    Employment Policies for a Green Economy at the European Union Level

    Sustainable economic development requires ensuring economic growth and development in terms of environmental protection by providing a bridge between sustainable economic growth, improvement in human health, social justice, employment and environmental protection. Our paper aims to study the situation of green jobs at the European Union level and the relationship between environment and employment, by analysing the link between employment and environmental policies. It highlights the main trends recorded at the European Union level in the field of employment policy to promote green jobs for sustainable economic development. Although there is little effect from environmental policies on employment, the effects are positive, which shows that the relationship between environmental and employment policy should be continued and improved by measures taken at both the macro- and microeconomic levels

    General practitioners’ perceptions on opportunistic single-time point screening for atrial fibrillation: A European quantitative survey

Objectives: This study evaluated the perception of general practitioners (GPs) about value and practicalities of implementing screening for AF, focusing on opportunistic single-time point screening with a single-lead electrocardiogram (ECG) device. Methods: A descriptive cross-sectional study was conducted with a survey developed to assess overall perception concerning AF screening, feasibility of opportunistic single-lead ECG screening and implementation requirements and barriers. Results: A total of 659 responses were collected (36.1% Eastern, 33.4% Western, 12.1% Southern, 10.0% Northern Europe, 8.3% United Kingdom & Ireland). The perceived need for standardized AF screening was rated as 82.7 on a scale from 0 to 100. The vast majority (88.0%) indicated no AF screening program is established in their region. Three out of four GPs (72.1%, lowest in Eastern and Southern Europe) were equipped with a 12-lead ECG, while a single-lead ECG was less common (10.8%, highest in United Kingdom & Ireland). Three in five GPs (59.3%) feel confident ruling out AF on a single-lead ECG strip. Assistance through more education (28.7%) and a tele-healthcare service offering advice on ambiguous tracings (25.2%) would be helpful. Preferred strategies to overcome barriers like insufficient (qualified) staff, included integrating AF screening with other healthcare programs (24.9%) and algorithms to identify patients most suitable for AF screening (24.3%). Conclusion: GPs perceive a strong need for a standardized AF screening approach.     This study assessed seasonal incidence, economic losses, the efficacy of therapeutic protocols, the recovery time of affected animals and specific prophylactic measures applied to sheep with acute infectious pododermatitis. The studies were conducted over a period of 12 months in 3 different sheep farms from private units in the same area. The results of the study showed an increased incidence of the disease in all 3 farms, with an average of 26.94% of the sheep flock. The incidence of the disease was increased in the months of April-May-June-July and September-October (30%), when there were heavy rains. The high morbidity led to economic losses through the decrease in milk production by approximately 30% and the decrease in the weight of the sheep by 10.58% (4.2 kg) of their normal weight. The therapeutic protocol applied locally as well as parenterally, combined with a foot bath with 10% zinc sulphate solution, were effective in treating acute infectious pododermatitis of sheep. The average recovery time (days) was approximately the same in the three groups of sheep (5.25 ± 0.68 days for cases with moderate diseases and 10.2 ± 0.22 for cases with severe diseases)

    Alternative medicine has no clear definition, consistent and universally accepted, but most often is defined as all products, practices and healthcare systems that are not part of conventional medical system. Complementary medicine or medicine called unconventional, alternative medicine can help treat various diseases where conventional medicine does not offer solutions. Joint diseases, particularly chronic ones, requires a long therapy with anti- inflammatory drugs, often resulting in the occurrence of secondary reactions, which require cessation of therapy, leaving the patient with pain and physical discomfort. For the most efficient and rapid recovery of locomotor or joint functions, it is recommended to apply alternative methods. In most cases of joint diseases, alternative medicine is not used alone, but together with allopathic medicine, conventional treatments to increase the efficiency and represents the combination of the most effective practices and methods of traditional medicine with conventional medicine

    Pediatric radiofrequency catheter ablation for supraventricular and ventricular tachyarrhythmias: a single center retrospective analysis

