more than 100 years, group A Streptococcus has been identified as a cause of severe and, in many cases, fatal infections of the female urogenital tract. Due to advances in hospital hygiene and the advent of antibiotics, this type of infection has been virtually eradicated. However, within the last three decades there has been an increase in severe intra- and post-partum infections attributed to GAS. METHODOLOGY: We hypothesized that GAS alters its transcriptome to survive in human amniotic fluid (AF) and cause disease. To identify genes that were up or down regulated in response to growth in AF, GAS was grown in human AF or standard laboratory media (THY) and samples for expression microarray analysis were collected during mid-logarithmic, late-logarithmic, and stationary growth phases. Microarray analysis was performed using a custom Affymetrix chip and normalized hybridization values derived from three biological replicates were collected at each growth point. Ratios of AF/THY above a 2-fold change and P-value <0.05 were considered significant. PRINCIPAL FINDINGS: The majority of changes in the GAS transcriptome involved down regulation of multiple adhesins and virulence factors and activation of the stress response. We observed significant changes in genes involved in the arginine deiminase pathway and in the nucleotide de novo synthesis pathway. CONCLUSIONS/SIGNIFICANCE: Our work provides new insight into how pathogenic bacteria respond to their environment to establish infection and cause disease

    Effects of memory load and distraction on performance and event-related slow potentials in a visuospatial working memory task

    Brain electrical activity related to working memory was recorded at 15 scalp electrodes during a visuospatial delayed response task. Participants (N = 18) touched the remembered position of a target on a computer screen after either a 1 or 8 sec delay. These memory trials were compared to sensory trials in which the target remained present throughout the delay and response periods. Distracter stimuli identical to the target were briefly presented during the delay on 30% of trials. Responses were less accurate in memory than sensory trials, especially after the long delay. During the delay slow potentials developed that were significantly more negative in memory than sensory trials. The difference between memory and sensory trials was greater at anterior than posterior electrodes. On trials with distracters, the slow potentials generated by memory trials showed further enhancement of negativity whereas there were minimal effects on accuracy of performance. The results provide evidence that engagement of visuospatial working memory generates slow wave negativity with a timing and distribution consistent with frontal activation. Enhanced brain activity associated with working memory is required to maintain performance in the presence of distraction. © 1997 by the Massachusetts Institute of Technolog

    HOPS 361-C's Jet Decelerating and Precessing Through NGC 2071 IR

    We present a two-epoch Hubble Space Telescope (HST) near-infrared (NIR) study of NGC 2071 IR highlighting HOPS 361-C, a protostar producing an arced 0.2 parsec-scale jet. Proper motions for the brightest knots decrease from 350 to 100 km/s with increasing distance from the source. The [Fe II] and Paβ\beta emission line intensity ratio gives a velocity jump through each knot of 40-50 km/s. We show a new [O I] 63 μ\rm \mum spectrum taken with the German REciever for Astronomy at Terahertz frequencies (GREAT) instrument aboard Stratospheric Observatory for Infrared Astronomy (SOFIA), which give a low jet inclination. Proper motions and jump velocities then estimate total flow speed throughout the jet. We model knot positions and speeds with a precessing jet that decelerates within the host molecular cloud. The measurements are matched with a precession period of a few thousand years and half opening angle of 15deg\rm\deg. The [Fe II] 1.26 μ\rm \mum to 1.64 μ\rm \mum line intensity ratio gives the extinction to each knot ranging from 5-30 mag. Relative to \sim14 mag of extinction through the cloud from C18^{18}O emission maps, the jet is well embedded at a fractional depth from 1/5 to 4/5, and can interact with the cloud. Our model suggests the jet is locally dissipated over 0.2 pc. This may be because knots sweep through a wide angle, giving the cloud time to fill in cavities opened by the jet.