In recent years radiofrequency catheter ablation (RFCA) has become a standard of care treatment for children and young adults with supraventricular and ventricular tachyarrhythmias. The aim of this study is to evaluate the short-term follow-up of RFCA for ventricular and supraventricular tachyarrhythmias in the pediatric age group. Materials and methods. A total of 63 pediatric patients (mean age at the time of the procedure 12.9±6.7 years, weight 47.98±13.2 Kg), were referred for electrophysiologic study (EPS) and RFCA between April 2004 and April 2018. The electrophysiologic study was performed in a tertiary center using a modified simple technique with the right femoral approach and 2 catheters 5 and 6 French one for stimulation and one for mapping. Referral for the RFCA procedure was done according to the recent guidelines: Class I: WPW syndrome and recurrent and/or symptomatic SVT and age >5 years; WPW syndrome and palpitations with inducible sustained SVT during EPS, age >5 years; Class IIb: WPW syndrome and recurrent and/or symptomatic SVT and age <5 years. Results. A total of 68 EPS procedures were performed in 63 patients. Referral for WPW syndrome in 22 patients (34.9%), atrioventricular node reentry in 20 patients (31.7%), concealed accessory pathways in 8 patients (12.7%), atrial tachycardia in 3 patients (4.8%), atrial flutter in 1 patient (1.6%), permanent ventricular tachycardia was the reason RF ablation in 1 patient (1.6%), premature ventricular beats in 2 patients (3.2%). RFCA was performed under general sedation in 5 patients (7.9%), the rest of the procedures were under exclusively local anesthesia. RFCA success rate was 97.1%, with an arrhythmia recurrence rate of 5.9%. No complications occurred during procedures. In 1 patient (1.6%) with WPW syndrome the mechanical induced atrial fibrillation with hemodynamical compromise needed electrical shock therapy for rhythm control. Mean duration of RFCA was 103.7 min, with a mean radiation exposure time of about 10.5 min and a mean dose of about 1582 μGy/m². A three-dimensional mapping system was needed in 3 RFCA procedures. Conclusion. Radiofrequency catheter ablation is a safe and highly successful option of treatment for the supraventricular and ventricular tachyarrhythmias in pediatric patients. The use of the modified simple technique reduces the risk for major complications

    Observations on the effectiveness of post-operative administration of nutrients and fluids in dogs and cats

    It is estimated that thousands of surgeries are taking place in dogs and cats every year. Today, major trauma associated with operations is minimal due to modern techniques, but nevertheless, the patient's body requires a recovery period, and nutrition is one of the important elements that can speed up the process of healing in many ways, by providing the necessary protein and energy to the body. Pain relief as well as balancing the body after surgery, especially in serious situations, is important for the patient's well-being and comfort, as they contribute to its faster and more effective recovery. The postoperative period in animals requires special attention to the pursuit of the operated patient and the immediate treatment of possible imbalances and complications that may occur. Pain, hypothermia, shock, vomiting, nerve phenomena (excessive vocalizations) are the main phenomena that unspeakable patients manifest in the postoperative period. The mechanisms for the production and maintenance of nocyceps are multiple, tolerance for pain is an individual parameter in relation to the age, gender, terrain and mental state of the patient. Because around postoperative care there is a multitude of opinions and controversies from the owners, sometimes even veterinarians, it was necessary that through this study we make known the benefits of specialized care and the need for permanent monitoring of the patient within the first 72 hours after surgery, by administering drug therapy to reduce pain, relieve the patient, as well as to support large functions

    Diagnostic methods and therapeutic options in dog skin allergies – a short review

    Allergy is an exaggerated reaction of the body, which occurs after the contact with various substances that the immune system considers foreign and acts against them. Like humans, dogs can be allergic to various substances which, when inhaled or absorbed through the skin, airways or gastrointestinal tract, stimulate the immune system by releasing histamine that induces inflammation, erythema, edema and itching. It is considered that all the cases of allergies, whether low, moderate or severe, are genetic in nature. Dogs that become allergic to drugs, vaccines, food, dust, pollen, fleas, various household substances, etc., are "genetically programmed" to have an immune system capable of producing an allergic reaction. The allergic reaction is not due to medication, biological products, food or the environment, but it is considered to be a genetic trait inherited from parents. Because there are different types of allergies and many conditions that can cause skin problems in dogs, the symptoms they present differ from one allergy to another, from one patient to another, requiring both diagnosis and treatment to be made differential for each type of allergy and for each individual. The symptoms of allergic reactions can be diagnosed through allergic tests (intradermal or blood tests), the individual removal of allergens from the dog's living environment (in case of parasitic allergy), or by reagent therapy. Fighting allergies involves the use of antihistamines, antiinflammatory corticosteroids, topical antipruritic substances, leukotrienes inhibitors, as well as various plant extracts with antiallergic effect

    Макросомия плода как прогностический критерий развития хронических неинфекционных экстрагенитальных заболеваний в период менопаузы

    The purpose of the study to elucidate whether maternal pathology during pregnancy can be a prognostic criterion for the occurrence of nontransmissive diseases during menopause. Material and methods. Within the project “Integration of diagnosis, treatment and prophylaxis of noncommunicable diseases in the continuity of maternal and perinatal care based on life cycle” was conducted the longitudinal observational study interviewing. In total, 153 women from several districts of the Republic of Moldova patients were interviewed in the current research, 13 of whom had no previous births. Among the 140 people interviewed with at least 1 birth in the anamnesis - 54.3% of women were in the reproductive period and 45.7% in the menopausal period Results. In the current study, out of 68 menopausal women interviewed, at least 1 birth gave 64 women. Of these 64 women - 18 (28.1%) had at least one birth with a foetus, the weight of which was equal to or exceeded 4000g vs. 46 women (71.9%) who gave birth weighing less than 4000 g. Among the interviewed patients who gave birth to at least one macrosomal fetus (n = 18) during menopause, 88.9% had hypertensive disorders compared to 58.7% in the control group (p <0.05). The thyroid gland pathology had 44.4% (n = 8) of women with a history of macrosomia compared to 19.6 (n = 9) in the group of patients who gave birth to children weighing less than 4000g (OR 3, 3, II 1.01-10.72). Conclusions. 1. Foetal macrosomia is the predictive marker of non-communicable diseases in the menopausal period, especially type II diabetes, hypertension and thyroid gland pathology. 2. Women with a history of macrosomia require screening for endocrinopathy during menopause. At the same time, this contingent of patients should be assigned to the increased risk group for cardiovascular disease, especially hypertensive disorders.Цель исследования. Выяснить, может ли патология матери во время беременности быть прогностическим критерием возникновения нетрансмиссивных хронических заболеваний во время менопаузы. Материал и методы. В рамках проекта «Интеграция диагностики, лечения и профилактики неинфекционных заболеваний в непрерывность материнской и перинатальной помощи на основе жизненного цикла» было проведено продольное обсервационное исследование. Всего в рамках настоящего исследования были опрошены 153 женщины из нескольких районов Республики Молдова, 13 из которых ранее не рожали. Из 140 опрошенных женщин, у которых в анамнезе имелись хотя бы 1 роды, 54,3% женщин были в репродуктивном периоде и 45,7% - в периоде менопаузы. Результаты. В текущем исследовании из 68 опрошенных женщин в менопаузе по крайней мере одни роды имелись у 64 женщин. Из этих 64 женщин- 18 (28,1%) имели одни и более родов плодом, вес которого был равен или превышал 4000 г vs. 46 женщин (71,9%), которые родили детей с массой тела менее 4000 г. Среди опрошенных пациенток, родивших хотя бы один крупный плод (n = 18), во время менопаузы 88,9% имели гипертонические расстройства по сравнению с 58,7% в контрольной группе (p <0,05). Патология щитовидной железы была зарегистрирована в 44,4% случаев (n = 8) родов крупным плодом в анамнезе по сравнению с 19,6 (n = 9) в группе пациенток, родивших детей с массой тела менее 4000 г (OR 3, 3, II 1.01- 10,72). Выводы. 1. Диагноз «крупный плод» в анамнезе является прогностическим маркером неинфекционных заболеваний в период менопаузы, особенно диабета II типа, гипертонии и патологии щитовидной железы. 2. Женщинам с макросомией в анамнезе требуется обследование на эндокринопатию во время менопаузы. При этом этот контингент пациентов следует отнести к группе повышенного риска сердечно-сосудистых заболеваний, особенно гипертонических расстройств

    Влияние социальных, поведенческих и медицинских факторов на развитие непередаваемых заболеваний во время беременности

    Noncommunicable Diseases (NCDs) on the Global Agenda for Sustainable Development Worldwide, adopted by the General Assembly on September 25, 2015 (and adapted to the context of the Republic of Moldova in 2017), identifies 17 goals and 169 targets on three dimensions (economic, social and environmental) of sustainable development. Goal 3 is to ensure “a healthy lifestyle and promote well-being for all, at all ages”, including: By 2030, a one-third reduction in premature mortality from noncommunicable diseases through prevention and treatment, and the promotion of mental health and well-being [ 1]. Purpose: to study the social, behavioral, and metabolic factors involved in the development or worsening of the course of noncommunicable diseases in pregnancy and their influence on pregnancy, childbirth, and perinatal outcomes. Material and methods: to achieve the proposed goal, through a prospective quantitative and qualitative case-control study, conducted within IMSP IMșiC during 2021, were included in the electronic database 35 cases, of which were divided according to the criteria of inclusion and exclusion in 2 groups: Lot I - study including 11 cases with hypertensive conditions in pregnancy, and Lot II - control that includes 24 cases (without NCDs). Results: The presence of 3 or more pregnancies and births in the anamnesis is associated with a high ratio of chances of having a subsequent pregnancy complicated by hypertensive conditions. Non-inflammatory pathologies of the genitals, thyroid gland pathology and obesity are associated with an increased risk of chances of assessing a pregnancy with hypertensive pathology. Snoring, heart palpitations, difficulty breathing on exertion in the daily low and medium activity before pregnancy can be associated with a relative risk of having a complicated pregnancy with hypertension. Maternal hypertension is associated with an increased chance of giving birth to a baby with a restricted growth of the fetus. The complication of pregnancy with hypertensive type of NCDs is associated with the need for oxytocic stimulation during labor 10 times more frequently than in the case of pregnancy without NCDs, and with an increased risk of acute fetal hypoxia.Непередаваемые заболевания (НПЗ) в Глобальной Повестке для Мирового Устойчивого Развития, принятой Генеральной Ассамблеей 25 сентября 2015 г. (и адаптировано к контексту Республики Молдова в 2017 г.), определены 17 целей и 169 задач по трем измерениям (экономическому, социальному и экологическому) устойчивого развития. Цель 3 - обеспечить «здоровый образ жизни и способствовать благополучию для всех в любом возрасте», включая: К 2030 году сокращение на одну треть преждевременной смертности от непередаваемых заболеваний за счет профилактики и лечения, а также содействие психическому здоровью и благополучию [1]. Цель: изучить социальные, поведенческие и метаболические факторы, влияющие на развитие или ухудшение течения непередаваемых заболеваний во время беременности, и их влияние на беременность, роды и перинатальные исходы. Материал и методы: для достижения поставленной цели посредством проспективного количественного и качественного исследования случай-контроль, проведенного в рамках IMSP IMșiC в течение 2021 года, в электронную базу данных были включены 35 случаев, из которых были разделены по критериям включения и исключения в 2 группы: Лот I - исследование, включающее 11 случаев с гипертонической болезню во время беременности, и Лот II - контроль, включающее 24 случая (без НПЗ). Результаты. Наличие в анамнезе 3 и более беременностей и родов связано с высоким соотношением шансов на последующую беременность, осложненную гипертонической болезнью. Невоспалительные патологии половых органов, патология щитовидной железы и ожирение связаны с повышенным риском оценки беременности с гипертонической патологией. Храп, учащенное сердцебиение, затрудненное дыхание при физической нагрузке при ежедневной низкой и средней активности перед беременностью могут быть связаны с относительным риском осложненной беременности с гипертоническими состояниями. Материнская гипертензия связана с повышенным риском рождения ребенка с ограниченным ростом плода. Осложнение беременности при НПЗ гипертонического типа связано с необходимостью стимуляции окситоцином во время родов в 10 раз чаще, чем в случае беременности без НПЗ, и с повышенным риском острой гипоксии плода
